Background: Advocacy is an important role of psychiatric nurses because their patients are ethically, socially, and legally vulnerable. This study of Japanese expert psychiatric nurses’ judgments of interventions for patient advocacy will show effective strategies for ethical nursing practice and their relationship with Japanese culture. Objectives: This article explores Japanese psychiatric nurses’ decision to intervene as a patient advocate and examine their ethical, cultural, and social implications. Research design: Using semi-structured interviews verbatim, themes of the problems that required interventions were (...) inductively summarized by a qualitative analysis and their contexts and nursing judgments were examined. Participants and research context: The participants were 21 nurses with 5 or more years of experience in psychiatric nursing. Ethical considerations: The research was approved by Institutional Review Board of research site and study facilities. The participants gave written informed consent. Findings: Analysis of 45 cases showed that nurses decided to intervene when surrounding people’s opinions impeded patients’ safety, healthcare professionals’ policies impeded patients’ decision-making, own violent behaviors impeded treatment and welfare services for patients, own or families’ low acceptance of illness impeded patients’ self-actualization, inappropriate treatment or care impeded patients’ liberty, and their families abused patients’ property. Discussion: To solve conflicts between patients and their surrounding people, the nurses sought reconciliation between them, which is in accordance with Japanese cultural norms respecting harmony. When necessary, however, they protected patients’ rights against cultural norms. Therefore, their judgments cannot be explained by cultural norms alone. Conclusion: The findings indicate that the nurses’ judgments were based on respect for patients’ rights apart from cultural norms, and they first sought solutions fitting the cultural norms before other solutions. This seems to be an ethical, effective strategy if advocates know the culture in depth. (shrink)

The pervasive use of distributional semantic models or word embeddings for both cognitive modeling and practical application is because of their remarkable ability to represent the meanings of words. However, relatively little effort has been made to explore what types of information are encoded in distributional word vectors. Knowing the internal knowledge embedded in word vectors is important for cognitive modeling using distributional semantic models. Therefore, in this paper, we attempt to identify the knowledge encoded in word vectors by conducting (...) a computational experiment using Binder et al.'s (2016) featural conceptual representations based on neurobiologically motivated attributes. In an experiment, these conceptual vectors are predicted from text‐based word vectors using a neural network and linear transformation, and prediction performance is compared among various types of information. The analysis demonstrates that abstract information is generally predicted more accurately by word vectors than perceptual and spatiotemporal information, and specifically, the prediction accuracy of cognitive and social information is higher. Emotional information is also found to be successfully predicted for abstract words. These results indicate that language can be a major source of knowledge about abstract attributes, and they support the recent view that emphasizes the importance of language for abstract concepts. Furthermore, we show that word vectors can capture some types of perceptual and spatiotemporal information about concrete concepts and some relevant word categories. This suggests that language statistics can encode more perceptual knowledge than often expected. (shrink)

AbstractThe Belmont Report continues to be held in high regard, and most bioethical analyses conducted in recent years have presumed that it affects United States federal regulations. However, the assessments of the report’s creators are sharply divided. Understanding the historic reputation of this monumental report is thus crucial. We first recount the historical context surrounding the creation of this report. Subsequently, we review the process involved in developing ethical guidelines and describe the report’s features. Additionally, we analyze the effect of (...) unfolding events on the subsequent creation of federal regulations, especially on gene therapy clinical trials. Moreover, throughout this paper we evaluate the ethical principles outlined in this report and describe how they overlap with the issue of protecting socially vulnerable groups. Based on the analysis, we conclude that the features of the Belmont Report cannot be considered as having affected the basic sections of the federal regulations for ethical reviews that were made uniform in 1981. Nevertheless, regarding the regulations on gene therapy clinical trials—which were at first expected to be applicable to research involving children—in addition to implementing policies regarding the public review of protocols that passed ethical review, this report’s principles are clearly reflected in the key notes that should have been referred to when the report was created. In political philosophy, reflective equilibrium is a standard method used to systematically reconcile intuitive judgments with theoretical principles. In this paper, we propose that survey experiments and a model selection method—i.e., the Akaike Information Criterion (AIC)-based model selection method—can be viewed together as a methodological means of satisfying the epistemic desiderata implicit in reflective equilibrium. To show this, we conduct a survey experiment on two theories of distributive justice, prioritarianism and sufficientarianism. Our experimental test case and AIC-based model selection method (...) demonstrate that the refined sufficientarian principle, a widely accepted principle of distributive justice, is no more plausible than the prioritarian principle. This tells us that some changes of certain intuitions revolving around sufficientarianism should be examined (separately) based on the findings of the survey experiment and AIC model selection. This shows the potential of our approach—both practically and methodologically—as a novel way of applying reflective equilibrium in political philosophy. (shrink)

It is often argued that relational egalitarianism has a fundamental problem with intergenerational justice when compared to other theories of justice such as utilitarianism, prioritarianism, and luck egalitarianism. Recently, Timothy Sommers argued that there is no such comparative disadvantage for relational egalitarianism. His argument is quite modest: it merely aims to reject the claim that there could be no way to extend relational egalitarianism to intergenerational justice. This may be called the ‘No Comparative Disadvantage Thesis’. The present article challenges Sommers’s (...) argument in two ways. First, I show that Sommers fails to provide a reasonable constraint on causal efficacy, which is crucial to his argument for our relation (not relationship) to future generations. Second, I show that the chain-relatedness problem casts a shadow over Sommers’s argument for the No Comparative Disadvantage Thesis. More specifically, compared to the other theories of justice, relational egalitarianism (as it stands) can less easily justify that present persons have duties to future others. This is because relational egalitarianism cannot appeal to a specific principle in regard to the duties in question, because its principle is sensitive to current context and practice. I conclude that the No Comparative Disadvantage Thesis does not hold much promise. (shrink)

BackgroundMost medical schools in Japan have incorporated mandatory courses on medical ethics. To this date, however, there is no established means of evaluating medical ethics education in Japan. This study looks 1) To develop a brief, objective method of evaluation for moral sensitivity and reasoning; 2) To conduct a test battery for the PIT and the DIT on medical students who are either currently in school or who have recently graduated (residents); 3) To investigate changes in moral sensitivity and reasoning (...) between school years among medical students and residents.MethodsQuestionnaire survey: Two questionnaires were employed, the Problem Identification Test (PIT) for evaluation of moral sensitivity and a portion of the Defining Issues Test (DIT) for moral reasoning. Subjects consisted of 559 medical school students and 272 residents who recently graduated from the same medical school located in an urban area of Japan.ResultsPIT results showed an increase in moral sensitivity in 4th and 5th year students followed by a decrease in 6th year students and in residents. No change in moral development stage was observed. However, DIT results described a gradual rising shift in moral decision-making concerning euthanasia between school years. No valid correlation was observed between PIT and DIT questionnaires.ConclusionThis study's questionnaire survey, which incorporates both PIT and DIT, could be used as a brief and objective means of evaluating medical students' moral sensitivity and reasoning in Japan. In this article, we address the problem of how people understand predicative metaphors such as “The rumor flew through the office,” and argue that predicative metaphors are understood as indirect (or two-stage) categorizations. In the indirect categorization process, the verb (e.g., fly) of a predicative metaphor evokes an intermediate entity, which in turn evokes a metaphoric category of actions or states (e.g., “to spread rapidly and soon disappear”) to be attributed to the target noun (e.g., rumor), rather than directly evoking (...) a metaphoric category as argued by CitationGlucksberg's (2001) categorization theory. We test our argument using two experiments, namely, offline comprehension and online priming. The offline comprehension experiment showed that interpretation of predicative metaphors had greater overlap with words indirectly associated with the verb than those directly associated with the verb. The online priming experiment demonstrated that indirectly associated words were activated during predicative metaphor comprehension, but directly associated words were not. These results provide convergent evidence for our argument, and thus the psychological validity of two-stage categorization as a process of predicative metaphor comprehension was confirmed. (shrink)

Was Nāgārjuna a thinker of philosophy or religion? This must be a question of the kind to which the answer depends heavily on the definition of “philosophy” and “religion”. Therefore, we may prefer to rephrase this question as: “Can Nāgārjuna legitimately be called a thinker of philosophy or religion?” Although it has been and may still be defined by its method or the objects of “philosophical thinking”, philosophy is, in most cases, expected to have the following characteristics: (1) Philosophy is (...) a type or product of human thought. (2) It is motivated by the human inclination for inquiry into the fundamental basis of being, whether human, social, or natural. (3) It is, theoretically at least, free from any types of dogmatic premises, whether traditional, social, or religious. (4) It requires a thoroughly logical and unequivocal usage of words and sentences. Likewise, although it has been and may still be defined by the existence or number of the absolute(s), be they god(s), spirit(s) or mental state(s), religion is, in most cases, expected to have the following characteristics: (1) Religion is a type of human beliefsystemor a product thereof. (2) It is motivated by the human inclination for ultimate reliance upon the absolute being or state. (3) It is, in practice, not completely free from some types of authoritative premises, whether traditional, customary, or founded by a certain individual. (4) It requires a purified mind, a deep understanding oftradition, constant practice and a careful observance of certain rules and regulations. Taking into account of the above characteristics of philosophy and religion, can we legitimately regard Nāgārjuna as a thinker of philosophy, religion, both, or neither? Let me focus on this question with an analysis of Nāgārjuna’s discussion as found in his magnum opus, Mūlamadhyamakakārikā, and related works. (shrink)

Compared to institutional and area-based ethics committees, little is known about the structure and activities performed by ethics committees at national medical organizations and societies. This five year follow-up study aimed to determine (1) the creation and function of ethics committees at medical organizations in Japan, and (2) their general strategies to deal with ethical problems. The study sample included the member societies of the Japanese Association of Medical Sciences (n=92 in 1998, n=96 in 2003). Instruments consisted of two sections: (...) (1) the structure, function and activities of ethics committees, and (2) the strategies for dealing with ethical problems. Response rates were 84.4% in 1998 and 64.4% in 2003. Findings showed a significant increase of ethics committees at medical organizations between 1998 (25.6%) and 2003 (50.0%). Members were mostly male, medical doctors in clinical or basic medicine, and members of the organization. The major functions of ethics committees were ethical reviews of research protocols, policy making and ethical reviews of manuscripts submitted for journal publication. Among organizations that did not have an ethics committee, a significant decrease was found in organizations that replied that they had never experienced an ethical problem which needed further investigation (p<0.01). Findings suggested an overall rise in awareness of the importance of ethical issues and also highlighted an increase in recognition of responsibility regarding ethical problems. (shrink)

Recent metaphor research has revealed that metaphor comprehension involves both categorization and comparison processes. This finding has triggered the following central question: Which property determines the choice between these two processes for metaphor comprehension? Three competing views have been proposed to answer this question: the conventionality view (Bowdle & Gentner, 2005), aptness view (Glucksberg & Haught, 2006b), and interpretive diversity view (Utsumi, 2007); these views, respectively, argue that vehicle conventionality, metaphor aptness, and interpretive diversity determine the choice between the categorization (...) and comparison processes. This article attempts to answer the question regarding which views are plausible by using cognitive modeling and computer simulation based on a semantic space model. In the simulation experiment, categorization and comparison processes are modeled in a semantic space constructed by latent semantic analysis. These two models receive word vectors for the constituent words of a metaphor and compute a vector for the metaphorical meaning. The resulting vectors can be evaluated according to the degree to which they mimic the human interpretation of the same metaphor; the maximum likelihood estimation determines which of the two models better explains the human interpretation. The result of the model selection is then predicted by three metaphor properties (i.e., vehicle conventionality, aptness, and interpretive diversity) to test the three views. The simulation experiment for Japanese metaphors demonstrates that both interpretive diversity and vehicle conventionality affect the choice between the two processes. On the other hand, it is found that metaphor aptness does not affect this choice. This result can be treated as computational evidence supporting the interpretive diversity and conventionality views. Informed consent, decision-making styles and the role of patient-physician relationships are imperative aspects of clinical medicine worldwide. We present the case of a 74-year-old woman afflicted with advanced liver cancer whose attending physician, per request of the family, did not inform her of her true diagnosis. In our analysis, we explore the differences in informed-consent styles between patients who hold an "independent" and "interdependent" construal of the self and then highlight the possible implications maintained by this position in the context (...) of international clinical ethics. Finally, we discuss the need to reassess informed-consent styles suitable to the needs of each patient regardless of whether he or she resides in the United States or in Japan. (shrink)

This paper aims to specify the precise conditions under which an agent is responsible for inequalities. Admittedly, the careful examination of the conditions in question has been the main focus of contemporary egalitarianism. As a matter of fact, contemporary political philosophers take responsibility to be a core conception which in principle justifies inequalities. In particular, they tend to flesh out the conception of responsibility in terms of choice, in such a way that we should hold individuals responsible for chosen inequalities (...) but not for unchosen inequalities. This core idea is intuitively appealing because, on the one hand, alleviation of inequalities that people do not choose would thereby be encouraged, and on the other hand, it avoids an egalitarian ‘moral hazard’: the situation in which people need care nothing for the consequences (economic or otherwise) of their own choice. G. A. Cohen thus goes so far as to say that egalitarianism successfully incorporates “within it the most powerful idea in the arsenal of the anti-egalitarian right: the idea of choice and responsibility.”. Myhill-Shepherdson property in recursive function theory states that extensional effective program transformations determine continuous operations on partial functions. Case showed that this property fails to characterize acceptability of numberings of partial recursive functions. In this note we present a higher type analogue to Myhill-Shepherdson property. Our purpose is to show that higher type Myhill-Shepherdson property characterizes weak acceptability under a natural condition.

Every acceptable numbering of an effective domain is complete. Hence every effective domain admits the 2nd recursion theorem of Eršov[1]. On the other hand for every effective domain, the 1st recursion theorem holds. In this note, we establish that for effective domains, the 2nd recursion theorem is strictly more general than the 1st recursion theorem, a generalization of an important result in recursive function theory.

The author points out that there is an essential difference as well as correspondence between the idea of the absolute in Fichte’s philosophy and that of the emptiness in Mahayana Buddhism. Both ideas treat the origin of the subject which goes beyond the relation of subject and object. In this point they are in agreement. But if we pay attention to the point that the absolute in Fichte’s philosophy is completely closed and therefore includes no negation in itself, it becomes (...) clear that it is, however, different from the Buddhist emptiness which consists in selfaffirmation through selfnegation. According to the author, the consciousness which works as a creator of the world in Fichte’s system remains out of the absolute. In this sense, Fichte’s idea of the absolute is insufficient as a principle of philosophy.Der Verfasser weist darauf hin, dass es zwischen dem Begriff des Absoluten bei Fichte und dem der Leere im Mahayanabuddhismus neben einer Übereinstimmung auch einen wesentlichen Unterschied gibt. Beide Begriffe stimmen darin überein, dass es ihnen um den Ursprung des Subjekts geht, der über die Relation von Subjekt und Objekt hinausgeht. Wenn man aber die Aufmerksamkeit darauf richtet, dass das Absolute bei Fichte als ein in sich Geschlossenes keine Negation in sich enthält, wird deutlich, dass es sich von der buddhistischen Leere unterscheidet, die in Selbstaffirmation durch Selbstnegation besteht. Bei Fichte hingegen bleibt das Bewusstsein als Weltschöpfer noch außer dem Absoluten. In diesem Sinne ist das Fichtesche Absolute als Prinzip der Philosophie unzureichend. (shrink)

The purpose of this paper is to show the plausibility of John Rawls’s treatment of efficiency within the system of justice. While in political philosophy efficiency is often treated as an independent condition for establishing justice, or more precisely, as a necessary condition for establishing justice, Rawls considers efficiency as a non-negligible factor that has normativity in general circumstances. This is similar to the view that efficiency is a presumptive condition for evaluating social arrangements. However, Rawls’s view is salient in (...) a more substantive way. This paper demonstrates the salience of Rawls’s view of efficiency by responding to G. A. Cohen’s Impure Justice Objection to Rawls’s theory of justice. This shows that there is no impure connection between Rawls’s justice and efficiency. Moreover, the combined thesis of Rawls’s justice and efficiency is superior to Cohen’s pluralist theory of justice, making it a fruitful approach. (shrink)

The trolley problem is a classic thought experiment that evokes an ethical dilemma. Thomson’s “bystander” and “footbridge” versions of the trolley problem induce different intuitive judgments about what to choose in the ethical dilemma. However, we can question how robust these intuitive judgments are. We thus conducted an online survey experiment of Thomson’s versions of the trolley problem which showed that more respondents tended to choose not pulling the lever in the bystander version and pushing a person off the bridge (...) in the footbridge version when they believed they were not being watched. These results illustrate that people’s decisions deviate from the social norms in the absence of the public’s attention. To activate the practice of holding the manufacturers to a normative expectation based on the social norms, we suggest the law must be changed. (shrink)
