Many problems that arise when providing pharmacy services may contain some ethical components and the aims of this study were to develop and validate a scale that could assess difficulties of ethical issues, as well as the frequency of those occurrences in everyday practice of community pharmacists. Development and validation of the scale was conducted in three phases: generating items for the initial survey instrument after qualitative analysis; defining the design and format of the instrument; validation of the instrument. The (...) constructed Ethical Issue scale for community pharmacy setting has two parts containing the same 16 items for assessing the difficulty and frequency thereof. The results of the 171 completely filled out scales were analyzed. The Cronbach’s α value of the part of the instrument that examines difficulties of the ethical situations was 0.83 and for the part of the instrument that examined frequency of the ethical situations was 0.84. Test–retest reliability for both parts of the instrument was satisfactory with all Interclass correlation coefficient values above 0.6,. The 16-item scale, as a self assessment tool, demonstrated a high degree of content, criterion, and construct validity and test–retest reliability. The results support its use as a research tool to asses difficulty and frequency of ethical issues in community pharmacy setting. The validated scale needs to be further employed on a larger sample of pharmacists. (shrink)

ABSTRACTAnxious individuals report disproportionately negative expectations concerning the future, termed the negative expectancy bias. The goal of the research was to question the efficiency of one-time educational programs for high school and college students in positive effects of dealing with ailment/disability. Before and after the educational programs we have gathered data from 11 high school students and 16 college students with semantic differential and measures for perceiving the stage of the handicap, disability and dependence as well as a possibility for a person with chronic illness/disability to lead a happy and fulfilled life. The data (...) have been analyzed by a one sided Sign Test and a one sided Wilcoxon Signed Ranks Test. In the final measurements we have determined more positive average descriptions of people with a chronic disease/disability by 7 high school and 13 college students. The biggest change in attitude has been recorded in a decreased number of high school students who perceive the challenged as unhappy and weak and in a decreased number of college students who perceive the challenged as unhappy and discouraged. A significant change in attitudes has also been recorded in a decreased number of participants who consider the challenged as weak, sad, passive and resigned and in an increased number of participants who perceive the challenged as productive. There hasn’t been a change in dominantly negative attitudes towards the challenged as people with handicaps, unattractive, disabled and dependent. After the educational programs more than 4/5 of high school and college students declared that due to the acquired knowledge they have a more positive attitude towards the ailment and the challenged and that this will help them improve the relationship towards the challenged and in coping with ailments in their life. In the final measurements the determined positive attitudes of participants towards ailments/disability and the challenged suggest that the educational programs have been relatively efficient and we recommend they are implemented further. The experience and attitudes of pharmacists towards research ethics through pharmacy practice research is largely unknown. This study sought to examine the pharmacists’ experience if they were research participants and their attitudes on the importance of informed consent in research practice. A cross-sectional survey was employed to achieve the aims of this study. The majority of 433 participating pharmacists were female ; the average age was 43.2 ± 9.5 years, and their average working experience was 15.0 ± 9.6 years. Almost (...) half of the respondents came from a medium chain pharmacy in Serbia. Older pharmacists reported the experience of being informed in detail by the researcher in their practice, had an informal agreement or a written agreement before the research process started, and believed it was necessary to know their rights in research. The more experienced pharmacists reported capturing objective and distant relationships with researchers when participating in pharmacy practice research. There was a significant difference between the experience of male and female pharmacists in the study. Those employed in medium chain pharmacies believed it was necessary to have informal agreement or a written agreement before beginning the research process. Results indicated that pharmacists show positive attitudes toward informed consent and its importance and these attitudes were more emphatically expressed by older and more experienced pharmacists in Serbia. Rare diseases are a heterogenic group of disorders with a little in common except of their rarity affecting by less than 5 : 10.000 people. In the world is registered about 6000-8000 rare diseases with 6-8% suffering population only in the European Union. In spite of rarity, they represent an important medical and social problem due to their incidence. For many rare diseases have no treatment, but if it exists and if started on time as being available to patients, there (...) is a good prognosis for them to be able for normal life. The problems of patients affected by rare diseases are related to the lack of diagnosis and timely undergoing as well as their treatment or prevention. Orphan drugs are products intended for treatment, diagnosis or prevention of rare diseases, but for their development and marketing the industry has not been interested in yet because of their marketing reasons. Patients suffering from a rare disease although belonging to the vulnerable group for their specific health needs, is becoming invisible in the health care system due to their additional needs un properly recognized. Ethical problems faced by patients, but also health care professionals are related to the allocation of medical diagnostics, unequal approach to health care, inappropriately specialized social services as well as therapy and rare orphan drugs unavailability. Ethical questions related to clinical trails on orphan drugs, population screening and epidemiology testing on rare diseases will also be discussed in this paper. U svetu je evidentirano izmedju 6.000 i 8.000 retkih bolesti koje pogadjaju sest do osam procenata stanovnistva samo u Evropskoj uniji. Dakle, iako retke, one predstavljaju vazan medicinski i socijalni problem s obzirom na broj obolelih. Za mnoge retke bolesti nema terapije, a ukoliko ona postoji i ako se na vreme zapocne i bude dostupna pacijentima, postoje dobri izgledi da ce oboleli moci da zive normalnim zivotom. Problemi s kojima se susrecu pacijenti sa retkim bolestima vezani su za dijagnostikovanje u smislu nemogucnosti ili nepravovremenog postavljanja dijagnoze, kao i same terapije ili prevencije pojave bolesti. Orphan lekovi su proizvodi koji su namenjeni za lecenje, dijagnozu ili prevenciju retkih bolesti, za ciji razvoj i marketing industrija nije zainteresovana iz trzisnih razloga. Pacijenti s retkim bolestima, iako pripadaju osetljivoj grupi, jer imaju specificne zdravstvene potrebe, ostaju nevidljivi u zdravstvenom sistemu, jer se njihove dodatne specificne potrebe ne prepoznaju. Eticki problemi s kojima se susrecu pacijenti, ali i zdravstveni radnici, ticu se alokacije zdravstvene dijagnostike, nejednakog pristupa zdravstvenoj zastiti i nezi, nedostatka specijalizovanih socijalnih usluga, kao i nepostojanja i nedostupnosti terapije orphan lekovima. Eticka pitanja retkih klinickih ispitivanja orphan lekova i populacionih i skrining ispitivanja u vezi sa retkim bolestima takodje su razmatrani u ovom radu. This paper attempts to philosophically articulate empirical evidence on the positive effects of illness within the wider context of a discussion of the positive aspects of vulnerability. The conventional understanding holds that to be vulnerable is to be open to harms and wrongs; it is to be fragile, defenseless, and of compromised autonomy. In this paper, we challenge the assumption that vulnerability consists of nothing but powerlessness and dependence on others. This paper attempts to: (1) outline the theoretical conceptualisation of (...) the positive aspects of coping with illness, while simultaneously showing how these findings provide empirical support for some classical philosophical ideas; (2) influence a change in the manners in which the ill are usually perceived (as weak, dependent, robbed of a good life) and (3) point to the importance of modernising the prevalent biomedical conception of illness with the subjective dimension of health. Biomedical understanding of illness focuses on the vulnerable experience of the sufferer, suggesting the importance of philosophical additions. (shrink)

Researching ethical problems and their frequency could give us a complex picture and greater insight into the types of ethical issues that pharmacists face in providing health care. The overall aim of this study was to assess the pharmacist’s perception of difficulty and frequency of selected ethical issues encountered by the community pharmacists in their everyday practice. A quantitative cross sectional multicenter study was performed using a validated survey instrument - Ethical Issue Scale for Community Pharmacy. The results of the (...) analysis of 690 completely filled out instruments showed the difference between the ethical issues which always occurred, and the ones that pharmacists found most difficult. The majority of respondents were familiar with the Code of Ethics but the correlation between the familiarity and the perceived usefulness of the code in resolving problems in everyday practice was negative. Results showed that patients’ well-being had a high influence on pharmacists’ behavior. The results provided quantitative data by the examination of specific ethical issues and their occurrence. Further empirical research is recommended in order to systematically identify the ethical issues faced by community pharmacists. (shrink)

Na početku članka daje se kratki pregled razvoja pojma trajanja u Bergsonovima djelima, s posebnim osvrtom na njegova djela Durée et simultanéité i Essai sur les données immédiates. Poslije toga izlaže se Bergsonovo poimanje teorije relativnosti i problema s njome povezanih, primjerice: problem istovremenosti teorije relativnosti i intuitivne ili apsolutne istovremenosti, potom problem referentnih sustava, problem jedinstvenoga vremena i mnoštva vremenâ, stvarnog vremena i simboličkog ili konvencionalnog vremena. U tom su smislu istaknute i neke važnije misli iz rasprave između Bergsona (...) i Einsteina, koja je bila 6. travnja 1922. godine. Na koncu se daje kratki pregled prosudaba i rasprava koje su pratile Bergsonovo tumačenje teorije relativnosti.The author presents a study on the meaning of duration and time in Bergson’s works Durée etsimultanéité and Essai sur les données immédiates de la conscience. It explains Bergson’s conceptof the theory of relativityand the difficulties relatedto it, such as: the theory of relativity’smeaningof coincidence and intuitive or absolute coincidence; difficulties regardingsupportingsystems,problemsof single time and multiple times, realtime and symbolic or conventionaltime.In this sense some important thoughts areemphasized regardingthe discussion betweenBergsonand Einstein, held on 6th April1922. The effective collection and management of personal data of rapidly migrating populations is important for ensuring adequate healthcare and monitoring of a displaced peoples’ health status. With developments in ICT data sharing capabilities, electronic personal health records (ePHRs) are increasingly replacing less transportable paper records. ePHRs offer further advantages of improving accuracy and completeness of information and seem tailored for rapidly displaced and mobile populations. Various emerging initiatives in Europe are seeking to develop migrant‐centric ePHR responses. This paper highlights their (...) importance and benefits, but also identifies a number of significant ethical, legal and social issues (ELSI) and challenges to their design and implementation, regarding (1) the kind of information that should be stored, (2) who should have access to information, and (3) potential misuse of information. We comment on the proposition “that lower temperatures and especially greater seasonal variation in temperature call for individuals and societies to adopt … a greater degree of self-control” (Van Lange et al., sect. 3, para. 4) for which we cannot find empirical support in a large data set with data-driven analyses. After providing greater nuance in our theoretical review, we suggest that Van Lange et al. revisit their model with an eye toward the social determinants of self-control.
