The authors report the first Italian experience of a research ethics committee (REC) audit focused on the evaluation of the REC’s compliance with standard operating procedures, requirements in insurance coverage, informed consent, protection of privacy and confidentiality, predictable risks/harms, selection of subjects, withdrawal criteria and other issues, such as advertisement details and justification of placebo. The internal audit was conducted over a two-year period (March 2009–February 2011) divided into quarters to better value the influence of the new insurance coverage regulation (...) that came into effect in March 2010 (Ministerial Decree of 14 July, 2009) and expand the requirements to safeguard participants in clinical drug trials including other critical items as information and consent and the risks to benefits ratio. Out of a total of 639 REC’s opinions and research studies, 316 were reviewed. Regarding the insurance policy requirements, Auditor/REC non-compliance occurred only in one case. The highest number of Auditor/REC non-compliance was in regard to information and consent, which should have incurred a suspended decision rather than a favorable opinion. This internal audit shows the importance and the difficulty of the review process. For this reason, specific courses for members of the research ethics committee and for those who aspire to become auditors will be provided. Para John Stuart Mill prazer e felicidade não são a mesma coisa, pois uma determinada quantidade de prazer desfrutado não significa necessariamente que a pessoa alcance a felicidade na mesma proporção. A felicidade seria um apanhado geral de bem estar presente não apenas no indivíduo, mas em toda a sociedade na qual ele vive. Desta forma Mill compreende que não pode haver uma sociedade onde todos os indivíduos tenham uma quantidade ilimitada de prazeres e que sendo assim existiram pessoas sofrendo (...) e infelizes dentro da sociedade. Contudo essa situação se alterna com o tempo entre os indivíduos, e seu ponto de vista valoriza o somatório do bem estar presente entre todas as pessoas, levando ao seu princípio da maior felicidade. Felicidade esta que não é igual a presente na escola das virtudes ou mesmo na deontológica, ela se diferencia por abordar outras questões como a razão para promover o bem, a implementação do princípio da prevenção de danos, da ajuda aos inocentes e a consideração de que os números efetivamente contam, ou seja, sua preocupação vai além do aspecto individual. Para tanto, a felicidade não pode estar desvinculada da moralidade, assim a análise das consequências das ações em pro da maior felicidade não deve contrapor os princípios morais da sociedade humana. Esses princípios morais servem como um guia para o julgamento das ações humanas, que por sua vez se são considerados corretos e bons, tendem ao caminho do prazer e consequentemente da felicidade do ponto de vista utilitarista de Mill. ABSTRACT Nanotechnologies are an expression of the human ability to control and manipulate matter on a very small scale. Their use will enable an even and constant monitoring of human organisms, in a new and perhaps less invasive way. Debates at all levels – national, European and international – have pointed out the common difficulty of giving a complete, clear definition of nanotechnologies. This is primarily due to the variety of their components, to the fact that there is not just (...) one technology but several. The most significant medical applications of nanotechnologies are in the diagnostic and the therapeutic fields, eg biosensors and molecular imaging, providing diagnosis and drug delivery with no invasive methods involved. Like any other emerging field, such technologies imply new possibilities for improving health but, on the other hand, they are still at an experimental stage and therefore should be implemented under rigorous safety testing before going on general release. For this purpose, the ethical, legal and social implications (ELSI) of nanotechnologies have been elaborated by study groups, in order to develop solutions before the results of the tests are diffused into medical practice. The aim of this paper is to define some of the ethical issues concerning biomedical applications and to evaluate whether there is a need for new or additional guidelines and regulations. (shrink)

Conscientious objection in the health care field—that is, refusal on the part of a medical professional to perform or cooperate in a procedure when it violates his or her conscience—is a growing concern for international legislators and a source of contentious debates among ethicists and the general public. Recognizing a general right to conscientious objection based on individual liberty, and thus a subjective right, could have negative consequences. Conscientious objection in health care settings should be fully protected, however, when the (...) objection is based on principles that are fundamental to the medical profession and the legal system. A Second level Master in “Clinical Bioethics Consultation” has been organized in Italy to offer an opportunity to offer an adequate training to carry out an ethics consultation in different health fields. The master has been promoted and realized by different institutions: Catholic University of Sacred Hearth in Rome, Insubria University in Varese, “Federico II” University in Naples, Lanza Foundation in Padua and the Local Health and Social Care Unit n.7 in Veneto Region. The aim of the master is train (...) ethics consultants to offer help regarding a correct decision, which will take by others. The choice must be as much as possible shared between patient, family and health care team. Ethics consultation can be primarily request by the physicians, nurses and health care staff such as support about a choice that wants to be the most appropriate and to clarify a dilemma between different options. Secondly an ethics consultation can be demand by the patient and the family members to find a clarification about their dilemmas and future choices. El artículo pone en evidencia la dimensión ético-antropológica de la historia de la medicina. En el pasado podemos encontrar aquellos elementos de relevancia ética que están en estrecha continuidad con el presente. Desde los orígenes el médico ha experimentado el conflicto entre el mundo del deseo y el mundo del límite. El cuidado de los enfermos comienza desde ahí, de la consciencia de compartir el mismo deseo, el mismo límite, el mismo destino. El artículo mira la historia de la medicina (...) como historia de la humanidad; dentro está toda la vida humana, según la definición del historiador Fielding Garrison . En esta perspectiva la dimensión ético-antropológica emerge sobre todo en las múltiples figuras de médicos ejemplares caracterizados por valores como el valor, la dedicación, la empatía, pero sobre todo por un gran sentido de humanidad y de solidaridad por sus enfermos. En fin, una lectura ético-social puede emerger del arte y de la literatura. Ellas son no sólo testimonio de una época sino también un sismógrafo de las dimensiones éticas de la medicina.This article highlights the ethical-anthropological level of the history of medicine. It explores the close connection between past and present regarding those elements of ethical relevance in medicine. Since the beginning, the physician experienced the conflict between hopes and limits. Medical care springs exactly from the awareness of sharing the same desire, the same limit, and the same destiny. This article regards the history of medicine as history of mankind; according to the historian Fielding Garrison, the history of medicine embodies the entire human life . In this perspective, the ethical-anthropological dimension emerges particularly in many exemplary figures of physicians, distinguished for courage, commitment, empathy, humanity and solidarity towards their patients. Finally, arts and literature can be regarded as instruments to get a cultural perspective, as well as guidelines for social and ethical key of interpretation. They are not only historical documents, but also a seismograph, registering the fundamental historical dimension of medicine. (shrink)

Las biotecnologías son ciencias destinadas a incidir en manera profunda en la sociedad. El alcance de los cambios que a través de ellas será posible realizar es notable, pero sobre todo se aleja de las ciencias de ingeniería, de las alimentarias, de las médicas. La bioética está decididamente llamada a acoger el reto representado por las biotecnologías. La presente contribución parte precisamente de aquí: captar los aspectos característicos y peculiares de algunas de las posibles aplicaciones biotecnológicas para encuadrar la singularidad (...) de las problemáticas bioéticas que de estas aplicaciones pueden derivar con el fin de leer tales contribuciones a la luz del valor al cual se refieren y por el cual están pensadas: la persona En lo específico, los autores se detienen en tres específicas aplicaciones biotecnológicas que representan sectores cuyas potencialidades preanuncian grandes promesas y, en ciertos aspectos, algunos retos verdaderamente cautivadores; en particular: los OGM, las nanotecnologías y la farmacogenética, temas en diversos modos considerados en la revista en el curso de los años.Biotechnologies, the use of living organisms or their products to modify human environment, are flourishing very rapidly. As part of the seminar Re-reading Petrarch in the Digital Age –taught at the University of Oregon in Winter 2014– a digital close reading of Francesco Petrarca’s Rerum Vulgarium Fragmenta led to a series of parallel and entwined activities and projects. Deeply integrated with the Oregon Petrarch Open Book Project, the course was oriented towards the encoding of Petrarca’s masterpiece based on the implementation of a network of different themes. The various occurrences and data obtained from the encoding were collected (...) into an online database that is still operating and updatable. The results of this qualitative data collection were also compared with a quantitative computer-based research of selected keywords extracted from the various themes. From the beginning of the seminar, the class was divided into groups and each of them read the Rvf focusing on one of themes chosen while at the same time paying attention to the others. The different groups considered these themes as interrelated and interdependent. The poet is the sphere in which all the topics emerge in their specific connotations. However, his sphere is related to, dependent on and intersected with the other four spheres. The encoding allowed measuring the different manifestations, levels and meanings of the intersecting relationships. The study of the visual interpretation of the Rvf in the Inc. Queriniano G V 15 paralleled the close reading and encoding activity and helped students to develop a visual rendering of their themes and a pointed and original and creative interpretation of Petrarca’s masterpiece. The main purpose of this paper is to investigate the role of social and emotional learning skills and resilience in explaining mental health in male and female adolescents, during the COVID-19 pandemic. Three self-report questionnaires were administered to 778 participants aged between 11 and 16 years and recruited from 18 schools in Northern Italy. The SSIS-SELb-S and the CD-RISC 10 assessed SEL and resilience skills respectively, while the Strengths and Difficulties Questionnaire was used to measure mental health in terms of (...) internalizing problems, externalizing problems, and prosocial behavior. We found that SEL and resilience skills were positively and significantly associated with each other, negatively associated with internalizing and externalizing problems, and positively related to prosocial behavior. Three linear regression analyses showed the significant role of resilience, age, and gender in explaining the variance of internalizing problems; the significant role of SEL skills, resilience, age, and gender in explaining the variance of externalizing problems; and the role of SEL skills, age, and gender in explaining prosocial behavior. Importantly, we found that resilience fully mediated the relationship between SEL skills and internalizing problems, partially mediated the relationship between SEL skills and externalizing problems and didn't mediate the relationship between SEL skills and prosocial behavior. The paper concludes with a discussion of the limitations of the study as well as its practical implications. (shrink)

El tema del fin de la vida está entre aquellos afrontados por la revista Medicina e Morale con mayor amplitud y profundización. Desde la fundación de la Revista hasta hoy se cuentan más de 70 publicaciones en la materia entre contribuciones científicas, editoriales y artículos colocados en la rúbrica El debate en Bioética. Las temáticas tratadas pueden ser de este modo clasificadas: muerte en general; eutanasia; muerte cerebral ; ensañamiento terapéutico; cuidados paliativos y asistencia al moribundo; rechazo actual de los (...) tratamientos; estados vegetativos; declaraciones anticipadas de tratamiento ; testamento biológico. Diversas son las perspectivas de reflexión y la integración entre los diversos conocimientos que se descubren en la lectura de las numerosas contribuciones presentes en la revista. Las cuestiones del "fin de la vida" han sido afrontadas desde un punto de vista ético, pero también clínico y médico-legal, biojurídico, deontológico, antropológico-filosófico, con una mirada siempre atenta a las controversias que han suscitado el debate público en el curso de los años.The theme about the end of life is discussed fullness and with deep analysis by the review Medicina e Morale. Over 70 articles, editorial and paper about end of life are published form the review foundation. The themes discussed may be classified like that: death, euthanasia, brain death, aggressive therapy, palliative cares, treatment refusal, post coma unresponsiveness, advanced directives, living will. Reading several articles published on this review is possible to find various useful remarks and integration between different branch of learning. A Educação Continuada de professores é reconhecida como indispensável e fundamental para a qualificação dos processos de ensino e aprendizagem. Para tanto, neste estudo, levaremos em consideração alguns aspectos dessa temática. Em primeiro lugar, precisamos ter conhecimento das diretrizes que tratam da Educação Continuada de professores, ou seja, compreender de onde viemos e qual é a proposta das legislações nacional e internacional. Em segundo lugar, explanaremos e discutiremos conceitos teóricos que tratam da temática, em especial a visão de Gert Biesta, (...) o importante filósofo da educação. Finalmente, com base nos dois itens citados, entrelaçaremos considerações e indagações para uma Educação Continuada que possa fazer diferença no cotidiano dos professores. The contribution describes a case report addressed in 2011 by the clinical ethics consultation service team of the Institute of Bioethics and Medical Humanities at the “Agostino Gemelli” School of Medicine of the Università Cattolica del Sacro Cuore, Rome. The clinical case regards ethical dilemmas about the patient’s prospects for receiving an orthotopic liver transplant, because she was a non-resident and lacked a caregiver to assist her during the follow-up period, as well as a place to stay after liver transplant (...) surgery. This article addresses some critical aspects of the relationship between aesthetic medicine (AM) and ethics and proposes a possible deontological ethical line to pursue based on current practices. The role of AM has always been controversial and suffers from unclear practical and moral boundaries, even within academic settings, since it aims to improve the appearance of individuals, not to cure a disease. Today, it is essential and pertinent to discuss these issues, as AM specialists are dealing with a growing and (...) increasingly demanding patient population that has undergone profound evolution in recent years. Current challenges within the field of AM include a lack of global uniformity concerning the education of AM specialists, an increasing number of physicians practicing AM with diverse training backgrounds, the spread of AM being practiced outside of medical practice or hospital settings, and the influence of social media where the success is modelled and dictated by the identification of a youthful appearance). By the field of action enriched by technologies that aim not only at enhancement per se but also at the preservation and regeneration of tissues, it is necessary to establish an active multidisciplinary discussion on the definition of shared ethical limits. This discussion would allow AM to fully reclaim its identity as a specialty that aims to improve patient well-being whilst maintaining respect for patient aesthetic harmony, the expertise of specialists who practice AM, the essential role of safety, and awareness of the importance of a confidential doctor−patient relationship. In this paper we analyze the resilience to decoherence of the Macroscopic Quantum Superpositions (MQS) generated by optimal phase-covariant quantum cloning according to two coherence criteria, both based on the concept of Bures distance in Hilbert spaces. We show that all MQS generated by this system are characterized by a high resilience to decoherence processes. This analysis is supported by the results of recent MQS experiments of N=3.5×104 particles.

El artículo analiza cómo la revista Medicina e Morale ha afrontado en su historia el tema de la ética en la experimentación clínica, que representa una parte muy amplia y compleja de la bioética y también de la deontología médica. Los aspectos que esta temática comprende son ya numerosos y la literatura producida ingentísima. La ética de la experimentación ha recibido particular impulso y atención a continuación de diversas experimentaciones no éticas que han turbado profundamente la opinión pública y hecho (...) necesario una fuerte reconsideración en el ámbito de la investigación clínica. En general, de los años 60's a los años 80's del siglo XX, ha prevalecido en la literatura un intenso empeño e interés en la elaboración de los principios de referencia y en la reflexión teórica relativa a varias cuestiones . De los años 90 en adelante se ha en cambio intensificado -probablemente por las exigencias nacidas en seno de los Comités de ética que precisamente en aquel período han iniciado a difundirse masivamentela elaboración de códigos, reglamentos, líneas-guía que han orientado el desarrollo de la disciplina más en sentido de procedimiento y con un acercamiento ético-deontológico. En particular, Medicina e Morale, ha estado siempre en estas temáticas, desde los inicios de su historia, constituyendo una voz original en el debate, además que un instrumento de difusión de documentos nacionales y sobre todo internacionales. La Revista ha representado, sobre todo en los últimos treinta años, un verdadero punto de referencia a nivel nacional y en algunos casos también internacional, proponiendo a veces orientaciones originales, que han sido luego acogidas en el ámbito de líneas-guía y de normativas.The article deals with how the journal Medicina e Morale has faced ethics in clinical trials, which represents a very large and complex bioethical issue, but also deontological one. This area comprises many questions and the literature is huge. It has risen after unethical clinical trials that have shocked the public and have produced a big rethinking about it. In general, from the '60s to the '80s of the XXth century, an intense commitment and interest have prevailed on the elaboration of the reference principies and on theoretical reflection on various issues, such as informed consent, the ethical significance of clinical research, the protection of vulnerable people, the risk acceptability). Since the '90s onwards the development of codes, regulations, guidelines -that have guided the development of the discipline in a procedural and also deontological sense- has increased, probably due to the large diffusion of ethics committees. Particularly, the journal "Medicina e Morale" has always been present on these issues, since the beginning of its history, creating an original voice in the debate, as well as a dissemination tool of national and especially international documentabon. The journal has been, especially in the last thirty years, a real national, and sometimes international, be nchmark, offering original suggestions which are then accepted in guidelines and regulations. Most contemporary approaches to meaning presume the limitation of semiotics (Didi-Huberman, Gumbrecht, Belting). The question of what kind of “semiotics” is required has not been asked. However, without some general science of meaning it is impossible to reform theory without committing past errors or ignoring progress. In the interest of reconnecting contemporary interests in “presence” to long-evolving needs, I review the ossification and decline of one theory of semiotics that serves as the tacit model rejected today. I return to problems (...) of the nature of the sign – whether it is “digital” or “analog” and conceived as “communication” or merely “meaning.” I then reconstitute a workable visual cognitive semiotics based on phenomenological premises. In lieu of an abstract, here is a brief excerpt of the content:164 BOOK REVIEWS "was presented with wine in the name of the whole University." That evening, one of the feasters recalled that this was the man who had written the foremost theological defense of the Royal Supremacy: the following morning, when Gardiner asked for vessels and vestments to say Mass before proceeding on his way, they were refused him, as to an excommunicate or a schismatic. This incident is (...) significant for a number of reasons: it shows that, whatever anyone else might have thought, Gardiner certainly thought he was a Catholic. The ambiguity of that label, and how far exactly it extended, is also quite evident. Some explanation of the difficulty and some consistency of usage in the text of the biography would have been welcome. The text itself appears to have a number of editorial flaws: p. 201 begins in the middle of a paragraph, and the opening lines are omitted. In the second paragraph of p. 176, numbers are transposed so that what is printed as 1534 should read 1543 (in the context, a significant difference). The book is, on the whole, a contribution to scholarship that is long overdue. That a man who was so powerful (not to say feared) during his own lifetime should wallow in oblivion is confusing. Redworth 's biography, for all its incompleteness, is of great value in piecing together the life of this leader, both political and ecclesiastical, who sought to fashion the emerging English Church and nation into a recognizably Catholic form. The religious order of the Dominicans (Order of Preachers) was founded in the thirteenth century by St. Dominic with the specific purpose of preaching the Gospel or " ministry of the Word." With the Franciscan Order it shared in the creative, artistic impulse that produced the medieval cathedrals. It is often said that the great Summa Theologiae of St. Thomas Aquinas, for all its intellectual rigor, is also a work of art comparable to the gothic masterpiece of Chartres cathedral. Dominicans and Franciscans thought that the Gospel should find expression not only in words, but in poetry, music, architecture, and BOOK REVIEWS 165 the plastic arts. Among the artists who were members of the Dominican Order are the recently beatified Fra Angelico (BL Giovanni da Fiesole ), Bartolommeo della Porta, an important influence on Raphael and other High Renaissance painters, Juan Mayno the gifted Spanish contemporary of El Greco, Pere Couturier, the advocate of modern church art and friend of Matisse, and the American sculptor, Thomas McGlynn. Carrying on this Dominican tradition, Angelico Rinaldo Zarlenga (1919-1986), a native of Italy, worked in the United States from 1949 until his death. The present work, edited by his brother Vincent, also a member of the Order, makes available a chronological catalogue of his paintings, sculptures, and designs in glass, with many illustrations in full color, along with a biography and critical appreciations by others, including Cardinals Joseph Bernadin and Mario Luigi Ciappi. It is an important contribution to the history of Catholic religious art in the United States. The noted German theologian Hans Urs von Balthasar built his whole theology not on the goodness and truth of God, but on His beauty, or in biblical terms, God's "glory." According to St. Thomas Aquinas, " beauty " is the " radiance of truth," that is, it is truth as truth is related well to our knowing powers. Like the fit between the key and the lock, beauty is the fit between known and the knower. That is why a landscape on a cloudy day looks dull and uninteresting, but when the sun comes out it appears strikingly beautiful. The sun's illumination has made the dim objects vividly visible, radiant to our eyes. This is what the artist does for us. En 1990, Georges Didi-Huberman, filósofo e historiador del arte contemporáneo, anuncia que va a elaborar una «estética del síntoma». La expresión está cargada de consecuencias epistemológicas. Nos proponemos exponer aquí las claves de tal filosofía del arte y aclarar los problemas que plantea. ¿Cuáles son, según Didi-Huberman, los síntomas de las imágenes? Antes de responder a esta pregunta es preciso hacer un rodeo por el pensamiento crítico de Freud mostrando lo que es para él un síntoma y evocando sobre todo (...) la figura de la histeria. El concepto de síntoma, tomado en su acepción freudiana y reactualizado en los confines de la obra de arte, permite a Didi-Huberman hacer ver de qué forma compleja significaciones heterogéneas pueden resumirse, articularse en una imagen. Designa esta articulación de sentidos mediante la noción de síntoma, cuyo «pan» es una circunstancia particular. Retomando los análisis de Didi-Huberman sobre la pintura de Fra Angelico, mostramos todo el poder de los síntomas de la pintura y la apertura epistemológica que imponen a los teóricos del arte. This article argues that the early Christian ?order of widows? provides a fruitful model for Christian ethicists struggling to address the medical and social problems of elderly women today. After outlining the precarious state of the ?almanah? - or widow - in biblical times, it describes the emergence of the order of widows in the early Church. Turning to the contemporary situation, it argues that demographics both in the United States and around the globe suggest that meeting the needs of (...) elderly women will become an enormous challenge in the years to come. The order of widows illustrates a three-fold conception of solidarity that has immediate implications today. That conception of solidarity encourages us: 1) to identify the unique medical needs of elderly women (e.g., osteoporosis); 2) to find ways of overcoming their societal isolation, which can increase their risk of medical and psychological problems; and 3) to develop strategies for enabling them to remain contributing members of the community for as long as possible. This essay is a slightly revised version of a keynote address given by the author at a conference on ?Women?s Health and Human Rights? held in Rome, Italy in February 1998 and sponsored by the Vatican, Georgetown University, and the Università Cattolica del Sacro Cuore. Al centro dell'attenzione degli studiosi nell'ultimo ventennio per l'importanza della testimonianza storica, per il carattere peculiare delle strutture compositive, per varie sfaccettature dell'impostazione ideologica, l'opera di Michele Attaliata non disponeva di un'analisi complessiva né tantomeno di un'edizione critica condotta sulla base degli attuali criteri ecdotici e di una traduzione in una lingua moderna. Lacuna avvertita dai bizantinisti con maggiore evidenza, dal momento che per gli altri storici di primo piano dell'XI secolo, da Michele Psello a Giovanni Scilitza, ci si avvale (...) di strumenti critici agguerriti. Lacuna colmata dal poderoso lavoro di I. Pérez Martín che si presenta come un'edizione dell' Ἱστοϱία di Attaliata, corredata da traduzione in spagnolo, commento, bibliografia, indici, tavole e preceduta da un'ampia introduzione. (shrink)

La ricognizione delle teorie medievali dell’analogia e dell’univocità consente di rilevare l’ineludibile valenza logico-linguistica che le caratterizza fin dagli esordi. Le teorie dell’analogia riguardano innanzitutto una teorizzazione dei rapporti di denominazione, specialmente del termine ‘ens’ e degli altri nomi divini. Solo quando la discussione filosofica, a partire dal XIV secolo, si arricchisce della distinzione tra concetto formale e concetto oggettivo, esse subiscono una revisione e una sistematizzazione nel senso di una prospettiva più marcatamente teologico-ontologica. Il confronto serrato delle tesi tomiste (...) con le posizioni univociste ed equivociste sollecita progressivamente una puntualizzazione della nozione e delle divisioni dell’analogia, fino alla ‘contaminazione’ scotistica più o meno esplicita che si ravvisa in alcuni tomisti del Tardo Rinascimento spagnolo e che ormai prelude all’età cartesiana.
