BackgroundThe preferable position of Deep Brain Stimulation electrodes is proposed to be located in the dorsolateral subthalamic nucleus to improve general motor performance. The optimal DBS electrode localization for the post-operative improvement of balance and gait is unknown.MethodsIn this single-center, retrospective analyses, 66 Parkinson’s disease patients were assessed pre- and post-operatively by using MDS-UPDRS, freezing of gait score, Giladi’s gait and falls questionnaire and Berg balance scale. The clinical outcome was related to the DBS electrode coordinates in x, y, z (...) plane as revealed by image-based reconstruction. Binomial generalized linear mixed models with fixed-effect variables electrode asymmetry, parkinsonian subtype, medication, age class and clinical DBS induced changes were analyzed.ResultsSubthalamic nucleus-deep brain stimulation improved all motor, balance and FoG scores in MED OFF condition, however there were heterogeneous results in MED ON condition. DBS electrode reconstructed coordinates impacted the responsiveness of axial symptoms. FoG and balance responders showed slightly more medially located STN electrode coordinates and less medio-lateral asymmetry of the electrode reconstructed coordinates across hemispheres compared to non-responders.ConclusionDeep brain stimulation electrode reconstructed coordinates, particularly electrode asymmetry on the medio-lateral axis affected the post-operative responsiveness of balance and FoG symptoms in PD patients. Através deles, fingindo-se um deles, o poeta apresenta-se múltiplo, como que habitado por várias pessoas, encerrando vários eus, num jogo literário em que entretanto não se identifica com nenhum deles. Sem desconsiderar a complexidade dessa criação literária, o artigo propõe-se a abordar alguns aspectos da obra do “Pessoa ele-mesmo”. Em certo sentido, Pessoa ele-mesmo é também um heterônimo. Poeta fingidor, nada nele é diretamente confessional (...) ou biográfico. Pessoa ele-mesmo apresenta-se como a expressão dramática de uma busca de identidade, aberta numa constelação de personalidades complexas e sofridas. A dor fingida que finge a dor sofrida e outras representações da dor existencial de Pessoa ele-mesmo podem ser lidas como angústia e mística. Through them, and pretending to be one of them, the poet presents himself as multiple, as if inhabited by several people, assuming several selves, in a literary game in which the poet does not identify himself with any of them. Without ignoring the complexity of such literary creation, the paper aims to address some aspects of the work of “Fernando Pessoa-himself”. In a sense, “Fernando Pessoa-himself” is also a heteronomous that carries a message that is neither confessional nor a bibliographic one. “Fernando Pessoa-himself” presents himself as a dramatic expression of an identity search within a constellation of some complex and suffered personalities. The “pretended pain” that pretends to be a true suffering as well other representations of the existential pain of “Fernando Pessoa-himself” can be comprehended as anxiety and mystique. The purpose of this article is twofold. First, on the basis of a re-examination of the sources and with new arguments, it discusses the meaning of a verse-fragment quoted anonymously by Servius ad Aen. 4.638 (Enn. Ann. Dub. Fr. V Sk. = Inc. 31 blänsdorf, p. 434 courtney), and supports the attribution of this fragment to Ennius’ “Annales”. Second, it demonstrates that this attribution is not undermined by the presence, in the fragment, of a parenthetical phrase that is “in apposition”, (...) a stylistic feature which, though commonly believed to date back to Virgil’s time, may, in fact, be also attested in another Ennian fragment from the “Annales” (Enn. Mind reading (i.e. the ability to infer the mental state of another agent) is taken to be the main cognitive ability required to share an intention and to collaborate. In this paper, I argue that another cognitive ability is also necessary to collaborate: representing others’ and ones’ own goals from a third-person perspective (other-centred or allocentric representation of goals). I argue that allocentric mind reading enables the cognitive ability of goal adoption, i.e. having the goal that another agent’s achieve p (...) because and as long as another agent has that goal that p. Having clarified the relevance of mutual goal adoption for acting jointly, I argue that when an intention is shared between several agents, each individual has an intention in favour of the joint action and one in favour of a joint mode of reasoning. This mode of reasoning is allocentric reasoning. Finally, I elaborate on the consequences of this view for the scientific study of human collaboration. (shrink)

O artigo pauta o desafio que a teologia tem diante de si na tarefa continua de nomear o Real numa cultura pluralista. Tal desafio coloca-se, sobretudo, à teologia fundamental que se encontra diante da afirmação da experiência como locus theologico . Para enfrentar essa temática o artigo propõe duas possíveis posturas de aproximação ao desafio de nomear o Real e, logo depois, partindo de uma das afirmações, enfrenta propriamente a temática da experiência como locus theologico, e suas conseqüências para a (...) teologia. Esta, por sua vez, será abordada diante do deslocamento que a leva da prática da tutela à vocação do discernimento. Assumindo a vocação do discernimento a teologia, em sua tarefa de nomear o Real, adentra o espaço epistemológico da relacionalidade. Tal espaço se afirma na superação de aproximações metafísicas e/ou racionalistas. Palavras-chave: Teologia fundamental – Pluralismo – Experiência.: The article guides the challenge that theology is faced with the task of naming the Real remains in a pluralistic culture. This challenge arises mainly to the fundamental theology which is before the affirmation of experience as a locus theologico. To address this issue the article proposes two possible positions of approaching the challenge of naming the Real, and soon after, leaving one of the statements, properly faces the issue of experience as theologico locus and its consequences for theology. This, in turn, will be addressed before the shift that leads to the practice of guardianship to the vocation discernment. Assuming the discernment of a vocation to theology, in its task of naming the Real, enters the epistemological space of relationality. Is there a circular relationship between trust and ethics? Is it possible to alter their relationship, changing the perception that social actors have of them? How has trust changed in the transition from modernity to post-modernity and how does it change in times of crisis? Starting from the epistemological assumption that progress in the social sciences is determined by the change in the theoretical horizon produced by “a reformulation of metaphysical assumptions” [1] and combining this path with the relational perspective, (...) according to which “not the facts, but the relationship between the facts is what requires analysis” [2], we will examine definitions, meanings, functions and relationships between trust and ethics. Following the theoretical logical method, we will understand that trust and ethical behaviour are particular forms of gift [3] that co-own each other. As such, they are ambivalent in nature and their circle can also produce dysfunctional outcomes that depend on the ability of social systems to modify collective perceptions through forms of communication, in the awareness that distrust constitutes an ineliminable and, paradoxically, preparatory element for the restoration of the trust circle. The relational circle between gift, trust, collective ethics and personal morality does not end with distrust, but changes in a contingent way, determining perverse effects: correct behaviour could produce, unintentionally, a disaster; incorrect actions could generate unforeseen positive effects.The perverse effects cannot be defined as exceptional - as is believed in the theory of rational choice - but recurrent because daily practices are marked by an intuitive, emotional and moralistic trust circle that prevails over logical reasoning, as ascertained by both relational theory and behavioural economics. Films, compared with emotional static pictures, represent true-to-life dynamic stimuli that are both ecological and effective in inducing an emotional response given the involvement of multimodal stimulation (i.e., visual and auditory systems). We hypothesized that a direct comparison between the two methods would have shown greater efficacy of movies, compared to standardized slides, in eliciting emotions at both subjective and neurophysiological levels. To this end, we compared these two methods of emotional stimulation in a group of 40 young adults (20 (...) females). Electroencephalographic (EEG) Alpha rhythm (8–12 Hz) was recorded from 64 scalp sites while participants watched (in counterbalanced order across participants) two separate blocks of 45 slides and 45 clips. Each block included three groups of 15 validated stimuli classified as Erotic, Neutral and Fear content. Greater self-perceived arousal was found after the presentation of Fear and Erotic video clips compared with the same slide categories. sLORETA analysis showed a different lateralization pattern: slides induced decreased Alpha power (greater activation) in the left secondary visual area (Brodmann Area, BA, 18) to Erotic and Fear compared with the Neutral stimuli. Instead, video clips elicited reduced Alpha in the homologous right secondary visual area (BA 18) again to both Erotic and Fear contents compared with Neutral ones. Comparison of emotional stimuli showed smaller Alpha power to Erotic than to Fear stimuli in the left precuneus/posterior cingulate cortex (BA 7/31) for the slide condition, and in the left superior parietal lobule (BA 7) for the clip condition. This result matched the parallel analysis of the overlapped Mu rhythm (corresponding to the upper Alpha band) and can be interpreted as Mu/Alpha EEG suppression elicited by greater motor action tendency to Erotic (approach motivation) compared to Fear (withdrawal motivation) stimuli. Correlation analysis found lower Alpha in the left middle temporal gyrus (BA 21) associated with greater pleasantness to Erotic slides (r38= –0.62,p= 0.009), whereas lower Alpha in the right supramarginal/angular gyrus (BA 40/39) was associated with greater pleasantness to Neutral clips (r38= –0.69,p= 0.012). Results point to stronger emotion elicitation of movies vs. slides, but also to a specific involvement of the two hemispheres during emotional processing of slides vs. video clips, with a shift from the left to the right associative visual areas. SUMMARY. The works of Alexander of Aphrodisias were written a few decades after the publication of the most successful -astrology hand- book in antiquity, Ptolemy's Tetrabiblos Syntaxis, which attempts to naturalize astrology, i.e. to make it agree with Aristotelian theory of science. A comparison of the doctrines between the Tetrabiblos and some passages of Alexander'p works on fate demostrates a noteworthy con¬vergere of the two scholars, and probably a dependence of the lasi great greek Aristotle's exegete on the theories of (...) Ptolemaic astrology, at least in reference to three points: theory of fate; justification of de- vining as a technical and naturai conjecture about man's fate; ana¬lysis of the stars' influence on the sublunary world in physical and phi¬losophical terms. It is argued that the preservation of truth by an inference relation is of little interest when premiss sets are contradictory. The notion of a level of coherence is introduced and the utility of modal logics in the semantic representation of sets of higher coherence levels is noted. It is shown that this representative role cannot be transferred to first order logic via frame theory since the modal formulae expressing coherence level restrictions are not first order definable. Finally, an inference (...) relation, calledyielding, is introduced which is intermediate between the coherence preservingforcing relation introduced elsewhere by the authors and the coherence destroying, inference relation of classical logic. Affective information can be retrieved simply by measuring words co‐occurrences in linguistic contexts. Lenci and colleagues demonstrate that the affective measures retrieved from linguistic occurrences predict words’ concreteness: abstract words are more heavily loaded with affective information than concrete ones. These results challenge the Affective grounding hypothesis, suggesting that abstract concepts may be ungrounded and coded only linguistically, and that their affective load may be a linguistic factor.

Background: Since the passage of the Patient Self-Determination Act, numerous policy mandates and institutional measures have been implemented. It is unknown to what extent those measures have affected end-of-life care, particularly with regard to the do-not-resuscitate order.Methods: Retrospective cohort study to assess associations of the frequency and timing of DNR orders with advance directive status, patient demographics, physician’s specialty and extent of documentation of discussion on end-of-life care.Results: DNR orders were more frequent for patients on a medical service than on (...) a surgical service and were made earlier in the hospital stay for medicine than for surgical patients . 22.18% of all patients had some form of an advance directive in their chart, yet this variable had no impact on the frequency or timing of DNR ordering. Documentation of DNR discussion was significantly associated with the frequency of DNR orders and the time from DNR to death .Conclusions: The physician’s specialty continues to have a significant impact on the frequency and timing of DNR orders, while advance directive status still has no measurable impact. Additionally, documentation of end-of-life discussions is significantly associated with varying DNR ordering rates and timing. (shrink)

Orthogonality in model theory captures the idea of absence of non-trivial interactions between definable sets. We introduce a somewhat opposite notion of cohesiveness, capturing the idea of interaction among all parts of a given definable set. A cohesive set is indecomposable, in the sense that if it is internal to the product of two orthogonal sets, then it is internal to one of the two. We prove that a definable group in an o-minimal structure is a product of cohesive orthogonal (...) subsets. If the group has dimension one, or it is definably simple, then it is itself cohesive. As an application, we show that an abelian group definable in the disjoint union of finitely many o-minimal structures is a quotient, by a discrete normal subgroup, of a direct product of locally definable groups in the single structures. Buddhism, one increasingly hears, is an 'eco-friendly' religion. It is often said that this is because it promotes an 'ecological' view of things, one stressing the essential unity of human beings and the natural world. Buddhism, Virtue and Environment presents a different view. While agreeing that Buddhism is, in many important respects, in tune with environmental concerns, Cooper and James argue that what makes it 'green' is its view of human life. The true connection between the religion and environmental thought (...) is to be found in Buddhist accounts of the virtues - those traits, such as compassion, equanimity and humility, that characterise the life of a spiritually enlightened individual. Central chapters of this book examine these virtues and their implications for environmental attitudes and practice. Buddhism, Virtue and Environment will be of interest not only to students and teachers of Buddhism and environmental ethics, but to those more generally engaged with moral philosophy. The Toronto experience suggests that there may be several general lessons for academic health sciences complexes to learn from the Olivieri/Apotex affair regarding the ethics, independence, and integrity of clinical research sponsored by for profit enterprises. From a local perspective, the OAA occurred when there already was a focus on the complex and changing relationships among the University of Toronto, its medical school, the fully affiliated teaching hospitals, and off campus faculty because of intertwined interests and responsibilities. The OAA became (...) a catalyst that accelerated various systemic reforms, particularly concerning academic/industry relations. In this article, the evolving governance framework for the Toronto academic health sciences complex is reviewed and these policy and process reforms discussed. These reforms have created collaborative activity among research ethics boards and contract research offices of the partner institutions, and allowed the joint university/hospital ethics centre to play a role in governance and policy, while respecting the missions and mandates of the involved institutions. Although few of the policies are dramatically innovative, what is arguably novel is the elaboration of an overarching governance framework that aims to move ethics to a central focus in the academic complex. Time alone will tell how sustainable and effective these changes are. (shrink)
