The last 10 years has seen the development and deployment of new biotechnologies not just as potential treatments but also as potential enhancements. The definition and differentiation of treatment from enhancement is an ongoing clinical, ethical and social debate that ranges across a proliferating number of convergent technologies. Many of these innovations will ‘come-on-line’ as present generations of young people will be reaching adulthood and considering parenthood. This paper reports on a project that explored the possibilities for human enhancement with (...) young people in order to gather their attitudes towards enhancement and the types of arguments/reasoning they employ when thinking about the possibilities and the techniques. The project focused on if/how distinctions are made between treatment and enhancement, between the different techniques that might be used for enhancement and perceptions of risks and benefits. The young people’s viewpoints, their methods of reasoning and underlying values are compared with those of bioethicists writing on the topic. (shrink)

Decision-making depends on bringing evidence together with values: decision theory for example employs probabilities and utilities; health economic decisions employ measures such as quality of life. The hypothesis guiding this chapter is that bringing evidence together with values in clinical decision-making requires an exercise of phronesis. Our aim however is not to justify our guiding hypothesis. It is rather to outline an account of phronesis that is in principle fit for the purposes of clinical decision-making if our guiding hypothesis is (...) correct. The chapter has three sections. Section 1 describes the growing gap between evidence and values in clinical decision-making: the missing link, we suggest, required to bridge this gap, is an appropriate account of phronesis. Section 2 provides an initial characterisation of the required account of phronesis via Michael Polanyi’s twin stipulation on the nature of tacit knowledge. Section 3 then fills out the required account using John McDowell’s characterisation of phronesis as a situation specific but at the same time conceptually structured form of practical discernment. A McDowellian account of phronesis, we argue, in satisfying Polanyi’s twin stipulation, provides an in-principle bridge between evidence and values. We conclude with a note on some of the further requirements if the gap is to be bridged not only in principle but in practice too. (shrink)

The Incarnate Ground of Christian Faith is addressed precisely to the epistemological questions posed by postmodernity. It begins by issuing an extended critique of one of the major approaches to pastoral theology and Christian education--Thomas Groome's Shared Praxis Approach. Martin's incisive analysis of shared praxis concludes that its implicit subjectivism and pedagogical narrowness cannot lend intellectual plausibility to the Christian faith among a postmodern generation. For an alternative vision of a holistic and plausible faith, Martin points in a different direction, (...) to an incarnational epistemology in which science and theology discover deep resonance in each other. The main features of this epistemological framework are developed by integrating the perspectives of scientist/philosopher Michael Polanyi and theologian Thomas F. Torrance. The resulting synthesis preserves the integrity of science and theology as distinct yet inherently related ways of making sense of the world, the purpose of which is to participate more fully in the world. The main thrust of the book is that a vibrant faith incarnationally grounded in experience and clarified theologically intensifies personal and social expressions of ecclesial life. Chrisoula Andreou argues that the predominant factor in the exalted and worldly views of human thriving involves a psychological measure of relative deprivation or advantage in relation to social competitors. This is the 'no avail' thesis: promoting self-sacrifice for the sake of conservation, in-and-of-itself, will remain ineffective as environmental policy. However, Andreou sets aside, to some extent, the applicability of philosophical discourse on happiness and human thriving, which is where this commentary is directed. Specifically, Aristotle's insights on social prestige (exousia) (...) being attained through philanthropy is discussed as a counter-narrative to present day materialist culture. For Aristotle, the Hellenic mentality was not to be of exclusionary competition but rather strengthening the community through competitive social power aimed at mutually enriching philia. This model provides a provocative case-study for insight on redirecting competitive human nature toward other ends. Additional supporting analysis is also provided on the emergence of the modern market economy using the works of economic historian Karl Polanyi. (shrink)

In _Michael Polanyi and His Generation_, Mary Jo Nye investigates the role that Michael Polanyi and several of his contemporaries played in the emergence of the social turn in the philosophy of science. This turn involved seeing science as a socially based enterprise that does not rely on empiricism and reason alone but on social communities, behavioral norms, and personal commitments. Nye argues that the roots of the social turn are to be found in the scientific culture and (...) political events of Europe in the 1930s, when scientific intellectuals struggled to defend the universal status of scientific knowledge and to justify public support for science in an era of economic catastrophe, Stalinism and Fascism, and increased demands for applications of science to industry and social welfare. At the center of this struggle was Polanyi, who Nye contends was one of the first advocates of this new conception of science. Nye reconstructs Polanyi’s scientific and political milieus in Budapest, Berlin, and Manchester from the 1910s to the 1950s and explains how he and other natural scientists and social scientists of his generation—including J. D. Bernal, Ludwik Fleck, Karl Mannheim, and Robert K. Merton—and the next, such as Thomas Kuhn, forged a politically charged philosophy of science, one that newly emphasized the social construction of science. (shrink)

Objectives—The development of predictive genetic tests provides a new area where consumers can gain knowledge of their health status and commercial opportunities. “Over-the-counter” or mail order genetic tests are most likely to provide information on carrier status or the risk of developing a multifactorial disease. The paper considers the social and ethical implications of individuals purchasing genetic tests and whether genetic information is different from other types of health information which individuals can obtain for themselves.Design—The discussion is illustrated by findings (...) from a questionnaire survey of university students as potential consumers. Topics covered included what health tests they had already used, expectations of genetic tests, willingness to pay, who should have access to the results and whether there need to be restrictions on such tests.Sample—Six hundred and fifteen first-year students in the universities of Leuven, Cardiff, Central Lancashire, Vienna and Nijmegen studying either medicine or a non-science subject.Results—Students were enthusiastic about genetic tests and had high expectations of their accuracy and usefulness but most thought they should be available through the health service and a minority thought that some tests, for example for sex selection, should not be available at all. There were few differences in responses by sex or subject of study but some by country. The paper also considers ethical and social issues outside the scope of a questionnaire survey of this type.Conclusion—To address some of these issues the sale of genetic tests to individuals can be made subject to ethical guidelines or codes of practice, for example to protect vulnerable groups, but there are fundamental social and ethical questions which such guidelines cannot address. This collection of essays, assembled by Marjorie Grene, exemplifies the development of Polanyi's theory of knowledge which was first presented in Science, Faith, and Society and later systematized in Personal Knowledge. Polanyi believes that the dilemma of the modern mind arises from the peculiar relation between the positivist claim for total objectivity in scientific knowledge and the unprecedented moral dynamism characterizing the social and political aspirations of the last century. The first part of Knowing and Being deals with this theme. Part two develops Polanyi's idea that centralization is incompatible with the life of science as well as his views on the role of tradition and authority in science. The essays on tacit knowing in Part Three proceed directly from his preoccupation with the nature of scientific discovery and reveal a pervasive substructure of all intelligent behavior. I propose to bring fresh evidence here for my theory of knowledge and expand it in new directions. We shall arrive most swiftly at the centre of the theory, by going back to the point from which I started about twenty years ago. Upon examining the grounds on which science is pursued, I saw that its progress is determined at every stage by indefinable powers of thought. No rules can account for the way a good idea is found for starting (...) an inquiry; and there are no firm rules either for the verification or the refutation of the proposed solution of a problem. Rules widely current may be plausible enough, but scientific enquiry often proceeds and triumphs by contradicting them. Moreover, the explicit content of a theory fails to account for the guidance it affords to future discoveries. To hold a natural law to be true, is to believe that its presence may reveal itself in yet unknown and perhaps yet unthinkable consequences; it is to believe that such laws are features of a reality which as such will continue to bear consequences inexhaustibly. There is a need to rapidly assess the impact of new technology initiatives on the Counter Improvised Explosive Device battle in Iraq and Afghanistan. The immediate challenge is the need for rapid decisions, and a lack of engineering test data to support the assessment. The rapid assessment methodology exploits available information to build a probabilistic model that provides an explicit executable representation of the initiative’s likely impact. The model is used to provide a consistent, explicit, explanation to decision makers on (...) the likely impact of the initiative. I propose to enquire here into the way we endow our speech with meaning and into the way by which we make sense of speech that we hear spoken. I shall show that, notwithstanding their informal character, these acts possess a characteristic pattern, a pattern that I shall call the structure of tacit knowing ; I shall show that to form such a structure is to create meaning . Both the way we endow our own utterances with meaning and our (...) attribution of meaning to the utterances of others are acts of tacit knowing. They represent sense-giving and sense-reading with in the structure of tacit knowing. My enquiry shall outline the total structure of language, comprising both its formal patterns successfully established by modern linguistics and its informal semantic structure, studied so far mainly by philosophy. On its appearance in 1946 the book quickly became the focus of controversy.

Since its introduction in the 1980s, futility as a concept has held contested meaning and applications throughout medicine. There has been little discussion within the psychiatric literature about the use of futility in the care of individuals experiencing severe and persistent mental illness, despite some tacit acceptance that futility may apply in certain cases of psychiatric illness. In this paper, we explore the literature surrounding futility and argue that its connotation within medicine is to describe situations where patients believe that (...) interventions will almost certainly provide no meaningful benefit. We then provide two arguments in support of the use of futility within the care of individuals experiencing SPMI: that some SPMI can be considered a terminal illness, and that the risk-benefit ratio is a dynamic entity such that futility can help describe what Gillett calls the ‘risk of unacceptable badness’ when it comes to considering how an intervention might impact a patient’s quality of life. We posit that capacity should not pose an obstacle to declaring futility when caring for individuals experiencing SPMI and explain how futility is not antithetical to recovery in mental health. Finally, we describe how using futility within psychiatric practice can allow for a reorientation of care by signalling the need to shift to a palliative approach. (shrink)

Technology is “unruly” because it operates in a social context where it is shaped by institutions, organisations and individuals in ways not envisaged when it was first developed. In the UK assisted reproductive technology has developed from strictly circumscribed beginnings as a treatment for infertility within the NHS, to a service which is more often offered by commercial clinics and purchased by clients who are not necessarily infertile. The article considers the process by which assisted reproductive technology has been created (...) and developed, a process which is ideological rather than technical, and the social implications of its ever expanding use. In a society where the discourse around reproduction and family life is one of choice and acceptance of diversity of life styles, the conditions are set for further “unruliness” supported by clinicians and commercial interests. The HFEA, public consultations and media coverage tend to subscribe to the way ethical issues are framed by those interested parties, an approach that favours increased liberalisation. (shrink)
