In this provocative contribution to both psychoanalytic theory and the philosophy of science, Louis Berger grapples with the nature of "consequential" theorizing, i.e., theorizing that is relevant to what transpires in clinical practice. By examining analysis as a genre of "state process formalism" - the standard format of scientific theories - Berger demonstrates why contemporary theorizing inevitably fails to explain crucial aspects of practice. His critique, in this respect, pertains both to the formal structure of psychoanalytic explanation (...) and the technical language through which this structure gains expression. The pragmatic recommendations that issue from this critique are illustrated with respect to a number of perennial problem areas besetting analysis and cognate disciplines. In a discussion that encompases theories of affect, issues in family therapy, the nature of first-language acquisition, and the philisophical topics of free will and determinism, Berger shows that certain systems of representation _can_ describe the psychological realm adequately, and that such systems necessarily follow modern physics in rejecting naive assumptions about the separability of theory and practice. His proposals culminate in a "nonhierarchical" conception of psychoanalytic theory that assigns a separate status to the clinically pragmatic level of theorizing. As such the use of information technologies in drug prescribing and administration has received considerable attention in recent years, with the hope of improving patient safety. Because of the complexity of drug regimens in renal transplant patients, occurrence of medication errors is inevitable even with a well adopted computerized physician order entering system. Our objective (...) was to quantify medication error type and frequency in an inpatient renal transplant unit.Methods: Systemic evaluation of all medication errors during an initial 10-day audit and a 28-day follow-up audit in an inpatient renal transplant unit. Each error was concurrently evaluated for potential to result in adverse patient consequences, error type and associated medication class.Results: A total of 103 clinically significant medication errors were detected during the 10-day and 28-day audit time periods. The most common errors were wrong medication dose ordered and wrong time of drug administration. Thirty-six out of 66 prescribing/ordering errors reached the patient.Conclusions: Even with utilization of computerized physician order entry system in an inpatient renal transplant unit, post-kidney transplant patients are at risk for adverse outcomes due to medication errors. Background Regulations on research ethics in France have evolved considerably over the past four years: the implementation of the Jardé law and of the General Data Protection Regulations have changed the landscape of research ethics for research involving or not involving human persons. In a context of creation of an Institutional Review Board at the University of Bordeaux, France, we sought to explore research ethics practices and perceptions in the medical community of our University Hospital. Methods A short questionnaire was (...) sent to all physicians of the University Hospital of Bordeaux. The questionnaire included closed questions and main topics were: physicians’ education in research ethics, ethics practices concerning researches non implying human persons, and physicians’ perceptions about current regulations. Results 86 questionnaires were sent back. If a majority of physicians have validated Good Clinical Practices trainings, there was a low rate of specific training on fundamental references in research ethics and a high proportion of responders do not consider themselves as educated in research ethics after completion of GCPs. Regulations on research ethics have many implications on medical research, especially by inducing changes in protocols in order to alleviate ethical requirements. Malpractices were acknowledged like false mention of positive opinion from an ethics committee. If If a majority of responders considers regulations as a positive answer to research ethics, a large majority considers it as a constraint and a complexification of research process. For 58%, regulations in research ethics are perceived as a hindrance for research initiatives. Conclusion Because of their impact on research process, regulations seem to constitute a scarecrow for physicians. Lack of training, bad representations and questionable practices highlight the need to improve education and to propose concrete guidance for medical researchers. (shrink)

Children diagnosed with type 1 diabetes mellitus (T1DM) are at risk for neurocognitive sequelae, including impaired attention functioning. The specific nature of the cognitive deficit varies; current literature underscores early age of diabetes diagnosis and increased disease duration as primary risk factors for this neurocognitive decline. Forty-three children with T1DM were evaluated for Attention Deficit/Hyperactivity Disorder (ADHD) symptomatology using the MOXO continuous performance test (MOXO-CPT) performed during a routine outpatient evaluation. The study cohort demonstrated a significant decline in all four (...) domains of attention functioning. The effect was most pronounced with early age at T1DM diagnosis, a longer disease duration and with poorer glycemic control (represented by higher HbA1c values). With increased disease duration (of 5 plus years), acute hyperglycemia was associated with inattention in the real-time setting. These findings highlight the need for routine screening of neurocognitive function in children with T1DM so that early intervention can be employed during this crucial period of cognitive development. Research on teachers’ professional identity integrates many constructs that are treated independently in most cases. This study described the associations between components of teacher professional identity and their association with teachers’ general pedagogical beliefs. Secondary teachers completed a survey about several components of their identity and general pedagogical beliefs. Multidimensional scaling revealed that the components could be mapped on two dimensions: form of motivation and degree of subject specificity. The resulting map revealed four meaningful groups of components. Methods Young healthy heterosexual couples underwent sleep-lab-based polysomnography of two sleeping arrangements: individual sleep and co-sleep. Individual and dyadic sleep parameters (i.e., synchronization of sleep stages) were collected. The latter were assessed using cross-recurrence quantification analysis. Additionally, subjective sleep quality, relationship characteristics, and chronotype were monitored. Data were analyzed comparing co-sleep vs. individual sleep. Interaction effects of the sleeping arrangement with gender, chronotype, or relationship characteristics were moreover tested. Results As compared to sleeping individually, co-sleeping was associated with about 10% (...) more REM sleep, less fragmented REM sleep (p = 0.008), longer undisturbed REM fragments (p = 0.0006), and more limb movements (p = 0.007). None of the other sleep stages was significantly altered. Social support interacted with sleeping arrangement in a way that individuals with suboptimal social support showed the biggest impact of the sleeping arrangement on REM sleep. Sleep architectures were more synchronized between partners during co-sleep (p = 0.005) even if wake phases were excluded (p = 0.022). Moreover, sleep architectures are significantly coupled across a lag of ± 5min. Depth of relationship represented an additional significant main effect regarding synchronization, reflecting a positive association between the two. Neither REM sleep nor synchronization was influenced by gender, chronotype, or other relationship characteristics. Conclusion Depending on the sleeping arrangement, couple's sleep architecture and synchronization show alterations that are modified by relationship characteristics. We discuss that these alterations could be part of a self-enhancing feedback loop of REM sleep and sociality and a mechanism through which sociality prevents mental illness. (shrink)

A more plausible function of argumentation, and thus of reasoning, is to advertise one's ability to detect lies and errors. Such selfish behavior is closer to what we should expect from a naturally selected competence.

RésuméLa sémiotique a inventé ou utilisé plusieurs modèles logiques pour représenter la structure élémentaire de la signification. Le carré sémiotique est sans doute l’un des plus célèbres de ces modèles. Il faut se demander, devant l’importance des phénomènes triadiques, si les modèles dyadiques sont (toujours) adaptés à leur description ou s’il ne faudrait pas se tourner (aussi) vers des modèles triadiques. Or, les modèles triadiques de la structure élémentaire de la signification nous apparaissent bien moins nombreux. À notre connaissance, seule (...) la sémiotique de Peirce et le templum de Boudon occupent cet espace autrement laissé vacant. Nous voulons ici à la fois présenter et exemplifier le templum et le compléter, en nous inspirant du carré sémiotique, pour le faire évoluer vers un « prisme sémiotique ». Nous proposerons également d’autres modèles logiques, comme le pentalemme et le cube sémiotiques. Au passage, nous étudierons plusieurs questions et problèmes – dont la décidabilité – entourant les modèles logiques. The term discourse is generally used either as a technical equivalent for 'verbal communication' or as referring to a particular scientific notion, where discourses are spans of texts or of utterances obeying specific principles of organisation. The aim of this paper is to suggest that an account of discourse is possible, in both cases, only through a theory of utterance-meaning construction. If discourse stands for verbal communication, then it can be explained only with regard to speaker's intended meaning. If discourse (...) stands for organised spans of texts or utterances, then they must be meaningful spans of texts or meaningful utterances. Yet it is argued that a pragmatic explanation of meaning provides all the elements that discourse analysis describes. In the end, the paper claims that a theory of context combined with a theory on the semantic-pragmatic interface should prove sufficient to explain discourse, in whichever sense, along the idea that discourse should be viewed as a process, not as a whole, following the claims of a number of scholars in the field. A possible way to tackle this process is proposed in terms of procedures through the approach of procedural pragmatics. (shrink)

This book offers a bold challenge to modern liberal ethics by exposing its inability to confront the inexorable advance of technology. Contemporary books on technology generally fall into three categories: those that offer optimist projections of a glorious future, those that provide radical critiques of specific techniques, and those that express alarm about the dehumanizing effects of a culture dominated by technology. The End of Ethics in a Technological Society offers a deeper assessment of the modern West's commitment to technological (...) progress. It argues that modern technology, ethics, and politics are all expressions of the enlightenment view that there are no principles of truth or goodness higher than the free human will. Technological advances are, on this view, merely extensions of the range of human freedom. Modern ethics thus fails to give voice to our often inchoate moral intuition that, in the realm of techno science, some possibilities simply ought not to be pursued. The authors develop their challenge by examining typical ethical approaches to such urgent contemporary concerns as environmental degradation, nuclear energy, high tech militarism, and fetal genetic testing. This essay results from a common interest in the history of emotions shared by an academic with appointments in philosophy and psychiatry (Charland) and a literary historian (White). Where our interests converge is in the early modern concept of 'the passions,' as explanatory of what we now call mental illness. The task we have set ourselves is to see how this might: (a) be exemplified in a 'case study' of the dramatic revelation of Leontes's jealousy in the first half of (...) William Shakespeare's The Winter's Tale, and his 'cure' which takes sixteen years. Both events have troubled literary critics as implausible, lacking in realistic motivation, and clumsy as narrative devices, but can be explained in terms of theories of the passions ; and (b) open up a greater, modern understanding of irrational, pathological states, their onset and termination, by using as an explanatory model the early modern understanding of passions as revived and refined in the nineteenth century by Théodule Ribot. Janus-like, this chapter faces into the past and the future (our present) via a play written in 1610 that is still performed to receptive audiences today. We hope to show that the theory can illuminate a playtext, and that the play can revivify the theory, and to contend that historical analysis can shed light on modern clinical problems. The particular historical distinction which lies at the heart of our approach derives from early modern terminology offered by Thomas Wright in his book, The Passions of the Minde in Generall (first published in 1601; corrected, enlarged and augmented in 1604), and presented in more modern fashion in the nineteenth century by Théodule Ribot, in his La Psycholgie des sentiments * We dedicate this essay to Philippa Maddern, whose inspiration has generated so much inter-disciplinary research in the history of emotions and has brought so many like-minded scholars into fertile contact—in our cases, linking Western Ontario and Western Australia. This paper takes a critical look at theory-free, statistical methodologies for processing and interpreting data taken from respondents answering a set of dichotomous (yes-no) questions. The basic issue concerns to what extent theoretical conclusions based on such analyses are invariant under a class of "informationally equivalent" question transformations. First the notion of Boolean equivalence of two question sets is discussed. Then Lazarsfeld's latent structure analysis is considered in detail. It is discovered that the best fitting latent model depends on which (...) one of the many informationally equivalent question sets is used. This fact raises a number of methodological problems and pitfalls with latent structure analysis. Related problems with other methodologies are briefly discussed. (shrink)

The South African Choice on Termination of Pregnancy Act 92 of 1996 gives women the right to voluntary abortion on request. The reality factor, however, is that five years later there are still more ‘technically illegal’ abortions than legal ones. Amongst other factors, one of the main obstacles to access to this constitutionally enshrined human right is the right to conscientious objection/refusal. Although the right to conscientious objection is also a basic human right, the case of refusal to provide abortion (...) services on conscientious objection grounds should not be seen as absolute and inalienable, at least in the developing world. In the developed world, where referral to another service provider is for the most part accessible, a conscientious objector to abortion does not really put the abortion seeker’s life at risk. The same cannot be said in developing countries even when abortion is decriminalised. This is because referral procedures are fraught with major obstacles. Therefore, it is argued that the right to conscientious objection to abortion should be limited by the circumstances in which the request for abortion arises. (shrink)

A great deal of thought has been given to the effects of information technology on reading, books and printed material. Its impact on writing, the production of texts, which is, however, the counterpart of reading, has not aroused the same interest. It is true that witnesses to the act of creation are less familiar objects than books or newspapers: in spite of the passion of the media and the educated public for writers’ manuscripts, these remain predominantly the prerogative of researchers (...) and are still rarely taken out of libraries and archive collections. In extremely literate Western societies, reading is an activity practised by large sectors of the population. The same cannot be said about writing as an active producer of texts, for this encompasses a much more limited group of professionals, and is thought, more or less, to require a special gift in its literary form. Written production is rather the hidden face of McLuhan's Gutenberg galaxy, and the commercial businesses which manufacture and distribute writing in its printed form are better known to the public than the mysterious secrets of creation. The unique aim of these pages is to attempt to answer, at least in certain essential features, this question: What does it mean to read these biblical Writings as inspired? Is it possible to separate in the act of reading a practice that is proper to it and to characterize it as such? This article is in no way about outlining a typology of interpretations of the Bible, nor a summary of the stages of the history of exegesis; it is only concerned with that which the presence of a book of divine origin –a group of books– demand of the reader. Dans la péninsule italienne, à une quarantaine d’années de guerres incessantes fait suite, à partir de 1494, une longue période de paix relative jusqu’à la fin du xviiie siècle. Florence, Venise et Rome sont alors les trois espaces culturels et politiques où naissent les réflexions les plus importantes – et les plus « européennes » – sur la question de la guerre et sur le déploiement d’un « après-guerre ». Après la pensée florentine qui articule politique de conquête et nécessité (...) de la conservation en temps de guerre, « l’après-guerre»s’inscrit dans une tension potentielle, sinon permanente, entre l’impossibilité de la paix et l’acceptation de la domination politico-militaire. La compréhension des effets de cette situation peut passer par l’éloge de la neutralité et de la pacification des rapports de force, notamment dans la pensée vénitienne. Dans cette perspective, la paix constitue un cadre à la fois imposé et en mouvement. Le questionnement concerne du coup ce que la guerre transmet à l’après-guerre. Puisque la guerre est une expérience radicale, une expérience de la limite, elle est du coup porteuse d’un savoir qui acquiert une nécessité : de ce fait, ce savoir continue d’informer la réflexion, même quand la conjoncture s’est modifiée du tout au tout. Ainsi, même lorsqu’on fait passer les raisons de la conservation avant celles de la conquête, on ne remplace pas la guerre par la paix. On a cessé de penser qu’après la guerre était la paix : bien au contraire, on a placé la guerre au principe de la paix. (shrink)

This special double issue of _Cybernetics and Human Knowing_ is comprised of a collection of papers devoted to the cybernetics and mathematics of Charles Sanders Peirce with a special focus on its synergies with George Spencer-Brown's thinking. Peirce was a truly original American philosopher and logician working in the late 1800s and early 1900s; Spencer-Brown is an English polymath, best known as the author of _Laws of Form_. The contributions reflect the extraordinary richness of Peirce's work and his relevance to (...) present concerns in cybernetics. The similarities in the focus on some of the deep foundational subjects are astonishing, amongst those especially the concept of the void or Firstness and the continuity of mind and matter. (shrink)

Cet article étudie l’apparente convergence qui s’opère, depuis quelques années, entre l’ approche en termes de Systèmes Nationaux d’Innovation et la théorie évolutionniste du changement technologique. Ce processus s’appuie sur une conception spécifique du processus d’innovation lui-même. Dans ce contexte, l’innovation est un processus cognitif, qui s’exprime à plusieurs niveaux – local, national, sectoriel et international – et dans de nombreuses dimensions – technologique, organisationnelle et institutionnelle. Cette approche attribue un rôle central aux acteurs publics et aux institutions mais révèle, (...) dans le même temps, les difficultés intrinsèques à toute tentative de synthèse entre des théories souvent hétérogènes.This paper examines the apparent convergence between the National Systems of Innovation approach and evolutionary theories of technological change. This convergence is organized around a complex and specific conception of the innovation process itself In this context, innovation is a cognitive, a multilevel – i.e. local, national, sectorial and international –, a historical and multidimensional – i.e. technological, organizational and institutional – process. This conception gives a central place to public actors and institutions but shows the strong difficulty inherent to the elaboration of a coherent evolutionary theory of economic change. The present study investigates the importance of emotional disclosure and vulnerability in the production of hegemonic masculinities. Of particular interest is the role that silence and invisibility play in how men talk about recent stressful life events. One-on-one interviews with men who experienced a stressful life event in the past year illustrate how men often talk about these events in simultaneously visible and invisible ways. We use the term “cloudy visibility” to describe this engagement, identified both in terms of what (...) men articulate in relation to their past stressful experiences and how they articulate these experiences within the present moment of the interview. The conversational consequences of these linguistic devices are analyzed to illustrate how men obscure their inner emotional lives, thus reproducing hegemonic masculine ideals of staying strong and stoic in the face of adversity, while they also seek to make aspects of their inner lives seen and heard to an interviewer. Dans maintes exégèses récentes des textes de Michel Foucault, sont sans doute négligés la place et les effets théoriques d’une étape particulière de sa pensée, celle qui croise ce que l’on peut appeler « les années 68 ». Le présent article tente d’aborder cette question en s’appuyant sur une attention à la chronologie précise et systématique des écrits et des différentes formes d’interventions de Foucault durant cette période (et durant les années qui suivent dès lors que les passages évoqués peuvent (...) être analysés, tout ou partie, comme des « effets » de ces années-là). Au contraire, la philosophie définie à ce moment-là par Foucault comme une « espèce de journalisme radical » permet de refuser l’attitude des intellectuels–prophètes dans la mesure même où il n’existe pas de philosophie conservatrice ou de philosophie révolutionnaire mais des outils que la philosophie fournit et dont chacun fait l’usage qui lui semble bon. Au fil des années, un réseau de plus en plus dense se mit en place pour penser l’inscription du travail de la pensée dans le présent. Cette tâche peut même remettre en cause l’écriture ou la réorienter radicalement : on en trouve une illustration lors de la constitution du GIP (Groupe information sur les prisons), en 1971. (shrink)
