The goals of vascular composite allotransplantation for hand are to maximize functional status and psychosocial wellbeing and to improve quality of life. Candidates are carefully vetted by transplant programs through an extensive evaluation process to exclude those patients with contraindications and to select those that are most likely to attain functional or quality of life benefit from transplant. Patient choice for any treatment, however, requires that candidates be able to understand the risks, benefits, and alternatives before choosing to proceed. This (...) study aimed to understand patients’ knowledge and perceptions about treatment options for hand loss, including hand transplant. This study will be used to inform a standardized education approach and develop conversation aids for use by clinicians and patients throughout the treatment decision process. Ten individuals who had experienced hand amputation or had congenital limb loss were interviewed to better understand previous and current decisions about treatment, experiences in adjusting to their treatment, and perceptions about hand VCA. From this qualitative interview data, four findings emerged: knowledge and education around VCA as a treatment option; adaptation of individuals with limb loss; fear of risk associated with transplantation; issues of aging and overuse injuries to existing limbs. Results suggests that there is opportunity for expanding education about all treatment options for patients with new loss, long-term loss, and congenital limb loss. Establishing a baseline of knowledge about all options–prosthetics, rehabilitative strategies, and VCA—can help patients evaluate their values and goals of treatment. Issues associated with aging, including overuse and injury, and adaptability over the life course should be included in considerations about treatment choices. Data indicate the need for routinely assessing patient preferences about treatment choice so patients can plan for their future as they adapt and age and as technology for treatments change. To assure that thorough information is provided for current and future decision-making, education about treatment choices and selection procedures for VCA should be standardized. (shrink)

Reconstructive allografts using Vascularized Composite Allotransplantation are providing individuals living with upper limb loss and facial disfigurement with new opportunities for a sensate, esthetically acceptable, and functional alternative to current treatment strategies. Important research attention is being paid to how best to assess and screen candidates for VCA, measure optimal patient outcomes, and support patient adherence to lifelong behaviors and medical regimens. Far less attention, however, has been dedicated to the team science required for these complex VCA teams to form, (...) prepare, and provide the highest quality clinical and psychosocial care to those receiving VCA. VCA teams are unique in that they require specialized team members whose scope of practice may not otherwise overlap. The team also needs to constantly negotiate balancing patient safety with multiple risks throughout the transplant process. This study aimed to elucidate the team science needed for this highly innovative and complex area of medicine. Using in-depth qualitative interviews with 14 VCA team members and observations at team meetings, we found that careful consideration of team composition, team structure, and organizational commitment influences team performance and patient outcomes, but that to be efficient and truly effective, teams need to commit to developing processes that foster collaboration. These processes are action-oriented, strategic and interpersonal. Dedication and commitment to team science allows teams to manage conflict under stress and exercise ways to leverage strengths to provide optimal performance or patient psychosocial and clinical outcomes. This study can provide insight into quality improvement efforts for VCA teams and guidance for other transplant programs that wish to consider expansion into VCA. (shrink)

Breast cancer is a major public health challenge. Organized mammography screening (OS) is considered one way to reduce breast cancer mortality. EU recommendations prone mass deployment of OS, and back in 2004, France introduced a national OS programme for women aged 50–74 years. However, in 2012, participation rate was still just 52.7%, well short of the targeted 70% objective. The aim of this paper is to show how systematic Charles Taylor’s philosophy is. It rejects two opposite readings: one claiming that Taylor’s thought is too diverse to have real unity; the other, that it is the product of a “monomaniac” (Taylor’s own word). I claim that his thought has a very distinct structure, comprising two levels. On the first, “meta-hermeneutic” level, Taylor defends a thesis about hermeneutics (namely, that it cannot be dispensed with): this unifies his anthropology, epistemology, moral (...) philosophy, philosophy of language and political philosophy. On the second, “hermeneutic” level, Taylor builds an impressive historic construal of modern identity and its dilemmas. More importantly, while these two levels are irreducibly distinct, they relate to each other in interesting ways, giving Taylor’s philosophy its systematicity. I finally confront this view with other readings, and argue that it is the best way to understand Taylor’s work. (shrink)

In this article, we first describe our general inhibitory-control theory and, then, we describe how we have tested its specific hypotheses on reasoning with brain imaging techniques in adults and children. The innovative part of this perspective lies in its attempt to come up with a brain-based synthesis of Jean Piaget’s theory on logical algorithms and Daniel Kahneman’s theory on intuitive heuristics.

This article illustrates instrumental variable (IV) estimation by examining an unexpected finding of the research on career adaptability and job satisfaction. Theoretical and empirical arguments suggest that in the general population, people’s abilities to adapt their careers are beneficial to their job satisfaction. However, a recent meta-analysis unexpectedly found no effect when personality traits are controlled for. We argue that a reverse effect of job satisfaction on career adaptability, originating from affective tendencies tied to personality, might explain this null effect. (...) Our argument implies that the estimates obtained with traditional ordinary least squares (OLS) regressions are biased by endogeneity, a correlation between an explanatory variable and the error term in a regression model. When experimental manipulations are impossible, IV estimations, such as two-stage least squares (2SLS) regressions, are one possible solution to the endogeneity problem. Analyzing three waves of data from a sample of 836 adults, the concurrent and time-lagged effect of job satisfaction on career adaptability was revealed to be more consistent than the reverse. Our results provide an explanation, rooted in affective dispositions, as to why recent meta-analytical estimates unexpectedly found that career adaptability does not predict job satisfaction at the interindividual level. We also discuss IV estimation in terms of its limits, weight the interpretation of its estimates against the temporality criterion for causal inference, and consider its possible extension to analyses of change. Background:Little is known about case managers’ ethical issues and professional values.Objectives:This article presents an overview of ethical issues in case managers’ current practice. Findings are examined in the light of nursing ethics, social work ethics and principle-based biomedical ethics.Research design:A systematic literature review was performed to identify and analyse empirical studies concerning ethical issues in case management programmes. It was completed by systematic content analysis of case managers’ national codes of ethics.Findings:Only nine empirical studies were identified, eight of them from (...) North America. The main dilemmas were how to balance system goals against the client’s interest and client protection against autonomy. Professional codes of ethics shared important similarities, but offered different responses to these two dilemmas.Discussion:We discuss the respective roles of professional and organizational ethics. Many state laws specify procedures for determining surrogate or proxy decision-makers for end-of-life care in the absence of an advance directive, living will, or other designation. Some laws also set forth criteria that the decision-maker must follow when making medical decisions for an incapacitated patient and determining whether to withdraw life-sustaining treatment. This article provides analysis of a medical ethics case on the question of how to address family allegations that the proxy decision-maker suffers from dementia and is unable to (...) make decisions for the patient. Cases such as this involve interwoven legal and ethical considerations including: how to address concerns that the proxy is making questionable or unreasonable decisions, how to evaluate the proxy's decisional capacity, strategies for enhancing communication, and standards for removing a proxy. This article suggests that “surrogacy ladders” in state law serve not only as a procedural mechanism, but also protect important ethical values such as tiers of moral authority for decision-making, relational autonomy, and privacy. Ces anecdotes qui mettent en scène des accouplements, voire des enfantements, entre hommes et bêtes, amènent à questionner les rapports troubles entre l’épistémè de la Renaissance et les savoirs ethnographiques acquis durant cette première époque coloniale, savoirs qui s’imprègnent parfois des récits mythologiques locaux. Inspirées par (...) une perspective anthropomorphique comme par le modèle de la fable, ces anecdotes zoophiles témoignent de la diversité des réflexions sur un possible interrègne, sur les zones de partage instables entre l’homme et l’animal, mais aussi sur la projection de stéréotypes animaliers sur des populations étrangères. La philosophie analytique récente a vu naître d’importants débats de méthode concernant la bonne manière de pratiquer l’ontologie, qui voient s’opposer les partisans déclarés de Quine à ceux de Carnap. Au cœur de ces débats se trouve l’idée que le monde possède une structure ontologique propre, indépendante de notre manière de le conceptualiser ou de l’organiser. C’est cette idée qui est ici plus précisément examinée, dans le but de la clarifier. Cet article défend la thèse que, si le monde possède (...) bien une structure intrinsèque, celle-ci est de nature physique et non ontologique. Quant à cette dernière, elle ne peut être conçue indépendamment d’une subjectivité consciente et constituante. The foundation of morality, and mainly, the possibility of moral sentiments that are universal and inalienable, is a central problem in Maine de Biran’s philosophy. Many studies focus on the part played by the self in Maine de Biran’s late philosophy, but relatively few consider with precision the importance of the concept of sympathy in this context. In this paper, I would like to show that this concept, which Biran mobilizes from his first to his last writings, is an important (...) conceptual resource in his enterprise of founding morality. I will draw on three main texts that cover almost the entire period in which he wrote, namely, his Mémoire sur l’influence de l’habitude sur la faculté de penser ([1802] 1987), his Mémoire sur les perceptions obscures ou sur les impressions générales affectives et les sympathies en particulier ([1807] 1984), and his Fragments relatifs aux fondements de la morale et de la religion ([1819] 1859). The main finding is that sympathy occupies a central place in this enterprise of moral foundation but undergoes profound and almost contradictory conceptual re-elaborations throughout Biran’s intellectual journey. Although in his first philosophical texts Biran concentrates on the fundamental dimension of organic sympathy, he rejects it almost entirely in his last texts in favor of an analysis of the sympathy between the selves. The question—what is a political constitution?—might seem, at first blush, fairly innocuous. At one level, the idea of a political constitution seems fairly well settled, at least insofar as most political constitutionalists subscribe to a similar set of commitments, arguments and assumptions. At a second, more reflective level, however, there remains some doubt whether a political constitution purports to be a descriptive or normative account of a real world constitution, such as Britain’s. By exploring the idea of a political constitution (...) as differently articulated by J.A.G. Griffith, Adam Tomkins and Richard Bellamy, this essay explores why the normativity of a political constitution may be indistinct and ill-defined, and how compelling reasons for this indistinctness and ill-definition are to be found in the very idea of a political constitution itself. A political constitution is here conceived as a ‘model’ which supplies an explanatory framework within which to make sense of our constitutional self-understandings. The discipline of thinking in terms of a model opens up a critical space wherein there need not be some stark, all-encompassing choice between constitutional models, which, in turn, allows for more subtle understandings of Britain’s constitution as neither exclusively ‘political’ nor ‘legal’. Purpose: This experimental cross-sectional research study examined the emotional reactivity and emotion regulation in preschool-age children who do and do not stutter by assessing their psychophysiological response during rest and while viewing pictures from the International Affective Picture System.Method: Participants were 18 CWS and 18 age- and gender-matched CWNS. Participants' psychophysiological responses were measured during two baselines and two picture viewing conditions. Skin conductance level and heart rate were measured to assess emotional reactivity. Respiratory sinus arrhythmia was measured to assess (...) emotional regulation. Participants' shyness and executive function were assessed via parent report and considered for their effects on participants' psychophysiological responses.Results: First, CWNS and CWS did not differ in their initial baseline SCL, heart rate, or RSA, but all participants had higher SCL and lower RSA in the second baseline, subsequent to the first challenge condition, compared to the first baseline. Second, during the challenge conditions, CWS did not differ from CWNS in their SCL, but showed a significantly higher heart rate than CWNS. Third, CWS exhibited a significantly lower RSA during the challenge conditions compared to CWNS. Lastly, the temperamental quality of shyness was associated with preschool-age children's SCL, such that participants who were rated higher in shyness had a higher SCL during the challenge conditions. Participants' executive function had a marginally significant effect on the RSA, such that the participants who had higher executive function composite scores exhibited lower RSA during the challenge conditions.Conclusions: Our findings suggest that CWS and CWNS did not differ in their emotional reactivity and emotional regulation abilities at rest. During challenge conditions, however, CWS tended to be more emotionally reactive, as indicated by a higher heart rate, and also employed more emotional regulation, indexed by a greater decrease in RSA, compared to CWNS. Preschool-age children's behavior is largely dominated by reactivity, but there is the emergence of regulation, which can help children adjust to various contextual demands. For CWS who are more emotionally reactive, regulatory skills may be particularly critical to their prognosis and treatment. (shrink)

Rips et al.'s proposition cannot account for the facts that (1) a historical look at the word number systems suggests that the concept of natural numbers has been progressively elaborated; (2) people from cultures without an elaborate counting system do not master the concept of natural numbers; (3) children take time to master natural numbers; and (4) the competing advantage of the postulated math schema in the natural selection process is not obvious.

The purpose of this dissertation is to clarify Aristotle's conception of the nature and utility of the dialectical tools, as well as to help in establishing proof for the existence of a coherence between his logical theory and his philosophical practice. The first book of the Topics is unanimously considered to be Aristotle's introduction to his dialectical method. Thus, a certain familiarity with this book is necessary for whomever undertakes a study of one of its fundamental elements, such as the (...) tools. The first part of the dissertation is, therefore, devoted to this task. After situating the tools in the Topics, we will study Aristotle's proemium , especially as concerns the presentation of the dialectical argument and the goals of dialectic. For it is in order to procure such arguments that the tools are said to be useful, and it is how Aristotle uses the method for one of the aforesaid purposes that we will be studying later. We will then examine the chapters devoted to the parts of the dialectical argument . Among these are the predicables, which enter into the constitution of the propositions that will be laid down as dialectical problems, or taken as dialectical premises. Now, it is precisely because the tools furnish such premises that they allow us to obtain an abundance of arguments apt to solve such problems. These arguments can be syllogisms or inductions. We will next analyze Aristotle's presentation of the four tools . After which, we will propose a more essential and complete definition of them than is usually assigned. They are in fact described by Aristotle and his commentators in terms of the abundance of dialectical arguments they serve for. To my mind, however, it is better to speak of them as abilities ordered to obtaining probable premises. In the second part, we will examine the use made of the tools in certain sections of the De Anima and the Metaphysica. We will thus be in a better position to verify that when he is arguing dialectically, he is really using the tools as he described them in the Topics and as we explained them. Finally, we will propose a few explanations of the use of the tools in those passages where it seems that it is more a question of knowing than of dialectical reasoning. (shrink)

Many physiotherapists are reluctant to treat a client with bone metastases, as treatment indications are not clearly defined. The paradigmatic case presented here, and a review of the literature, allow us to highlight some ethical and clinical issues in physiotherapy care at the end of life, particularly for patients with bone metastases. The objective of this article is not to create an ethical decision making model but rather to serve as additional guidance to existing decision models in rehabilitation. This guidance (...) is based on principlism and is specific to the oncology clients in palliative care. The proposed principles, without being exhaustive, are beneficence, non-maleficence, respect for autonomy and justice. With a review of clinical indications and precautions, the article also helps to clarify both the theoretical and the evidence-based risks. These principles provide avenues for reflection to guide the management of these patients. The physiotherapist will then be able to conduct a critical analysis, in collaboration with the interdisciplinary team, whose goal is to respect the client’s right to appropriate care at the right time and that is based on clinical need. This article offers a critical assessment of Cristina Bicchieri and Jon Elster’s recent attempt to distinguish between social, moral, and quasi-moral norms. Although their typologies present interesting differences, they both distinguish types of norms on the basis of the way in which context, and especially other agents’ expectations and behavior, shapes one’s preference to comply with norms. We argue that both typologies should be abandoned because they fail to capture causally relevant features of norms. We nevertheless emphasize that both Bicchieri (...) and Elster correctly draw attention to important and often neglected characteristics of the psychology of norm compliance. (shrink)

Background: Studies on autistic strengths are often focused on what they reveal about autistic intelligence and, in some cases, exceptional and atypical reasoning abilities. An emerging research trend has demonstrated how interests and strengths often evident in autism can be harnessed in interventions to promote the well-being, adaptive, academic and professional success of autistic people. However, abilities in certain domains may be accompanied by major limitations in others, as well as psychiatric and behavioral issues, which may challenge their inclusion in (...) support programs. Objectives: To provide an in-depth, pragmatic, real-life example of the psychological and psychiatric management of interests and strengths in an autistic adolescent. Method: An autistic teenager, C.A., with above-average calendar calculation and musical abilities, received psychiatric, neuropsychological, and language standardized and clinical assessments, combined with a measurement of his musical and calendar calculation abilities. C.A. and his parents then received psychiatric and psychological support over a 14-month period, targeting their perceptions of C.A.’s interests, strengths, and co-occurring difficulties. Results: C.A. had a verbal IQ within the intellectual disability range and a non-verbal IQ in the low mean range. Modest calendar calculation, absolute pitch, and matrix abilities coexisted with severe receptive and expressive language disorder. The discrepancy between his abilities in areas of strengths and his limitations in other domains led to anxiety, frustration, and sometimes behavioral issues. Displacing the focus from academic performance to interests, as well as promoting the use of his strengths to develop new skills independently of their short-term adaptive benefits yielded positive effects on C.A.’s self-assessment, quality of life, and behavior at follow up. Discussion: The appealing idea that abilities mostly found in autistic people, such as calendar calculation, can be directly harnessed into academic achievement and lead to paid employment may have detrimental effects, especially when such abilities are modest and associated with other limitations. These abilities should be primarily used to maximize well-being and quality of life, independently of their short-term adaptive function, which may or may not be positive. (shrink)

Nous proposons, dans cet article, une analyse communicationnelle des différentes postures de traitement muséal de controverses socioscientifiques pour interroger leur signification en termes d’évolution des modèles mobilisés d’éducation scientifique et d’empowerment des citoyens. Plusieurs travaux montrent que certains centres de sciences privilégient une posture d’externalité en confinant les controverses à leurs dimensions scientifiques. Nous mettons ici l’accent sur des approches alternatives qui intègrent des problématiques controversées. Que ce soit en se positionnant explicitement comme acteur, comme organisateur ou comme témoin du (...) débat public, les centres de sciences traduisent une tendance forte de médiation qui prend acte des reconfigurations contemporaines de la gouvernance des sciences en société. Il nous semble que les expériences dont nous rendons compte intègrent l’évolution des rapports de légitimité entendus en termes de choix scientifiques et techniques et inscrivent une prise de distance vis-à-vis du projet originel de valorisation des sciences.In this article, we offer a communicational analysis of the different stances taken by museums in their treatment of socio-scientific controversies, in order to explore their significance in terms of changing patterns in scientific education and citizens’ empowerment. Several studies have shown that some science centres tend to adopt a distanced stance by confining controversies to their scientific dimensions. Here, we discuss alternative approaches in which the controversial issues are included. Whether explicitly taking up their stance as protagonists, organisers or witnesses to a public debate, science centres display a strong tendency towards mediation, thereby acknowledging contemporary reconfigurations in the governance of science in society. We believe that the experiences reported on here reflect awareness of a change in the balance of legitimating powers in terms of scientific and technical choices, and have effectively distanced themselves from the original idea of highlighting the value of science. Focusing on the differences between two alternative stories of Tenmu’s departure from the Ōmi capital to Yoshino, I argue that the Nihon shoki contains traces of several competing historical narratives that are the expression of a historical process: the political struggles over the historical record and the representation of Tenmu’s legitimacy in the early eighth century when the (...) L'A. retrace tout d'abord la destinée exceptionnelle de ce pape, de son vrai nom Tebaldo Visconti, né à Plaisance en 1210 et mort à Arezzo en 1276. Il étudie ensuite l'action de Grégoire X dans son redressement spirituel de (...) l'Orient latin. Son désir profond de venir en aide à la Terre Sainte s'exprima aussi dans des mesures financières. Les limites de l'action du Saint-Siège sont étudiées ainsi que le soutien logistique des colonies franques de Syrie apporté au pape. L'évolution de la conjoncture internationale compromit sérieusement à partir de 1273 les retombées ses efforts. (shrink)

C'est pendant toute sa vie d'évêque que Grégoire a rédigé ses Histoires dont l'aventure éditoriale posthume fut complexe. Si elles présentent une forte part hagiographique, elles ne donnent pas moins une importante information à propos des Mérovingiens notamment. Cependant, une telle information ne doit pas faire oublier les ambitieux desseins théologiques et exégétiques de l'auteur. Ainsi le Christ se révèle-t-il dans cette oeuvre comme fondement et finalité des structures typologiques, et l'Eglise comme effet du Christ dans l'histoire. Ainsi n'y a-t-il (...) rien d'étonnant à ce que, dans le contexte de ce vocabulaire typologique employé dans les Histoires, le résultat le plus étonnant ait d'abord été la forte présence des « Prophètes », leur rôle s'étant révélé essentiel pour présenter un Christ maître de toute l'histoire et confirmant l'accomplissement des prophéties.It was during his whole life as a bishop that Gregory composed his Histories, of which the posthumous editorial adventure was complex. If they present a strong hagiographic portion, they also give important information, especially about the Merovingians. Such information, nevertheless, should not make us forget the author’s ambitious theological and exegetical project. Thus Christ reveals himself in this work as the foundation and finality of typological structures, and the Church as an effect of Christ in history. We should not be surprised, therefore, that, in the context of this typological vocabulary used in the Histories, the most surprising result had first been the strong presence of the “Prophets.” Their role was reveled as essential for presenting Christ as master of all history and confirmed what the prophecies accomplished. The rhetorical and demonstrative function of images and comparisons in Gregory of Nyssa’s De anima et resurrectione is well known. They aim at warranting the faith in resurrection and making it desirable. The prospect of this study is to show that they belong to the progress of the debate such as Gregory has composed it. Their quality changes while the author moves from the philosophical likelihood to the truth of the Scriptures. He opposes one secular image to a biblical one (...) : at the beginning of the dialogue he refuses the comparison which reduces man to a bubble ; in the last part he chooses the solid composition (pukasmos) of the skènopègia (Ps. 117, 27). Other pictures are partly accepted, specially those which are borrowed from the sphere of tekhnè, whereas Macrina dismisses the Platonic myth of the soul’s chariot. Nevertheless, the end of the dialogue becomes pregnant with images derived from the Bible, when Macrina and his brother are discussing resurrection itself. Then by the means of the comparisons which they select, the biblical revelation and the facts of the phusis finally unite. Nature in some way supplants tekhnè, which has been honoured in the first half of the dialogue. (shrink)
