Lifetime quality-adjusted life-year (QALY) prioritarianism has recently been defended as a reasonable specification of the prioritarian view that benefits to the worse off should be given priority in health-related priority setting. This paper argues against this view with reference to how it relies on implausible assumptions. By referring to lifetime QALY as the basis for judgments about who is worse off lifetime QALY prioritarianism relies on assumptions of strict additivity, atomism and intertemporal separability of sublifetime attributes. These (...) assumptions entail that a health state at some period in time contributes with the same amount to how well off someone is regardless of intrapersonal and interpersonal distributions of health states. The paper argues that this is implausible and that prioritarians should take both intrapersonal and interpersonal distributions of goods into account when they establish who is worse off. They should therefore not accept that lifetime QALY is a reasonable ground for ascribing priority and reject lifetime QALY prioritarianism. (shrink)

Two principles form the basis for much priority setting in health. According to the greater benefit principle, resources should be directed toward the intervention with the greater health benefit. According to the worse off principle, resources should be directed toward the intervention benefiting those initially worse off. Jointly, these principles accord with so-called prioritarianism. Crucial for its operationalisation is the specification of the worse off. In this paper, we examine how the worse off can be defined as those with the (...) fewer lifetime Quality-Adjusted Life Years (QALYs). This chapter opens the question of aggregating wellbeing across time to determine the value of a single person’s life. It explains the imperfect analogy between this sort of aggregation and aggregation across time to determine the overall value of a world. It assesses the principle of temporal good, the analogue of the principle of personal good. It shows there are plausible grounds for doubting the principle, which derive from various putative pattern goods. Nevertheless, this chapter eventually adopts the principle of (...) temporal good as a default position. Despite Fisher's (1930) psychological intuitions of and the formal treatment given by Yaari (1965, Review of Economic Studies 32, 137), the intertemporal model of choice is mainly a model with certain lifetime. The purpose of this paper is to reconsider this assumption, starting from a very simple two-period model of choice with lifetime uncertainty. We examine the comparative statics of the model at the first two orders and replace the concept of `pure time preference' by taking into account (...) the subjective treatment of the probability of survival. Residual useful lifetime prediction plays a key role of failure prediction and health management in equipment. Aiming at the problems of residual life prediction without comprehensively considering multistage and individual differences in equipment performance degradation at present, we explore a prediction model that can fit the multistage random performance degradation. Degradation modeling is based on the random Wiener process. Moreover, according to the degradation monitoring data of the same batch of equipment, we apply the expectation maximization algorithm to estimate (...) the prior distribution of the model. The real-time remaining life distribution of the equipment is acquired by merging prior information of real-time degradation data and historical degradation monitoring data. The accuracy of the proposed model is demonstrated by analyzing a practical case of metalized film capacitors, and the result shows that a better RUL estimation accuracy can be provided by our model compared with existing models. A new lifetime distribution, called exponential doubly Poisson distribution, is proposed with decreasing, increasing, and upside-down bathtub-shaped hazard rates. One of the reasons for introducing the new distribution is that it can describe the failure time of a system connected in the form of a parallel-series structure. Some properties of the proposed distribution are addressed. Four methods of estimation for the involved parameters are considered based on progressively type II censored data. These methods are maximum likelihood, moments, least squares, (...) and weighted least squares estimations. Through an extensive numerical simulation, the performance of the estimation methods is compared based on the average of mean squared errors and the average of absolute relative biases of the estimates. Two real datasets are used to compare the proposed distribution with some other well-known distributions. The comparison indicates that the proposed distribution is better than the other distributions to match the data provided. (shrink)

In this memoir I track the people, events and influences which have had a bearing on my ability to work alongside Māori and Pacific students and academics. From a childhood in rural Aotearoa New Zealand, through sporadic and prolonged university studies, to teaching in Schools of Education at the University of Waikato and Auckland University of Technology, I have always been fascinated by the possibilities of thinking differently. Political egalitarians tend to defend equal distributions of voting power at specific times, as in ‘one election, one vote’. Appealing as it is, the principle seems incompatible with distributing power proportionally to the stakes voters have at different elections, as in ‘one stake, one vote’. This article argues that the tension above stems from the temporal scope ascribed to political equality, as at specific moments of democratic decision-making instead of over entire lives. More specifically, ascribing a lifetime view to (...) political equality renders equality compatible with proportionality at different elections. I first show that storable votes differ from standard votes in their distinctive commitment to lifetime political equality. I then argue that storable voting schemes are compatible with three key reasons to value political equality: equal consideration of interests, relational equality, and non-domination. Finally, storable votes are also consistent with proportionality at specific times. I conclude that the neglected idea of lifetime political equality can, through storable votes, deliver proportionality without inequality. (shrink)

Bell, John I've been invited to share with you my experiences of a lifetime's acquaintance with Shakespeare, and how that acquaintance has led to what might be loosely termed a humanist philosophy.

Solving the 4-d Einstein equations as evolution in time requires solving equations of two types: the four elliptic initial data (constraint) equations, followed by the six second-order evolution equations. Analytically the constraint equations remain solved under the action of the evolution, and one approach is to simply monitor them (unconstrained evolution). The problem of the 3-d computational simulation of even a single isolated vacuum black hole has proven to be remarkably difficult. Recently, we have become aware of two publications that (...) describe very long term evolution, at least for single isolated black holes. An essential feature in each of these results is constraint subtraction. Additionally, each of these approaches is based on what we call “modern,” hyperbolic formulations of the Einstein equations. It is generally assumed, based on computational experience, that the use of such modern formulations is essential for long-term black hole stability. We report here on comparable lifetime results based on the much simpler (“traditional”) ${\dot g} - {\dot K}$ formulation. With specific subtraction of constraints, with a simple analytic gauge, with very simple boundary conditions, and for moderately large domains with moderately fine resolution, we find computational evolutions of isolated non-spinning black holes for times exceeding 1000 GM/c2. We have also carried out a series of constrained 3-d evolutions of single isolated black holes. We find that constraint solution can produce substantially stabilized long-term single hole evolutions. However, we have found that for large domains, neither constraint-subtracted nor constrained ${\dot g} - {\dot K}$ evolutions carried out in Cartesian coordinates admit arbitrarily long-lived simulations. The failure appears to arise from features at the inner excision boundary; the behavior does generally improve with resolution. (shrink)

This chapter defines a quantitative notion of a person’s lifetime wellbeing. It does so on the basis of betterness among uncertain prospects, using expected utility theory and a theorem of John Harsanyi. It adopts the assumption of Daniel Bernoulli that wellbeing is risk-neutral. It gives an account of interpersonal comparisons of wellbeing.

Violence is a known risk factor for health problems. In this epidemiological study across 5,385 male patients, we investigate the prevalence of perpetrated violence, exposure to violence, their overlap and the relationship between violence, mental, and psychosomatic health, as well as adverse health behaviors, such as self-harming behavior and the consumption of drugs. Participants completed an anonymous questionnaire addressing violence experience, age of victimization/perpetration, frequency, and perceived severity of violence exposure. We considered physical, psychological as well as sexual violence. Information (...) on health status and adverse health behaviors complemented the data. Results showed that 48.4% of the sample reported having experienced violence. The victim-perpetrator overlap formed the largest group, in which the incidence of having experienced multiple types of violence was significantly higher compared to victims and perpetrators. The age-crime curve flattened more slowly with increasing age in this group. Although the perceived severity of exposure to violence is lower in the overlap group, its health status and adverse health behaviors were worse. Interventions should focus on this group since they constitute a burden for the healthcare system. (shrink)

This paper is concerned with the interpretation of velocity eigenstates for unstable quantons, their relationship to space-like momentum eigenstates for such quantons and the explanation of Shirokov’s contracting lifetimes for such velocity eigenstates. It is an elaboration of a portion of the authors earlier study.

Having children is probably as old as the first successful organism. It is often done thoughtlessly. This book is an argument for giving procreating some serious thought, and a theory of how, when, and why procreation may be permissible.Rivka Weinberg begins with an analysis of the kind of act procreativity is and why we might be justifiably motivated to engage in it. She then proceeds to argue that, by virtue of our ownership and control of the hazardous material that is (...) our gametes, we are parentally responsible for the risks we take with our gametes and for the persons that develop when we engage in activity that allows our gametes to unite with others and develop into persons. Further argument establishes that when done respectfully, and in cases where the child's chances of leading a life of human flourishing are high, procreation may be permissible. Along the way, Weinberg argues that the non-identity problem is a curiously common mistake. Arguments intending to show that procreation is impermissible because life is bad for people and imposed on them without their consent are shown to have serious flaws. Yet because they leave us with lingering concerns, Weinberg argues that although procreation is permissible under certain conditions, it is not only a welfare risk but also a moral risk. Still, it is a risk that is often permissible for us to take and impose, given our high level of legitimate interest in procreativity. In order to ascertain when the procreative risk is permissible to impose, contractualist principles are proposed to fairly attend to the interests prospective parents have in procreating and the interests future people have in a life of human flourishing. The principles are assessed on their own merits and in comparison with rival principles. They are then applied to a wide variety of procreative cases. (shrink)

Michael Psellos (1018-1081) read texts of the Neoplatonist Proclus (412-485) throughout his life. His interest may have started as early as 1034, but the first direct references can be dated to ca 1041 and the last occur towards the end of his life, notably the Omnifaria Doctrina. Psellos’ interest in Proclus evolved over time: 1. 1034-1043 hermeneutical problems, 2. 1043-1059 theurgy and interest in relation between body and soul, 3. 1059-1081 physiology and interest in Proclus’ philosophical principles. Freedom is one of the central values in political and moral philosophy. A number of theorists hold that freedom should either be the only or at least one of the central distribuenda in our theories of distributive justice. Moreover, many follow Mill and hold that a concern for personal freedom should guide, and limit, how paternalist public policy can be. For the most part, theorists have focussed on a person’s freedom at one specific point in time but have failed to (...) give proper attention to freedom across time. Given that we care about personal freedom now, we have reason to care about future freedom too. But what kind of distribution of freedom across a person’s lifetime should we promote as a matter of legislation and public policy? I argue that none of the candidate principles for the distribution of freedom across time is plausible. Neither a starting gate view, nor a maximisation nor a sufficientarian view is satisfactory, because none adequately reflects our various reasons to value freedom. I show that this result presents a tough challenge for theories of distributive justice and paternalism that set great store by personal freedom. (shrink)

Nagle, Cormac At the 2015 national conference of Catholic Health Australia, Fr Cormac Nagle OFM was awarded CHA's highest honour, the Maria Cunningham Lifetime Contribution Award. After receiving this award, Fr Cormac reflected on his years of ministry at the Chisholm Centre's 2015 Annual General Meeting. We are pleased to present a slightly edited version of his speech here. Fr Cormac reminds us that Catholic health and aged care follows in the footsteps of Jesus Christ, and that both our (...) service and our ethics are based on a profound respect for human dignity. He advises how our ethical standards should always be interpreted so that they reveal rather than obscure the Good News which Christianity proclaims. Fr Cormac also highlights the need for a holistic and multidisciplinary approach to healthcare. Rather than treating only physical or medical problems, this holistic approach cares for the whole person and also attends to their emotional, social and spiritual needs. (shrink)

In the current model of visual transduction, the lifetime of active cGMP phosphodiesterase depends upon the period of its interaction with GTP-bound transducin. If recoverin regulates the lifetime of light-activated cGMP phosphodiesterase through inhibition of rhodopsin phosphorylation, rhodopsin should directly interact with cGMP phosphodiesterase and/or GTP-bound transducin complexed with cGMP phosphodiesterase. Is this true?

A new continuous version of the inverse flexible Weibull model is proposed and studied. Some of its properties such as quantile function, moments and generating functions, incomplete moments, mean deviation, Lorenz and Bonferroni curves, the mean residual life function, the mean inactivity time, and the strong mean inactivity time are derived. The failure rate of the new model can be “increasing-constant,” “bathtub-constant,” “bathtub,” “constant,” “J-HRF,” “upside down bathtub,” “increasing,” “upside down-increasing-constant,” and “upside down.” Different copulas are used for deriving many (...) bivariate and multivariate type extensions. Different non-Bayesian well-known estimation methods under uncensored scheme are considered and discussed such as the maximum likelihood estimation, Anderson Darling estimation, ordinary least square estimation, Cramér-von-Mises estimation, weighted least square estimation, and right tail Anderson Darling estimation methods. Simulation studies are performed for comparing these estimation methods. Finally, two real datasets are analyzed to illustrate the importance of the new model. (shrink)

Following its determination of a finding of scientific misconduct the Office of Research Integrity (ORI) will seek redress for any injury sustained. Several remedies both administrative and statutory may be available depending on the strength of the evidentiary findings of the misconduct investigation. Pursuant to federal regulations administrative remedies are primarily remedial in nature and designed to protect the integrity of the affected research program, whereas statutory remedies including civil fines and criminal penalties are designed to deter and punish wrongdoers. This commentary discusses the available administrative and statutory remedies in the context of a specific case, that of former University of Vermont nutrition researcher Eric Poehlman, and supplies a possible rationale for the legal result. The quality of the products coming from different lines of production requires some tests called comparative life tests. For lines having the same facility, the lifetime of the product is distributed by Burr XII, the lifetime distribution, and units are tested under type-I generalized hybrid censoring scheme. The observed censoring data are used under maximum likelihood and the Bayes method to estimate the model parameters. The theoretical results are discussed and assessed through data analysis and Monte Carlo simulation (...) study. Finally, we reported some brief comments obtained from numerical computation. Proponents of temporal neutrality have challenged the intuitive appeal of future-bias: The intuitive appeal of future-bias is limited to a set of isolated cases that involve only hedonic and self-regarding goods and harms. They suggest that we should treat future-bias as irrational in self-regarding hedonic cases too, or at least not treat the intuitive appeal as evidence for future-bias's permissibility, since hedonic and non-hedonic cases are relevantly similar. This paper defends the rationality of future-bias against this concern. Firstly, hedonic goods (...) and harms mostly concern well-being at a time, while the proposed non-hedonic goods concern lifetime goodness. This is a relevant structural difference between hedonic and non-hedonic goods and harms that warrant different attitude patterns, including the applicability of future-bias. Secondly, the degree of idealisation required to make intuitions favour temporal neutrality obscures the weight of intuitions to a level that makes it unreliable as evidence against the permissibility of future-bias. As temporal neutralists need this degree of idealisation of their cases, the argument fails. In summary, we can keep using intuitive support as evidence for future-bias's rationality, at least when it comes to hedonic goods, but to some extent for its general permissibility. (shrink)

Scarce healthcare resources can be allocated in many ways. The National Institute for Health and Clinical Excellence in the UK focuses on the size of the benefit relative to costs, yet we know that there is support among clinicians and the general public for reducing inequalities in health. This paper shows how the UK general public trade-off these sometimes competing objectives, and the data we gather allow us to show the weight given to different population groups, for example, 1 extra (...) year of life in full health to someone who would otherwise die at the age of 60 years is worth more than twice as much as an additional year of life to someone who would otherwise die at the age of 70 years. Such data can help inform the rationing decisions faced by all healthcare systems around the world. (shrink)

Scholarly correspondence can be as insightful as scholarly work itself, as it often documents the motivating forces of its writers’ intellectual ideas while illuminating their lives more clearly. The more complex the authors’ scholarly works and the more troubled the eras in which they lived, the more substantial, and potentially fascinating, their correspondence. This is especially true of the letters between Alfred Schutz and Eric Voegelin. The scholars lived in incredibly dramatic times and produced profound, complex works that continue to (...) confound academics. The communication between these two giants of the social sciences, as they sent their thoughts to one another, was crucial to the work of both men. _A Friendship That Lasted a Lifetime: The Correspondence between Alfred Schutz and Eric Voegelin _demonstrates that Schutz and Voegelin shared a remarkable friendship: they first met as students in Vienna in the 1920s and found themselves great partners in discussion; years later they were pushed out of Europe by Nazi pressure and went to work at separate American universities. For twenty years they wrote each other, developing their respective scientific works in that dialogue. The letters bear witness to their friendship during the years they spent in exile in the United States, and they document the men’s tentative attempts at formulating the theories of “lifeworld” and “gnosis” associated with Schutz and Voegelin today. The entire collection of 238 letters was printed in German in 2004, but this edited volume is the first to present their correspondence in English and offers a selection of the most important letters—those that contributed to the thinkers’ theoretical discussions and served as background to their most significant thoughts. Editors Gerhard Wagner and Gilbert Weiss do not analyze Schutz’s and Voegelin’s works in light of the correspondence—rather, they present the collection to create a framework for new interpretations. _A Friendship That Lasted a Lifetime_ takes a unique look at two major social scientists. This volume is a valuable resource in the study of Voegelin’s political philosophy and Alfred Schutz’s contribution to American sociology and marks an important addition to the literature on these remarkable men. Showing how scholarly discourse and the dialogue of everyday life can shed light on one another, the book finally presents this correspondence for an American audience and is not to be missed by scholars of philosophy and sociology. This paper provides a critical examination of the strongest defenses of the pure lifetime view, according to which justice requires taking only people's whole lives as relevant when assessing and establishing their distributive entitlements and obligations. The paper proposes that we reject a pure lifetime view and replace it with an alternative view, on which some time-specific considerations--that is to say, considerations about how people fare at specific points in time--have nonderivative weight in determining what our obligations are (...) to them. Each argues for surprising and controversial conclusions: Boonin argues that we should solve the non-identity problem by accepting its apparently unacceptable (...) conclusion that actions cannot be wrong due to their effects on future people whose lives are worth living and who would not exist were the actions not performed; Weinberg argues that there is no non-identity problem, that future children have no interest in existing, but significant interest in avoiding harm and that procreation is, therefore, a much more morally risky business than we tend to think. This paper performs a critical analysis of Irving Gottesman’s 1991 “Figure 10,” which lists the lifetime risks of developing schizophrenia among the relatives of people diagnosed with schizophrenia. Figure 10, which has been cited in numerous psychiatry and abnormal psychology textbooks, is almost always discussed in support of important genetic influences on schizophrenia. However, the pooled results in Figure 10 can also be explained by environmental factors. Moreover, the risk percentages Gottesman reported are derived from biased research designs, some (...) of which are based on implausible theoretical assumptions, while some results are not included. It is concluded that a closer look at the studies used to compile Figure 10 might lead psychiatrists, psychologists, and genetic researchers to decide that, in addition to problems in schizophrenia adoption research and the ongoing failure to find postulated “schizophrenia genes,” the evidence supporting a genetic basis for schizophrenia is far weaker than is currently believed.
