This essay explores the matter of hegemony in the global system from the standpoint of global capitalism theory, in contrast to extant approaches that analyse this phenomenon from the standpoint of the nation‐state and the inter‐state system. It advances a conception of global hegemony in transnational social terms, linking the process of globalisation to the construction of hegemonies and counter‐hegemonies in the twenty‐first century. An emergent global capitalist historical bloc, lead by a transnational capitalist class, rather than a particular nation‐state, (...) bloc of states, or region, is pursuing a hegemonic project. The US state is seen as the point of condensation for pressures from dominant groups to resolve problems of global capitalism. US‐led militarisation is a contradictory political‐military response to the crisis of global capitalism, characterised by economic stagnation, legitimacy problems and the rise of counter‐hegemonic forces. The book begins with an overview of globalization, showing how wealth and power are concentrated in the hands of a transnational elite while ever increasing numbers of people are being marginalised. Institutions such as the World Trade Organisation and the International Monetary Fund are intent upon exercising a new hegemony over individuals as the role of the traditional nation state is transformed. At the centre of this power shift is a group of high-tech robber barons who dominate the Information Age (...) and exploit the technologies of globalization for their own narrow interests. The second part of the book explores the rise of the new grass roots oppositional movements around the world. Manifest in such diverse struggles as the uprising of the Zapatistas in Mexico and the battle of Seattle against the World Trade Organisation, this new postmodern politics is "de-centred" and has little interest in the old ideologies that dominated much of the twentieth century. The final section of the book contextualizes postmodern politics by drawing on contemporary examples. The authors discuss the demise of socialist and proto-socialist experiments in Chile, Grenada, Nicaragua and Cuba and the emergence of postmodern movements in Latin America. Behind the economic turbulence and political transformations of recent decades is the transition from the nationstate phase of world capitalism to a new transnational phase. While many detractors of globalization focus on global trade, the process is driven by the transnationalization of capital ownership, which in turn leads to the rise of a transnational bourgeoisie that sits at the apex of the global order. Parallel to the transatlantic and transpacific integration of capital there has been an integration of Southern capitalists (...) into the emergent system of transnational capital. A transnational hegemony is replacing a declining U. S. hegemony, although supra-national structures are still not capable of providing the economic regulation and political conditions for global capitalism to function smoothly. In this period of extraordinary conflict, upheaval, and uncertainty, the role of popular classes will be crucial. But their struggles must take on a transnational perspective and engage in transnational organizing. (shrink)

Objective: To assess the adequacy of the process of informed consent for surgical patients at the University Hospital of the West Indies. Method: The study is a prospective, cross-sectional, descriptive study. 210 patients at the University Hospital of the West Indies were interviewed using a standardised investigator-administered questionnaire, developed by the authors, after obtaining witnessed, informed consent for participation in the study. Data were analysed using SPSS V.12 for Windows. Results: Of the patients, 39.4% were male. Of the surgical procedures, (...) 68.6% were scheduled, 7.6% urgent and 23.8% emergency, 35.2% were minor and 64.8% major. Information imparted/received was acceptable in 40% of cases, good in 24% and inadequate (unacceptable) in 36% of cases. Almost all (97.6%) patients stated that they understood why an operation was planned and 93.3% thought that they had given informed consent. Most (95.2%) thought that they had free choice and made up their own mind. A quarter (25.2%) of all patients were told that it was mandatory for them to sign the form. There was a discussion of possible side effects and complications in 56.7% of patients. Conclusions: This study clearly indicates that surgical patients at the University Hospital of the West Indies feel that they have given informed consent. However, it also suggests that more information should be given to patients for consent to be truly informed. (shrink)

A transnational capitalist class has emerged as that segment of the world bourgeoisie that represents transnational capital, the owners of the leading worldwide means of production as embodied in the transnational corporations and private financial institutions. The spread of TNCs, the sharp increase in foreign direct investment, the proliferation of mergers and acquisitions across national borders, the rise of a global financial system, and the increased interlocking of positions within the global corporate structure, are some empirical indicators of the transnational (...) integration of capitalists. The TCC manages global rather than national circuits of accumulation. This gives it an objective class existence and identity spatially and politically in the global system above any local territories and polities. The TCC became politicized from the 1970s into the 1990s and has pursued a class project of capitalist globalization institutionalized in an emergent transnational state apparatus and in a "Third Way" political program. The emergent global capitalist historic bloc is divided over strategic issues of class rule and how to achieve regulatory order in the global economy. Contradictions within the ruling bloc open up new opportunities for emancipatory projects from global labor. (shrink)

Previous research has suggested that individuals who identify as being more religious request more aggressive medical treatment at end of life. These requests may generate disagreement over life-sustaining treatment (LST). Outside of anecdotal observation, however, the actual role of religion in conflict over LST has been underexplored. Because ethics committees are often consulted to help mediate these conflicts, the ethics consultation experience provides a unique context in which to investigate this question. The purpose of this paper was to examine the (...) ways religion was present in cases involving conflict around LST. Using medical records from ethics consultation cases for conflict over LST in one large academic medical centre, we found that religion can be central to conflict over LST but was also present in two additional ways through (1) religious coping, including a belief in miracles and support from a higher power, and (2) chaplaincy visits. In-hospital mortality was not different between patients with religiously versus non-religiously centred conflict. In our retrospective cohort study, religion played a variety of roles and did not lead to increased treatment intensity or prolong time to death. Ethics consultants and healthcare professionals involved in these cases should be cognisant of the complex ways that religion can manifest in conflict over LST. (shrink)

We examine the problem of deducing the geodesic motion of test particles from Einstein's vacuum field equations and its extension to include gravitational radiation reaction. In the latter case we obtain an equation of motion for a particle which incorporates radiation reaction of the electrodynamical type, but due to shearing radiation, together with a mass-loss formula of the Bondi-Sachs type.

It is time for a paradigm shift in our study of world capitalism and the global ruling class. The statecentrism informing much theorization and analysis of world politics, political economy, and class structure is less and less congruent with 21st century world developments. Global capitalism represents a new stage in the ongoing and open-ended evolution of world capitalism, characterized by the rise of transnational capital and a globally integrated production and financial system commanded by a transnational capitalist class, or TCC, (...) that attempts to exercise its class power through transnational state apparatuses. The TCC, as the new global ruling class, has been attempting, albeit with limited success, to construct a global hegemonic bloc in which one group, the TCC, exercises leadership and imposes its project through the consent of those drawn into the bloc, while those from the majority who are not drawn into this hegemonic project, either through material rewards or ideological mechanisms, are contained or repressed. Global capitalism is in crisis. A popular revolt is spreading worldwide, thus posing many challenges. Transformative struggles must be informed by an accurate analysis of global capitalism and its ruling class. (shrink)

The current moment must be seen from a stadial perspective on capitalist development. A new transnational stage is marked by the rise of transnational capital, a transnational capitalist class and state, and novel relations of power and inequality in global society. Recent events do not represent a new U. S. bid for hegemony amidst heightened inter-imperialist rivalry. Faced with increasingly dim prospects for a viable transnational hegemony, transnational elites have mustered up fragmented and incoherent responses involving heightened military coercion, the (...) search for a post-Washington Consensus, and acrimonious internal disputes. This militarized globalization is less a campaign for U. S. hegemony than a contradictory political response to the explosive crisis of global capitalism. Yet the power of collective agencies to influence history is enhanced at such times of crisis, rather than of stability and equilibrium. (shrink)

Virtues are acquirable, so if intellectual humility is a virtue, it’s acquirable. But there is something deeply problematic—perhaps even paradoxical—about aiming to be intellectually humble. Drawing on Edward Slingerland’s analysis of the paradoxical virtue of wu-wei in Trying Not To Try (New York: Crown, 2014), we argue for an anti-individualistic conception of the trait, concluding that one’s intellectual humility depends upon the intellectual humility of others. Slingerland defines wu-wei as the “dynamic, effortless, and unselfconscious state of mind of a person (...) who is optimally active and effective” (Trying Not to Try, 7). Someone who embodies wu-wei inspires implicit trust, so it is beneficial to appear wu-wei. This has led to an arms race between faking wu-wei on the one hand and detecting fakery on the other. Likewise, there are many benefits to being (or seeming to be) intellectually humble. But someone who makes conscious, strategic efforts to appear intellectually humble is ipso facto not intellectually humble. Following Slingerland’s lead, we argue that there are several strategies one might pursue to acquire genuine intellectual humility, and all of these involve commitment to shared social or epistemic values, combined with receptivity to feedback from others, who must in turn have and manifest relevant intellectual virtues. In other words, other people and shared values are partial bearers of a given individual’s intellectual humility. If this is on the right track, then acquiring intellectual humility demands epistemic anti-individualism. (shrink)

Promises to perform supererogatory actions present an interesting puzzle. On the one hand, this seems like a promise that one should be able to keep simply by performing some good deed or other. On the other hand, the only way to keep it is to do something that exceeds one’s duties. But any good deed that one performs, which might otherwise have been supererogatory, will not go above and beyond what one is morally required to do in such a case (...) because one has an obligation that one does not normally have—namely, an obligation to do something supererogatory. Thus, some scholars have argued that promises of this sort cannot possibly be kept and so are wrong to make. I show that, far from being impossible, keeping promises to supererogate is easy, and so there is nothing wrong with making promises of this sort. (shrink)

While a significant literature has appeared discussing theoretical ethical concerns regarding COVID-19, particularly regarding resource prioritization, as well as a number of personal reflections on providing patient care during the early stages of the pandemic, systematic analysis of the actual ethical issues involving patient care during this time is limited. This single-center retrospective cohort mixed methods study of ethics consultations during the first surge of the COVID 19 pandemic in Massachusetts between March 15, 2020 through June 15, 2020 aim to (...) fill this gap. Results indicate that there was no significant difference in the median number of monthly consultation cases during the first COVID-19 surge compared to the same period the year prior and that the characteristics of the ethics consults during the COVID-19 surge and same period the year prior were also similar. Through inductive analysis, we identified four themes related to ethics consults during the first COVID-19 surge including (1) prognostic difficulty for COVID-19 positive patients, (2) challenges related to visitor restrictions, (3) end of life scenarios, and (4) family members who were also positive for COVID-19. Cases were complex and often aligned with multiple themes. These patient case-related sources of ethical issues were managed against the backdrop of intense systemic ethical issues and a near lockdown of daily life. Healthcare ethics consultants can learn from this experience to enhance training to be ready for future disasters. (shrink)

Background: Breaches of publication ethics such as plagiarism, data fabrication and redundant publication are recognised as forms of research misconduct that can undermine the scientific literature. We surveyed journal editors to determine their views about a range of publication ethics issues. Methods: Questionnaire sent to 524 editors-in-chief of Wiley-Blackwell science journals asking about the severity and frequency of 16 ethical issues at their journals, their confidence in handling such issues, and their awareness and use of guidelines. Results: Responses were obtained (...) from 231 editors (44%), of whom 48% edited healthcare journals. The general level of concern about the 16 issues was low, with mean severity scores of <1 (on a scale of 0–3) for all but one. The issue of greatest concern (mean score 1.19) was redundant publication. Most editors felt confident in handling the issues, with <15% feeling “not at all confident” for all but one of the issues (gift authorship, 22% not confident). Most editors believed such problems occurred less than once a year and >20% of the editors stated that 12 of the 16 items never occurred at their journal. However, 13%–47% did not know the frequency of the problems. Awareness and use of guidelines was generally low. Most editors were unaware of all except other journals’ instructions. Conclusions: Most editors of science journals seem not very concerned about publication ethics and believe that misconduct occurs only rarely in their journals. Many editors are unfamiliar with available guidelines but would welcome more guidance or training. (shrink)

Contemporary empiricism has attempted to ground its analysis of science in a falsificationism based in selection theory. This paper links these evolutionary epistemologies with commitments to certain epistemological and ontological assumptions found in the later work of K. Popper, D. Campbell, and D. Hull, I argue that their assumptions about the character of contemporary empiricism are part of a shared paradigm of epistemological explanation which results in unresolved tensions within their own projects. I argue further that their claim to be (...) doing a science of science is not defensible. Hull's selectionism is analyzed to show how this epistemological agenda has played itself out in late empiricism. I suggest some directions that Hull might take toward an historical epistemology. The improvement of the welfare of inhabitants of arid and semi-arid lands, either through the enhancement of existing livelihoods or the promotion of alternative ones, and their potential constraints are discussed. Alternative livelihoods are discussed under regenerative and extractive themes with respect to environmental stability. Regenerative (i.e., non-extractive) livelihoods include activities like apiculture, poultry keeping, pisciculture, silkworm production, drought tolerant cash cropping, horticulture, community wildlife tourism, processing of livestock and crop products, agro-forestry for tree products, and micro-enterprises in the informal (...) sector. Examples of livelihoods that are extractive or potentially so include timber production, woodcarving, basketry, brick making, sand scooping, and charcoal making. Suggestions to improve these livelihoods in a sustainable manner are offered. (shrink)

Modern ethics has been chiefly concerned with the analysis of the conditions and principles of morality; and, in particular, one of its most important achievements has been the further elucidation of the Kantian dictum that “I ought” implies that I can. On the face of it Kant's contention seems perfectly straightforward, but, on examination, it becomes apparent that the simple word “can” covers a somewhat complicated ambiguity. When it is said that I ought to do act A, it may rightly (...) be taken that I can do act A; but the question arises whether or not this in turn involves that I am aware that I can do act A. There is clearly some significance in the assertion that I can do many acts which, however, never occur to me, either because I am ignorant of certain facts or because I have a mistaken opinion about the facts. On the other hand, it may quite properly be said that I cannot perform any act of the possibility of which I am unaware. The problem then is to determine and examine the sense of the word “can” which is a pre-condition of moral obligation. (shrink)

Systematic study of the intersection of ethics consultation services and solid organ transplants and recipients can identify and illustrate ethical issues that arise in the clinical care of these patients, including challenges beyond resource allocation. This was a single-centre, retrospective cohort study of all adult ethics consultations between January 1, 2007, and December 31, 2017, at a large academic medical centre in the north-eastern United States. Of the 880 ethics consultations, sixty (6.8 per cent ) involved solid organ transplant, thirty-nine (...) (65.0 per cent) for candidates and twenty-one (35.0 per cent ) for recipients. Ethics consultations were requested for 4.3 per cent of heart, 4.9 per cent of lung, 0.3 per cent of liver, and 0.3 per cent of kidney transplant recipients over the study period. Nurses were more likely to request ethics consultations for recipients than physicians (80.0 per cent vs 20.0 per cent, p = 0.006). The most common reason for consultation among transplant candidates was discussion about intensity of treatment or goals of care after the patient was not or was no longer a transplant candidate. The most common reason for ethics consultation among transplant recipients was disagreement between transplant providers and patients/families/non-transplant healthcare professionals over the appropriate intensity of treatment for recipients. Very few consultations involved questions about appropriate resource allocation. Ethics consultants involved in these cases most often navigated communication challenges between transplant and non-transplant healthcare professionals and patients and families. (shrink)

Critical care society guidelines recommend that ethics committees mediate intractable conflict over potentially inappropriate treatment, including Do Not Resuscitate status. There are, however, limited data on cases and circumstances in which ethics consultants recommend not offering cardiopulmonary resuscitation despite patient or surrogate requests and whether physicians follow these recommendations. This was a retrospective cohort of all adult patients at a large academic medical center for whom an ethics consult was requested for disagreement over DNR status. Patient demographic predictors of ethics (...) consult outcomes were analyzed. In 42 of the 116 cases, the patient or surrogate agreed to the clinician recommended DNR order following ethics consultation. In 72 of 74 of the remaining cases, ethics consultants recommended not offering CPR. Physicians went on to write a DNR order without patient/surrogate consent in 57 of those cases. There were no significant differences in age, race/ethnicity, country of origin, or functional status between patients where a DNR order was and was not placed without consent. Physicians were more likely to place a DNR order for patients believed to be imminently dying. The median time from DNR order to death was 4 days with a 90-day mortality of 88.2%. In this single-center cohort study, there was no evidence that patient demographic factors affected ethics consultants’ recommendation to withhold CPR despite patient/surrogate requests. Physicians were most likely to place a DNR order without consent for imminently dying patients. (shrink)

The result of many years of painstaking scholarship, the new volume will replace the now nearly 100 year old original edition, and is destined to become just as long-lasting a classic.

Being a medically qualified patient can be an unpleasant experience for a person who is used to making decisions. For the most part, this applies to the vast majority of doctors and other healthcare professionals. Becoming passive and surrendering the decision-making process to others is alien to the medical culture we were taught. However, when as a hospitalised medically qualified patient, one sees fellow patients in difficulty, or deteriorating clinically, unnoticed by medical staff, the question of whether it is ethical (...) to intervene arises. I report my views on this as a largely passive, but still actively thinking patient. (shrink)

RESUMO: Hilary Putnam procurou solapar o relativismo cognitivo, mediante acusações de incoerência autodestrutiva. A concepção de Thomas Kuhn de desenvolvimento do conhecimento científico ocupa um lugar de destaque nesse empreendimento crítico, e a incomensurabilidade entre paradigmas rivais constitui o núcleo da disputa. Putnam afirmou que a incomensurabilidade é autorrefutante, levando em conta apenas sua dimensão semântica. Este artigo examina essa investida antirrelativista. Considero dois sentidos de autorrefutação, o material e o formal, e defendo que essa acusação não atinge a referida (...) formulação semântica. Adicionalmente, mostro que a dimensão epistemológica da incomensurabilidade também não é afetada. ABSTRACT: Hilary Putnam sought to undermine cognitive relativism by charging it with self- destructive incoherence. Thomas Kuhn's conception of the development of scientific knowledge occupies a prominent place in that critical endeavor, and the incommensurability between rival paradigms constitutes the core of the dispute. Putnam claimed that incommensurability is self- refuting, taking into account only its semantic dimension. This article examines this anti-relativist attack. I consider two senses of self-refutation, the material and the formal, and argue that they do not affect the semantic formulation of incommensurability. Additionally, I show that the epistemological dimension of incommensurability is also unaffected. (shrink)

Daniel Dennett (1991) has advanced a mild realism in which beliefs are described as patterns “discernible in agents’ (observable) behavior” (p. 30). I clarify the conflict between this otherwise attractive theory and the strong realist view that beliefs are internal states that cause actions. Support for strong realism is sometimes derived from the assumption that the everyday psychology of the folk is committed to it. My main thesis here is that we have sufficient reason neither for strong realism nor for (...) the supporting assumption about the commitments of folk psychology. Several generally implicit arguments in support of the latter assumption are considered. Explicit arguments for it by Ramsey et al. (1990) and Wellman (1990) are examined and judged unsuccessful. An explicit argument for strong realism by Cummins (in conversation) is also found inadequate. Consideration of this latter argument helps to explain why we cannot be satisfied with Dennett's own very brief discussion of causation by beliefs. (shrink)
