The paper assesses labour market integration opportunities for the groups of country’s population that are highly contrasting in social terms, i.e. young people and older adults, and their dependence on demographic changes. Since the beginning of the 21st century up to 2050 in the future, demographic changes have demonstrated and will continue to have an increasing impact on the social and economic development of European countries. In this context, it is particularly appropriate to monitor the demographic prospects (...) and labour market situation of young people and older adults. These marginal social groups of economic activity are the best reflection of the overall labour market prospects of an individual country. The paper provides a detailed analysis of the situation in Lithuania. Transformations of the situation of young people and older adults in the Lithuanian labour market are closely linked to highly dynamic changes in demographic structures. Using a variety of labour market indicators, the paper identifies trends characterising the situation of the target groups at issue, with a particular emphasis on the observed changes in unemployment and employment (economic activity) during crisis periods. The results of the analysis help to highlight weaknesses and strengths of the labour market situation of the target groups. The strengths include the positive dynamism of most of the observed labour market indicators in Lithuania in the long term and the fact that the situation is, in many cases, relatively better than the average for EU countries. The weaknesses in the labour market situation of young people and older adults are related to the relatively higher vulnerability of these social groups during economic downturns; they appear to lose jobs faster and need longer time to find a suitable job during economic recoveries. Current local and national demographic trends have deepened the existing and formed new global demographic processes that have received a new historical reasoning that requires deep scientific research taking into account the influence of the multifactorial global dimension of the modern society development. The purpose of the article is to study the development of global demographic processes and to define the causes of their occurrence, manifestations, implications and prospects for implementation in the first half of the 21st (...) century. The authors have identified and characterized four global demographic processes, namely population growth, migration, increase of tourism, and change in population structure. It is projected that in the 30’s of the 21st century, the number and growth rates of the world population will reach the objective growth and these dynamics over the next two decades will begin to change in the direction of reducing the growth rates, which will lead to gradual stabilization, and eventually reduce the size of the world population. By the middle of the 21st century, one can observe the preservation of the growth rates of international and domestic migration, the growth of international migration flows from the South to the North and from the East to the West, the strengthening of new economically developed centers of gravity (Canada, Australia and New Zealand), the increase in migration of rural population to cities, as well as urbanization and activation of the metropolises development. The share of international tourists in comparison with the world population will be constantly increasing, and the annual growth rate of the number of international tourists will significantly depend on the world economy and may vary at the several percent level. Permanent change will occur in the age, religious-cultural and socio-economic structure of the population. (shrink)

Demography is typically defined as the study of human populations and the changes in their quantity associated with migration, fertility, and mortality. The term demography comes from Greek word and means “describing people.” Thus, this discipline deals with the characteristics of the population, taking into account features such as, sex ratio, age structure, composition, spatial distribution, and population density. In addition, sometimes a distinction is made between “formal demography” or “demographic analysis,” which includes the statistical analysis of population (...) parameters and their dynamics, and “population studies,” that is, the analysis of the causes and effects of changes in the structure of the population in a broader context and in connection with other phenomena and processes. The rapid uptake of digital technologies is constantly transforming the modern culture of sports spectating; however, relatively little is known about the impact of digitalization on the changing face of global sports-consuming audiences, particularly from a qualitative perspective. In this article, the relationship between modern mass digital technologies and audiences of sports spectators is described and explained by taking a customer-centric approach to grounded theory using a literature review and in-depth qualitative semi-structured interviews with sports marketing, management, and technology professionals. The qualitative approach permits the gathering of in-depth insights into a problem, generates new concepts through data synthesis and analysis, and captures changing attitudes within the sports industry. Moreover, the qualitative approach to research is not bound by the limitations of quantitative methods and focuses on the primary questions of “how” and “why” digital technology’s mass deployment and diffusion have transformed content consumption in the sports industry. The analysis first systematizes and codes the collected data. Second, all the materials are iteratively read and the key points are outlined. Using the iterative analysis, the theoretical and empirical insights and findings are synthesized in order to achieve the results. Finally, dimensions of the transformation of sports spectators’ consumption are identified and explained. The research implications highlight “how” and “why” modern digital technologies have changed the sports-consuming audience by making it more inclusive in terms of age, gender, demographics, social and health status. Drawing on theories of institutional evolution, this article contends that despite the centrality of occupationally based social insurance in postwar Korea and Taiwan, the welfare state has in fact deepened considerably. The analysis is structured around three distinct eras of social policy reform in Korea and Taiwan: the developmental state, democratic transition, and postindustrialism. The authors contend that during each of these eras, the institutional purposes of social policy were altered to meet certain socioeconomic objectives. New institutional purposes were grafted (...) onto the prevailing social insurance model, changing the outcomes of social policy. The developmental state era was productivist in purpose, democratic reform during the 1980s reoriented social insurance toward universalist and redistributive principles, and the post-1997 era refocused social insurance to meet the imperatives of flexible labor markets, demographic shifts, and economic globalization. Demographic change, change in family structures, growing ethnic plurality resulting from migration, social inequality and so on require new ways of addressing spiritual and social needs in many Western European countries. In view of these current social changes, increasingly more effort is being put into strengthening cooperation between church congregations and diaconal institutions at the local level. This article will focus on the reciprocal relationship between the church and its immediate local context by focusing on one of (...) the church’s ministries – diaconia – as a service of God’s people to the world. Although diaconia is part of the church, it has developed in organisations separate from the church and thus as a parallel structure to the institutionalised church structure in many countries. The current changes offer opportunities for the church and diaconia to join forces and to overcome cooperational difficulties at the local level. The objective of this article is to draw attention to the potential of the social space for collaboration, to analyse hindrances to collaborative efforts on a practical level and to point out what steps the church and diaconia may take to further local collaboration.Contribution: The contribution of this article is to analyse the social space with regard to the collaboration between local churches and diaconal organisations, and thereby to point out its potential for their further development in line with the journal’s focus on original ideas in practical theology. In response to recent developments in the labour market, in occupational structure and in occupational practice, many aspects of vocational education and training are subjects of discussion and in transition. The tertiary sector is growing, some occupations are integrating while others are differentiating. New methods of production and organization require new types of employee competencies: problem-solving and social-communicative skills are becoming more and more important. The article focuses on the importance and the possibilities of shaping these developments from a (...) gender perspective. First, the significance of changing qualification requirements is discussed for the technical sector, the service and care sector and the economic-administrative sector. Social determinants of health are a subject of growing interest, yet criticisms have emerged about the way determinants are conceptualized in nursing. A tendency to focus on readily observable living conditions and measurable demographic characteristics can divert attention, it has been said, from the less visible underlying processes which shape social life and health. To illustrate how the analytic perspective determines what becomes visible or invisible as a “determinant” in health, this paper presents a case exemplar. Drawing from news (...) reports and research in real estate economics and urban policy analysis, it explores a single local infectious illness outbreak through a series of progressively more abstract units of inquiry, considering mechanisms of lending and debt financing, housing supply, property valuation, tax policy, change in the structure of the financial industry, and international patterns of migration and capital flow, among other factors, which contributed in various ways to creating unsafe living conditions. An analytic exercise calling attention to dynamism and complexity in social processes, the paper offers a political‐economy‐based approach that serves as a cautionary note against oversimplification in discussions of health causality. In the late 1980s workforce became more diverse in terms of demographic changes, cultural differences and other characteristics of organizational members. This diversity was a reflection of changing global markets. Workforce diversity has both positive and negative effects on organizational performance. Therefore, it is becoming important especially for medium- and large-scale businesses. In order to manage increasingly workforce diversity and to prevent discrimination, diversity management is now considered as a major part of strategic human resource management. The purpose of (...) this study is to establish the dimensions of discrimination that occur due to demographic differences in Turkish manufacturing industry. The findings of the research indicate that demographic characteristics, socio-cultural structure, managerial policy and behaviors, union tendency and regional differences, laws and local community, gender, educational and age differences, and political opinions have influence on discrimination. According to the results, discrimination has been observed mostly in job processes such as promotions and appointments, human resource selection, job/employment examination and interviews, and performance appraisal. In aging humans, aerobic exercise interventions have been found to be associated with more positive or less negative changes in frontal and temporal brain areas, such as the anterior cingulate cortex (ACC) and hippocampus, relative to no-exercise control conditions. However, individual measures such as gray-matter (GM) probability may afford less reliable and valid conclusions about maintenance or losses in structural brain integrity than a latent construct based on multiple indicators. Here, we established a latent factor of GM structural integrity based (...) on GM probability assessed by voxel-based morphometry, magnetization transfer saturation, and mean diffusivity. Based on this latent factor, we investigated changes in structural brain integrity during a six-month exercise intervention in brain regions previously reported in studies using volumetric approaches. Seventy-five healthy, previously sedentary older adults aged 63–76 years completed an at-home intervention study in either an exercise group (EG;n= 40) or in an active control group (ACG;n= 35). Measures of peak oxygen uptake (VO2peak) taken before and after the intervention revealed a time-by-group interaction, with positive average change in the EG and no reliable mean change in the ACG. Significant group differences in structural brain integrity changes were observed in the right and left ACC, right posterior cingulate cortex (PCC), and left juxtapositional lobule cortex (JLC). In all instances, average changes in the EG did not differ reliably from zero, whereas average changes in the ACG were negative, pointing to maintenance of structural brain integrity in the EG, and to losses in the ACG. Significant individual differences in change were observed for right ACC and left JLC. Following up on these differences, we found that exercising participants with greater fitness gains also showed more positive changes in structural integrity. We discuss the benefits and limitations of a latent-factor approach to changes in structural brain integrity, and conclude that aerobic fitness interventions are likely to contribute to brain maintenance in old age. The concept of reproductive labor is central to an analysis of gender inequality, including understanding the devaluation of cleaning, cooking, child care, and other “women's work” in the paid labor force. This article presents historical census data that detail transformations of paid reproductive labor during the twentieth century. Changes in the organization of cooking and cleaning tasks in the paid labor market have led to shifts in the demographics of workers engaged in these tasks. As the context for cleaning and (...) cooking work shifted from the dominance of private household servants to include more institutional forms, the gender balance of this reproductive labor workforce has been transformed, while racial-ethnic hierarchies have remained entrenched. This article highlights the challenges to understanding occupational segregation and the devaluation of reproductive labor in a way that analyzes gender and race-ethnicity in an intersectional way and integrates cultural and structural explanations of occupational degradation. Ecological theory is built upon assumptions about the fundamental nature of organism-environment interactions. We argue that two mutually exclusive sets of such assumptions are available and that they have given rise to alternative approaches to studying ecology. The fundamentally different premises of these approaches render them irreconcilable with one another. In this paper, we present the first logical formalisation of these two paradigms.The more widely-accepted approach - which we label the demographic paradigm - includes both population ecology and community (...) ecology (synecology). Demographic ecology assumes that the environment is relatively stable and that biotic processes, governed predominantly by resource availability, are the most important of ecological and evolutionary influences. Moreover, ecological processes are assumed to translate into directional selection pressures that drive significant evolutionary change on a local scale through the process of optimisation. (shrink)

Lexicalized theories of syntax often assume that verb-structure regularities are mediated by lemmas, which abstract over variation in verb tense and aspect. German syntax seems to challenge this assumption, because verb position depends on tense and aspect. To examine how German speakers link these elements, a structural priming study was performed which varied syntactic structure, verb position, and verb overlap.structural priming was found, both within and across verb position, but priming was larger when the verb position was the (...) same between prime and target. Priming was boosted by verb overlap, but there was no interaction with verb position. The results can be explained by a lemma model where tense and aspect are linked to structural choices in German. Since the architecture of this lemma model is not consistent with results from English, a connectionist model was developed which could explain the cross-linguistic variation in the production system. Together, these findings support the view that language learning plays an important role in determining the nature of structural priming in different languages. (shrink)

ObjectiveThis study aimed to investigate the morphometric alterations in the cortical and subcortical structures in multiple system atrophy patients with mild cognitive impairment, and to explore the association with cognitive deficits.MethodsA total of 45 MSA patients and 29 healthy controls were recruited. FreeSurfer software was used to analyze cortical thickness, and voxel-based morphometry was used to analyze the gray matter volumes. Cortical thickness and gray matter volume changes were correlated with cognitive scores.ResultsCompared to healthy controls, both MSA subgroups exhibited widespread (...) morphology alterations of brain structures in the fronto-temporal regions. Direct comparison of MSA-MCI and MSA-only patients showed volume reduction in the left superior and middle temporal gyrus, while cortical thinning was found in the left middle and inferior temporal gyrus in MSA-MCI patients. Cortical thinning in the left middle temporal gyrus correlated with cognitive assessment and disease duration.ConclusionStructural changes in the brain occur in MSA-MCI patients. The alteration of brain structure in the left temporal regions might be a biomarker of cognitive decline in MSA-MCI patients. (shrink)

This article refers to the main features of contemporary criminality in Lithuania. The period of analysis of those main features is 2004-2011. From 2004, a period of stable state registration of criminality, i.e. a period without significant changes in criminal laws commenced. The article deals with the analysis of spreading criminality in Lithuania, and the main socio-demographical features of persons charged with criminal offences. The registered number of criminal offences in 2011 decreased by about 15%, compared to 2004. The largest (...) decrease was recorded in 2007. Yet during the economic crisis, after 2007, the number of registered criminal offences slightly increased, however, as has already been mentioned, the registered number of criminal offences in 2011 was 15% less. Nevertheless, we must bear in mind that the changes in criminality came two-three years after certain social changes, in this case – the economic crisis. The characteristic features of the contemporary criminality in Lithuania are: the tendency of decrease in violent crimes (homicides, rapes, serious bodily injuries). During the researched period, the number of homicides decreased by 39%, of serious bodily harms – by 56%; and of rapes and attempts – by 26.5%. Another tendency is the decrease in property crimes. The most widespread property crime is theft. The registered number of thefts decreased by 25%. Among them, the registered number of car thefts decreased the most – by 65%; and the decrease of domestic burglaries amounted to 55.5%. The registered number of robberies decreased by 53%. During the researched period, destruction of or damage to property and swindling – their registered number – increased by 80% and 92% respectively. In the countries with much more developed market economy than in Lithuania (e. g. in Scandinavian countries), the indicators of those crimes are 2 to 3 times higher. This means, firstly, that in future the number of those crimes in Lithuania will increase; secondly, the decrease in violent crimes and the increase of swindling means that market economy in Lithuania has become more developed than before. In Lithuania, the number of persons charged with criminal offences during the researched period tended to decrease, but not much, only by 12%. Some socio-demographic characteristics of persons charged with criminal offences are: juveniles make up 15%; traditionally, they are men, but in contemporary criminality in Lithuania we observe a small (only by 3%) increase of women; the increase in the number of persons aged from 18 to 24 is observed, and it seems that the socio-economic situation of this age group has become more complicated; during the economic crisis in Lithuania, the number of unemployed and not studying persons increased significantly (up to 60% in the structure of all registered persons charged with criminal offences) and almost reached a problematic situation that existed during the transitional period from socialist economy to market economy, from totalitarian society to democratic society (before 2000). Knowledge of factors that trigger human response to climate change is crucial for effective climate change policy communication. Climate change has been claimed to have low salience as a risk issue because it cannot be directly experienced. Still, personal factors such as strength of belief in local effects of climate change have been shown to correlate strongly with responses to climate change and there is a growing literature on the hypothesis that personal experience of climate (...) change explains responses to climate change. Here we provide, using survey data from 845 private forest owners operating in a wide range of bio-climatic as well as economic-social-political structures in a latitudinal gradient across Europe, the first evidence that the personal strength of belief and perception of local effects of climate change, highly significantly explain human responses to climate change. A logistic regression model was fitted to the two variables, estimating expected probabilities ranging from 0.07 to 0.81 for self-reported adaptive measures taken. Adding socio-demographic variables improved the fit, estimating expected probabilities ranging from 0.022 to 0.91. We conclude that to explain and predict adaptation to climate change, the combination of personal experience and belief must be considered. (shrink)

: This paper seeks to contribute to the development of a sociological understanding of scientific change. It first presents a conceptual framework for defining and understanding the conditions that give rise to episodes of cumulative change (both as the selective reconstruction of events and as the patterned structuring of innovations over time and across different settings). It argues that one of the most powerful structuring mechanisms is the existence of standardized research technologies. Then, the development of electroencephalography (EEG) (...) is presented as an illustrative case study. The EEG supported a dramatic episode of development in brain science, and did so primarily through technological standardization and reliability. The case shows how standard and manipulable technologies provide a framework for orderly change both in retrospective accounts and in specific historical periods of scientific practice. (shrink)

In Bangladesh due to improved quality of life the number of people over 60 years is increasing rapidly. This should be seen as an emerging challenge as the elderly will have special needs and require different care-giving services. Since Bangladesh does not have a social welfare system there will be competition for inadequate resources specially health and medical services. It is envisaged that due to more elderly population the demographic structure will undergo a slow change from the (...) present pyramid structure. The growing trend towards nuclear family or where children live abroad will put the elderly parents in a dilemma-the financial and social support that is essential for them has not yet emerged. The nutrition and health status of elderly people depend on adequate food safe water, proper sanitation facilities and maintaining hygienic standards. To provide special medical care for the elderly there is a need to establish WHO recommended Age-Friendly Primary Health Care centers and separate wards /units are to be set up in the hospitals. To reduce vulnerability of older women there is a need to distribute assets and properties according to the law.. Our new generations have to be responsive, informed and attentive about their duties and responsibilities towards the elderly people. Taking proper care of the elderly is our ethical duty and responsibility. (shrink)

In our attempts to track changes in geological practice over time and to isolate the source of these changes, we have found that they are largely connected with the germination of new geologic subdisciplines. We use keyword and title data from articles in 68 geology journals to track the changes in influence of each subdiscipline on geology over all. Geological research has shifted emphasis over the study period, moving away from economic geology and petroleum geology, towards physics- and chemistry-based topics. (...) The Apollo lunar landings had as much influence on the topics and practice of geological research as the much-cited plate-tectonics revolution. These results reflect the barely-tangible effects of the changes in vocabulary and habit of thought that have pervaded the substance of geology. Geological literature has increased in volume and specialization, resulting in a highly fragmentary literature. However, we infer that "big science," characterized by large amounts of funding, collaboration, and large logistical investments, makes use of this specialization and turns "twigging" into a phenomenon that enhances, rather than inhibits, the progress of science. Serpins are members of a family of structurally related protein inhibitors of serine proteinases, with molecular masses between 40 and 100kDa. In contrast to other, simpler, proteinase inhibitors, they may interact with proteinases as inhibitors, as substrates, or as both. They undergo conformational interconversions upon complex formation with proteinase, upon binding of some members to heparin, upon proteolytic cleavage at the reactive center, and under mild denaturing conditions. These conformational changes appear to be critical in determining the properties of the (...) serpin. The structures and stabilities of these various forms may differ significantly. Although the detailed structural changes required for inhibition of proteinase have yet to be worked out, it is clear that the serpin does undergo a major conformational change. This is in contrast to other, simpler, families of protein inhibitors of serine proteinases, which bind in a substrate‐like or product‐like manner. Proteolytic cleavage of the serpin can result in a much more stable protein with new biological properties such as chemo‐attractant behaviour. These structural transformations in serpins provide opportunities for regulation of the activity and properties of the inhibitor and are likely be important in vivo, where serpins are involved in blood coagulation, fibrinolysis, complement activation and inflammation. Traditional approaches to theory structure and theory change in science do not fare well when confronted with the practice of certain fields of science. We offer an account of contemporary practice in molecular biology designed to address two questions: Is theory change in this area of science gradual or saltatory? What is the relation between molecular biology and the fields of traditional biology? Our main focus is a recent episode in molecular biology, the discovery of enzymatic RNA. (...) Tirana, immediately after it was declared the capital of Albania on 11 February 1920, has undergone many changes in its morphology and city context. The capital is located in the heart of the country. During its lifespan, Tirana has adopted four important regulatory plans starting from 1923. The Western ideologies of the time influenced drastically the city development. The influence of such ideologies was stopped immediately though the imposition of communist ideas, after the Second World War. Rational building forms were (...) created which could be standardized in order to reduce construction costs. Meanwhile, in Western Europe, this current was implemented during the 1920s, as a response to social problems. The communist regime established strict anti-urbanization policy that aims to foster the development of rural areas and the suburban industrial districts in order to control urban growth and demographic mobility. All the regulatory master plans of Tirana, climate data of the city, the actual situation of the specific neighborhood, its orientation toward north, are considered in order to gather information and to implement the correct bioclimatic eco-renovation in the area. A special attention will be given to the underground tunnels which were constructed during the communist regime all over the city (1945–1983) as a response to a hypothetic nuclear attack, in order to shelter the inhabitance against bombing. These reinforced concrete tunnels will serve not only as an area for water collection and recycling strategies but also for cooling with zero energy expenditure for the inhabitants as a consequence of differences in temperature and air pressure. This strategy benefits from the underground air temperature in both seasons (winter and summer). It envisages the handover of these tunnels by the community. Nowadays, such structures are abandoned and denigrated. The realization of these structures required large amounts of money from the state budget. Considering that fact they need to be vitalized and reused, using different methodologies and strategies as part of the city. After the communist regime in the city of Tirana, two other regulatory plans were implemented which observe the city from another perspective, determining and evaluating Tirana’s real values as a city of the future, but not defining yet Tirana as an Ecocity. The ecological potential of Tirana including its outskirts is to be considered. Its flora and fauna are very rich, containing a mixt of species and plants. Regardless, the city of Tirana is suffering from different kinds of pollution such as air and noise pollution which are above EU and WHO norms. Tirana benefits from a Temperate Mediterranean climate. Furthermore, the benefits of the solar radiation are enormous. Unfortunately, nowadays, the city of Tirana is not profiting much from it. From August 2000 to February 2001, I conducted an online survey of what eventually became 140 self-identified Satanists. A report detailing my findings from that questionnaire research was published in the Marburg Journal of Religion under the title “Who Serves Satan? A Demographic and Ideological Profile.” Eight years later, from June through December of 2009, a comparable online survey of 300 Satanists was conducted. However, because of certain problems with the second questionnaire, a third online survey was launched in (...) 2011 – a third survey which, as this article went to press, was still in process. The present paper compares findings from the first survey with the second, using preliminary statistics from the third survey to counterbalance inadequacies in certain of the statistics from the second. Comparing results from the first with results from the second, the average age of respondents rose from twenty-five to twenty-nine. Partly as a consequence of higher average age, the new sample exhibited more diversity – in terms of respondents having a broader range of educational backgrounds, an increased likelihood of being a parent, and the like. Similarly, while the majority of respondents to the new survey were still broadly within the LaVeyan tradition,a far greater percentage than in the old survey professed some variety of theistic or esoteric Satanism. Carey argues that the aspects of categorization that are diagnostic of deep conceptual structure and, by extension, narrow conceptual content, must be distinguished from those aspects that are incidental to categorization tasks. For natural kind concepts, discriminating between these two types of processes is complicated by the role of explanatory stance and the causal history of features in determining category structure.

The escape from the ‘Malthusian trap’ is shown to tend to generate in a rather systematic way quite serious political upheavals. Some demographic structural mechanisms that generate such upheavals have been analyzed, which has made it possible to develop a mathematical model of the respective processes. The forecast of political instability in Sub-Saharan African countries in 2015– 2050 produced on the basis of this model is presented.

The aim of this academic symposium was to provide an answer to the question whether “youth quotas” offer a solution to changes in age demographics and a looming gerontocracy. Based on the premise that young people have the potential to act as change agents, especially with regard to ecological sustainability, it was our aim to stimulate a societal discussion and to raise public awareness on the topic of youth quotas, whilst providing the discussion with a scientific basis. The question (...) of a power shift between generations is already discussed in many facets in the literature. Many commentators state that a shift is already visible and that the problem requires careful political management. In light of this, the implementation of youth quotas could be a possible method of protecting the interests of younger generations in politics and beyond. The symposium was investigating a topic that is greatly under-researched. Some key questions to be addressed at the symposium were: Should youth quotas be limited to the political arena or should they also be implemented in other fields? Can youth quotas ensure that a greater sense of urgency is applied to the problem-solving process of future concerns like global warming? Can young people really be relied upon to represent the interests of the young generation as a whole, or will they just follow their own individual interests? Are youth quotas in general an effective instrument to strengthen the rights of the young generation or do we need other and more effective instruments? Intense debates arose over the question of whether or not youth quotas are an effective means to strengthen the rights of future generations. Some junior researchers suggested that young people can be thought of as the “trustees of posterity” as they tend to be fiercer defenders of long-termist policies, especially regarding the environment, since the environmental crisis will have a more concrete impact on their lifespan. But other speakers rejected the causality that young people will have a stronger determination to solve future problems, or will necessarily add a new “young” perspective in the epistemic process of finding solutions to future problems. The indication of these latter speakers was that environmental issues are not the top priority of young people. Regarding the composition of party lists, one speaker pointed to the problem of legitimacy in the outcome of an election. The positive discrimination of youth within a societal group has to be justified because other groups could feel disadvantaged by the implementation of such a strong instrument. Some speakers challenged the analogy of youth quotas to quotas for women or ethnic minorities, because women and ethnic minorities can’t change their status whereas today’s young people, in the normal course of life, will be old in the future. This means that the disadvantage of a person in his or her young age is just temporal. Generational effects were pitted against age effects in this context. Some experts pointed to alternatives to youth quotas. In their view, lowering the voting age and better political education in schools, especially, would produce better results. Another proposed strategy was the implementation of proxy votes for the parents. A vote at the end of the symposium sparked interesting results. Although several reservations were noted, most of the speakers voted for the implementation of youth quotas. All speakers voted for lowering the voting age. The conclusion reached by this academic symposium was that a package of measures is required to give adequate answers to demographic change. Youth quotas could be part of this package. This study contributes to the first-hand primary research on the topic exploring DBS clinicians’ views on post-DBS personality change among their patients and its underlying cause. Semi-structured interviews were conducted with sixteen clinicians from various disciplines working in Australian DBS practice for movement (...) disorders and/or psychiatric conditions. Thematic analysis of the interviews revealed five primary themes: 1) types, frequency and duration of personality change, 2) causes of personality change, 3) impact on patient and family, 4) communication, comprehension and awareness, and 5) management. Clinicians described a variety of personality changes in Parkinson’s disease following DBS including irritability, impulsivity and impaired decision-making. The frequency of personality change seen in patients varied amongst clinicians, but changes were overwhelmingly transient. Clinicians considered both DBS stimulation and additional factors as inducing personality change. For DBS patients with major depressive disorder, a restoration of pre-morbid personality was associated with alleviation of illness. Considerations for future research of personality change following DBS include selecting suitable tools for quantitative examination and developing a common language between the scientific and ethics communities. In the care of patients with disorders of consciousness (DOC), some ethical difficulties stem from the challenges of accurate diagnosis and the uncertainty of prognosis. Current neuroimaging research on these disorders could eventually improve the accuracy of diagnoses and prognoses and therefore change the context of end-of-life decision making. However, the perspective of healthcare professionals on these disorders remains poorly understood and may constitute an obstacle to the integration of research. We conducted a qualitative study involving healthcare professionals from (...) an acute care university medical center. A short questionnaire captured demographic data as well as the experience of participants with DOC patients. A semi-structured interview was used to explore attitudes toward ethical issues identified in a previous literature review. Qualitative content analysis of interviews was conducted with the NVivo software. Accurate diagnosis among DOC is often regarded as a challenge, but this was generally not the case for our participants because most reported high confidence in DOC diagnoses. However, participants reported struggling with prognosis, especially because of its essential role for end-of-life decision making and communication with families. Variability of opinion between healthcare professionals was reported and identified by some as a minor issue while others stressed how families struggle with different medical opinions. End-of-life decision making encompassed a large proportion of ethical challenges in these patients, and the removal of artificial nutrition and hydration created significant discomfort in a minority of participants. The concept of futility was subject to wide-ranging understandings with both favorable and unfavorable opinions. Our data suggest that to ensure the incorporation of new evidence-based advances, attention should be directed to the real-world practices and challenges of accurate diagnosis and prognosis. Given pervasive challenges in end-of-life care, we recommend improved training of healthcare professionals in the care of patients with DOC, particularly in end-of life care, understanding the context of decision making, and determining how to optimally integrate new neuroscience research on the care of patients with DOC. An examination of the salient literature on hospital clinical directorates (CDs) is presented. A critique of the largely managerialist, instrumental, hortatory and normative extant literature about CDs is offered. In analysing the literature this way the earlier promotional and critical literature is eschewed in favour of an evaluative approach. CDs are then reconceptualised by locating them within two overarching accounts of social structure—formalised, prescribed frameworks, and enacted, patterned interactions—following the kinds of distinctions made by Giddens, Weick, social action and (...) institutional theorists. Social structure as it relates to culture is also considered, following Martin. Such an approach facilitates an understanding of the general weaknesses of health service perspectives and methods of analysis, and exposes the strengths of Giddens–Weick type paradigms. (shrink)
