This paper presents an analysis of the characteristics of men who become stepfathers, and their subsequent fertility patterns and lifetime reproductive success. Because women who already have children are ranked lower in the marriage market than women without children, men who marry women with children (e.g., stepfathers) are likely to have lower rankings in the marriage market as well. Using retrospective fertility and marital histories from the Panel Study of Income Dynamics (PSID), I show that men who become stepfathers have (...) lower levels of education, less income, and are more likely to have been divorced before and to already have children, all characteristics that lower their rankings in the marriage market. Men with one or two stepchildren are just as likely to have children within a marriage as non-stepfathers, although men with three stepchildren show decreased fertility. Among men age 45 and older, stepfathers have lower lifetime fertility than non-stepfathers, although the difference disappears when men’s age at first marriage is controlled for. Additionally, stepfathers have significantly higher fertility than men who never marry. The results suggest that some men become stepfathers to procure mates and fertility benefits that they would otherwise have been unlikely to obtain; for these men, raising other men’s children serves as a form of mating effort. Independent of ecology, subsistence strategy, social complexity, or other aspects of socioecology, the altricial nature of young humans requires mothers to have help raising their offspring. What seems to be context-dependent, however, is who the helpers are, how they invest, and what the impacts of that investment are. Life history theory predicts that greater extrinsic mortality will lead to earlier and higher fertility. To test this prediction, I examine the relationship between life expectancy at birth and several proxies for life history traits (ages at first sex and first marriage, total fertility rate, and ideal number of children), measured for both men and women. Data on sexual behaviors come from the Demographic and Health Surveys (DHS). Two separate samples are analyzed: a cross-sectional sample of 62 countries and a (...) panel sample that includes multiple cross-sectional panels from 48 countries. Multivariate regression analysis is used to control for potential confounding variables. The results provide only partial support for the predictions, with greater support among women than men. However, the prediction is not supported in sub-Saharan African countries, most likely owing to the nonequilibrium conditions observed in sub-Saharan Africa with respect to life expectancy. The applicability of the model to understanding HIV/AIDS risk behaviors is discussed. (shrink)

The hypothesis that father absence during childhood, as well as other forms of childhood psychosocial stress, might influence the timing of sexual maturity and adult reproductive behaviors has been the focus of considerable research. However, the majority of studies that have examined this prediction have used samples of women of European descent living in industrialized, low-fertility nations. This paper tests the father-absence hypothesis using the Cape Area Panel Study (CAPS), which samples young adults in Cape Town, South Africa. The sample (...) contains multiple racial groups (blacks, coloureds [mixed race], and whites) and includes both males and females. Dependent variables include age at menarche, age at first sexual intercourse, and age at first pregnancy. Childhood stress is measured by father absence by age six (either never lived with father or lived with father some but not all years) and an index of childhood exposure to violence (measuring threatened or actual verbal or physical abuse). The hypothesis received no support for effect on age at menarche but was supported for age at first sex and first pregnancy. The model showed stronger support for coloureds and whites than blacks and had no predictive power at all for black males. (shrink)

Teaching bioethics with visual education tools, such as movies and comics, is a unique way of explaining the history and progress of human research and the art and science of medicine to high school students. For more than a decade, bioethical concepts have appeared in movies, and these films are useful for teaching medical and research ethics in high schools. Using visual tools to teach bioethics can have both interpretational and transformational effects on learners that will enhance their overall understanding (...) of complex moral and legal issues in medicine and research.High school students are uniquely suited to learn bioethics because they will soon become legal adults. As adults, they will make moral decisions that may affect their health and wellbeing as well as that of their communities and societies.However, not all visual education tools are appropriate for bioethics pedagogy in high school. Bioethics film and comic producers must consider the specifics of student age, race, gender, belief, level of education, and sexual orientation. Such tools must not be dominated by either dystopic or utopic genres, must aim for objectivity, and must consider the complexity of ethical decision making. It is critical that the teacher, who is the final arbiter regarding the use of visual tools in the classroom, determines that the visual learning tool is acceptable for students in any particular education context. In addition, during the conceptualization and creation of these tools, bioethics film and comic producers must work harder to ensure that these visual tools are devoid of any form of stereotyping. Experience with electronic communication in ethics committees at two hospitals is reviewed and discussed. A listserver of ethics committee members transmitted a synopsis of the ethics consultation shortly after the consultation was initiated. Committee comments were sometimes incorporated into the recommendations. This input proved to be most useful in unusual cases where additional, diverse inputs were informative. Efforts to ensure confidentiality are vital to this approach. They include not naming the patient in the e-mail, requiring a password for access to (...) the listserver, and possibly encryption. How this electronic communication process alters group interactions in ethics committees is a fruitful area for future investigation. (shrink)

The WHO Simulation Initiative is a transnational project looking to support the establishment of simulations of the WHO World Health Assembly across all WHO regions.

Context Physicians are regularly confronted with research that is funded or presented by industry. Objective To assess whether physicians discount for conflicts of interest when weighing evidence for prescribing a new drug. Design and setting Participants were presented with an abstract from a single clinical trial finding positive results for a fictitious new drug. Physicians were randomly assigned one version of a hypothetical scenario, which varied on conflict of interest: ‘presenter conflict’, ‘researcher conflict’ and ‘no conflict’. Participants 515 randomly selected (...) Fellows in the American College of Obstetricians and Gynecologists' Collaborative Ambulatory Research Network; 253 surveys (49%) were returned. Main object measures The self-reported likelihood that physicians would prescribe the new drug as a first-line therapy. Results Physicians do not significantly discount for conflicts of interest in their self-reported likelihood of prescribing the new drug after reading the single abstract and scenario. However, when asked explicitly to compare conflict and no conflict, 69% report that they would discount for researcher conflict and 57% report that they would discount for presenter conflict. When asked to guess how favourable the results of this study were towards the new drug, compared with the other trials published so far, their perceptions were not significantly influenced by conflict of interest information. Conclusion While physicians believe that they should discount the value of information from conflicted sources, they did not do so in the absence of a direct comparison between two studies. This brings into question the effectiveness of merely disclosing the funding sources of published studies. (shrink)

This article focuses on mirror self-recognition, the ability to recognize one's own image in a mirror. It presents the result of the first experiment on mirror self-recognition which showed that chimpanzees are able to learn that the chimps they see in the mirror are not other chimps, but themselves, as evidenced by self-directed behaviour. It reviews evidence for neural network for self-recognition and self-other differentiation and cites evidence that frontal cortex and cortical midline structures are implicated in self-recognition tasks. Most of the time when historians study chemistry the subject dealt with is what might be called élite chemistry. This is chemistry at the cutting edge, chemistry which makes a difference to how we come to understand the properties of matter, molecules, reactions, and so on. Other associated matters which may be explored by historians of chemistry concern social, economic or political relationships with élite chemistry. In this Debus Lecture I want to consider what possibilities there were that the working-class (...) would be able to learn about chemistry in the earlier part of the nineteenth century, and how and why this was achieved. What is a computer? What distinguishes computers from other artificial or natural systems with alleged computational capacities? What does use of a physical system for computation entail, and what distinguishes such use from otherwise identical transformation of that same system when it is not so used? This paper addresses such questions through a theory of information processing artifacts, the class of technical artifacts with physical capacities that enable agents to use them as means to their computational ends. Function ascription, use (...) plan requirements, malfunction, and efficacy of IPAs are all addressed in this theory, with emphasis on artifacts that can be used—reliably or otherwise—for digital computation. By explicitly distinguishing physically grounded computational capacities from user-ascribed computational functions, and by recognizing the distinct roles of each for the implementation of computations in artifacts, this theory clearly distinguishes the use of physical systems for computation from the transformations of physical system states that enable such use. As such, it provides a rigorous basis for distinguishing “computers” from other artificial and natural systems—a distinction whose nature and legitimacy faces ever-evolving challenges from multiple disciplines. This theory, and the associated “instrumental” view of computation in artifacts, naturally accommodates the openminded but scrupulous consideration of radically unconventional physical systems as potential substrates for future computers. According to pancomputationalism, all physical systems – atoms, rocks, hurricanes, and toasters – perform computations. Pancomputationalism seems to be increasingly popular among some philosophers and physicists. In this paper, we interpret pancomputationalism in terms of computational descriptions of varying strength—computational interpretations of physical microstates and dynamics that vary in their restrictiveness. We distinguish several types of pancomputationalism and identify essential features of the computational descriptions required to support them. By tying various pancomputationalist theses directly to notions of what counts as (...) computation in a physical system, we clarify the meaning, strength, and plausibility of pancomputationalist claims. We show that the force of these claims is diminished when weaknesses in their supporting computational descriptions are laid bare. Specifically, once computation is meaningfully distinguished from ordinary dynamics, the most sensational pancomputationalist claims are unwarranted, whereas the more modest claims offer little more than recognition of causal similarities between physical processes and the most primitive computing processes. Background Institutional Review Boards (IRBs) are federally mandated to include both nonscientific and unaffiliated representatives in their membership. Despite this, there is no guidance or policy on the selection of unaffiliated or non-scientist members and reports indicate a lack of clarity regarding members’ roles. In the present study we sought to explore processes of recruitment, training, and the perceived roles for unaffiliated and non-scientist members of IRBs.Methods We distributed a self-administered REDCap survey of members of the Association for the Accreditation (...) of Human Research Protection Programs familiar with IRB member recruitment. The survey included closed and open-ended questions regarding: the operation of the HRPP/IRB(s), how unaffiliated and non-scientist members are recruited, whether they had faced challenges recruiting for these roles, and training and mentorship offered. The survey also collected information regarding the perceived value and roles of unaffiliated and non-scientist members.Results 76 responses were included in the analysis (38% completion rate). The most common approach for recruitment was referral from current IRB members, with almost half of respondents indicating challenges recruiting unaffiliated members. Over 75% indicated no additional training was provided to unaffiliated or non-scientist members compared to affiliated or scientist members. Most common supports provided were travel/parking expenses and honoraria. Commonly perceived roles were to provide an independent voice from the participant perspective, notably regarding consent processes and materials.Conclusions Respondents indicated challenges in defining unaffiliated and non-scientist members and limited practices toward recruitment and support. Future work should more closely examine the challenges in defining these roles and applying the definitions in practice, as well as strategies that may improve recruitment and retention of unaffiliated and non-scientist members. RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These findings extend existing opinion-based recommendations and should guide interventions to improve communication about prognosis in ICUs. (shrink)

This article places the issue of quoting practices in journalism - widely debated in public and professional forums since the Masson-Malcolm (Masson v. New Yorker Magazine, 1991) dispute - into both practical and ethical contexts. It suggests that the multitude of ethical dilemmas facing journalists in the handling of quotations can be addressed by adapting Bok's (1979) test of publicity, which requires that journalists willingly imagine themselves under scrutiny. The spirit of the test asks journalists to embrace this central orienting (...) rhetorical question: Is my behavior such that I'd be comfortable having all my audiences - interview subject, editor, public, and self - simultaneously monitor my choice(s), knowing that I might later be called upon to justify my actions. This collection of articles by top scholars offers fresh interpretations of Mill's ideas about happiness, moral obligation, justice, and rights. Applying contemporary philosophical insights, the articles challenge the conventional readings of Mill, and, in the process, contribute to a deeper understanding of utilitarian theory as well as the complexity of moral life.

This report describes the rationale, approaches, organization, and resource development leading to a large-scale deletion bin map of the hexaploid wheat genome. Accompanying reports in this issue detail results from chromosome bin-mapping of expressed sequence tags representing genes onto the seven homoeologous chromosome groups and a global analysis of the entire mapped wheat EST data set. Among the resources developed were the first extensive public wheat EST collection. Described are protocols for sequencing, sequence processing, EST nomenclature, and the assembly of (...) ESTs into contigs. These contigs plus singletons were used for selection of distinct sequence motif unigenes. Selected ESTs were rearrayed, validated by 5′ and 3′ sequencing, and amplified for probing a series of wheat aneuploid and deletion stocks. Images and data for all Southern hybridizations were deposited in databases and were used by the coordinators for each of the seven homoeologous chromosome groups to validate the mapping results. Results from this project have established the foundation for future developments in wheat genomics. (shrink)
