Dans notre société de plus en plus digitalisée, avons-nous vraiment le choix d’adopter ou non les technologies? Comment cette digitalisation impacte-t-elle les personnes âgées en particulier et son écosystème? Quels sont les enjeux éthiques soulevés par cette digitalisation? Ce texte vise à amener des éléments de réflexions en lien avec ces enjeux selon le point de vue de divers experts des domaines de la technologie, du vieillissement et de la bioéthique. Ces experts se sont rencontrés lors d’un symposium (...) ayant eu lieu à Angers, France, en octobre 2019. Le texte est un compte-rendu des échanges et points de vue de ces experts, ainsi que des discussions ouvertes qu’ils ont eues avec l’assistance, portant sur les principaux enjeux soulevés par cette digitalisation selon la perspective des personnes âgées, des proches-aidants, des soignants, de la société et de la recherche. L’accompagnement de fin de vie est devenu une pratique usuelle, conseillée, en France, dans les établissements d’hébergement pour personnes âgées dépendantes. Il fait l’objet de formation continue et est inscrit dans les recommandations des sociétés de soins palliatifs. Mais l’accompagnement de fin de vie est-il un concept ? Une pratique ? Une fonction ? Qui sont les acteurs de cet accompagnement ? Est-ce une attribution des professionnels de santé ou de tout un chacun dans la société ? Nous (...) tenterons, d’abord, de cerner le concept d’accompagnement dans le champ des pratiques sociales. Nous verrons qu’il s’agit bien d’une pratique qui s’appuie sur des fondements théoriques du souci de l’autre, de la bienveillance, de l’être-avec. Puis la description sociohistorique de l’accompagnement et du soin palliatif nous montrera l’idéal humain qu’il recouvre. Cependant les valeurs communes à l’un et l’autre tendent à la confusion entre accompagnement de fin de vie et soin palliatif. C’est pourquoi certains auteurs nous invitent à une réflexion éthique sur l’accompagnement. Ils nous proposent de préciser la fonction et la posture professionnelle qu’il requiert. Selon différentes conceptions du genre et des parcours de transition, toujours singulières mais empreintes d’attentes sociales, l’enveloppe corporelle du sujet transgenre se métamorphose. Lors d’entretiens de recherche et d’épreuves projectives (Rorschach et tat ), deux jeunes adultes transgenres aux profils psychopathologiques névrotiques présentaient une sensibilité marquée aux normes, à la morale et aux attentes sociétales. La légitimité, la culpabilité et la honte étaient des problématiques redondantes. Différents temps de transitions subjectives, familiales, sociales et corporelles se sont trouvés en jeu : (...) le passage à l’âge adulte, une vie étudiante hors du giron familial. Ces deux situations illustrent une discussion sur les transitions du corps et des enveloppes chez des sujets transgenres, entre seconde peau et moi-peau corsets. (shrink)

La vulnérabilité due à l’âge ou à la maladie conduit à mettre en place des dispositifs juridiques de protection. La famille et le juge et, dans une certaine mesure, le notaire, en sont les acteurs traditionnels. La protection du patrimoine est une des dimensions non négligeables de cette protection assurée prioritairement par la famille du protégé, conjoint et enfants qui sont aussi ses héritiers et ses débiteurs d’aliments. En réexaminant la place assignée à chacun – famille, juge, notaire, professionnel – (...) dans un nouveau texte réformant la loi plus que trentenaire qui régissait jusqu’alors la protection des majeurs, le législateur a semble-t-il pris acte de l’évolution de la famille et d’une nouvelle perception de son rôle par les différents acteurs de la protection. Comme toujours, l’argent est un puissant révélateur de la place assignée à chacun dans la famille. Les règles récemment modifiées qui régissent la protection, notamment patrimoniale, de la personne vulnérable et qui organisent sa représentation ne pouvaient donc être totalement neutres sur ce terrain. Etudier le probleme de la personne, c'est s'interesser a un probleme qui traverse a peu pres tous les domaines de la philosophie, depuis la psychologie jusqu'a la theologie en passant par l'ethique et par le droit, mais dont le denominateur commun est la definition du soi-meme. Etudier ce probleme chez Maimonide, c'est voir comment un philosophe juif du Moyen-Age pouvait elaborer une representation coherente de l'homme en s'appuyant a la fois sur la pensee greco-arabe et sur la tradition hebraique. Ce (...) livre espere contribuer a une meilleure comprehension d'un philosophe dont on a trop souvent fragmente la pensee, apparemment dechiree entre des sources contradictoires. En fait, Maimonide a su developper un dialogue critique entre deux heritages qui demeurent aux fondements de la civilisation occidentale. This article presents a multimodal video corpus with the principal aim to model and predict the effects of aging in Mild Cognitive Impairment situation on pragmatic and communicative skills. We take as observable variables the verbal pragmatic markers and non-verbal pragmatic markers. This approach, at the interface of the psycholinguistics, cognitive sciences and rehabilitation medicine (speech-language pathology and therapy) is part of a longitudinal research process in an ecological situation (interviews conducted by close intimate of the elderly).In the first part (...) of the article we present the linguistic, cognitive and social characteristics of aging in its continuum up to mild cognitive impairment and pathological disorders such as Alzheimer’s disease. In the second part, we develop a multimodal approach, in particular to inform and enrich speech and language therapy knowledge. Finally, we present our experimental design and preliminary results on two female participants over 75 years of age with mild cognitive impairment.Our general findings indicate that with aging, verbal pragmatic markers acquire an interactive function that allows people with Mild Cognitive Impairment to maintain intersubjective relationships with their interlocutor. In addition, at the non-verbal level, gestural manifestations are increasingly mobilized over time with a preference for non-verbal pragmatic markers with a referential function and an interactive function. One such non-verbal manifestation compensates for naming deficits, planning difficulties, discursive hitches; while another optimizes and maintains the interaction with the interlocutor.Clinicians have a duty to develop their professional practice through an evidence-based clinical approach whose main objective is to reconcile clinical practice with the best evidence from research (Dollaghan 2007). In the case of speech-language pathology, clinicians consider themselves very limited in this approach (Lof 2011; McCurtin 2011), especially for patients with Mild Cognitive Impairment (Mungas et al. 2010; Hopper 2013; Morello 2017) and more specifically when it comes to assessing or supporting language functions (Cummings 2014).The studies focusing on Mild Cognitive Impairment require longitudinal corpora i) to understand the naturally occurring evolutions in subjects, ii) the implication of the cognitive reserve in each individual, and iii) to take advantage of these parameters as evidence for research and earlier rehabilitation. Autant que la position de la femme dans la société, le principe de la matrilinéarité juive est révélateur des normes qui régissent le lien social. Le système de filiation matrilinéaire s’est installé dans le judaïsme sur la base de l’interprétation du dit talmudique affirmant Ton fils né d’une idolâtre n’est pas ton fils mais le sien (Qid. 68b). Il entérinait un processus enclenché quelques siècles plus tôt, à l’époque de la Mishna, autour du premier siècle de l’ère, mettant un terme (...) à la longue tradition biblique qui ne percevait de filiation que patrilinéaire, et instaurait, ce faisant, un modèle de transmission de la judéité qui perdure jusqu’à nos jours. Cette transformation majeure était-elle due à une évolution interne de la pensée rabbinique rationalisant la judéité au regard des affiliations? Ou à des contraintes extérieures liées à l’imposition de nouvelles réglementations du statut de la personne? Les deux hypothèses d’interprétations sont débattues – et parfois se rejoignent – dans les recherches menées sur la filiation ou l’identité juive. Cet article examine les conditions de la transmission de l’identité de la personne, telles qu’elles ont été traitées et définies au long des siècles par l’institution de normes juridiques et sociales régulant la filiation, la conversion et l’héritage, toutes normes qui éclairent les évolutions du statut de la femme dans l’histoire du judaïsme entre l’Antiquité tardive et le Moyen Âge. Cette étude envisage les notions de sujet, d’individu et de personne dans une perspective d’anthropologie historique. La période considérée va de la disparition de l’Empire romain d’Occident à la mutation féodale, et son analyse est centrée sur la convergence de la culture savante – c’est-à-dire chrétienne – avec ce que nous pouvons savoir de la culture populaire à propos des trois notions précitées. Inaugurée par saint Augustin dans ses Confessions, l’émergence de la notion de personne coïncide avec l’oblitération du sujet (...) dans le De Trinitate de l’évêque d’Hippone. Cette conception rejoint celle du corps grotesque qui traverse la culture populaire de cette époque. If, for example, both A and B need treatment and there (...) are only enough resources for one, the fundamental interests of these two people compete against each other. (2) In clinical medicine, the individual case might demand a different approach than that suggested by theory. Similarly, an ethicist who is confronted by a diversity of medical cases becomes aware of the fact that solutions drawn from ethical theory can be irrelevant in practice. (3) The moral philosopher has often focused on the individual and his or her universal commitments. The fact that ethical dilemmas can have deep roots in practical challenges and must be understood within a certain context has been "forgotten." (4) In clinically relevant ethics, it is important to find solutions that all involved parties find reasonable. This revitalizes central Aristotelian schools of thought on, for example, ethical judgment and situational understanding.Today, there is a common understanding that in both the education of doctors and in the implementation of medical knowledge and technology there needs to be ethical guidelines and control. At the same time, we are expected to handle similar cases equally. Consideration of the concrete, individual case must be balanced with the principle of similar treatment. Second, as a committee member, one must interpret and respect ethical principles that cannot be easily defined. Every principle must be interpreted and implemented within a specific and concrete context. Third, one must consider the law, but seen isolated, the implications of relevant rules can be difficult to determine. Fourth, the status of principles and rules are open for interpretation. The most important example of this is the Declaration of Helsinki, which was established through cooperation between the worlds' doctors unions. An example: Decisions about confidentiality can prevent data gathering that is crucial in ensuring that a research project is scientifically acceptable. Two important ethical considerations are then put up against one another. A certain kind of externalism—"the view from nowhere"—lies at the heart of evidence-based medicine (EBM). As a consequence, the individual case glides out of focus. However, to judge to what extent external knowledge is applicable to an individual case, the clinician has to rely on some sort of knowledge of the case at hand. The article focuses on the tension between the externalism of EBM and the "internal evidence" one has to presuppose when making clinical judgments.

Combining information from two or possibly several blocks of data is gaining increased attention and importance in several areas of science and industry. Typical examples can be found in chemistry, spectroscopy, metabolomics, genomics, systems biology and sensory science. Many methods and procedures have been proposed and used in practice. The area goes under different names: data integration, data fusion, multiblock analyses, multiset analyses and a few more. This book is an attempt to give an up-to-date treatment of the most used (...) and important methods within an important branch of the area; namely methods based on so-called components or latent variables. These methods have already obtained an enormous attention in for instance chemometrics, bioinformatics, machine learning and sensometrics and have proved to be important both for prediction and interpretation. The book is primarily a description of methodologies, but most of the methods will be illustrated by examples from the above-mentioned areas. The book is written such that both users of the methods as well as method developers will hopefully find sections of interest. In the end of the book there is a description of a software package developed particularly for the book. This package is freely available in R and covers many of the methods discussed. To distinguish the different type of methods from each other, the book is divided into five parts. Part I is introduction and preliminary concepts. Part II is the core of the book containing the main unsupervised and supervised methods. Part III deals with more complex structures and, finally, Part IV discusses alternative unsupervised and supervised methods. The book ends with Part V discussing the available software. Our recommendation for reading the book are as follows. A minimum read of the book would involve chapters 1, 2, 3, 5 and 7. Chapters 4, 6 and 8 are more specialized and chapters 9 and 10 contain methods we think are more advanced or less obvious to use. We feel privileged to have so many friendly colleagues who were willing to spend their time on helping us to improve the book by reading separate chapters. Change can be difficult to achieve, and system inertia may be considered relevant. There is a tendency for dynamic systems to enter into specific states characterized by stabilizing factors. The present work attempts to define inertia processes and explores these with regard to pro-environmental behaviour and decision-making. Inertia processes can be considered both within an organizational context and from the level of the individual, and may involve a number of psychological processes and aspects of the decision-making process. The main aim of this study is to test the validity of the Motivation, Engagement, and Thriving in User Experience model in higher education. We propose a process model in which we investigate how the need-satisfaction of digital learning tools within the interface sphere and task sphere accounts for engagement, learning, and well-being. A total of 426 higher education students drawn from two subsamples participated in this cross-sectional study. A structural equation model shows that interface autonomy and competence satisfaction positively (...) predict task autonomy and competence. Task competence, in turn, negatively predicts focused attention and positively predicts perceived usability and well-being. Task autonomy positively predicts perceived usability and reward. Based on our results, we provide some initial support for the METUX model in higher education. However, more validation work is needed to improve the scale that measures need-satisfaction in the interface and task spheres. Moreover, we find no support for the effect of task sphere on learning. Further investigations are needed into how METUX can be used in domain- and situation-specific contexts to account for increases in engagement, learning, and well-being. Finally, future studies need to include all aspects of the METUX model in order to fully test its validity. (shrink)

The mobility restrictions related to COVID-19 pandemic have resulted in the biggest disruption to individual mobilities in modern times. The crisis is clearly spatial in nature, and examining the geographical aspect is important in understanding the broad implications of the pandemic. The avalanche of mobile Big Data makes it possible to study the spatial effects of the crisis with spatiotemporal detail at the national and global scales. However, the current crisis also highlights serious limitations in the readiness to take the (...) advantage of mobile Big Data for social good, both within and beyond the interests of health sector. We propose two strategical pathways for the future use of mobile Big Data for societal impact assessment, addressing access to both raw mobile Big Data as well as aggregated data products. Both pathways require careful considerations of privacy issues, harmonized and transparent methodologies, and attention to the representativeness, reliability and continuity of data. The goal is to be better prepared to use mobile Big Data in future crises. (shrink)

Next SectionThe principle of respect for autonomy has shaped much of the bioethics' discourse over the last 50 years, and is now most commonly used in the meaning of respecting autonomous choice. This is probably related to the influential concept of informed consent, which originated in research ethics and was soon also applied to the field of clinical medicine. But while available choices in medical research are well defined, this is rarely the case in healthcare. Consideration of ordinary medical practice (...) reveals that the focus on patient choice does not properly grasp the moral aspects involved in healthcare. Medical decisions are often portrayed as if doctors and patients in confidence confront specific decisions about examinations or treatment, yet the reality often involves many different participants, with decisions being made over time and space. Indeed, most of the decisions are never even presented to patients, as it would be unethical to suggest something that is not medically justifiable. The options patients do confront are somewhat arbitrarily constructed within the narrow framework of both what is deemed to be medically appropriate and how the healthcare system is organised practically. While the autonomy discourse has proven valuable, a failure to distinguish between the fields of medical research and clinical medicine has generated a focus on patient choice that does not reflect what is really at stake in healthcare settings. This is alarming, because the current discourse misrepresents medical practice in a way that actually contributes to bioethical self-delusion. (shrink)

La sociedad atraviesa una serie de transformaciones en sus relaciones entre instituciones y comunidad; en este marco la universidad, desde su misión académica centrada en la formación e investigación para el desarrollo de conocimientos, está promoviendo una mayor implicación hacia las necesidades de la sociedad, a lo que se denomina responsabilidad social universitaria (RSU). Este artículo presenta cómo, desde una perspectiva humanista, se constituye una comunidad social de investigación entre la Unidad de Investigación Enfoque Centrado en la Persona de la (...) Universidad de Valencia –España-, el grupo Prisma de la Universidad Autónoma de Bucaramanga –Colombia- y la Fundación Albeiro Vargas (FAVAC) –Colombia- cuya finalidad es la atención al adulto mayor. Mediante una metodología de investigación-acción participativa, se genera una base de cooperación institucional para la promoción del bienestar psicológico del adulto mayor. El voluntariado, a través de sus docentes y sus alumnos, se concreta así en una acción de responsabilidad social universitaria cuya primera actividad es la valoración del bienestar psicológico del adulto mayor en la fundación Albeiro Vargas. Para la ejecución de este trabajo se entrevistaron a 122 adultos mayores de 60 años de la ciudad de Arica (52,5% hombres y 47,5% mujeres). A éstos se les administró un cuestionario de Satisfacción Vital SWLS (Diener, Emmons, Larsen y Griffin, 1985) junto con un cuestionario demográfico, para determinar cuáles de estas variables de contexto por si solas o en su conjunto, explican la Satisfacción Vital. El objetivo principal de este estudio es evidenciar cómo los indicadores de la Satisfacción Vital, (...) se ven influidas por la situación de pareja comparando las puntuaciones medias del cuestionario. BackgroundBurnout is a syndrome that results from stressors in the work environment that have not been successfully managed. The prevalence of burnout among schoolteachers was always controversial. COVID-19 pandemic added more stressors to teachers since they had to change their working styles in response to the pandemic lockdowns or curfews. In Saudi Arabia, the prevalence and determinants of burnout among school teachers were not measured by any other group during the COVID-19 pandemic stressors.MethodsA cross-sectional survey was conducted among 879 teachers (...) in the Jazan region, Saudi Arabia, using the Maslach Burnout Inventory, during April 2021. Multistage cluster random sampling was used.ResultsThe mean age of the participants was 41.4 years. Male teachers represented 52.6% of the participants while females represented 47.4%. Most teachers showed burnout symptoms. Consequences of burnout were observed, such as using psychotropic medications, absenteeism, lack of job satisfaction, and changing schools. Using the MBI scale, most teachers showed medium or high emotional exhaustion, low depersonalization, and low personal accomplishment.ConclusionMost teachers showed symptoms of burnout during the COVID-19 pandemic. Being an expert and ability to adapt to technology during the COVID-19 pandemic proved to effectively reduce burnout symptoms. Increasing incentives, early detection, and improving the work environment is recommended to diminish burnout consequences. (shrink)
