Variant Creutzfeldt–Jakob disease is a fatal, transmissible, neurodegenerative disorder for which there is currently no effective treatment. vCJD arose from the zoonotic spread of bovine spongiform encephalopathy. There is now compelling evidence for human to human transmission through blood transfusions from presymptomatic carriers and experts are warning that the real epidemic may be yet to come. Imperatives exist for the development of reliable, non-invasive presymptomatic diagnostic tests. Research into such tests is well advanced. In this article the ethical implications of (...) the availability of these tests are elaborated and comparisons drawn with predictive genetic testing for Huntington’s disease and screening for HIV. Paramount to considerations is the issue of whom to test, weighing up respect for personal autonomy against obligations to benefit and protect society. A paradigm is proposed similar to that used for HIV screening but with unique features: compulsory testing of all blood/organ donors and individuals undergoing surgery or invasive procedures who have a significant risk of disease transmission. I can be aware of myself, and thereby come to know things about myself, in a variety of different ways. But is there some special way in which I—and only I—can learn about myself? Can I become aware of myself by introspecting? Do I somehow show up in my own conscious experiences? David Hume and most contemporary philosophers say no. They deny that the self shows up in experience. However, in this paper I appeal to research on schizophrenia—on thought insertion, (...) in particular—to argue that Hume and his follows are wrong: The self does, in fact, show up in experience. Many people believe that the mind is an epistemic refuge of sorts. The idea is that when it comes to certain core mental states, one’s being in such a state automatically puts one in a position to know that one is in that state. This idea has come under attack in recent years. One particularly influential attack comes from Timothy Williamson (2000), who argues that there is no central core of states or conditions—mental or otherwise—to which we are guaranteed epistemic (...) access. In Williamson’s words, we are cognitively homeless. In this paper I will argue that Williamson’s argument for the conclusion that we are cognitively homeless fails. Then I will show that there is a class of phenomenal states that constitutes a substantial cognitive refuge. When all is said and done, I will have both defended and shed light on our cognitive home. We recognize a paradox of power and promise in the context of legislative and organizational changes in nurse regulation which poses constraints on nursing's capacity to bring voice and influence to pressing matters of healthcare and public policy. The profession is at an important crossroads wherein leaders must be well informed in political, economic and legislative trends to harness the profession's power while also navigating forces that may put at risk its central mission to serve society. We present a critical (...) policy analysis of the impact of recent regulatory trends on what the International Council of Nurses considers nursing's three ‘pillars’ – the profession of nursing, socioeconomic welfare of nurses and nurse regulation. Themes surfacing from this analysis include regulatory discontinuity, a tightening of regulatory control, and an increasingly managerial governance culture. These themes illuminate insights and strategies required to renew and revitalize the social mandate of our profession amidst a climate of urgency in the questioning of nurse scholars with respect to the future of the profession. At this historic juncture, nurses must clearly understand the implications of legislative and organizational regulatory changes to ensure the profession contributes to full capacity in achieving health and health equity globally. In recent years, the target of reaching “net zero” emissions by 2050 has come to the forefront of global climate politics. Net zero would see carbon emissions matched by carbon removals and should allow the planet to avoid dangerous climate change. But the recent prominence of this goal should not distract from the fact that there are many possible versions of net zero. Each of them will have different climate justice implications, and some of them could have very negative consequences (...) for the world's poor. This article demonstrates the many ambiguities of net zero, and argues in favor of a net zero strategy in which those who can reasonably bear the burden adopt early and aggressive mitigation policies. We also argue for a net zero strategy in which countries place the lion's share of their faith in known emissions reduction approaches, rather than being heavily reliant on as-yet-unproven “negative emissions techniques.” Our overarching goal is to put net zero in its place, by providing a clear-sighted view of what net zero will achieve, and where the “net” in net zero needs to be tightened further if the world is to achieve climate justice. Problem: Medical student mistreatment, as well as patient and staff mistreatment by all levels of medical trainees and faculty, is still prevalent in U.S. clinical training. Largely missing in interventions to reduce mistreatment is acknowledgement of the abuse of power produced by the hierarchical structure in which medicine is practiced. Approach: Beginning in 2001, Yale School of Medicine has held annual “Power Day” workshops for third year medical students and advanced practice nursing students, to define and analyse power dynamics within (...) the medical hierarchy and hidden curriculum using literature, guest speakers, and small groups. During rotations, medical students write narratives about the use of power witnessed in the wards. In response to student and small group leader feedback, workshop organizers have developed additional activities related to examining and changing the use of power in clinical teams. Outcome: Emerging narrative themes included the potential impact of small acts and students feeling “mute” and “complicit” in morally distressing situations. Small groups provided safe spaces for advice, support, and professional identity formation. By 2005, students recognized residents that used power positively with Power Day awards and alumni served as keynote speakers on the use of power in medicine. By 2010, departments including OB/GYN, surgery, psychiatry, and paediatrics, had added weekly team Power Hour discussions. Next Steps: The authors highlight barriers, benefits, and lessons learned. Barriers include the notion of clinical irrelevance and resistance to the word “power” due to perceived accusation of abuse. Benefits include promoting open dialogue about power, fostering inter-professional collaboration, rewarding positive role modelling by residents and faculty, and creating a network of trainee empowerment and leadership. Furthermore, faculty have started to ask that issues of power be addressed in a more transparent way at their level of the hierarchy as well. (shrink)

Este artículo es una aproximación al carácter político de las mafias que protegen el tráfico de drogas desde una perspectiva mencionada pero poco tratada dentro del concepto mismo de mafia: la articulación de intereses de amplios grupos sociales dentro de su oferta de protección. Tanto las mafias de la droga que gozan de dominación social como las que no tienen mayor interacción social toman decisiones dirigidas a la imposición de sus intereses. La gran diferencia está en que las decisiones de (...) las primeras tienden a involucrar la estructura social que ha sido transformada por el narcotráfico. Cualquier intento por reprimir a las mafias involucra ahora un ataque a los intereses estructurales de amplios sectores sociales. Un caso concreto servirá para ilustrar esta lectura política del narcotráfico: la guerra de Pablo Escobar contra el estado. (shrink)

One increasingly popular view in the philosophy of perception isexternalismabout sensible qualities, according to which sensible qualities such as colors, smells, tastes, and textures are features, not of our minds, but of mind-independent, external objects in the world. The primary motivation for this view is that perceptual experience seems to betransparent—that is, when we attend to sensible qualities, it seems like what we are attending to are features of external objects, not our own minds. Most (if not all) externalists are (...) either naïve realists or externalist representationalists. However, in this article, I argue that those who are moved by the primary motivation for externalism should instead be sense-datum theorists, for externalists’ primary motivation supports the sense-datum theory, not their actually favored views. I argue that externalists should focus on different motivations, get new ones, or become sense-datum theorists. (shrink)

In this book encounter with Kostas Axelos' Introduction to Future Ways of Thought: On Marx and Heidegger, I introduce Axelos as an imaginative reader of Marx and Heidegger. His approach draws on their parallels to imagine what is possible. I argue that Axelos makes three unique contributions to the Heideggerian-Marxist project. First, Axelos’ understanding of the planetary epoch relies on a reading of Marx that is heavily influenced by Heidegger’s concept of “world”. Second, he builds on the idea of an (...) “open world” meant to combat the technological over-determination characteristic of the planetary era. Third, Axelos preserves the significance of Heidegger and Marx to outline the possibilities of a thinking that, as of yet, remain a future undertaking. I conclude with some ideas about how Axelos' thinking may be further developed. (shrink)

In this essay, I argue that it is morally wrong for police to lie to suspects in interrogations and that it should be legally prohibited. I base my argument on broadly Kantian considerations about respect for autonomy: Respect for rational agency forbids lying to suspects and there is no plausible and compelling rationale for allowing police to lie to suspects in typical cases of interrogation.

En examinant les limites et les potentiels du blocage propre à « l’action éco-directe », nous pouvons dégager certaines leçons qui pourraient être généralisées à d’autres luttes. En particulier, dans une phase capitaliste de circulation, une tactique consistant à arrêter les flux peut être utile dans des scénarios impliquant d’aller au-delà du (...) simple arrêt de la construction d’un oléoduc. Mais, si une interruption peut ralentir la reproduction du capital, elle pose des défis à la reproduction du social. Ces défis peuvent être surmontés par des blocages qui cherchent non seulement à mettre fin à la situation génératrice du conflit, mais également à créer de nouveaux rapports sociaux. (shrink)
