In medical practice, physicians are often faced with tough ethical and moral dilemmas, one such example is the reoccurring conflict between a patient’s hope and the truth. This paper explores two ethical dilemmas centered on compassion and the reduction of suffering: truth-telling with terminal patients and the clinical use of placebos. In each case the disclosure of truthful information could interfere with hope and suffering relief.

Medicine has made great strides with advances in technology and outcomes. However, compassion is an element that often is missing from medical care and ethics. The paper discusses why compassion is the ideal physician and why it is important to medicine. The benefit of compassion in biomedical ethics by exploring three ethical frameworks is also explored. Compassion is an important concept that has a place in both medical care and ethical practice.

According to material plenitude, every material object coincides with an abundance of other material objects that differ in the properties they have essentially and accidentally. Although this kind of plenitude is becoming increasingly popular, it isn't clear how to make sense of the view beyond its slogan form. As I argue, it turns out to be extraordinarily difficult to do so: straightforward attempts are either inconsistent or fail to capture the target idea. Making progress requires us to engage in more (...) delicate metaphysics than we might have expected and, along the way, reveals substantive constraints on the material world. In this article, I argue that any attempt to develop a coherent version of plenitude is subject to two under-appreciated challenges, and I develop a version of plenitude (global plenitude) capable of overcoming both. Material Plenitude is the view that there is an abundance of coincident objects wherever there is any material object. Although plenitude has garnered increased attention from metaphysicians in recent years, it has yet to be well‐understood beyond its slogan from. The goal of this article is to explore a few places for puzzlement about plenitude; in particular, how we ought to motivate and formulate the target view. In his recent book, Daniel Korman contrasts ontological conservatives with permissivists and eliminativists about ontology. Roughly speaking, conservatives admit the existence of ‘ordinary objects' like trees, dogs, and snowballs, but deny the existence of ‘extraordinary objects', like composites of trees and dogs. Eliminativists, on the other hand, deny many or all ordinary objects, while permissivists accept both ordinary and extraordinary objects. Our aim in this paper is to outline some of our reasons for being drawn to permissivism, as well as (...) some of our misgivings about conservative metaphysics. In the first section, we discuss a tempting epistemic line of argument against conservatism. This isn’t a line of argument we find especially promising. Our most basic complaint against conservatism is not that conservatism has poor epistemic standing even if true, but instead that conservatism is weird. We develop this thought in the second part of the paper. In the final section we discuss some larger methodological issues about the project of ontology. (shrink)

Radically permissive ontologies like mereological universalism and material plenitude are typically motivated by concerns about arbitrariness or anthropocentrism: it would be objectionably arbitrary, the thought goes, to countenance only those objects that we ordinarily take there to be. Despite the prevalence of this idea, it isn't at all clear what it is for a theory to be “objectionably arbitrary,” or what follows from a commitment to avoiding arbitrariness in metaphysics. This paper aims to clarify both questions, and examines whether arguments (...) from arbitrariness really are the proper foundations for one or both varieties of ontological permissivism. I argue that these considerations (even when made more precise) are far less successful at motivating radical forms of permissivism than we often take them to be. To do better, permissivists must either supply a much more developed metaphysics of material objects, or a controversial (but tempting) conception of what we're doing when we do metaphysics. In the face of the puzzles of material constitution, some philosophers have been moved to posit a distinction between an object's matter and its form. A familiar difficulty for contemporary hylomorphism is to say which properties are eligible as forms: for example, it seems that it would be intolerably arbitrary to say that being statue shaped is embodied by some material object, but that other complex shape properties aren't. Anti-arbitrariness concerns lead quickly to a plenitudinous ontology. The usual complaint is (...) that the super-abundance of material objects is too extraordinary to accept, but I want to raise a different worry: I argue that the most natural way of developing this picture is already inconsistent. I show that a simple version of plenitudinous hylomorphism is subject to a Russellian argument, but argue that we cannot treat the problem straightforwardly as an instance of Russell's Paradox of Sets. Fairchild (2019) advertised the humility of material plenitude, arguing that despite the profligate ontology of coincident objects it entails, the best version of plenitude is one that takes no stand on a range of nearby questions about modality and coincidence. Roughly, the thought is that plenitude says only that there are coincident objects corresponding to every consistent pattern of essential and accidental properties. Plenitude says (or should say) nothing about which patterns those might be, and so should be compatible (...) with any reasonable hypothesis about which combinations of properties it is possible for something to have. I argued in the earlier paper that a particular formulation of the target view (Global Plenitude) has exactly that virtue. But, unfortunately, Global Plenitude turns out not to be very humble at all. Global Plenitude is incompatible with an exceptionally compelling hypothesis about coincidence: that there are some things which coincide, but might not have. Scandal ensues. (shrink)

In a contribution to a symposium on xenophilia, this essay — a study of Brian Friel’s 1980 play Translations — raises the question of whether all xenophilia is by nature doomed to fail. Set in Ireland in 1833, the drama centers on the tension arising from a young British lieutenant’s falling in love with an Irish-speaker while he is in her country to translate Irish place-names into English for an imperial cartographic survey. While the lieutenant is referred to in the (...) play as a Hibernophile, the essay interprets his love as xenophilic: love for the foreignness rather than the Irishness of what he encounters. The lieutenant’s love of foreign places and their names impedes his effort to systematize Ireland for imperial ends, and his love for an Irish woman brings about his own undoing. Applying Simone de Beauvoir’s view of alterity to the lieutenant’s xenophilia, the essay questions whether the English written over the Irish in this play and the lieutenant’s desire written over the objects of his love obscure enough of the other’s otherness to render his xenophilia no longer viable. ABSTRACT As bioethics emerged in the 1960s and 1970s and began to have enormous impacts on the practice of medicine and research – fuelled, by broad socio‐political changes that gave rise to the struggle of women, African Americans, gay men and lesbians, and the antiauthoritarian impulse that characterised the New Left in democratic capitalist societies – little attention was given to the question of the ethics of public health. This was all the more striking since the core values and practices (...) of public health, often entailing the subordination of the individual for the common good, seemed opposed to the ideological impulses of bioethics. Of what relevance is autonomy‐focused bioethics for public health, with its mix of justifications including those that are either implicitly or explicitly paternalistic or that seek to impose strictures on individuals and communities in the name of collective welfare? To examine the deep divide between the central commitments of bioethics and the values that animate the practice of public health, we focus on a series of controversies implicating the concepts of privacy, liberty, and paternalism. Recognising the role of moral values in decision‐making was a signal contribution of bioethics in its formative period. Over the past three decades a broad array of perspectives emerged under the rubric of bioethics but individualism remains central. As we commence the process of shaping an ethics of public health, it is clear that bioethics is the wrong place to start when thinking about the balances required in defence of the public's health. In sentences such as “Some dogs are mammals,” the literal semantic meaning (“Some and possibly all dogs are mammals”) conflicts with the pragmatic meaning (“Not all dogs are mammals,” known as a scalar implicature). Prior work has shown that adults vary widely in the extent to which they adopt the semantic or pragmatic meaning of such utterances, yet the underlying reason for this variation is unknown. Drawing on theoretical models of scalar implicature derivation, we explore the hypothesis that the cognitive (...) abilities of executive function (EF) and theory of mind (ToM) contribute to this observed variation. In Experiment 1, we show that individuals with better ToM are more likely to compute a scalar implicature and adopt the pragmatic meaning of an utterance; however, EF makes no unique contribution to scalar implicature comprehension after accounting for ToM. In Experiment 2, we replicate this finding and assess whether it generalizes to the comprehension of other pragmatic phenomena such as indirect requests (e.g., “It's hot in here” uttered to ask for something to be done) and metaphor (e.g., “to harvest courage”). This is the first evidence that differences in ToM are associated with pragmatic competence in neurotypical adults across distinct pragmatic phenomena. (shrink)

It permits us to recognize disease outbreaks, to track the incidence and prevalence of threats to public health, and to monitor the effectiveness of our interventions. But surveillance also challenges our understandings of the significance and role of privacy in a liberal democracy. In this paper we trace (...) the century-long history of public health surveillance in the United States situating that history in the context of the broad social, political, and ideological forces that have shaped our conceptions of privacy. Although we focus here on the United States, the debates over privacy that unfolded in the 1960s were repeated in many European nations. Prior research has investigated how narcissistic executives affect firm policies and outcomes and how these executives influence colleagues and followers. However, almost no research exists concerning the impact of narcissistic executives on external agents. We examine the case of credit ratings—where analysts are required to assess management competence and where undue management influence is a concern—to determine whether narcissistic CEOs exert an effect on their firm’s rating. Using the size of the CEO’s personal signature to measure narcissism, we find that (...) CEO narcissism is associated with worse credit ratings. This effect is attenuated when firms face greater financial constraints, higher industry competition, and more concentrated institutional ownership. Our study contributes to the growing body of literature on CEO narcissism and suggests that these manipulative individuals are unable to exert an obvious upward influence on credit rating agencies. In the context of health care system complexity, nurses need responsive leadership and organizational support to maintain intrinsic motivation, moral sensitivity and a caring stance in the delivery of patient care. The current complexity of nurses’ work environment promotes decreases in work motivation and moral satisfaction, thus creating motivational and ethical dissonance in practice. These and other work-related factors increase emotional stress and burnout for nurses, prompting both new and seasoned nurse professionals to leave their current position, or even the (...) profession. This article presents a theoretical conceptual model for professional nurses to review and make sense of the ethical reasoning skills needed to maintain a caring stance in relation to the competing values that must coexist among nurses, health care administrators, patients and families in the context of the complex health care work environments in which nurses are expected to practice. A model, Nurses’ Ethical Reasoning Skills, is presented as a framework for nurses’ thinking through and problem solving ethical issues in clinical practice in the context of complexity in health care. This paper outlines a defense of hybrid contingentism: that it is contingent which individuals there are, but not contingent what properties there are. Critics pursue two main lines of complaint. First, that the hybrid contingentist’s treatment of haecceitistic properties is metaphysically mysterious, and second, that hybrid contingentism involves an unjustified asymmetry in the associated modal logic. I suggest that these complaints may be too quick, at least in the setting of higher-order metaphysics. It is not at all obvious whether and (...) to what extent we should expect particular "symmetries" across the orders, and so whether (as Williamson (2013) argues) “the default preference is for a uniform metaphysics, on which being is contingent at all orders or none.”. What motivates manufacturing companies to make costly investments in producing in an environmentally clean manner? The traditional argument is that such behaviour is value reducing, and that therefore, firms must be forced by regulation to invest in "green" production processes. A counter-argument is that firms have an incentive to make environmental investments in an attempt to attract "green" consumers and investors, hence gaining competitive advantage over their rivals. In this paper, we employ a game-theoretic approach that demonstrates that competing firms' (...) incentives to make voluntary investments in environmental "clean-up" are affected by the size of the investment costs and the extent of consumer and investor "green" awareness. We argue that an increase in green behaviour can be induced by a combination of governmental subsidies for firms that invest in environmentally clean production processes, together with an education program that promotes "green" awareness amongst consumers, investors and the managers themselves. As therapeutic advances in the treatment of AIDS began to emerge in the late 1980s and public health began to have more to offer than just the threat, or the perceived threat, of quarantine or partner notification, fissures began to appear in the alliance against named HIV reporting that had emerged a few years earlier. In 1989, New York City’s Health Commissioner stated that the prospects of early clinical intervention warranted “a shift toward a disease-control approach to HIV infection along (...) the lines of classic tuberculosis practices,” including the “reporting of seropositives.”Although his proposal met with fierce and effective resistance, it is now clear that his call represented part of a national trend. The CDC continued to press for cases of HIV to he reported by name to health departments, an effort that assumed the dimensions of a campaign. ArgumentIn this paper I examine the mass medical inspections of immigrants to the United States from the 1890s through the 1920s. I show how, framed as it was not only by nativism and eugenics but also by national industrial imperatives and priorities, scientific medicine served dual purposes. On the one hand, the medical exam was a tool for managing cultural and biological threats to the nation. There were regional variations in medical inspections that reflected the politics of race. On the (...) other hand, the medical exam played an important role in the process of building an unskilled, highly mobile labor force. The industrial demands of the nation provided a rationale for drawing and absorbing millions of European immigrants into the labor force. It was thus a distinct product of the political economy of immigration. It was this second function that characterized the exam for the majority of immigrants entering the nation. (shrink)

Historically, land tenure theory tends to present the relationships between agricultural landowners and their renter as either a dominant renter-subordinate landlord relationship where the renter holds the power in decision-making on the land, or a dominant landlord-subordinate renter relationship where the landlord maintains the power over decisions on the land. However, these relationships are much more complex and nuanced, as more recent studies have begun to emphasize. In our study, we contribute to this evolving re-orientation in land tenure theory by (...) showing the varying ways women landowners manage their renter relationship. Using qualitative interview data from 56 women agricultural landowners in the Midwestern U.S., we add detail to the nuances that exist in the landowner-renter relationship, helping to re-orient land tenure theory by increasing the understanding of the power dynamics at play within the patriarchal structure of U.S. agriculture.
