Objective:The purpose of this study was to describe the current situation of moral distress and to explore its influencing factors among Chinese nurses.Methods:This is an exploratory, descriptive design study. A total of 465 clinical nurses from different departments in three Grade-III, Level-A hospitals in Jinan, Shandong Province, completed the questionnaires, including demographics questionnaire, Chinese version of Moral Distress Scale–Revised, and Job Diagnostic Survey.Ethical considerations:The study was approved by the university ethics board and the local health service director.Results:The total score of (...) Moral Distress Scale–Revised was 36.01 ± 24.02 points. The mean frequency and intensity scores of moral distress were 1.13 ± 0.49 and 1.09 ± 0.58, respectively. The level of moral distress among Chinese clinical nurses was low, and the frequency and intensity of moral distress were on low to moderate level. The level of moral distress experienced by clinical nurses is associated with demographics features and job characteristics, including age, education degree, department, task significance, autonomy, and dealing with others.Conclusion:Our conclusion suggests that hospital and organizational administrations should attach much importance to the moral distress experienced by clinical nurses in China. Further studies should focus on interventions about how to reduce the levels of the frequency and intensity of moral distress among clinical nurses. (shrink)

BackgroundThe Basal ganglia (BG) played a crucial role in the brain-level mechanisms of chronic pain disorders. However, the functional changes of BG in chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) are still poorly understood. This study investigated the BG subregions’ resting-state functional connectivity (rs-FC) in CP/CPPS patients compared with healthy controls.MethodsTwenty eight patients with CP/CPPS and 28 age- and education-matched healthy males underwent clinical measurements and 3T brain MR imaging, including T1-weighted structural images and resting-state functional imaging. The data were analyzed (...) by the seeded-based rs-FC analysis. Then, a machine learning method was applied to assess the feasibility of detecting CP/CPPS patients through the changed rs-FC.ResultsCompared with healthy males, patients presented decreased rs-FC between the BG subregions and right middle cingulate cortex, and correlated with pain (r = 0.51, p-uncorrected = 0.005) and urinary symptoms (r = –0.4, p-uncorrected = 0.034). The left superior temporal gyrus and right supramarginal gyrus showed decreased rs-FC with the BG subregions as well. The area under the receiver operating characteristic curve of 0.943 (accuracy = 80%, F1-score = 80.6%) was achieved for the classification of CP/CPPS patients and healthy males with support vector machine (SVM) based on the changed rs-FC.ConclusionThese findings provide evidence of altered BG subregions’ rs-FC in CP/CPPS, which may contribute to our understanding of the BG’s role in CP/CPPS. Regarding the continuous development of high-speed trains and the increase of running speeds, the aerodynamic design of high-speed trains has become significantly important, while reduction of drag and noise comprises a significant challenge in order to optimize aerodynamic design of high-speed trains. The design form factor of a high-speed train is highly influenced by aerodynamic aspects including aerodynamic drag, lift force, and noise. With the high-speed train as the object, the paper aims to take bionic concept as the entry point, (...) selecting the hummingbird as the bionic prototype and extracting bionic elements to establish a bionic train model. Then, the finite volume method was used for numerical simulation and analysis of the aerodynamic performance and aerodynamic noise of the bionic high-speed train. Computational results prove that drag and noise of the bionic head type were lower than those of the original train; drag of the head train of the bionic model was reduced by 2.21% in comparison with the original model, while the whole-train drag was reduced by 3.53%, indicating that drag reduction effects are available and implying that the bionic head type could reduce drag and noise. Noise sources of the bionic train are mainly located at positions with easy airflow separation and violent turbulence motion. Large turbulence energy is in bogie areas and mainly exists at the leeward side of the bogie area. Obviously, the bogie area is the major noise source of the train. Aerodynamic noise of the bionic train in far-field comprises a wide-frequency range. Noises were concentrated within 613 Hz~3150 Hz. When the bionic high-speed train ran at 350 km/h, through comparative analysis of total noise levels at observed points of the high-speed train, it is found that this position with the maximum noise level was 25 m away from the head train nose tip, with the maximum value of 88.4 dB. When the bionic train ran at 600 km/h, the maximum sound pressure level at the longitudinal point was 99.7 dB and the average noise level was 96.6 dB. When the running speed increased from 350 km/h to 600 km/h, the maximum noise level increased by 11.3 dB and the average noise level increased by 11.6 dB. Computation results of aerodynamic noise at the point which is 7.5 m away from the rail center show that the maximum aerodynamic noise level existed at the first-end bogie of the head train, while the noise level was larger at the position closer to the ground. Career planning and career decision are important tasks for college students. The process may be affected by career stress, career exploration, and future work self, with some students facing career indecision. Combining both construction career and proactive motivation model, this study investigated the relationships between career stress, career exploration, future work self, career planning and career indecision among 1,012 Chinese college students using the Structure equation model to test the proposed mediation model. Results show that career stress negatively predicts career (...) planning, while career exploration positively predicts career planning. The mediating role of future work self has on career stress and career exploration's effects on both career planning and career indecision was verified. Moreover, the study also found future work self's positive effects on both career planning and career indecision in Chinese college students. Finally, the study's theoretical and practical significance and implications are discussed. (shrink)

The organization and operation mechanism of innovation and entrepreneurship education is studied according to the current situation of IEE in Chinese art colleges and universities. The IEE system of art colleges and universities is optimized, and a new teaching model of IEE with the characteristics is explored. In addition, the research methods are theoretical analysis, comparative analysis, and empirical analysis. The objects are students from some (...) domestic art academies. The influence of innovation and entrepreneurship on the modern art teaching model is explored. It mainly investigates the awareness and needs of college students’ innovation and entrepreneurship, the professional knowledge of students participating in related activities, the transformation of achievements, and the system and management of IEE. Based on this, a new teaching model of innovation and entrepreneurship is proposed. The six dimensions of creativity, initiative, interest, ideation, independence, and concentration of students in this model are evaluated. The results show that the spirit of innovation and entrepreneurship has a significant role in improving the contemporary art education model on these six dimensions. It is found that this spirit is vital for the development of contemporary art education. In art education, the application of entrepreneurship improves students’ thinking and practical ability. And this spirit is an important part of the construction of contemporary art education. (shrink)

ObjectiveTo investigate the instant impact of hemodialysis on the cerebral morphological measurements of patients with end-stage renal disease.Materials and methodsTwenty-five patients undergoing maintenance HD and twenty-eight age-, sex-, and education-matched healthy control were included. The HD group and HC group had 3D high-resolution structural magnetic resonance imaging scans twice and once, respectively. Both groups underwent neuropsychologic tests. The morphological measurements of structural MRI were measured using CAT12 and these measures were compared among three groups. The relationship between morphological measures and (...) clinical parameters and neuropsychological tests were investigated through multiple regression analysis.ResultsCompared to the HC group, the cortical thickness before HD significantly decreased in the bilateral temporal lobe and significantly decreased in the left superior temporal gyrus after HD. The cortical thickness significantly increased in the bilateral temporal lobe, frontal lobe and occipital lobe after HD compared to before HD. The sulcus depth in the bilateral insula, frontal lobe, and parietal lobe after HD significantly increased compared to before HD. No significant differences in sulcus depth between HD and HC were detected. After HD, the cortical thickness of the right parsopercularis was positively correlated with the number connection test-A. Cortical thickness in multiple regions were positively correlated with blood flow velocity and cortical thickness in the left parahippocampal gyrus was negatively correlated with ultrafiltration volume. Patients showed better performance in the digit symbol test and line tracing test after HD compared to before HD, but there were no significant differences in the comparison of neuropsychologic tests between patients and HC.ConclusionThe instant morphological changes were captured during a single hemodialysis in HD patients. There was an association between these instant changes in the brain and clinical parameters and neuropsychologic tests. This work implied the instant impact of a single hemodialysis impact on the brain in HD patients. (shrink)

BackgroundCognitive frailty includes reversible and potentially reversible subtypes; the former is known as concurrent physical frailty and pre-mild cognitive impairment subjective cognitive decline, whereas the latter is known as concurrent PF and MCI. The diagnoses of pre-MCI SCD and MCI are based on clinical criteria and various subjective cognitive decline questionnaires. Heterogeneous assessment of cognitive impairment results in significant variability of CI, CF, and their subtype prevalence in various population-based studies.ObjectiveThis study aimed to compare the classification differences in CI and (...) CF subtypes from PF and normal cognition by applying clinical and objective cognitive criteria. Clinical criteria comprised Fried PF and clinical MCI criteria combined with the SCD questionnaire, whereas objective criteria comprised Fried PF and objective cognitive criteria based on the norm-adjusted six neuropsychological test scores.MethodsOf the 335 volunteers in this study, 191 were diagnosed with CI based on clinical cognitive diagnosis criteria, and 144 were identified as robust normal based on objective cognitive assessment from the community-dwelling older adult cohort. Individuals with clinical CI, including 94 with MCI and 97 with pre-MCI SCD, were reclassified into different z-score-derived MCI, pre-MCI SCD, and normal subgroups based on objective cognitive criteria. The classification diagnostic accuracy of normal cognition, PF, pre-MCI, MCI, CF, and CF subtypes based on clinical and objective criteria was compared before and after adjusting for age, sex, and education level.ResultsThe reclassification of objective assessments indicated better performance than that of clinical assessments in terms of discerning CI severity among different subgroups before adjusting for demographic factors. After covariate adjustment, clinical assessments significantly improved the ability to cognitively discriminate normal individuals from those with pre-MCI SCD and MCI but not the z-score-derived pre-MCI SCD and MCI groups from the robust normal group. Furthermore, the adjustment did not improve the ability to discriminate among individuals with reversible CF from those with potentially reversible CF and pre-MCI only SCD from MCI only SCD.ConclusionsObjective criteria showed better performance than clinical criteria in the diagnosis of individuals with CI or CF subtypes. Rapid clinical cognitive screening in combination with normative z-scores criteria is cost effective and sustainable in clinical practice. (shrink)

BackgroundThe COVID-19 pandemic called for a new ethical climate in the designated hospitals and imposed challenges on care quality for anti-pandemic nurses. Less was known about whether hospital ethical climate and nurses’ ethical sensitivity were associated with care quality. This study examined the association between the perceived hospital ethical climate and self-evaluated quality of care for COVID-19 patients among anti-pandemic nurses, and explored the mediating role of ethical sensitivity in this relationship.MethodsA cross-sectional study was conducted through an online survey. A (...) total of 399 anti-pandemic nurses from ten designated hospitals in three provinces of China were recruited to fill out an online survey. Multiple linear regression analysis and a bootstrap test were used to examine the relationships between ethical climate, ethical sensitivity and care quality.ResultsNurses reported mean scores of 4.43 ± 0.577 (out of 5) for hospital ethical climate, 45.00 ± 7.085 (out of 54) for ethical sensitivity, and 5.35 ± 0.661 (out of 6) for self-evaluated care quality. After controlling for covariates, perceived hospital ethical climate was positively associated with self-evaluated care quality (direct effect = 0.710, 95% confidence interval [CI] 0.628, 0.792), and was partly mediated by ethical sensitivity (indirect effect = 0.078, 95% confidence interval [CI] 0.002, 0.145).ConclusionsChinese nurses who cared for COVID-19 patients perceived high levels of hospital ethical climate, ethical sensitivity, and self-evaluated care quality. Positive perceptions of hospital ethical climate were both directly associated with a higher level of self-evaluated care quality and indirectly associated, through the mediation effect of ethical sensitivity among anti-pandemic nurses.
