The global response to managing the spread of HIV has recently undergone a significant shift with the advent of ‘treatment as prevention’, a strategy which presumes that scaling-up testing and treatment for people living with HIV will produce a broader preventative benefit. Treatment as prevention includes an array of diagnostic, technological and policy developments that are creating new understandings of how HIV circulates in bodies and spaces. Drawing on the work of Michel Foucault, we contextualize these developments by linking them (...) to systems of governance and discursive subjectivation. The goal of this article is to problematize the growing importance of viral suppression in the management of HIV and the use of related surveillance technologies. For people living with HIV, we demonstrate how treatment-as-prevention’s emphasis on individual and collective viral load is transforming the performative dimensions of embodied risk, affect, subjectivity and sex. In this paper, we extend Michel Foucault’s final works on the ‘care of the self’ to an empirical examination of research practice in community-based research (CBR). We use Foucault’s ‘morality of behaviors’ to analyze interview data from a national sample of Canadian CBR practitioners working with communities affected by HIV. Despite claims in the literature that ethics review is overly burdensome for non-traditional forms of research, our findings suggest that many researchers using CBR have an ambivalent but ultimately productive relationship (...) with institutional research ethics review requirements. They understand and use prescribed codes, but adapt them in practice to account for the needs of participating community members, members of their research teams and the larger communities with whom they work. Complying with ethics protocols was seen as only the beginning, a minimum standard; our research suggests that the real ethical work happens in the field, where CBR practitioners encounter community members in diverse public roles and must forge ethical consensus across communities. CBR represents an ethical terrain in which practitioners challenge themselves to work differently, and as a result they care for themselves—and others—in ways that often resist the propensity for domination through public health research.‘…there are different ways to “conduct oneself” morally, different ways for the acting individual to operate, not just as an agent, but as an ethical subject of action.’ (Foucault, 1985: 26). Background There has been growing interest in the use of incentives to increase the uptake of health-related behaviours and achieve desired health outcomes at the individual and population level. However, the use of incentives remains controversial for ethical reasons. An area in which incentives have been not only proposed but used is HIV prevention, testing, treatment and care—each one representing an interconnecting step in the "HIV Cascade." Methods The main objective of this qualitative case study was to document the experiences (...) of health care and service providers tasked with administrating incentivized HIV testing, treatment, and care in British Columbia, Canada. A second objective was to explore the ethical and professional tensions that arise from the use of incentives as well as strategies used by providers to mitigate them. We conducted interviews with 25 providers and 6 key informants, which were analyzed using applied thematic analysis. We also collected documents and took field notes. Results Our findings suggest that incentives target populations believed to pose the most risk to public health. As such, incentives are primarily used to close the gaps in the HIV Cascade by getting the "right populations" to test, start treatment, stay on treatment, and, most importantly, achieve viral suppression. Participants considered that incentives work because they "bring people through the door." However, they believed the effectiveness of incentives to be superficial, short-lived and one-dimensional—thus, failing to address underlying structural barriers to care and structural determinants of health. They also raised concerns about the unintended consequences of incentives and the strains they may put on the therapeutic relationship. They had developed strategies to mitigate the ensuing ethical and professional tensions and to make their work feel relational rather than transactional. Conclusions We identify an urgent need to problematize the use of incentives as a part of the "HIV Cascade" agenda and interrogate the ethics of engaging in this practice from the perspective of health care and service providers. More broadly, we question the introduction of market logic into the realm of health care—an area of life previously not subject to monetary exchanges. (shrink)

In this paper, I aim to discuss E. J. Lowe's view of the synergy between metaphysics and natural science. In doing so, I will extend Lowe’s synergistic model to develop a realist account of theological claims thereby responding to Byrne’s strong form of eliminativism and agnosticism about theological claims. The paper is divided up as follows. In section 1, I will discuss Lowe’s view of metaphysics. In section 2, I will explain how Lowe thinks metaphysics and natural science are related. (...) In section 3, I will respond to objections against Lowe’s conception of metaphysics. In section 4, I will discuss the implications of Lowe’s conception of metaphysics for a realist account of theological claims. In section 5, I will conclude this paper by claiming that there are excellent reasons to extend the synergy between metaphysics and natural science to that of theology as well. (shrink)

Here are the three questions: -/- 1. If the soul is constantly acting upon the brain, then why don't we see physically uncaused spikes in the energy level of the brain? 2. Are the neurons in the brain sufficiently sensitive to respond to such tiny stimuli as would be within (...) the range of energy fluctuations permitted by the Uncertainty Principle? For Lowe, the self is an agent that is capable of carrying out intentional actions. Call this the thesis about the self (TS). In this paper my purpose is to develop and defend TS and argue that Frank Jackson’s serious metaphysics (SM) fails to account for the nature of (...) the self. This paper is outlined as follows: Section I presents Lowe’s theory of the self. In section II, I present Jackson’s central claims of SM. In section III, I develop Jackson’s SM as an objection against Lowe’s TS. In section IV, I respond to objections raised against Lowe’s TS and then critique Jackson’s claim that if an entity is not locatable within the framework of physicalism it should be eliminated. This book presents engaging and informative analysis of three interrelated notions, namely: selfhood, the first person pronoun ‘I’ and the first person perspective. Philosophers have long debated about these notions on non-empirical grounds often focusing on the question of whether the first person pronoun ‘I’, beyond its role as a grammatical term, has an underlying implication for the ontology of selfhood. Philosophers continuously grapple with whether the first person pronoun ‘I’ is a referring expression and if it is, what its (...) referent is or could be. To give an adequate treatment of such questions, philosophers have begun working across the relevant disciplines. This book highlights some excellent examples of the complex nature of the first person thoughts as they figure in linguistics, autism, thought insertion in schizophrenia and the phenomenon of mental autonomy. In Selfhood, Autism and Thought Insertion, many of the leading philosophers working on this issue, as well as a few emerging scholars, have written 12 new essays addressing questions besetting the ontology of selfhood. The essays address topics as diverse as reflections on E. J. Lowe’s non-Cartesian substance dualism, physical determinism, the metaphysics and anti-metaphysics of the self, animalism, neo-Lockean persons, rationality and the first-person, whether the first person is essentially a linguistic concept, first-person and third-person perspectives and autism, consciousness, first person perspective and neuroimaging, thought insertion and mental autonomy. The contributors to this volume do not agree in all of the details associated with the notion of selfhood, yet they all have a common conviction that the central questions besetting it must be taken seriously. Community-based participatory research is growing in popularity as a research strategy to engage communities affected by health issues. Although much has been written about the benefits of using CBPR with diverse groups, this research has usually taken place in community-based organizations which offer social services and programs. The purpose of this article is to explore the opportunities and challenges encountered during a CBPR project conducted in a small hospital serving people living with HIV and addictions issues. The structure of hospital-based (...) care delivery required the team to account for participants’ health-related limitations, including adopting recruitment strategies built on clinical care, and modifying the original research design to better protect participant confidentiality in a small space. Although CBPR offers an important strategy for researching with members of hospitalized communities, it requires research teams to develop context-relevant approaches to ethically engaging community members that recognize the ways clinical space mediates the research process. A. J. Ayer was one of the foremost analytical philosophers of the twentieth century, and was known as a brilliant and engaging speaker. In essays based on his influential Dewey Lectures, Ayer addresses some of the most critical and controversial questions in epistemology and the philosophy of science, examining the nature of inductive reasoning and grappling with the issues that most concerned him as a philosopher. This edition contains revised and expanded versions of the lectures and two additional essays. The criterion of truth is the measure of the truthfulness and reliability of our knowledge. It is also the basis for determining the correctness of our concepts and how much our perceptions, ideas, and concepts accord with objective reality. Idealism holds to the idea that the criterion of truth does not involve the integration between theory as created by human intelligence and objective reality, but rather that the criterion of truth involves the "clarity and correctness" of perception, viewpoints, and concepts (...) by the subject. For instance, the Machists 1 think that the criterion of truth is experience, however, they neither interpret experience from a materialistic viewpoint, nor view experience as the result of humans interacting with nature as they reform it. The Machists view experience as a summary of perceptions and as the subjective experience of humans. In this sense, perception must be tested by perception itself. In attempting to escape the trap of solipsism , they proposed "collective experience" as the criterion of truth. According to such a view, anything that involves "common significance," that is, anything acknowledged by everyone, is the truth. Lenin exposed the absurdity of idealist theory by pointing out that by following the view of "socially formed experience" it is very easy to consider as normal the most absurd and farcical notions, such as ghosts, for such beliefs are also a form of human "experience." Religion also possesses a "common significance," for innumerable people believe in ghosts and miracles, etc. Nevertheless, religion does not become the truth because of this. The concept of the "criterion of truth" held by the Machists played a dominant role in modern bourgeois philosophy. Jean Jacques Rousseau and John Dewey, along with other reactionaries in philosophy all denied scientific criteria. They would rather that the masses remain ignorant of how to understand and determine the truth. (shrink)

It proposes new conjectures and revives several old ones. Throughout there are notes on punctuation, conjecture attribution and an analysis of the structure of Attis’ lament.
