Background:The ethics and value bases in healthcare are widely acknowledged. There is a need to improve and raise awareness of ethics in complex systems and in line with competing needs, different stakeholders and patients’ rights. Evidence-based strategies and interventions for the development of procedures and practice have been used to improve care and services. However, it is not known whether and to what extent ethics can be developed using interventions.Objectives:To examine ethics interventions conducted on healthcare professionals and healthcare students to (...) achieve ethics-related outcomes.Research design:A systematic review.Methods:Five electronic databases were searched: CINAHL, the Cochrane Library, Philosopher’s Index, PubMed and PsycINFO. We searched for published articles written in English without a time limit using the keywords: ethic* OR moral* AND intervention OR program OR pre-post OR quasi-experimental OR rct OR experimental AND nurse OR nursing OR health care. In the four-phased retrieval process, 23 full texts out of 4675 citations were included in the review. Data were analysed using conventional content analysis.Ethical consideration:This systematic review was conducted following good scientific practice in every phase.Findings:It is possible to affect the ethics of healthcare practices through professionals and students. All the interventions were educational in type. Many of the interventions were related to the ethical or moral sensitivity of the professionals, such as moral courage and empowerment. A few of the interventions focused on identifying ethical problems or research ethics.Conclusion:Patient-related outcomes followed by organisational outcomes can be improved by ethics interventions targeting professionals. Such outcomes are promising in developing ethical safety for healthcare patients and professionals. (shrink)

The epistemological problems of unification of two distinct theories are discussed. An approach related to the work of Soviet authors (Stepin, Podgoretzky and Smorodinsky) is used and developed. The notion of ‘crossbred objects’—theoretical objects with contradictory properties which are part of the domain of application of two independent theories—is introduced which helps to describe the dynamics of revolutionary theory change. The occurrence of the cross-contradiction of two theories is reconstructed and the reductionistic and the synthetic means of its elimination are (...) proposed. Moral injury emerged in the healthcare discussion quite recently because of the difficulties and challenges healthcare workers and healthcare systems face in the context of the COVID-19 pandemic. Moral injury involves a deep emotional wound and is unique to those who bear witness to intense human suffering and cruelty. This article aims to synthesise the very limited evidence from empirical studies on moral injury and to discuss a better understanding of the concept of moral injury, its importance in the healthcare (...) context and its relation to the well-known concept of moral distress. A scoping literature review design was used to support the discussion. Systematic literature searches conducted in April 2020 in two electronic databases, PubMed/Medline and PsychInfo, produced 2044 hits but only a handful of empirical papers, from which seven well-focused articles were identified. The concept of moral injury was considered under other concepts as well such as stress of conscience, regrets for ethical situation, moral distress and ethical suffering, guilt without fault, and existential suffering with inflicting pain. Nurses had witnessed these difficult ethical situations when faced with unnecessary patient suffering and a feeling of not doing enough. Some cases of moral distress may turn into moral residue and end in moral injury with time, and in certain circumstances and contexts. The association between these concepts needs further investigation and confirmation through empirical studies; in particular, where to draw the line as to when moral distress turns into moral injury, leading to severe consequences. Given the very limited research on moral injury, discussion of moral injury in the context of the duty to care, for example, in this pandemic settings and similar situations warrants some consideration. This book provides the first detailed account of Gramsci's work in the context of current critical and socio-cultural debates. Renate Holub argues that Gramsci was ahead of his time in offering a theory of art, politics and cultural production. Gramsci's achievement is discussed particularly in relation to the Frankfurt School (Adorno, Horkheimer, Benjamin, Bloch, Habermas), to Brecht's theoretical writings and to thinkers in the phenomenological tradition especially Merleau-Ponty. She argues for Gramsci's continuing relevance at a time of retreat from (...) Marxist positions on the postmodern left. The article begins by addressing the main conceptual issues. Use of this term is the hallmark of an approach that is critical of the explanatory deficits of correlational analysis and of the covering-law model, advocating instead the causal reconstruction of the processes that account for given macro-phenomena. The term "social mechanisms" should be used to refer to recurrent processes generating (...) a specific kind of outcome. Explanation of social macro-phenomena by mechanisms typically involves causal regression to lower-level elements, as stipulated by methodological individualism. While there exist a good many mechanism models to explain emergent effects of collective behavior, we lack a similarly systematic treatment of generative mechanisms in which institutions and specific kinds of structural configurations play the decisive role. This paper discusses ethics in the context of Aboriginal Studies. Taking the example of a late-nineteenth century missionary work, a collection of out-of-print Mi’kmaq stories, it examines the ethical implications of the potential re-publication of such a text. It is argued that the Baptist missionary Silas T. Rand, who translated and transcribed the narratives, did his work from a Eurocentric perspective. The biases of a colonial ideology built into his translations/interpretations which are often quoted as authoritative would be further perpetuated (...) if his work is republished without critical commentary. As Aboriginal oral traditions generally form the basis of Aboriginal cultures and contemporary Aboriginal literatures but have been demeaned for centuries thereby further colonizing the peoples, an edition of Rand’s work informed by a postcolonial ethics is crucial. The paper therefore concludes with the suggestion that the stories collected by the missionary should be repatriated in the respective Mi’kmaq communities in a way that the people can decide how to go about their publication (if they want them published at all). A non-Aboriginal scholar may facilitate the process but should follow the directions of the communities. (shrink)

Linking arbitrary shapes to personal labels or reward values results in immediate processing benefits for those stimuli that happen to be associated with the self or high rewards in perceptual matching tasks. Here we further explored how social and reward associations interact with multisensory stimuli by pairing labels and objects with tones. We also investigated whether self and reward biases persist for multisensory stimuli with the label removed after an association had been made. Both high reward stimuli and those associated (...) with the self, resulted in faster responses and improved discriminability, which persisted for multisensory stimuli even when the labels were removed. However, these self- and reward-biases partly depended on the specific alignment between the physical tones and the conceptual order. Performance for reward associations improved when the endpoints of low or high rewards were paired with low or high tones; meanwhile, for personal associations, there was a benefit when the self was paired with either low or high tones, but there was no effect when the stranger was associated with either endpoint. These results indicate that, unlike reward, social personal associations are not represented along a continuum with two marked endpoints but rather with a single reference point. (shrink)

Interesting observations and problems emerge as the author pursues a complete calculus of an adverbial logic. Author Bartsch establishes a total of 18 adverbial subcategories and proposes an equal number of logico-semantic basic constructions in a predicate logical notation to explain them. The logico-semantic basic constructions bring out certain aspects of adverbial semantics, insofar as they can be specified by predicate logical means. However, for a logical and linguistic analysis of adverbials to be complete, sentence semantic analysis - provided it (...) is carried out in a clear-cut manner with clear paraphrasing criteria, unambiguous notation and complete interpretation rules - must be supplemented by and combined with 'word semantics' and syntax in a more elaborate way. The topic of adverbial semantics is a complicated and rewarding field, and this book contains enough suggestions, challenging hypotheses and stimulating observations, so that anybody directly concerned with the study of adverbials will want to have a look. Renate Holub provides a critical introduction to the philosophical foundations of human rights as developed by the Italian philosopher Giambattista Vico [1668-1744]. She demonstrate the profoundly innovative principles he contributed to and his contemporary relevance for a global theory of justice. Leading twentieth century transatlantic intellectuals, like Joseph Schumpeter, Arthur Nussbaum, Robert Cox who, though for quite different ultimate purposes, were variably working at the intersections between sociology, economic analysis, and international legal thought, squarely recognized the gravitas of the (...) unprecedented conceptual reach of the principles governing Vico’s social science. What is central to Vico’s international philosophy is an understanding of civilizational history not marked by permanent violence between individuals or groups of people. Rather, it is an understanding which outlines a broad historical tendency towards the reduction of violence in the internal and external organizations of social relations of peoples in all global regions. Vico’s political thought developed significant elements for a global theory of justice based not on legitimations of violence, as ‘just war’ theories, but for structuring, however minimally, the conditions of ‘permanent just global peace.’. Klein, Renate The practice of surrogacy in Australia has been controversial since its beginning in the late 1980s. In 1988, the famous 'Kirkman case' in the state of Victoria put surrogacy on the national map. This was a two-sisters surrogacy - Linda and Maggie Kirkman and the resulting baby Alice - in which power differences between the two women were extraordinarily stark: Maggie was the glamorous and well spoken woman of the world; Linda who carried the baby, was the (...) demure school teacher in child-like frocks and pig tails. Their IVF doctor applauded altruistic surrogacy. He called it 'gestational surrogacy' and proclaimed that if the so-called surrogate mother didn't use her own eggs, thus wasn't the baby's 'genetic' mother, no attachment would ensue! This statement is haunting us to this day. It is patently absurd: as a baby grows in a woman's body over the nine months of the pregnancy, it is hard to see why the 24/7 presence of the baby inside her body, its growth, its interaction with her would be any different whether s/he has the mother's genes! Es geht vielmehr um Konzeptionen des moralisch-individuellen Gewissens und der darin konturierten Beziehung zum politischen System. Zentrales Thema ist mithin die Frage nach dem Verhältnis von individueller Freiheit und politischer Ordnung. Dabei wird das semantische Feld im Kontext der Termini Gewissen, Moral und Ethik neu formuliert. Mittels einer Analyse der Gewissensdiskurse in der Frühmoderne sowie in der Spätmoderne läßt sich (...) die These entfalten, daß die Erkenntnis der neuartigen normativen Prob-lemstrukturen im technischen Zeitalter eine politologische Aufklärung erfordern, welche die zu-nehmend erkennbar werdenden Grenzen des Wissens in die Theoriebildung einbezieht. Die Poli-tik- und Sozialwissenschaften befinden sich gegenwärtig in einer Schwellenphase, in der die herkömmlichen normativen Erklärungsmuster, die in der Tradition des ethischen Humanismus verwurzelt sind, zu erodieren beginnen - doch werden die weitreichenden Konsequenzen des gegenwärtig sich vollziehenden sozio-technischen Umbruchs für die politische Ethik allenfalls halbherzig gezogen. Precision medicine and digital phenotyping are two prominent data-based approaches within digital medicine. While precision medicine historically used primarily genetic data to find targeted treatment options, digital phenotyping relies on the usage of big data deriving from digital devices such as smartphones, wearables and other connected devices. This paper first focusses on the aspect of data type to explore differences and similarities between precision medicine and digital phenotyping. It outlines different ways of data collection and production and the consequences thereof. (...) Second, it shows how these sorts of data influence dominant beliefs in the field: The field of precision medicine relying on the dominant understanding of ‘genetic determinism’ imported from genetics, digital phenotyping building on the logic of ‘data fundamentalism’. In the end, the analysis shows how digital data informs potentials as well as challenges of precision medicine and digital phenotyping: a better health care for individuals connected with individualisation and responsibilisation for all, with a prognosed shift from reactive to preventive medicine. Additionally, data-based approaches might facilitate epistemological and ontological redirections for the whole field of medicine that will also affect knowledge production and a reassessment of the value of different types of knowledge with all its consequences. Institutionally, it might lead to shifts in distribution of power to experts in big data related technologies, i.e. private companies. (shrink)

Patients' wishes regarding health care and dying must be taken into consideration by their physicians. Competent patients need to record directives about their care in advance of a crisis situation. The primary care physician, seeing the patient at the time of a routine office visit, is in a favorable position to explore and record attitudes. A patient's value system should be part of a medical history before hospital admission. Details in a Value History Questionnaire facilitate guiding an incompetent patient through (...) a terminal illness in accordance with wishes previously expressed.An instrument in the form of a questionnaire was designed to record the attitudes of 200 patients regarding health care and dying. Respondents ranged in age from 17 to 84 years, and all were members of one family practice. They reacted positively to the opportunity to record their values, opinions, and wishes about their health care and process of dying. They clearly indicated that, in the absence of prior directives, they would want their families consulted about crucial decisions. (shrink)

"This book addresses recent developments in the study of tense from a cross-paradigm and cross-linguistic point of view. Leading international scholars explore challenging ideas about tense at the interfaces between semantics and syntax as well as syntax and morphology. The book is divided into three main subsections: 1) Tense in tenseless languages; 2) Tense, mood, and modality, and 3) Descriptive approaches to some tense phenonema. Although time is a universal dimension of the human experience, some languages encode reference to time (...) without any grammatical tense morphology of the verb. Some of these exceptional "tenseless" languages are investigated in this volume: Kalaallisut, Paraguayan Guaraní and Movima. Modal verbs are polyfunctional in the sense that they express both tense and modality. In this volume, an untypical modal is analyzed, a modal analysis of imperatives is argued for, and sentential mood, which is closely related to modality, is analyzed. It is always interesting to look at the expression of tense in understudied languages, which is done here for Scottish Gaelic, Austronesian Rukai and German dialects. The aim of this review was to analyse the empirical studies that focus on ethics in nursing care for older people, scoping the need and areas for further study. A search of the MEDLINE and CINAHL databases (earliest to August 2009) was conducted using the the keywords: ethic* and nursing or care or caring and elderly or aged or older. After a four-stage process, 71 empirical articles were included in the review, with informants ranging from elderly people to relatives, caregivers, (...) managers and students in care settings. The review focusses on the concepts, contexts, methods and validity of these studies. Based on the analysis, the reviewed research seems to be fragmented and multifaceted, focussing on selected issues such as autonomy, self-determination and informed consent. No large research programs or research traditions were found so it was not possible to draw any conclusions about suitable methods, study designs or instruments of measurement for use in this research area. This study of the workings of neural networks in perception and understanding of situations and simple sentences shows that, and how, distributed conceptual constituents are bound together in episodes within an interactive/dynamic architecture of sensorial and pre-motor maps, and maps of conceptual indicators (semantic memory) and individuating indicators (historical, episodic memory). Activation circuits between these maps make sensorial and pre-motor fields in the brain function as episodic maps creating representations, which are expressions in consciousness. It is argued that all consciousness (...) is episodic, consisting of situational or linguistic representations, and that the mind is the whole of all conscious manifestations of the brain. Thought occurs only in the form of linguistic or image representations. The book also discusses the role of consciousness in the relationship between causal and denotational semantics, and its role for the possibility of representations and rules. Four recent controversies in consciousness research are discussed and decided along this model of consciousness: • Is consciousness an internal or external monitoring device of brain states? • Do all conscious states involve thought and judgement? • Are there different kinds of consciousness? • Do we have a one-on-one correspondence between certain brain states and conscious states. The book discusses also the role of consciousness in the relationship between causal and denotational semantics, and its role for the possibility of representations and rules. (Series A). (shrink)

This book offers a concise overview of the development of intercultural philosophy since the early 1990s, focusing on one of its key pioneers Heinz Kimmerle (1930-2016). Building on influences from Gadamer, Heidegger and Derrida, Kimmerle's approach to intercultural philosophy is radical and fosters epistemic justice. Kimmerle critically reflected on his own western philosophical tradition, highlighting the problems of a discourse based on a dominant concept of rationality, and of excluding different approaches and participants. Instead, Kimmerle developed an alternative way of (...) thinking, emphasising the importance of recognising philosophies of different cultures. He focused particularly on African philosophies in academic discourse. In the book, the many layers of Kimmerle's intercultural philosophy are revealed, exploring how dialectics, hermeneutics, deconstruction and decolonization can contribute to epistemic justice. The author goes beyond Kimmerle and demonstrates how Kimmerle's approach can be further enhanced by using an intersectional approach and by engaging in dialogue with female philosophers and artists. This new study, which also introduces unpublished and untranslated texts from Kimmerle's work in German and Dutch, will be of considerable interest to researchers of continental philosophy, intercultural and African philosophy, political philosophy, decolonial and feminist studies. Wahrnehmen - was ist das? Worauf kommt es an bei der Wahrnehmung sozialer Wirklichkeit? Welche Zugange sind produktiv fur die Annaherung an die Wirklichkeit sozialer Berufe? Soziale bzw. helfende Berufe haben sehr viel damit zu tun, andere Menschen in ihrer Lebensbewaltigung zu unterstutzen und zu begleiten. Hierfur sind hilfreiche Strukturen in der Gesellschaft zu kultivieren. Die helfenden Professionen greifen dabei auf Menschenbilder, Gesellschaftsbilder, soziale Gegebenheiten, Strukturen und Ziele fur ein "gelingendes Leben" zu, bezogen auf den jeweils Anderen in seinen spezifischen (...) Kontexten sozialer Wirklichkeit. Den Anderen in seiner sozialen Wirklichkeit wahrnehmen ist damit Grundkompetenz in sozialen bzw. helfenden Berufen. (shrink)

Democracy depends on a vivid public sphere, where ideas disseminate into the public and can be discussed – and challenged - by everyone. Journalism has contributed significantly to this social mediation by reducing complexity, providing information on salient topics and (planned) political solutions. The digital transformation of the public sphere leads to new forms of media provision, distribution, and use. Journalism has struggled to adapt to the new conditions. Journalistic news values, relevant to democracy, are being replaced by ones relevant (...) to social media platforms’ attention seeking business model. We plead for a broad public debate about the ongoing platformization and about possible policies to ensure a media system that serves and strengthens democracy. (shrink)
