In the last decades it has become clear that medicine must find some way to combine its scientific and humanistic sides. In other words, an adequate notion of medicine requires an integrative position that mediates between the analytic-reductionist and the normative-holistic tendencies we find therein. This is especially important as these different styles of reasoning separate “illness” (something perceived and managed by the whole individual in concert with their environment) and “disease” (a “mechanical failure” of a biological element within the (...) body). While the demand for an integrative view has typically been motivated by ethical concerns, we claim that it is also motivated, perhaps even more fundamentally, by epistemological and methodological reasons. Evidence-based bio-medicine employs experimental and statistical techniques which eliminate important differences in the ways that conscious humans evaluate, live with, and react to disease and illness. However, it is precisely these experiences that underpin the concepts and norms of bio-medicine. Humanistic disciplines, on the other hand, have the resources to investigate these experiences in an intersubjectively testable way. Medicine, therefore, cannot afford to ignore its nature as a human science; it must be concerned not only with disease and illness, but also with the ways in which patients as persons respond to malady. Insofar as attitudes and expectations influence the criteria of illness and disease, they must be studied as part of the genuine subject matter of medicine as a human science. In general, we urge that this is a necessary step to overcome today's trend to split evidence-based and clinical medicine. (shrink)

Falls are frequent in Myotonic Dystrophy type 1, but the pathophysiology of the balance impairment needs further exploration in this disease. The current work aims to provide a richer understanding of DM1 imbalance. Standing balance in 16 patients and 40 controls was tested in two posturographic tests. In the Sensory Organization Test, standstill balance was challenged by combining visual and environmental conditions. In the “react” test, reflexes induced by sudden shifts in the support base were studied. Oscillations of the body (...) centre of mass were measured. In the SOT, COM sway was larger in patients than controls in any condition, including firm support with eyes open. On sway-tuned support, COM oscillations when standing with closed eyes were larger in patients than controls even after taking into account the oscillations with eyes open. In the “react” paradigm, balance reflexes were delayed in patients. Results in both experimental paradigms are consistent with leg muscle weakness. This, however, is not a sufficient explanation. The SOT test highlighted that patients rely on vision more than controls to maintain static balance. Consistently enough, evidence is provided that an impairment of proprioceptive and vestibular systems contributes to falls in DM1. Rehabilitation programs targeted at reweighting sensory systems may be designed to improve safe mobility in DM1. (shrink)

The precarious rights of senior citizens, especially those who are highly educated and who are expected to counsel and guide the younger generations, has stimulated the creation internationally of advocacy associations and opinion leader groups. The strength of these groups, however, varies from country to country. In some countries, they are supported and are the focus of intense interest; in others, they are practically ignored. For this is reason we believe that the creation of a network of all these associations (...) is essential. The proposed network would act as a support for the already-existing policies of the United Nations’ High Commission for Human Rights, of independent experts, and of the Global Alliance for the Rights of Older People. All three have long ago recommended the creation of a recognized instrument for uniting presently scattered efforts. The proposed network, therefore, will seek to promote the international exchange of relevant expertise, and it will reinforce the commitments and actions that single countries are currently taking to meet these objectives. For example, informative public events can be organised to promote particular support initiatives and to provide an opportunity for new members of the network to be presented. The network will promote health for senior citizens, disease prevention, senior mobility, safe free time for seniors, alimentary education, protection against new risks and dangers, as well as equity in the services necessary for seniors to adopt new information and communication technologies. In the case of retired academic members, the network will promote equality with respect to continuing use of digital technologies (particularly email), continuing access to research libraries, and the guaranteed ability for seniors to fund their own research programs and to deliver free seminars. In this paper we investigate ignorance in relation to our capacity to justify our beliefs. To achieve this aim we specifically address scientific misconceptions, i.e. beliefs that are considered to be false in light of accepted scientific knowledge. The hypothesis we put forward is that misconceptions are not isolated false beliefs, but rather form part of a system of inferences—an explanation—which does not match current scientific theory. We further argue that, because misconceptions are embedded in a system, they cannot be (...) rectified simply by replacing false beliefs with true ones. To address our misconceptions, we must rather act on the system of beliefs that supports them. In the first step of our analysis, we distinguish between misconceptions that are easy to dispel because they represent simple errors that occur against the background of a correct explanatory apparatus and misconceptions that are, on the contrary, very difficult to dispel because they are the product of pseudo explanations. We show that, in the latter case, misconceptions constitute an integral part of an incorrect explanation and the reasons that support such misconceptions are deeply misleading. In the second step, we discuss various approaches that have been adopted to address the problem of misconceptions. Challenging the notion that directly addressing and criticizing specific misconceptions is an effective approach, we propose that critical thinking is the most fruitful means to deal with misconceptions. We define the core competences and knowledge relevant for the practice of critical thinking and discuss how they help us avoid misconceptions that arise from accepting beliefs that form part of a mistaken explanation. Il concetto di "libertà" descrive uno dei valori fondamentali della società occidentale contemporanea. Tuttavia, sebbene tutti concordino circa la necessità di difendere la libertà, persone che incarnano convinzioni politiche, morali e sociali diverse interpretano la libertà in maniere differenti, se non perfino contrapposte. Nonostante queste evidenti divergenze, è opinione diffusa che questo concetto si caratterizzi alla sua base per una sorta di denominatore comune, a proposito del quale sussiste un’unanimità pervasiva e che delinea l’orizzonte di una comune battaglia. Questo studio (...) investiga questa opinione da un punto di vista cognitivo allo scopo di determinare come sia costituito e di quale informazione si componga il concetto astratto di “libertà”. La ricerca – che si rifà in parte alla teoria di G. Lakoff – identifica un nucleo cognitivo comune concreto del concetto e chiarisce come questo si contamini ideologicamente nel corso del suo sviluppo da concetto concreto a concetto astratto. Le conclusioni di questo studio evidenziano tuttavia come il nucleo universale del concetto sia eccessivamente indeterminato per costituire un denominatore comune politicamente rilevante che fornisca una base di accordo a proposito di un valore condiviso ed evidenzia alcuni limiti dell'impostazione data da Lakoff al problema, avanzando delle ipotesi per futuri sviluppi della ricerca su questi temi. In recent years, there has been increasing interest in investigating science communication. Some studies that address this issue attempt to develop a model to determine the level of confidence that an author or a scientific community has at a given time towards a theory or a group of theories. A well-established approach suggests that, in order to determine the level of certainty authors have with regard to the statements they make, one can identify specific lexical and morphosyntactical markers which indicate (...) their epistemic attitudes. This method is considered particularly appealing because it permits the development of an algorithmic model based on the quantitative analysis of the occurrence of these markers to assess automatically and objectively the opinion of an author or the predominant opinion of a scientific community on a topic at a given time. In this contribution we show that this line of research presents many kinds of problems especially when it is applied to research articles. To this aim, we propose two main lines of reasoning. The first one relies generally on the argumentative structure of scientific articles and shows that certainty/uncertainty markers are used differently in different argument forms and that therefore their number/frequency of use does not offer reliable indications for determining whether the topic at issue is considered by the authors to be more or less factual/speculative. The second one is based on the analysis of a sample of psychiatric research articles on homosexuality written over a long time span and taken from The British Journal of Psychiatry. Since the psychiatric perspective on homosexuality changed radically during the decades in which these articles were published, they offer an inventory of various kinds of argumentative strategies directed both at defending and confuting dominant as well as marginal positions. We focus especially on uncertainty markers and show that frequently the positions stated using expressions indicating uncertainty are actually not considered as conjectural or speculative by their authors, but that the use of uncertainty markers is motivated by a number of different and often incongruent rhetorical strategies. We present a generalisation of the type-theoretic interpretation of constructive set theory into Martin-Löf type theory. The original interpretation treated logic in Martin-Löf type theory via the propositions-as-types interpretation. The generalisation involves replacing Martin-Löf type theory with a new type theory in which logic is treated as primitive. The primitive treatment of logic in type theories allows us to study reinterpretations of logic, such as the double-negation translation.

This article takes up the commonplace antagonism between ‘second wave’ lesbian feminism and ‘third wave’ queer theory and politics, and argues that the antagonism itself is both historically and politically reductive. First, I make the case that ‘third wave’ queer theory actually shares its central concern – namely, accountability for intra-group inequalities – with lesbian feminism. However, I argue that ‘third wave’ queer theories ultimately founder in their bid for a more reflexive political praxis by tending to hold others – (...) lesbian feminists – accountable for ongoing inequalities rather than grappling with them directly. By contrast, I show that lesbian feminists from the late 1970s to the late 1980s developed a reparative politics that succeeds where ‘third wave’ theories stumble by developing relationships of mutual accountability around issues of race and racism, and by establishing processes by which to repair these relationships when they founder. I conclude by arguing that a fuller attention to the history of lesbian feminism, in fact, offers important resources for feminists and queers dealing with issues of intra-group marginalization, such as transphobia, in the present. We define and investigate Heyting-valued interpretations for Constructive Zermelo–Frankel set theory . These interpretations provide models for CZF that are analogous to Boolean-valued models for ZF and to Heyting-valued models for IZF. Heyting-valued interpretations are defined here using set-generated frames and formal topologies. As applications of Heyting-valued interpretations, we present a relative consistency result and an independence proof.

Summary In the epistemic frame of the biocultural turn and of the neuroaesthetics, we have developed neurohermeneutics as an approach to literature that aims at contributing to the current debate about the linkage between literary, cognitive and neuroscientific studies, focusing on the relationship between mindbrain processes mirrored in the formal features of the text and the strategies activated by the author in a text in order to guide the reader in imagining, emotionally feeling and cognitively getting meanings out of the (...) literary experience. The aim of the neurohermeneutical approach is to grasp and describe phenomenologically the mirroring process between the two extremes of the literary experience, i.e. the writer’s creative process as it is mirrored in the formal features of the literary work and the reader’s imaginative reconfiguration of the text, and what they share in common. The reader revives the mental imaginative processes of the author by creating his/her unrepeatable individual experiences of the text and subjectively redesigning it in an endless loop of features that trigger the imagination and its creative potential both while writing and by reading literature. Syphilis occupies a unique position in the history of U.S. medicine and medical ethics. Given its widespread prevalence and variable presentation, syphilis was a major professional concern among late nineteenth‐ and early twentieth‐century physicians. Syphilis was also at the center of perhaps the most famous example of medical racism in our history, the U.S. Public Health Service Syphilis Study at Tuskegee, in which officials followed the natural history of the disease in a cohort of black men for forty years without (...) offering treatment, even once penicillin became available. In this essay, I call attention to a less well‐known aspect of syphilis and its history. I examine one of the disease's most dreaded manifestations, neurosyphilis (also known as general paresis or general paralysis of the insane), and its treatment via malarial inoculation from the 1920s into the 1950s. The goal of this treatment, dramatic even to observers at the time, was to produce spiking fevers that would kill the syphilitic spirochete and arrest the disease process. My focus will be on developments at St. Elizabeths Hospital, a large‐scale federal psychiatric facility in the District of Columbia that served the city's residents and members of the U.S. military. Physicians at St. Elizabeths were the first in the United States to experiment with malarial fever therapy, treating their initial cohort of patients in December of 1922.Malarial fever therapy raised a host of ethical questions, rendered all the more complex from today's vantage point by efforts to understand them within the context of the time. What sorts of risks, both from malaria and via indirect exposure, might be deemed acceptable in the treatment of a devastating and almost invariably fatal disease? How ought researchers and clinicians to have approached the question of consent, itself an evolving concept in the early decades of the twentieth century, when the disease they were targeting impaired a patient's ability to reason effectively? Finally, at a time when segregated and inferior care for black Americans was the norm and when the black community bore a disproportionate burden of disease, who would have had access to malarial fever therapy, and at what cost? We prove a version of the associated sheaf functor theorem in Algebraic Set Theory. The proof is established working within a Heyting pretopos equipped with a system of small maps satisfying the axioms originally introduced by Joyal and Moerdijk. This result improves on the existing developments by avoiding the assumption of additional axioms for small maps and the use of collection sites.

Following the murder of George Floyd, the Black Lives Matter-movement (BLM) took to the streets to protest against institutional racism. In these protests, one could often hear the slogan “No Justice, No Peace”. Drawing on legal theory, speech act theory and phenomenology, this article investigates what kind of justice and peace are called upon and how the slogan functions as a claim addressed to the legal order. First, the article shows that the rule of law provides a comprehensive normative framework (...) to evaluate what went wrong during the arrest of George Floyd. Second, the article maintains that the slogan should be understood as a passionate utterance in the sense of Stanley Cavell. Finally, drawing on a responsive phenomenology of legal order inspired by the work of Bernhard Waldenfels, the article concludes that the slogan is an extraordinary claim questioning the legal order, showing its contingent origins. A meta-analysis and an experiment show that the degree of compression of the in-depth dimension of visual space relative to the frontal dimension increases quickly as a function of the distance between the stimulus and the observer at first, but the rate of change slows beyond 7 m from the observer, reaching an apparent asymptote of about 50 %. In addition, the compression of visual space is greater for monocular and reduced cue conditions. The pattern of compression of the in-depth (...) dimension as a function of distance is similar to the ratio of in-depth to frontal visual angles of stimuli, but is not as extreme as this ratio would suggest, implying that observers are incapable of fully ignoring size information provided by cues to depth. Size and distance judgments may be described by an Affine transformation of physical space; however, the compression parameter in this model changes as a function of distance from the observer and other experimental conditions. We discuss how transgenerational communitarianism deals with public decisions involving tradeoffs between different generations’ wellbeing and having global consequences. Policies for tackling climate change are an example. Although there is a natural, evolutionary, basis for intergenerational altruism, most people lack the competencies for constituting a transgenerational community. Moreover, greater attention to future generations’ wellbeing need not substitute for collective action: a lower discount rate reflecting a stronger concern for future generations may even worsen their wellbeing. Finally, in a world of (...) irreducible value pluralism, there is no community of persons sharing moral values that can legitimize common policies for addressing global problems: only the common interest of avoiding destructive consequences may motivate collective action to face problems like climate change. Queer theorists have long staked their politics in an engagement with intersectionality. Yet intersectional scholars have been some of queer theory’s most vocal critics, decrying its failure to adequately engage persistent inequalities. I approach this seeming paradox in three parts. First, I situate intersectionality within the field of critical theory, arguing that it shares critical theory’s view of power. Both traditions, I argue, understand power to generate the very marginalized figures that it subordinates. Second, while intersectional and queer theories share (...) this critical insight, the two frameworks offer fundamentally different understandings of what constitutes a democratic politics of redress. Where intersectional theorists promote coalition-building between differently marginalized subjects, queer theorists tend to figure sexually marginalized subjects as exemplary democratic agents. Finally, I argue that this slippage in conceptions of democracy has had negative consequences for critical theory and highlights the difficult but essential role of coalition as a political resource. The paper aims to explore the phenomenon of the spread in democracy of new powers – produced by inexhaustible technological developments – from the perspective of the philosophy of Institutions. It traces the original idea of democracy, in which the «government of the people» arises from the conversion of natural liberty into social and political liberty, dwells on the political and juridical meaning of authority, analyses the traditional instruments used to condition human opinions and behaviours, and reconstructs – in light (...) of this itinerary – the functioning and new grammar of the digital order. What opens before us is a fluid and disorganized scenario, dominated by digital systems, algorithms and artificial intelligence, that draws the attention of philosophers and sociologists, jurists and scholars of language and of anthropology. The old single order, outlined by the political and juridical machine of the modern State – which, through an aloof and solemn language, aimed to impart regularity to human behaviour and to give society direction – is replaced by multifarious models of order, each of which is generated by its own logic, practices, and autonomous control techniques. Under the omnipotence of technology, concepts such as authority, liberty, truth and power undergo a vortex of semantic transformations that penetrate a new symbolic space into human reasoning and actions.
