IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process. The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus with (...) clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information but significantly reduced for potential complications and biopsy description. Only 17.8% of the candidates mentioned the legal perspective of ICP. Death was omitted by the majority of the candidates ; after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior, but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

The concept of Smart Healthy Age-Friendly Environments (SHAFE) emphasises the comprehensive person-centred experience as essential to promoting living environments. SHAFE takes an interdisciplinary approach, conceptualising complete and multidisciplinary solutions for an inclusive society. From this approach, we promote participation, health, and well-being experiences by finding the best possible combinations of social, physical, and digital solutions in the community. This initiative emerged bottom-up in Europe from the dream and conviction that innovation can improve health equity, foster caring communities, and sustainable development. (...) Smart, adaptable, and inclusive solutions can promote and support independence and autonomy throughout the lifespan, regardless of age, gender, disabilities, cultural differences, and personal choices, as well as promote happier and fairer living places. The New European Bauhaus initiative gathers “beautiful, sustainable, and inclusive projects and ideas” to inspire a positive transformation around us. The New European Bauhaus 2024 Festival took place between 9-13 April. “Designing the perfect New European Bauhaus neighbourhood: New European Bauhaus meets SHAFE” was a Satellite Event of the Festival, held as an online workshop, organised by the SHAFE Foundation on April 9th, 2024. At the event, a group of 50 participants with a multidisciplinary background discussed how to meet the challenges of the European Garcia family to enable them to live in a perfect neighbourhood. The White Paper presents the findings of the participants with the New European Bauhaus inspiring projects and ideas to house the European Garcia family in their neighbourhood. The White Paper recommends developers, designers, planners, and policymakers to include person-centred design of social and physical environments and technologies. Although awareness of person-centred design is growing, many initiatives still hamper the inclusion of citizens or end-users throughout the development of products and services. (shrink)

This article reconstructs the reception of Fichte’s philosophy in the works of the physician and philosopher A.K.A. Eschenmayer between 1796 and 1801. In 1796/97, Eschenmayer was working on his project of a metaphysics of nature which would be capable of constituting a middle term between the empirical sciences and the transcendental philosophy. In doing so, he explicitly engaged with Kant, on the one hand, and with scientists of the time, on the other hand, while the influence of Fichte is comparatively (...) slight and less easily discerned. In 1798, however, he introduced into his studies of magnetism an explicit recognition of Fichte and a reference to the third principle of the Wissenschaftslehre. In 1799, Eschenmayer adopted a higher standpoint from which it was possible to conceive the genesis of the concepts which up to that point had been merely analysed. This change of viewpoint coincided with a deeper engagement with the works of Fichte. The analysis of the Dedukzion and of Eschenmayer’s explicit references to Fichte that is attempted in the present study leads to the conclusion that Eschenmayer not only adopted some terminology typical of Fichtean philosophy but also integrated into his deduction several theoretical elements developed by Fichte. The reconstruction of the influences that Fichte exercised on Eschenmayer’s thought is fundamental to an understanding of the theoretical position of Eschenmayer and the point of view from which in 1801 he formulated his criticism of Schelling’s idea of nature as autarchic and as its own legislator.Der Beitrag rekonstruiert die Rezeption der Philosophie Fichtes in den Werken des Arztes und Philosophen A.K.A Eschenmayer aus den Jahren 1796 bis 1801. 1796/97 verfolgt Eschenmayer das Ziel, eine Naturmetaphysik zu entwickeln, die als Mittelglied zwischen den empirischen Wissenschaften und der Transzendentalphilosophie fungieren soll. Dabei setzt er sich einerseits mit Kant und andererseits mit den Wissenschaftlern seiner Zeit auseinander. Der Einfluss von Fichte auf Eschenmayer ist hingegen vergleichsweise gering und nicht leicht festzustellen. Dennoch erkennt Eschenmayer bereits in seiner Studie zum Magnetismus aus dem Jahr 1798 das Verdienst Fichtes an und verweist auf den dritten Grundsatz der Wissenschaftslehre. Im Jahr 1799 erhebt sich Eschenmayer insofern zu einem höheren Standpunkt, als er nun die Genese der Begriffe entfaltet, die er vorher bloß analysiert hatte. Diese Änderung des Standpunktes führt Eschenmayer zu einer tieferen Auseinandersetzung mit den Werken Fichtes. Die in diesem Beitrag durchgeführte Untersuchung der Passagen aus Eschenmayers Dedukzion, in denen sich explizite Verweise auf Fichte finden, hat zu dem Schluss geführt, dass Eschenmayer sich nicht nur Fichtes Terminologie zu eigen macht, sondern auch mehrere von Fichte aufgestellte Theorien in seine eigene Deduktion integriert. Die Rekonstruktion der Einflüsse Fichtes auf Eschenmayer ist nicht nur für ein besseres Verständnis der Philosophie Eschenmayers unentbehrlich, sondern trägt auch zum Verständnis des Standpunktes bei, von dem aus Eschenmayer im Jahre 1801 Kritik an Schellings Idee der Natur als ihrer eigenen Gesetzgeberin übt. This article re-examines current definitions of populism, which portray it as either a powerful corrective to or the nemesis of liberal democracy. It does so by exploring a crucial but often neglected dimension of populism: its redemptive character. Populism is here understood to function according to the logic of resentment, which involves both socio-political indignation at injustice and envy or ressentiment. Populism promises redemption through regaining possession: of a lower status, a wounded identity, a diminished or lost control. Highly moralized (...) images of the past – historical or archetypal – are mobilized by populist leaders to castigate the present and accelerate the urgency of change in it. The argument is illustrated with Caesar’s Column, a futuristic novel written by the Minnesota populist leader Ignatius Donnelly. The complex and ambivalent structure of this dystopian novel – a textual source for the Populist Party manifesto in the 1890s, which stands in contrast with agrarian populism as everyday utopia – enables us to move beyond the polarized positions dominating the current debate. Reading Caesar’s Column ultimately shows that populism can be both a corrective and a danger to democracy, but not for the reasons usually stated in the literature. (shrink)

The self- vs. external-regulation behavior theory, SR-ER Theory model has postulated the Self-Regulation /Non or De-Regulation/Dys-regulation continuum in the person and in their context. The model also generates a behavioral heuristic that allows us to predict and explain the variability of other dependent behavioral variables in a range of scenarios. Consequently, the objective of this study was to validate the different scales prepared on the basis of the theory presented. A total of 469 students voluntarily completed at different times the (...) five questionnaires presented, to give a total of 1,385 completed questionnaires. Using an ex post facto design, descriptive, correlational, confirmatory factorial analysis, reliability, and concurrent validity analyses were carried out. The scales were analyzed individually and as a whole. The results showed the acceptable structure of scale and consistent levels of reliability. The five levels generated by the SR-NR-DR combinatory heuristic that arises from the theoretical model determined significant differences in the levels of the variables analyzed for each psychological context. We discuss the theoretical implications and the implications for the assessment and improvement of the behaviors analyzed in function of the personal and contextual regulation levels evaluated. Objective: Quality of Life is an important predictor of patient's recovery and survival in lung cancer patients. The aim of the present study is to identify 1-year trends of lung cancer patients' QoL after robot-assisted or traditional lobectomy and investigate whether clinical and sociodemographic variables may predict these trends.Methods: An Italian sample of 176 lung cancer patients undergoing lobectomy completed the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire—Core 30 at the pre-hospitalization, 30 days, 4 months, (...) 8 months, and 12 months after surgery. Sociodemographic and clinical characteristics were also collected. The individual change over time of the 15 dimensions of the EORTC QLQ-C30 and the effects of pre-surgery scores of QoL dimensions, type of surgery, perioperative complications, and age on patients' QoL after surgery were studied with the individual growth curve models.Results: Patients had a good recovery after lobectomy: functioning subscales improved over time, while most of the symptoms became less severe over the care process. Perioperative complications, type of surgery, pre-surgery status, and age significantly affected these trends, thus becoming predictors of patients' QoL.Conclusion: This study highlights different 1-year trends of lung cancer patients' QoL. The measurement of pre- and post-surgery QoL and its clinical and sociodemographic covariables would be necessary to better investigate patients' care process and implement personalized medicine in lung cancer hospital divisions. The psychological factors influencing a return to sport has gained increased research attention. In the current investigation, we explored professional basketball players’ perceptions of the psychological factors facilitating a return to performance equal to or exceeding previous performance standards. We also sought to describe athletes’ experiences – both positive and negative – of returning to sport following injury recovery. Ten Italian professional male basketball players (age range 22-36 years), were retrospectively interviewed in relation to three time-periods: (1) from the commencement (...) of rehabilitation to their first official competition, (2) the first official competition, and (3) the six-months following the initial competition. Hierarchical content analysis of the data revealed numerous themes across the three time periods. In regards to Period 1, participants indicated that social support, investment in rehabilitation and training programs, coping skills and motivation were fundamental in reaching pre-injury performance levels. During their first official game (i.e. Period 2), athletes reported that realistic performance expectations, focusing on the performance, positive emotions, motivation, arousal and social support facilitated their return to sport. Athletes however, also described a predominance of factors that hindered their return to pre- injury levels (i.e., low confidence in personal abilities, decrements in skill execution and dysfunctional physical sensations). Moreover, participants typically described a substandard level of performance during their first competition back following injury. In recounting experiences during the six months following their first official game, basketballers reported improvements in skill execution and highlighted the importance of coping skills, motivation and social support. The process of restoring self-confidence in one’s ability to successfully perform was perceived as crucial in enabling participants to move beyond a mere return to sport to a return to high performance ─ that is, to reach a level of proficiency equal to or exceeding previous performance standards. Findings support the relevance of cognitive, emotional and behavioral responses highlighted in the Integrated Model and suggest the importance of addressing psychological factors throughout the return-to-sport process. Finally, results from the present study hold a number of practical implications for athletes’ aiming to achieve a return to pre-injury levels. (shrink)

Josefa Amar y Borbón, une intellectuelle espagnole du XVIIIe siècle, défendit dans ses ouvrages et dans sa vie la raison des femmes, à partir de sa propre tradition culturelle mais simultanément avec d’autres intellectuelles européennes de son temps. Elle se forgea un prestige intellectuel et intervint publiquement dans le débat sur l’admission des femmes dans les sociétés réformistes, contre l’opinion négative d’une partie des hommes des élites éclairées espagnoles. La lecture de ses textes et leur comparaison avec la pensée de (...) sa contemporaine Mme d’Épinay montre que, dans des contextes sociaux et culturels différents, ces deux femmes avait des idées concordantes sur la raison, l’éducation et la condition social des femmes, loin des opinions plus répandues, celles des auteurs comme Thomas, Rousseau et d’autres contemporains espagnols. Elles avaient aussi des stratégies vitales communes, qui visaient à occuper et à revendiquer pour les femmes des possibilités et des espaces dans l’activité culturelle et la sociabilité des Lumières. ObjectiveIndividuals with an evening chronotype prefer to sleep later at night, wake up later in the day and perform best later in the day as compared to individuals with morning chronotype. Thus, college students without ADHD symptoms with evening chronotypes show reduced cognitive performance in the morning relative to nighttime. In combination with symptoms presented in attention deficit hyperactivity disorder, we predicted that having evening chronotype renders impairment in attention during the morning, when students require optimal performance, amplifying desynchrony.MethodFour hundred (...) college students were surveyed for evening chronotype and symptoms of ADHD. Of those surveyed, 43 students with evening chronotype performed laboratory attention tasks and were queried about fatigue during morning and evening sessions.ResultsStudents with ADHD symptoms demonstrated a greater decrement in sustained attentional vigilance when abstaining from stimulants and asked to perform cognitive tests at times misaligned with natural circadian rhythms in arousal compared to their non-ADHD counterparts with the same chronotype. While individuals with ADHD symptoms had slower reaction-times during sustained attention tasks in the morning session compared to those without symptoms, there was no significant group difference in working memory performance, even though both groups made more errors in the morning session compared to the evening session.ConclusionThese findings suggest that evening chronotype students with ADHD symptoms are at a greater disadvantage when having to perform sustained attention tasks at times that are not aligned to their circadian rhythm compared to their neuro-typical peers. The implications of this finding may be useful for the provision of disability accommodations to college age students with ADHD when they are expected to perform tasks requiring sustained attention at times misaligned with their circadian rhythms. O presente estudo objetiva apresentar o direito e a prática jurídica como categorias cognitivas e linguísticas expressas metaforicamente, bem como a importância da metáfora para a capacidade humana de pensar e construir sentidos. Para tanto, utiliza-se de metodologia qualitativa mediante a abordagem da Teoria da Metáfora Conceptual de George Lakoff e Mark Johnson, apresentada na obra “Metáforas da Vida Cotidiana” (1980[2002]), tudo com amparo em instrumentais bibliográficos, notadamente literatura nacional e estrangeira. Os resultados alcançados indicam que a retórica jurídica é (...) estruturada metaforicamente para ocultar seu caráter, expressando uma pretensa objetividade, mas que o discurso emergente é interacional e negociado. Resumen: El presente trabajo intenta analizar los elementos críticos a la base de aquella suerte de prescripción que Edward Said formulara a los intelectuales bajo la célebre consigna de “decir las verdades al poder”, esto es, de interpelar públicamente al poder -político, económico, religioso, militar- frente a toda evidencia de injusticia, inconsistencia o turbia manipulación en su operar. En tanto tal, y a partir de nuestra lectura de Said, delimitamos cinco dilemas que el intelectual ha de resolver, en tanto requisitos (...) para decir las verdades al poder: orientación intramundana versus extramundana, rol profético versus sacerdotal, libertad universalista versus organicidad, racionalidad sustantiva versus instrumental y arrojo versus temor. Se comentan las implicaciones de estos dilemas a la luz de los desafíos y oportunidades que las sociedades contemporáneas -en particular, las latinoamericanas- presentan para el rol del intelectual.: This work analyzes the critical foundations of that quasi-rule posed by Edward Said to intellectuals through the famous motto “speaking truth to power”, that is, of publicly interpellating to power -political, economic, religious, military- whenever its exercise may involve injustice, inconsistency or underhanded manipulation. So, according to our interpretation of Said’s work, we identify five dilemmas the intellectual should resolve in order to be able to speak truth to power: a worldliness orientation versus an otherworldliness one, a prophetic role versus a priestly one, universalist freedom versus organic compromise, a substantive rationality versus an instrumental one, and courage versus fear. Involvements of these dilemmas are discussed according to the challenges and opportunities posed by current societies -in particular, the Latin American ones- to the intellectual’s role. (shrink)

En este trabajo se analiza críticamente la literatura sobre el capital social, con énfasis en la situación actual del tema. En la primera parte se presenta el concepto, se da cuenta de algunos antecedentes relacionados con el contexto teórico en que surgió y se indican los momentos más importantes de la discusión posterior. Aquí también se enuncia la tesis del trabajo, que sostiene que parte de la confusión e indeterminación conceptual existente deriva de la manera como fue inicialmente establecido el (...) enfoque. A continuación se analizan críticamente las estrategias para delimitar el campo teórico y de aplicación del capital social, se presentan los componentes básicos en el plano de los indicadores y se resumen las elaboraciones más importantes que han caracterizado su desarrollo. Orphan drug policy often gives ‘special treatment’ to rare diseases, by giving additional priority or making exceptions to specific drugs, based on the rarity of the conditions they aim to treat. This essay argues that the goal of orphan drug policy should be to make prevalence irrelevant to funding decisions. It aims to demonstrate that it is severity, not prevalence, which drives our judgments that important claims are being overlooked when treatments for severe rare diseases are not funded. It shows (...) that prioritising severity avoids problems caused by prioritising rarity, and that it is compatible with a range of normative frameworks. The implications of a severity-based view for drug development are then derived. The severity-based view also accounts for what is wrong with how the current system of drug development unfairly neglects common diseases that burden the developing world. La Educación para la Justicia Global es una propuesta educativa crítica y transformadora impulsada por la Federación de entidades de Justicia global catalana, aunque es un enfoque extendido su marco conceptual está poco desarrollado. El objetivo del artículo es definir criterios temáticos/metodológicos que permitan identificar y caracterizar las prácticas educativas de justicia global. La metodología corresponde a un proceso de sistematización y organización de las experiencias de las entidades utilizando entrevistas cualitativas, focus-group y análisis documental. Los resultados muestran seis ejes (...) temáticos y cinco criterios teóricos/metodológicos que pueden ilustrar y orientar a quienes deseen conocer o adoptar el enfoque. (shrink)

El trabajo busca situar el análisis de los movimientos indígenas latinoamericanos dentro del desarrollo global del capitalismo y el sistema-mundo moderno, tratando de seguir una lectura decolonial de dichos procesos. Se postula que el neoliberalismo representa la encarnación actual de dicho sistema, pero que además simboliza un proyecto cultural que otorga criterios para el diseño de políticas de gobernabilidad en materia indígena. Se revisa la trayectoria actual de los movimientos indígenas en relación con las diferentes estrategias de gobernabilidad Estatal implementadas (...) en tres regiones latinoamericanas: Mesoamérica, los países Andinos y el Cono Sur. Finalmente se analiza el potencial transformador y contrahegemónico de los movimientos indígenas contemporáneos, en cuanto intentan renovar valores y significados culturales, cuestionar el sistema de relaciones de poder y promover una comprensión alternativa del desarrollo. Why, when and where are some moral systems supported and followed whilst others are condemned? Are moral values relative or universal? Can immoral actions be tolerated in times of crisis? Is the dream of becoming better sufficient for prompting virtuous behavior, or should we dream about what is best? Do moral values last? The divergence in practices and codes of moral belief and action present significant challenges but also offer opportunities to anthropologists for understanding social life. In this book, (...) class='Hi'>Monica Heintz explores these questions, drawing on case studies from Eastern Europe that encompass migration, religion, economic and social policies and paying particular attention to the way morality works in communities undergoing rapid social change. She uses these examples to reflect on the wider question of societal conflict and change, showing how they are driven by moral values. By highlighting the centrality of such values as engines for action and questioning the limits of universal moral values, she argues that anthropology has the capacity to shed light on the study of morality and moral values generally. Blockchain is an open digital ledger technology that has the capability of significantly altering the way that people operations operate in organizations. This research takes a first step in proposing several ways in which the blockchain technology can be used to improve current organizational practices, while also considering the ethical implications. Specifically, the paper examines the role that blockchain technology plays in three primary areas of people operations: entry to the organization, intraorganizational processes, and exit. In each section, the paper (...) reviews the ethical implications from the lenses of virtue ethics, utilitarianism, deontology and contractarianism. The paper concludes that in whole the implementation of blockchain technology in people operations processes can create a more ethical work environment. In this work we present the TULSI system (so named after Turin University Legal Semantic Interpreter), a system to produce automatic annotations of normative documents through the extraction of modificatory provisions. TULSI relies on a deep syntactic analysis and a shallow semantic interpreter that are illustrated in detail. We report the results of an experimental evaluation of the system and discuss them, also suggesting future directions for further improvement.

This article exposes four arguments on the debate about the nature and conceptual range of social intervention, understanding it equally as action, interpretation, distinction and finally as a discursive dispositive. This debate has relevance for social sciences disciplines in which intervention notion has an important place in its theoretic-methodological structure. This review sets a counterpoint with technological perspectives that reduce intervention to action’s execution. In this way, there is a discussion about epistemic and ontological social sciences communities’ subordination in which (...) intervention is a hallmark. En este artículo se presentan cuatro argumentos expresados en el debate sobre la naturaleza y alcance conceptual de la intervención social, entendiéndola indistintamente como actuación, interpretación, distinción sistémica y finalmente como dispositivo discursivo. Este debate es relevante para disciplinas de las ciencias sociales en las que la noción de intervención ocupa un lugar importante en su configuración teórico-metodológica. La revisión de estas perspectivas establece un contrapunto con las perspectivas tecnológicas que sostienen que la intervención se reduce a la ejecución de acciones. De este modo, se discute la subordinación epistémica y ontológica de las comunidades de las ciencias sociales, en las cuales intervenir constituye su sello de identidad. (shrink)

Se presentan los aspectos básicos de un modelo de procesamiento sintáctico-semántico denominado Sintaxis Dinámica. Para cumplir con este objetivo, presentamos definiciones e ilustraciones de los componentes esenciales del metalenguaje, y ejemplificamos este modelamiento dinámico presentando el proceso sintáctico completo de composición del significado de una oración simple. Finalmente, se señalan sucintamente algunas características prominentes del modelo.

This text, commissioned by the OSUN Global Observatory of Academic Freedom, discusses how the concept of academic freedom is codified in Colombian Law and regulations of public and private higher education institutions. It also explores common conceptions of academic freedom among Colombian scholars, as well as commonly observed threats to it.

Representation is integral to the functioning and legitimacy of modern government. Yet political theorists have often been reluctant to engage directly with questions of representation, and empirical political scientists have closed down such questions by making representation synonymous with congruence. Conceptually unproblematic and normatively inert for some, representation has been deemed impossible to pin down analytically and to defend normatively by others. But this is changing. Political theorists are now turning to political representation as a subject worthy of theoretical investigation (...) in its own right. In their effort to rework the theory of political representation, they are also hoping to impact how representation is assessed and studied empirically. This volume gathers together chapters by key contributors to what amounts to a "representative turn" in political theory. Their approaches and emphases are diverse, but taken together they represent a compelling and original attempt at re-conceptualizing political representation and critically assessing the main theoretical and political implications following from this, namely for how we conceive and assess representative democracy. This paper examines folk theories of algorithmic recommendations on Spotify in order to make visible the cultural specificities of data assemblages in the global South. The study was conducted in Costa Rica and draws on triangulated data from 30 interviews, 4 focus groups with 22 users, and the study of “rich pictures” made by individuals to graphically represent their understanding of algorithmic recommendations. We found two main folk theories: one that personifies Spotify and another one that envisions it as a (...) system full of resources. Whereas the first theory emphasizes local conceptions of social relations to make sense of algorithms, the second one stresses the role of algorithms in providing a global experience of music and technology. We analyze why people espouse either one of these theories and how these theories provide users with resources to enact different modalities of power and resistance in relation to recommendation algorithms. We argue that folk theories thus offer a productive way to broaden understanding of what agency means in relation to algorithms. (shrink)
