Pressure is mounting to hold researchers and research institutions accountable for the protection of human subjects. When subjects or their family members believe they have been injured, they are increasingly willing to file lawsuits. Recent cases indicate that institutional review boards and their members may be pulled more and more into the legal fray.On September 17, 1999, 18-year-old Jesse Gelsinger died while participating in research conducted by the University of Pennsylvania's Institute for Gene Therapy. Gelsinger was involved in a Phase (...) I clinical trial testing a new approach to treatment of ornithine transcarbamylase deficiency, a rare metabolic disorder. Although infants affected by the severe form of OTC die shortly after birth, Gelsinger suffered from a relatively mild form. It appears undisputed that the cause of Gelsinger's death was not the disorder itself, but multiple organ system failure triggered by the virus used to carry new genetic material into his system. Oral health is a critical part of overall health. The current COVID-19 pandemic has highlighted the importance of oral health. In this article, we describe how dental practice has been impacted by COVID-19, identify the public health response to COVID-19, and explain the gradual resumption of dental care after the initial disruption due to the pandemic. Finally, we discuss how long-standing health disparities in oral health have been exacerbated by the current pandemic.

In this paper, we will consider the role of oaths and codes of ethics in undergraduate medical education. Studies of ethics syllabi suggest that ethics educators typically use well-known bioethics texts such as Beauchamp and Childress. Yet, many issues that medical students will face are addressed by codes of ethics and oaths. We will first provide a historical survey of oaths and codes and then address how these sources of ethical guidance can be effectively used in ethics education of medical (...) students. Oaths and codes can be engagingly taught using a range of techniques including visual narrative. Excerpts from television and film can be used to highlight challenging ethical dilemmas in a variety of settings, taking the learning from the theoretical to the more applied while offering context. Tantrums don’t vanish after the age of three. Aggression is a way of life. Simple communication is a constant challenge. And dreams of a child’s (...) future have to be altered and adapted. In the narratives that accompany this commentary, we see that the challenges of raising a child with autism do not necessarily diminish with age; they may, however, change because children with autism become adults with autism and this presents a new set of both challenges and opportunities.The current epidemic of young children with autism is giving rise to yet another epidemic—the rising number of young adults who are autistic. As one of the authors here mentioned, “the system is in for a tsunami of new individuals needing medical care of all types, needing continued supports for housing and vocational training, communication, occupational and physical therapy” (Cornell). According to a recent study published in Pediatrics, “[y]outh with an ASD have poor postsecondary employment and education outcomes, especially in the first 2 years after high school” (Shattuck, 2012). With the rising number of children being diagnosed with autism, this employment disparity is likely to be exacerbated until or unless the workforce adapts to and accommodates the needs and skills of persons with autism.The needs of young adults with autism are only recently beginning to receive more attention from the media. But for these children’s parents, this issue has always been personal. The emotions on display in these narratives are heartfelt—even raw at times—and seem to swell like the wave of a tsunami itself as parents are now confronting what should be the independence of adulthood. With barriers to employment, health care and housing, young adults with autism remain largely dependent upon their parents. While the topic of transition to adulthood for those with autism is a vast one, implicating the need for wide scale policy change, certain themes emerge in these personal narratives that we wish to focus on here, which may well serve as a guide for that change.The Advocate MotherThe Advocate Mother is a common theme in many narratives of autism and the ones presented here are not different. In fact, nearly all of the narratives presented are written by mothers. Mothers in these narratives exemplify sacrifice, dashed hopes, and even heartbreak. Although there is steely determination in many of these narratives, there is also [End Page 207] self–doubt, anxiety and guilt. As Laura Shumaker recounts the story of taking her adult son to Washington, DC, she experiences great anxiety when her son is mocked. She speaks of “the heavy feeling in [her] chest that mothers get when people laugh at their children.”The advocate mother can easily become the overly consumed mother. Take, for example, the powerful narrative of Nancy Coughlin. She confesses that her daughter’s autism took over her life and the life of her family: “my own mental health was disintegrating, as was my marriage, and in our family’s last–ditch effort to ease the burden we bought a second home; a cheap little place just a few blocks down the road from where we lived. We called it our ‘respite house’.”Others confess that raising a child with autism becomes a “full time job.” (Van der Poel). And the demands of this job do not diminish simply because the child reaches adulthood; it may, in fact, become not only a personal job for some parents but their vocation as well. Den offentlige beslutningsprosessen i bioetiske spørsmål i Norge preges av stor bruk av etiske komiteer. I denne artikkelen reiser jeg spørsmålet om hvordan denne bruken kan begrunnes. Mens en god offentlig prosess krever at det finnes organer som tar seg av nemndenes drøftende og informasjonsgivende funksjon, er det mindre sikkert hvorvidt vi trenger deres rådgivende funksjon, som det til en viss grad er mulig å skille fra de to andre funksjonene. Nemndenes rådgivende funksjon kjennetegnes ved at deres råd gis en (...) spesiell vekt i høringsrunder, uavhengig av argumentene bak rådene, og slik må det også være hvis man skal ha rådgivende nemnder overhodet. Men det er uklart hva som rettferdiggjør at nemndenes råd gis spesiell vekt.Jeg ser på to måter å svare på dette spørsmålet på, enten med utgangspunkt i en moralsk epistemologi som sannsynliggjør at nemndene finner moralsk riktige svar, eller med utgangspunkt i en teori om politisk legitimitet som tilsier at nemndenes konklusjoner har politisk legitimitet. Jeg drøfter og forkaster to former for moralsk epistemologi; én som ser på nemnder som en samling etikkeksperter som kan resonnere seg frem til riktig svar, og én som ser på nemnder som en samling vismenn med spesiell moralsk innsikt. Mer plausibelt er en mellomløsning, som ser på nemndsmedlemmene som de kompetente dommere i en reflektiv likevektprosess. I en slik prosess spiller imidlertid skjønnsvurderinger en uunngåelig rolle, og jeg argumenterer for at beslutninger basert på subjektive skjønnsvurderinger ikke vil være politisk legitime. I den grad nemnder bidrar til politisk legitime beslutninger, er det mer fordi de bidrar til å oppdage løsninger som av uavhengige grunner har politisk legitimitet, enn at de selv gir slik legitimitet. Neither the historical tradition of American constitutionalism nor those who have theorized about it have promoted political or theoretical designs hospitable to the valorization or promotion of democratic virtue. This article illustrates this point by canvassing practical interpretations of the American constitution, from the document of 1787–1791 to Bush v. Gore, and theoretical interpretations from Madison to Rawls, Dworkin, Ackerman, Elster, Holmes, and other contemporary theorists of liberal constitutionalism and natural law. Exposing these roadblocks to the theory and practice (...) of democratic virtue in America, it argues, provides a critical corrective to current debates about the relationship between constitutionalism and democracy. The codification of international human rights law and international humanitarian law, and the accession to those treaties by a large majority of states, does not at first glance seem to have any significant effect upon states' behavior in situations of crisis. Any understanding of the prospects for such law in these situations requires an appraisal of both the motivations of states in concluding these treaties and the pressures on them to ignore them. This paper analyzes those motivations and temptations through (...) the framework of precommitment theory, a component of rational choice theory, originally articulated by Jon Elster and Thomas Schelling. The metaphor of Ulysses offers a useful way to address three different dimensions of the purposes that states join treaties: beliefs, i.e., the state's attitude toward the norms in the treaty; predictions, the state's concerns about its own future behavior; and interactions, i.e., the extent to which the state is trying to influence or bind other states. And it permits linking those motivations to the propensity of states to comply at a later time. The paper offers a four-part typology for why states commit to such treaties, contrasting it with notions from contemporary international relations theories. It then explores the contrasting approaches of compliance theories and international law doctrine to the temptation of states to violate treaties and elaborates the contributions of the precommitment framework. Building upon that framework, it proposes a set of strategies that those concerned with compliance might explore for making international law more relevant when governments seem most inclined to discount it. When we raise children what we are typically aiming for is a kind of flourishing; we want childrento live well as children, and to grow to become adults who live well too. Undoubtedly, part of what we are aiming forwhen we aim for a child’s flourishing is that they meet their developmental milestones well, and that they succeedamong their peers. We are also generally interested in how a child regards themselves; we want children tobelieve that they have value, and that (...) what they care about has value too. We also typically want children to havean appreciation for their relative value among others; that they are important among many other important beings,and we want them to act well in that knowledge. Of course, not all persons who raise children aim for those ends.Some do not care for how a child conceives of themselves, or whether the child flourishes at all. Others care fortheir children, and how they conceive of themselves, but aim poorly. As result, some children come to believe thatthey only deserve love and kindness when they succeed at meeting a caregiver’s ends. Some children come tobelieve that they have no worth at all. And yet other children, rather than believing they are unworthy, come toregard themselves as of superior worth; as children, for example, who bully and sneer at other children for being adifferent race, a different class, gender, ability, etc. may. When a child comes to understand themselves in theseterms, we typically see it at best as morally problematic, and at worst as a tragedy. Despite our interest inchildren’s self-conceptions, and the vast literature on how to conceive of and regard children, there is relatively littleregarding how children should conceive of and regard themselves. In order to explain how children shouldconceive of themselves it is my contention that a new account of self-respect is needed for the context of childhood;one that is responsive to the moral value of children, as well as their developmental capacities to recognize thatvalue. Given that developmental psychology is complex and subject to significant variation as children develop, myaim in this paper is to offer a snapshot of what I take self-respect to amount to in early childhood. This article focuses on the influence of male role models on the lives of adolescents in the U.S. National Longitudinal Lesbian Family Study. Half of the adolescents had male role models; those with and those without male role models had similar scores on the feminine and masculine scales of the Bem Sex Role Inventory, as well as on the trait subscales of the State-Trait Personality Inventory and the Child Behavior Checklist. A positive association was found between feminine gender role traits (...) and curiosity, and a negative correlation between this trait and internalizing problem behavior; these associations were independent of the gender of the adolescents and the presence of male role models. In sum, the absence of male role models did not adversely affect the psychological adjustment of adolescents reared by lesbian mothers. (shrink)
