Due to the potential ethical and psychological implications of screening, and especially inregard of screening on children without available and acceptable therapeutic measures, there is a common view that such procedures are not advisable. As part of an independent research- and bioethical case study, our aim was therefore to explore and describe bioethical issues among a representative sample of participant families (n = 17,055 children) in the ABIS (All Babies In South-east Sweden) research screening for Type 1 diabetes (IDDM).The primary (...) aim is the identification of risk factors important for the development of diabetes and other multifactorial immune-mediated diseases. Four hundred, randomly chosen, participant mothers were asked to complete a questionnaire exploring issues of information, informed consent, bio-material, confidentiality and autonomy, and of prevention/intervention. 293 completed the questionnaire, resulting in a response rate of 73.3%. The majority of questions had the form of 6-point Likert-type response scales (1–6).We found that the majority of respondents felt calm in regard of samples and written material, and also concerning the possibility of their child in the future being identified as having high risk of developing Type 1 diabetes. An important finding concerning access and control of mainly biological data was indicated, with the respondents expressing concern for potential future use. We believe our findings indicate that this kind of empirical studies can substantially contribute to our understanding of bioethical issues of medical research involving genetics. Issues, such as safeguards ensuring theethical criteria of autonomy and respect, were emphasised by our respondents. We believe theissues brought up may promote further discussion, and do suggest issues for consideration by, among others, researchers, bioethicists and Institutional Review Boards. (shrink)

Based on extensive clinical questionnaire data, this paper explores the relation between research subjects’ self-assessed understanding and actual knowledge of a large-scale predictive screening study, and its implications for the proper handling of information and consent routines in longitudinal studies. The intitial data show that low self-assessed understanding among participants was correlated with limited knowledge, concern over participation and collected samples, less satisfaction with information, and feeling passive or negative towards the study. Among those reporting high understanding, a non-negligible number (...) displayed a lack of knowledge regarding central aspects of the study. Regarding high assessed understanding, the multivariate analysis identified the main predictor variables to be knowledge, having a positive attitude towards participation and the study itself, being satisfied with information, having a stable psychosocial background and feeling calm regarding the handling of samples. These findings indicate that to evaluate participants' understanding through self-assessment may not be reliable method. Self-assessed understanding may rather be a good indicator of general attitudes than a tool for analysing content. The data also show that actual understanding varies considerably among participants, suggesting that more effort needs to be put into adjusting the information to the needs of different subgroups. It is argued that when doing this, researchers in longitudinal studies must be careful not to exhaust participants with excessive information that they do not want and that may cause them to drop out. The ethical relevance of obtaining repeated consents in longitudinal research is discussed. (shrink)

The set-theoretic and individual-theoretic analyses of collections make the same mistake: their ontologies are far removed from language. Our basic notion, the collector, has a clear linguistic counterpart: the definite article. Our theory furnishes a theory of definite descriptions as a special case. We use 3-valued valued logic in order to come to grips with the existential presuppositions of the collections.

The knowledge of an object under observation will have no concrete meaning unless we can work out a logical model the similarities of which are known in relation to the analysed structures. The logical model, when dealing with complex structures, is drawn out by similarity starting from an analogical model worked out in relating local dimensions in the object to physical dimensions of a different nature which restrict the number of variables and can be arranged and modified as required.Any operating (...) similarity and behaviour comparisons come actually down to an apparent functional analogy. The difference between the thin structure of the natural and artificial systems makes it illusive to hope that a strict isomorphism between these two fields may thus be sorted out. However, the fact of achieving analogical models widens our knowledge and shows new possibilities to the human creative mind. (shrink)

Over the last few decades there has been a substantially higher percentage of successful organ transplants but also a significant imbalance between the demand for and the supply of organs, creating the basis for a highly profitable black market trade in human organs. Sometimes there are reports that children have been kidnapped, only to reappear later lacking one kidney, or that they simply disappear and are subsequently killed to have all their transplantable organs removed for profit. The European Union feels (...) that there is a need for action and that it has a duty to act in this field, especially for ethical reasons. There is now established close co-operation between the various European transplant organizations. The legal protection of children with regard to organ transplantation is not specifically mentioned in the existing conventions because this issue was not foreseen at the time of their preparation. However, the issue is covered in a broader sense by more general provisions. There are endless rumours surrounding this area. Members of various organizations who travel in the suspected countries say that the trafficking in children who are sold for transplantation is well known, but it is too difficult and very dangerous to catch the people involved. We have to conclude that it has been impossible to prove or disprove the rumours, but they are consistent and we all, especially in the health care professions, have an obligation to be keenly aware of how and where organs are obtained. Penrose proved that a computational or formalizable theory of the brainís cognitive functioning is impossible, but suggested that a physical non-computational and non-formalizable one may be viable. Arguments as to why Penroseís program is unrealizable are presented. The main argument is that a non-formalizable theory should be verbal. However, verbal paradoxes based on Cantorís diagonal processes show the impossibility of a consistent verbal theory of the brain comprising its arithmetical cognition. It is suggested that comprehensive theories of the human brain (...) and of physical experience are Kantian rather than Platonic ideas. This suggestion is likewise based on arguments related to diagonal processes. (shrink)

The author discusses a number of perspectives concerning the politicization of public administration. Firstly, he argues that it be hardlyadequate to confine the concept of politicization to the increasing penetration of political par ties into the sphere of public administration. The politicization of public administration refers to more intensive patternsof bureaucratie polities as well. Secondly, he warns against a too easy acceptance of the idea that a politicization of public administration has taken place. One might say that what is aften (...) considered as a growing penetration of political factors into the administrative sphere actually pertains to the increasing awareness and visibility regarding the frequent and intensive relations between polities and administration.
