Objective We sought to understand ethics and education needs of emergency nurses and physicians in paediatric and adult emergency departments in order to build ethics capacity and provide a foundation for the development of an ethics education programme. Methods This was a prospective cross-sectional survey of all staff nurses and physicians in three tertiary care EDs. The survey tool, called Clinical Ethics Needs Assessment Survey, was pilot tested on a similar target audience for question content and clarity. Results Of the (...) 123 participants surveyed, 72% and 84% of nurses and physicians fully/somewhat agreed with an overall positive ethical climate, respectively. 69% of participants reported encountering daily or weekly ethical challenges. Participants expressed the greatest need for additional support to address moral distress, conflict management with patients or families and resource issues. Of the 23 reported occurrences of moral distress, 61% were associated with paediatric mental health cases. When asked how the ethics consultation service could be used in the ED, providing education to teams was the most desired method. Conclusions Nurses report a greater need for ethics education and resources compared with their physician colleagues. Ethical challenges in paediatric EDs are more prevalent than adult EDs and nurses voice specific moral distress that are different than adult EDs. These results highlight the need for a suitable educational strategy, which can be developed in collaboration with the leadership of each ED and team of hospital ethicists. (shrink)

A consideration of Confucian ethics as a living ethical tradition with contemporary relevance.

Determining the compositional properties of surfaces in the environment is an important visual capacity. One such property is specular reflectance, which encompasses the range from matte to shiny surfaces. Visual estimation of specular reflectance can be informed by characteristic motion profiles; a surface with a specular reflectance that is difficult to determine while static can be confidently disambiguated when set in motion. Here, we used fMRI to trace the sensitivity of human visual cortex to such motion cues, both with and (...) without photometric cues to specular reflectance. Participants viewed rotating blob-like objects that were rendered as images (photometric) or dots (kinematic) with either matte-consistent or shiny-consistent specular reflectance profiles. We were unable to identify any areas in low and mid-level human visual cortex that responded preferentially to surface specular reflectance from motion. However, univariate and multivariate analyses identified several visual areas; V1, V2, V3, V3A/B, and hMT+, capable of differentiating shiny from matte surface flows. These results indicate that the machinery for extracting kinematic cues is present in human visual cortex, but the areas involved in integrating such information with the photometric cues necessary for surface specular reflectance remain unclear. A Markov model of categorization followed by decision making provides reasonable fits to Part I data. A Markov (...) model predicting decision making followed by categorization is rejected. In Part II, a no-parameter model predicts decisions using categorization and decision responses collected in separate trials, suggesting that Part 1 results are not an artifact of the presentation of categorization and decision questions within a single trial. Fodor’s Informational Semantics states that the content of a representation depends on the counterfactual relation between the representation and the represented. However, his theory suffers from the psychological explanation problem and the indeterminacy problem raised by twin cases. In response to these problems, Fodor has introduced narrow content and a mixed theory of content that combines a historical account with the counterfactual account. In The Elm and the Expert, he drops both of them for the reason that twin cases are (...) nomologically impossible. I argue that Fodor underestimates the persistence of the problems raised by twin cases. Consequently, I contend that Fodor has to keep both the narrow content and the historical account. (shrink)

The divine attributes of omniscience and omnipotence have faced objections to their very consistency. Such objections rely on reasoning parallel to semantic paradoxes such as the Liar or to set-theoretic paradoxes like Russell's paradox. With the advent of paraconsistent logics, dialetheism—the view that some contradictions are true—became a major player in the search for a solution to such paradoxes. Those who accept the necessity of mereological universalism face what has come to be known as the ‘junk argument’ due to Bohn [2009], which proceeds from the incompatibility of junk with universalism and the possibility of junk, to conclude that mereological universalism isn't metaphysically necessary. Most attention has focused on ; however, recent authors have cast doubt on . This paper undertakes a defence of premise against three main objections. The first is a new objection to the effect that Bohn's (...) defence of that premise presupposes far too much. I show that one can defend premise from a much weaker set of assumptions. The second objection, due to Contessa [2012], is that those who accept unrestricted composition should only accept the existence of binary sums rather than infinitary fusions. I argue that this conception of unrestricted composition is problematic: it is in conflict with an intuitive remainder principle. The final objection is .. (shrink)

Mereological principles are often controversial; perhaps the most stark contrast is between those who claim that Weak Supplementation is analytic—constitutive of our notion of proper parthood—and those who argue that the principle is simply false, and subject to many counterexamples. The aim of this paper is to diagnose the source of this dispute. I’ll suggest that the dispute has arisen by participants failing to be sensitive to two different conceptions of proper parthood: the outstripping conception and the non-identity conception. I’ll (...) argue that the outstripping conception (together with a specific set of definitions for other mereological notions), can deliver the analyticity of Weak Supplementation on at least one sense of ‘analyticity’. What is the proper role of logic in analytic theology? This question is thrown into sharp relief when a basic logical principle is questioned, as in Beall’s ‘Christ – A Contradiction.’ Analytic philosophers of logic have debated between exceptionalism and anti-exceptionalism, with the tide shifting towards anti-exceptionalism in recent years. By contrast, analytic theologians have largely been exceptionalists. The aim of this paper is to argue for an anti-exceptionalist view, specifically treating logic as a modelling tool. IntroductionThe purpose of this essay is to work constructively with Michael S. Hogue's groundbreaking American Immanence: Democracy for an Uncertain World to demonstrate how psychodynamic analyses of religion are essential theoretical allies in the fight for resilient democracy. The "revolution in mind"1 that psychodynamic approaches contribute, especially in their analyses of religion, (...) both complements and complicates socio-political theory as a whole. In the improvisational spirit of "yes-and," the goal here is not to identify and decry the failures and shortcomings of Hogue's theopolitical vision but rather to inspire the recovery of a sibling discourse—a kind of "rebirth of a forgotten alternative"2—that also draws deeply from the American immanental tradition in order to add psychodynamic depth to Hogue's timely and important analysis. Psychodynamic theories of religion shed light on how symbolic identifications, including and especially ultimate concerns, structure personal and collective experience. By engaging religion and theology sympathetically, constructively, and creatively, theorists such as Rollo May, Gordon Allport, Karen Horney, Erich Fromm, and Ernest Becker departed from the skepticism, reductionism, and dismissal that characterized psychoanalytic analyses of religion in the wake of Freud.3 Each of these figures, and especially Becker whose work will be a focus in this essay, are noteworthy interlocutors for extending Hogue's theopolitical theorizing of resilient democracy, particularly in how they drew unique inspiration from the American immanental tradition [End Page 44] in their social analyses. In this respect, they provide a more robust theorization of the psychosocial conditions of the associational ethos needed to connect Hogue's conception of resilient democracy with forms of agonistic organization and democratic activism. Briefly stated, psychodynamic approaches are a helpful bridge for connecting these proposals to strategies of organization, their impediments, and collective action.In this way, both psychodynamic analyses of religion and Hogue's theopolitical innovation insist that no revolutionary political change is possible, and certainly not of the kind or scale needed to counter our wickedly entangled global crises, without a deep engagement with theological imaginaries. They differ, however, in their theoretical aims. Whereas Hogue compellingly argues for the political upshot of a theology deriving from the American immanental tradition, psychodynamic analysts of religion focus on the function of ultimate concerns in individual and collective life. In other words, American Immanence rests on a methodology of analyzing the philosophical and theological assumptions that have contributed to our climate crises and then presenting clear and compelling articulation of an alternative paradigm meant to open up new ways of being. In contrast psychodynamic analyses leverage conceptual clarity of psychic reality for the fundamentally therapeutic aim of transformation. At their best, both approaches share what could be called an etiological orientation to ontological questions in order to provide liberative alternatives through rational understanding and clarification, but the terrain, and thus the problems they hope to mitigate in their analyses are focused differently. Extending the visual metaphor, they are different lenses highlighting different areas of the same landscape. Furthermore, it must be said that neither are sufficient in themselves, but the psychodynamic theorists of religion to be highlighted in this essay share deeply with Hogue's heading in American Immanence.II. Priest’s 2014 theory of unity and identity, based on a paraconsistent logic, has a wide range of applications. In this paper, I apply his theory to some puzzles concerning mereology and topology. These puzzles suggest that the classical mereotopology needs to be revised. I compare and contrast the Priest-inspired solution with another, based on classical logic, that requires the co-location of boundaries. I suggest that the co-location view should be preferred on abductive grounds.

Respecting and encouraging autonomy in the elderly is basic to the practice of geriatrics. In this paper, we examine the practice of cardiopulmonary resuscitation (CPR) and "artificial" feeding in a geriatric unit in a general hospital subscribing to jewish orthodox religious principles, in which the sanctity of life is a fundamental ethical guideline. The literature on the administration of food and water in terminal stages of illness, including dementia, still shows division of opinion on the morality of withdrawing nutrition. We (...) uphold the principle that as long as feeding by naso-gastric (N-G) or percutaneous endoscopic gastrostomy (PEG) does not constitute undue danger or arouse serious opposition it should be given, without causing suffering to the patient. This is part of basic care, and the doctor has no mandate to withdraw this. The question of CPR still shows much discrepancy regarding elderly patients' wishes, and doctors' opinions about its worthwhileness, although up to 10 percent survive. Our geriatric patients rarely discuss the subject, but it is openly ventilated with families who ask about it, who are then involved in the decision-making, and the decision about CPR or "do-not-resuscitate" (DNR) is based on clinical and prognostic considerations. This essay contains material which will hopefully be of interest not only to philosophers, but also to those social scientists whose research concerns the analysis of communication, verbal or non-verbal. Although most of the topics taken up here are central to issues in the philosophy of language, they are, in my opinion, indistinguishable from topics in descriptive social psychology. The essay aims to provide a conceptual framework within which various key aspects of communication can be described, and it presents a (...) formal language, using techniques from modern modal logic, in which such descriptions can themselves be formulated. It is my hope that this framework, or parts of it, might also turn out to be of value in future empirical work. There are, therefore, essentially two sides to this essay: the development of a framework of concepts, and the construction of a formal language rich enough to express the elements of which that framework is composed. The first of these two takes its point of departure in the statement quoted from Lewis (1972) on the page preceding this introduction. The distinction drawn there by Lewis is accepted as a working hypothesis, and in one sense this essay may be seen as an attempt to explore some of the consequences of that hypothesis. (shrink)

Morgan has argued that attitudes to the medicalisation of sports are historically conditioned.While the history of doping offers contested versions of when the sports world turned againstconservative forces, Morgan has argued that these attitudes are out of step with prevailingnorms and that the World Anti Doping Agency's policy needs to be modified to better reflectthis. As an advocate of critical democracies in sports, he argues that anti-doping policy mustacknowledge and reflect these shifts in order to secure their legitimacy. In response, (...) wecritically present the World Anti-Doping Agency's policy that incorporates the Prohibited Listof Substances and Methods for athletes. We evaluate the validity of the therapy-enhancementdistinction in relation to its role in both justifying and sustaining the operation of theProhibited List. In particular, we focus on the case of thyroxine, which has been the subjectof controversy in athletic doping. While thyroxine is not currently banned, critics haveclaimed that its use in the absence of a relevant pathology is tantamount to doping. Wechallenge Morgan's claim that a conventionalist defence of the therapy-enhancementdistinction is the best available, and his conclusion that this properly supports a morepermissive stance towards performance-enhancing drug use. Furthermore, we reject hisconventionalist support for democratic line drawing in relation to doping and in particular thestatus of thyroxine with regard to the prohibited list. We offer a modified defence of the statusquo, a qualified, naturalist account of health and disease, where athletes may be prescribeddrugs that are genuine responses to medical necessity that do not, or do not typically, threatenthe goods of athletic excellence.
