Familial determination, replete with its frequent usurping of patient autonomy, propagation of collusion, and circumnavigation of direct patient involvement in their own care deliberations, continues to impact clinical practice in many Asian nations. Suggestions that underpinning this practice, in Confucian-inspired societies, is the adherence of the populace to the familial centric ideas of personhood espoused by Confucian ethics, provide a novel means of understanding and improving patient-centred care at the end of life. Clinical experience in Confucian-inspired Singapore, however, suggests that (...) personhood is conceived in broader terms. This diverging view inspired a study of local conceptions of personhood and scrutiny of the influence of the family upon it. From the data gathered, a culturally appropriate, clinically relevant and ethically sensitive concept of personhood was proposed: the Ring Theory of Personhood that better captures the nuances of local conceptions of personhood. The Ring Theory highlights the fact that, far from being solely dependent upon familial centric ideals, local conceptions of personhood are dynamic, context dependent, evolving ideas delineated by four dimensions. Using the Ring Theory, the nature of familial influences upon the four dimensions of personhood – the Innate, Individual, Relational and Societal – are examined to reveal that, contrary to perceived knowledge, conceptions of personhood within Confucian societies are not the prime reason for the continued presence of this decision-making model but remain present within local thinking and practices as a sociocultural residue and primarily because of inertia in updating ideas. Drawing upon the insights of Rabindranath Tagore, who coined the term viswasahitya to express his own understanding of comparative literature, this essay resituates translation as the cornerstone for new directions in world literature. While conventional understandings of world literature tend to reconfirm existing power structures and hierarchies, translation opens up the possibility of thinking beyond the national/global binary by interrogating the lines along which such binaries are conceptualized. Translation operates at the borders that are seen to divide cultures, languages, worldviews (...) and geographies. This essay explores the dynamic relationship between translation and world literature within contemporary South Asian writing, through an analysis of heteroglossia, multilingualism and ‘translatedness’ in selected texts by Mahasweta Devi and Amitav Ghosh, opening up larger questions about multilingualism and also about the very discipline of comparative literature. Highlighting the role that translation has historically played in shaping power relations in the world, this paper projects the transformative potential of translation as the key to a radical reconceptualization of a world literature for the future. Does religiosity help or hinder the exercise of agency? This article brings new evidence to bear on this long-standing debate, examining the life and work of the indigenous activist and follower of liberation theology, Rigoberta Menchú, in Guatemala, and the experiences of a millenarian community in Brazil, particularly one of its leaders, Dona Dodô. The two cases elucidate the dynamics of agency and piety, challenging the idea that pious individuals lack agency. In particular, the article interrogates the construction of pious (...) women as doubly oppressed by the forces of religion and patriarchy, and argues that, on the contrary, it was in the course of religious observance that Menchú and members of the millenarian community mounted challenges to ecclesiastical as well as political orders. Thus, the article underscores the possibilities for resistance and contention through piety rather than at odds with it. In studying these historical figures, the article looks beyond the Global North, which has inspired much of the theorising on religion and agency, to women and men marginalised by their ethnicity, poverty and rurality. In doing so, it demonstrates how religion can enable action among those far from traditional centres of power. (shrink)

‘Terminal discharges’ are carried out in Singapore for patients who wish to die at home. However, if due diligence is not exercised, parallels may be drawn with euthanasia. We present a theoretical discussion beginning with the definition of terminal discharges and the reasons why they are carried out in Singapore. By considering the intention behind terminal discharges and utilising a multidisciplinary team to deliberate on the clinical, social and ethical intricacies with a patient- and context-specific approach, euthanasia is avoided. It (...) is hoped that this will provide a platform for professionals in palliative medicine to negotiate challenging issues when arranging a terminal discharge, so as to avoid the pitfall of committing euthanasia in a country such as Singapore where euthanasia is illegal. We present a game semantics for Linear Logic, in which formulas denote games and proofs denote winning strategies. We show that our semantics yields a categorical model of Linear Logic and prove full completeness for Multiplicative Linear Logic with the MIX rule: every winning strategy is the denotation of a unique cut-free proof net. A key role is played by the notion of history-free strategy; strong connections are made between history-free strategies and the Geometry of Interaction. Genericity is the idea that the same program can work at many different data types. Longo, Milstead and Soloviev proposed to capture the inability of generic programs to probe the structure of their instances by the following equational principle: if two generic programs, viewed as terms of type , are equal at any given instance A[T], then they are equal at all instances. They proved that this rule is admissible in a certain extension of System F, but finding a semantically (...) motivated model satisfying this principle remained an open problem.In the present paper, we construct a categorical model of polymorphism, based on game semantics, which contains a large collection of generic types. This model builds on two novel constructions:•A direct interpretation of variable types as games, with a natural notion of substitution of games. This allows moves in games A[T] to be decomposed into the generic part from A, and the part pertaining to the instance T. This leads to a simple and natural notion of generic strategy.•A “relative polymorphic product” which expresses quantification over the type variable Xi in the variable type A with respect to a “universe” which is explicitly given as an additional parameter B. Facing the possibility of a surge of COVID-19-infected patients requiring ventilatory support in Intensive Care Units, the Singapore Hospice Council and the Chapter of Palliative Medicine Physicians forward its position on the guiding principles that ought to drive the allocation of ICU beds and its role in care of these patients and their families.

“At-own-risk discharges” or “self-discharges” evidences an irretrievable breakdown in the patient–clinician relationship when patients leave care facilities before completion of medical treatment and against medical advice. Dissolution of the therapeutic relationship terminates the physician’s duty of care and professional liability with respect to care of the patient. Acquiescence of an at-own-risk discharge by the clinician is seen as respecting patient autonomy. The validity of such requests pivot on the assumptions that the patient is fully informed and competent to invoke an (...) at-own-risk discharge and that care up to the point of the at-own-risk discharge meets prevailing clinical standards. Palliative care’s use of a multidisciplinary team approach challenges both these assumptions. First by establishing multiple independent therapeutic relations between professionals in the multidisciplinary team and the patient who persists despite an at-own-risk discharge. These enduring therapeutic relationships negate the suggestion that no duty of care is owed the patient. Second, the continued employ of collusion, familial determinations, and the circumnavigation of direct patient involvement in family-centric societies compromises the patient’s decision-making capacity and raises questions as to the patient’s decision-making capacity and their ability to assume responsibility for the repercussions of invoking an at-own-risk discharge. With the validity of at-own-risk discharge request in question and the welfare and patient interest at stake, an alternative approach to assessing at-own-risk discharge requests are called for. The welfare model circumnavigates these concerns and preserves the patient’s welfare through the employ of a multidisciplinary team guided holistic appraisal of the patient’s specific situation that is informed by clinical and institutional standards and evidenced-based practice. BackgroundAmidst expanding roles in education and policy making, questions have been raised about the ability of Clinical Ethics Committees (CEC) s to carry out effective ethics consultations (CECons). However recent reviews of CECs suggest that there is no uniformity to CECons and no effective means of assessing the quality of CECons. To address this gap a systematic scoping review of prevailing tools used to assess CECons was performed to foreground and guide the design of a tool to evaluate the quality (...) of CECons.MethodsGuided by Levac et al’s (2010) methodological framework for conducting scoping reviews, the research team performed independent literature reviews of accounts of assessments of CECons published in six databases. The included articles were independently analyzed using content and thematic analysis to enhance the validity of the findings.ResultsNine thousand sixty-six abstracts were identified, 617 full-text articles were reviewed, 104 articles were analyzed and four themes were identified – the purpose of the CECons evaluation, the various domains assessed, the methods of assessment used and the long-term impact of these evaluations.ConclusionThis review found prevailing assessments of CECons to be piecemeal due to variable goals, contextual factors and practical limitations. The diversity in domains assessed and tools used foregrounds the lack of minimum standards upheld to ensure baseline efficacy.To advance a contextually appropriate, culturally sensitive, program specific assessment tool to assess CECons, clear structural and competency guidelines must be established in the curation of CECons programs, to evaluate their true efficacy and maintain clinical, legal and ethical standards. BackgroundThe COVID-19 pandemic has changed the healthcare landscape drastically. Stricken by sharp surges in morbidity and mortality with resource and manpower shortages confounding their efforts, the medical community has witnessed high rates of burnout and post-traumatic stress amongst themselves. Whilst the prevailing literature has offered glimpses into their professional war, no review thus far has collated the deeply personal reflections of physicians and ascertained how their self-concept, self-esteem and perceived self-worth has altered during this crisis. Without adequate intervention, this may (...) have profound effects on their mental and physical health, personal relationships and professional efficacy.MethodsWith mentions of the coronavirus pervading social media by the millions, this paper set out to collate and thematically analyse social media posts containing first-person physician reflections on how COVID-19 affected their lives and their coping mechanisms. A consistent search strategy was employed and a PRISMA flowchart was used to map out the inclusion/exclusion criteria.ResultsA total of 590 social media posts were screened, 511 evaluated, and 108 included for analysis. Salient themes identified include Disruptions to Personal Psycho-Emotional State, Disruptions to Professional Care Delivery, Concern for Family, Response from Institution, Response from Society and Coping Mechanisms.ConclusionIt is evident that the distress experienced by physicians during this time has been manifold, multi-faceted and dominantly negative. Self-concepts were distorted with weakened self-esteem and perceived self-worth observed. The Ring Theory of Personhood (RToP) was adopted to explain COVID-19’s impact on physician personhood as it considers existential, individual, relational and social concepts of the self. These entwined self-concepts serve as ‘compensatory’ to one another, with coping mechanisms buffering and fortifying the physician’s overall personhood. With healthcare institutions playing a vital role in providing timely and targeted support, it was further proposed that a comprehensive assessment tool based on the RToP could be developed to detect at-risk physicians and evaluate the presence and effectiveness of established support structures. The application of continuous deep sedation in the treatment of intractable suffering at the end of life continues to be tied to a number of concerns that have negated its use in palliative care. Part of the resistance towards use of this treatment option of last resort has been the continued association of CDS with physician-associated suicide and/or euthanasia, which is compounded by a lack clinical guidelines and a failure to cite this treatment under the aegis of a palliative care (...) approach. I argue that reinstituting a palliative care-inspired approach that includes a holistic review of the patient’s situation and the engagement of a multidisciplinary team guided by clearly defined practice requirements that have been lacking amongst many prevailing guidelines will overcome prevailing objections to this practice and allow for the legitimization of this process. Die indische Philosophin und Aktivistin Radha d’Souza begegnet dem Begriff Weltzivilgesellschaft mit größter Vorsicht und Skepsis. Selbst jahrelang in existenzielle Auseinandersetzungen um Grundrechte auf natürliche Ressourcen wie Wasser und Ackerland und höchstgerichtliche Entscheidungen verwickelt, sieht d’Souza im Begriff der Weltzivilgesellschaft in der Hauptsache den Versuch, einer neuerlichen Bemäntelung imperialistischer, westlicher Interessen, die sich nun nicht nur durch Entscheidungen von Regierungen legitimieren, sondern gar durch den Willen ihrer zivilen Bevölkerung.

This review essay takes three very different types of books, one on new social movements, the second on global governance and the third on dystopia, to reflect on methodological questions in knowledge production for social change which is the professed aim of critical and radical scholarship. The essay reflects on the methodological problems of making connections between philosophical, sociological and empirical analyses in ways that can guide action. The treatment of facts and events, omission to consider gaps and absences in (...) accounts of experiences, the obfuscation of the desirable and the possible in legal liberalism, problems of connecting everyday life to ontological questions of being and reality, perspectives and unity of micro and macrocosms, are some of the themes considered. The essay engages the methodological strategies that are needed to make the moves from space/time in philosophy to spatio-temporality in sociological analysis to here and now in empirical studies. It argues that all levels are interpenetrated in manifest experiences and that it is important to grasp the institutional constraints on knowledge production if knowledge is to provide guidance for transformative action. Abstract:On one hand, Heidegger is one of the most referenced philosophers in environmental ethics, on the other, there is an ongoing debate regarding the formulation of any kind of ethic based on Heidegger's philosophy as he himself was skeptical about the same. In such context, this review teases out why environmental ethics borrows extensively from Heidegger philosophy and how that in turn provides the necessary underpinnings of different schools of environmental ethics. This essay delineates the import of Heidegger's phenomenology, critique (...) of technology, and post-metaphysical conception of nature, in schools of environmental ethics, particularly, the radical schools like deep ecology. This also highlights that overemphasis on the Heidegger's later work considerably limits the contribution of his philosophy to this discipline and on that note, it concludes that there is a need to bring in the hermeneutic phenomenology of early Heidegger in order to reinterpret his later celebrated concepts in environmental ethics. IntroductionClinical ethics committees (CECs) support and enhance communication and complex decision making, educate healthcare professionals and the public on ethical matters and maintain standards of care. However, a consistent approach to training members of CECs is lacking. A systematic scoping review was conducted to evaluate prevailing CEC training curricula to guide the design of an evidence-based approach.MethodsArksey and O’Malley’s methodological framework for conducting scoping reviews was used to evaluate prevailing accounts of CEC training published in six databases. Braun and Clarke’s (...) thematic analysis approach was adopted to thematically analyse data across different healthcare and educational settings.Results7370 abstracts were identified, 92 full-text articles were reviewed and 55 articles were thematically analysed to reveal four themes: the design, pedagogy, content and assessment of CEC curricula.ConclusionFew curricula employ consistent approaches to training. Many programmes fail to provide CEC trainees with sufficient knowledge, skills and experience to meet required competencies. Most programmes do not inculcate prevailing sociocultural, research, clinical and educational considerations into training processes nor provide longitudinal support for CEC trainees. Most CEC training programmes are not supported by host institutions threatening the sustainability of the programme and compromising effective assessment and longitudinal support of CEC trainees. While further reviews are required, this review underlines the need for host organisations to support and oversee a socioculturally appropriate ethically sensitive, clinically relevant longitudinal training, assessment and support process for CEC trainees if CECs are to meet their roles effectively. Value creation has long been hailed as the major objective of business firms by many management researchers. Some authors state that a firm must create value for its shareholders; some insist that value must be created not just for shareholders but also for all stakeholders. However, most discussions of value creation do not address an important question: "For whom the value is created?" The purpose of this paper is to take a first step to fill this void and propose a (...) model of value creation along three dimensions: financial, nonfinancial, and time. It is hoped that the model will contribute to a better understanding of how strategic and operational decisions of managers may create value for some stakeholders while destroying it for others. Leopold’s land ethic is a watershed event in environmental ethics as it is the first one to provide an alternative conceptualization of land to transcend its “Abrahamic conception.” However, if Leopold had employed phenomenological methods to formulate his land ethic, then his conceptualization of land and the understanding of its relation with its dwellers could have been more nuanced. From an analysis of the Sundarbans islanders’ phenomenological accounts of land, collected during a field study, it can be shown that phenomenological (...) understanding of land negates the Abrahamic notions of the Sundarbans’s land, which are deeply rooted in developmental as well as ecological conservation programs that the region is presently witnessing. As an alternative, such a phenomenological approach can enhance Leopold’s land ethic by providing an opportunity to see land in its true form, instead of conceptual­izing land “in order to” maintain the integrity of a land pyramid. In this manner, it helps to broaden the horizon of the land ethic and transforms the conceptualization of land itself. (shrink)

Iain Thomson proposes that Heidegger’s notion of “being as such” should be regarded as the core concept of ecophenomenology. Here, we attempt to tease out further nuances of this concept by juxtaposing Thomas Sheehan’s interpretation of “being as such” with that of Ian Thomson. We demonstrate that Sheehan’s reading of “being as such” as the intrinsic-hidden-clearing aligns with Thomson’s interpretation, and further adds a nuanced hermeneutic-phenomenological understanding of the concept in Heidegger scholarship. We suggest that this reconciliation—which portrays that “being (...) as such” qua ex-sistence qua the intrinsic-hidden-clearing denotes the same transcen­dental realm—is imperative to guide ecophenomenology to proceed further towards attaining its core philosophy of “back to the thing itself.” This reconciliation helps us go beyond safeguarding a particular thing or an ecosystem. Alternatively, it emphasizes the manner in which a respectful awareness of the “being as such” can build empathy toward the excess that a thing always possesses in our relation to it. It is said that the transition from hydrology to the hydrosocial system has the potential for transforming the way currently water is seen as a natural object. The hydrosocial cycle denotes that we need to think about water beyond the definition of natural objects as the meaning of water emerges from the socio-cultural-political nexus it is embedded in. In this essay by drawing upon Martin Heidegger’s philosophy, I explore whether this transition is capable of changing the way we think about (...) water. To philosophically capture the status of water and the thinking that is associated with it in this transitional moment, I engage with the notion of inceptual thinking, examining its possibilities within the context of this transition. My deliberation will establish that even though the hydrosocial cycle provides us with a unique space and opportunity from which to initiate inceptual thinking about water, the present orientation of hydrosocial scholarship fails to accomplish this objective. I further argue that the possibility to initiate an inceptual thinking arises from the rupture in our thinking, and our empathy toward the ‘in-between’ space of the actor and the thing. This inceptual thinking would lead toward understanding thing as gathering. (shrink)

Abstract:Insights from phenomenological narratives of environmental change, led me to inquire on how to capture the dynamicity and relational reality of the Sund-arbans's environment, as articulated by the islanders. Existing concepts to represent one's surroundings, I argue, have their own limitations and I propose the term saṃsāra from Indian philosophy as an alternative. I contend that the hermeneutic of saṃsāra could craft out a new dimension of the concept and thus could very well capture the experience of the islanders. The (...) hermeneutic of saṃsāra and its translation to English, teases out deeper phenomenological nuances of the human—environment relationship. I also demonstrate the implications of this conceptualization in the literature of ecophenomenology and advocate that the notion of saṃsāra illuminates some of the key features of the human—environment relationship, which till now were not duly acknowledged in this literature. Abstract:Sundarbans attracts worldwide attention for being the largest single block halophytic mangrove forest and for Royal Bengal Tiger. Along with ecological conservation, recent scholarly works demonstrate the importance of mangrove preservation for withstanding climate change-induced natural calamities. These conservation programs following the trend of the West separate human settlements from the forest and restrict human access to forest for maintaining wilderness; this I mark as the first wave of territorialization. Based on a case study of one of the village islands (...) in Sundarbans, adjacent to the core forest, I illustrate how in this dynamic delta the recent changes in the islanders' outlook toward life and livelihoods, as induced by the first wave of territorialization, leading them to embrace a second wave of territorialization and to what extent that is impacting the sustainability of human settlements in this dynamic delta. It also elaborates as soon as islanders’ living practice turns into livelihoods, they become reluctant to take up those traditional livelihoods as being perceived ‘risky,’ ‘painful,’ ‘uncertain,’ and ‘destructive for Sundarbans.’ The villagers' intention to find a better and more respected livelihood directs them toward the city and leads them to normalize and in turn concretize the already-imposed forest—settlement territorial division. This concretization of the boundary is termed here as the second wave of territorialization. I posit the upshot of this is a double layer of distancing, which is detrimental to the longterm sustainability of nature—human relationship in this dynamic delta. In a nutshell, this article argues that the inherent understanding about this liminal landscape that these islanders gather while engaging with traditional livelihoods are crucial to combat the massive climate change-induced alterations that this delta is going to witness in the near future. Therefore, this disengagement is not only devastating for the ecology of the Sundarbans; it is going to be even more threatening to the human settlements of this area. (shrink)

The three most-referred to concepts of Heidegger in environmental philosophy—Ereig­nis, “let things be,” and Gelassenheit—need to be reinterpreted in the light of Thomas Sheehan’s interpretation of them. Environmental philosophy conceives of these concepts as his suggestive treatments for transcending technological nihilism. Following Sheehan, this reinterpretation reveals that these concepts instead of delineating a radical way out of the technological nihilism evokes the need to realize the presence of the intrinsic hidden clearing as the fundamental-limiting reality of human existence. Rather than (...) considering this technological revelation as a singular determining factor and consequently, striving to realize the transcendental source of meaning for overcoming the nihilistic revelation of the environment as mere resources, environmental philosophy should identify the role of the hermeneutic structure of human existence in intricately shaping our relationship with the environment to discover a new ground for understanding this relationship and eventually, for crafting a new pragmatic form of environmental ethics. Rapid changes in the environment are far from being a new phenomenon, especially for vulnerable zones like the Sundarbans, India. In the era of climate change, when these islands are witnessing a lot of initiatives to combat the increasing negative impacts of various environmental changes, this article showcases why it is imperative to study the everyday phenomenological experiences of the islanders to be able to go beyond the climate-affected narratives and generate a deeper understanding of the phenomenon itself—‘environmental change’. This (...) article provides the islanders’ phenomenological accounts of environmental change and borrowing from Heidegger’s concept of ‘Being-in-the-world’ explicates the reason behind the disparity in acknowledging environmental change across individuals. Making this interpretive phenomenological account of environmental change as the ground, this article offers a rubric of community-based ethic which perhaps can prove to be effective in addressing any place-specific environmental change, over and above the prevalent approach of place-based ethic. In this age of environmental crisis, Jainism is regarded worldwide as one of the first religions to have developed an environmental ethic, based on its practice of ahiṃsā. This article attempts to critically engage with the concept of ahiṃsā in its recently evolving forms—from a religious concept to its current portrayal as an environmental ethic. By explaining how ahiṃsā becomes the central concept of Jainism, tying together its ethics, theology, and ecology, this article establishes that the current global portrayal of (...) ahiṃsā by Jains, more than being driven by environmental concerns, is directed toward attaining liberation through reducing karmic impressions on souls. The article discerns the differences between Jain practice of ahiṃsā and ahiṃsā as an environmental ethos; it argues that to recognize ahiṃsā as an environmental ethic a broader reconceptualization is required beyond the way it is currently conceptualized in Jainism. It is evident, in the face of the COVID-19 pandemic that has physicians confronting death and dying at unprecedented levels along with growing data suggesting that physicians who care for dying patients face complex emotional, psychological and behavioural effects, that there is a need for their better understanding and the implementation of supportive measures. Taking into account data positing that effects of caring for dying patients may impact a physician’s concept of personhood, or “what makes you, ‘you’”, we adopt (...) class='Hi'>Radha Krishna’s Ring Theory of Personhood (RToP) to scrutinise the experiences of physicians working in intensive care units (ICU) using a fictional scenario that was inspired by real events. The impact of death and dying, its catalysts, internal constituents, external factors, dyssynchrony, and buffers, specific to ICU physicians, were identified and explored. Such a framework allows for ramifications to be considered holistically and facilitates the curation of strategies for conflict resolution. This evaluation of the RToP acknowledges the experience and wide-ranging effects it has on ICU physicians. As such, our findings provide insight into their specific needs and highlight the importance of support on a personal and organisational level. Although further research needs to be conducted, the RToP could serve as the basis for a longitudinal assessment tool supported by the use of portfolios or mentorship due to their provision of personalised, appropriate, specific, timely, accessible and long-term support. Background Supporting physicians in Intensive Care Units s as they face dying patients at unprecedented levels due to the COVID-19 pandemic is critical. Amidst a dearth of such data and guided by evidence that nurses in ICUs experience personal, professional and existential issues in similar conditions, a systematic scoping review is proposed to evaluate prevailing accounts of physicians facing dying patients in ICUs through the lens of Personhood. Such data would enhance understanding and guide the provision of better support for (...) ICU physicians. Methods An SSR adopts the Systematic Evidenced Based Approach to map prevailing accounts of caring for dying patients in ICUs. To enhance the transparency and reproducibility of this process, concurrent and independent use of tabulated summaries, thematic analysis and directed content analysis is adopted. Results Eight thousand three hundred fifty-eight abstracts were reviewed from four databases, 474 full-text articles were evaluated, 58 articles were included, and the Split Approach revealed six categories/themes centered around the Innate, Individual, Relational and Societal Rings of Personhood, conflicts in providing end of life care and coping mechanisms employed. Conclusion This SSR suggests that caring for dying patients in ICU impacts how physicians view their personhood. To resolve conflicts within individual concepts of personhood, physicians use prioritization, reframing and rely on accessible, personalized support from colleagues to steer coping strategies. An adapted form of the Ring Theory of Personhood is proposed to direct timely personalized, appropriate and holistic support. (shrink)
