Although self‐testing apps, a form of mobile health (mHealth) apps, are often marketed as empowering, it is not obvious how exactly they can empower their users—and in which sense of the word. In this article, I discuss two conceptualisations of empowerment as polar opposites—one in health promotion/mHealth and one in feminist theory—and demonstrate how both their applications to individually used self‐testing apps run into problems. The first, prevalent in health promotion and mHealth, focuses on internal states and understands empowerment as (...) an individual process. However, this version of empowerment has been accused of paternalism and responsibilisation. The second, feminist version considers structural conditions and foregrounds collective, political change, whose realisation is not obviously attainable for an individually used app. By pointing out the flaws of the positions that focus on either internal states or external conditions, and by engaging with theory from critical phenomenology, I argue that the interplay between them is where empowerment can take place. I propose to formulate empowerment in phenomenological terms as a shift in being‐in‐the‐world and discuss how this conceptualisation of empowerment would avoid the criticism of previous empowerment narratives while being realisable by self‐testing apps. In his telling, psychological tests seek to quantify population-based traits on which every person has a position and which tend to be conceptualized as being stable. Personality traits are often conceptualized as dispositions. Diagnostic categories represent (...) not trait-like properties of populations but episodic states consisting of clusters of symptoms experienced by individuals with disorders. Ratings, scales, he notes, are hybrids between the two. They are used to quantify psychiatric symptom clusters so that change over time can be measured. Le Moigne argues that these distinctions were initially suggested during the development of measures that could assess outcomes in psychopharmacological treatment studies.With respect to depression, Le Moigne writes that in the 1970s and 1980s something akin to depressive personality was reconceptualized as a temperament. Most phase III clinical trials today are explanatory. Because explanatory, or efficacy, trials test hypotheses under “ideal” conditions, they are not well suited to providing guidance on decisions made in most clinical care contexts. Pragmatic trials, which test hypotheses under “usual” conditions, are often better suited to this task. Yet, pragmatic, or effectiveness, trials are infrequently carried out. This mismatch between the design of clinical trials and the needs of health care professionals is frustrating for everyone involved, and (...) explains some of the challenges inherent in attempts to enhance knowledge translation and encourage evidence-based practice. The situation is more than simply frustrating, however; it is potentially unethical. Clinical trials must be socially valuable in order to (1) warrant the risks they impose on human research subjects and (2) fairly and efficiently assess new clinical interventions. Most bioethicists would agree that trials that have no social value, for instance, because their results do not have the potential to advance clinical care, should not be performed. What is less widely appreciated is that given limited research resources, trials that are more socially valuable should be preferred to trials that are less socially valuable when all else is equal. With respect to clinical trial design, I argue that while explanatory trials often have some social value, many have less social value than their pragmatic counterparts. On the basis of this general ethical assessment, I provide a preliminary defense of the position that clinical researchers should aim to conduct pragmatic trials, that is, that researchers face a burden of justification related to any idealizing elements added to trial designs. Background: Positive effects of acute exercise on cognitive performances in general inspired research that investigated the effects of acute exercise on specific cognitive subdomains. Many existing studies examined beneficial effects of acute exercise on subsequent set shifting performance in healthy adults. Set shifting, a subdomain of executive function, is the ability to switch between different cognitive sets. The results of existing studies are inconsistent. Therefore, a meta-analysis was conducted that pooled available effect sizes. Additionally, moderator analyses were carried (...) out to identify covariates that determine the magnitude of exercise-induced set shifting benefits.Methods: Medline, PsycINFO, and SPORTDiscus were searched for eligible studies. Hedges' g corrected standardized mean difference values were used for analyses. Random-effects weights were applied to pool effects. Potential moderation of the effect of acute exercise on subsequent set shifting performance by exercise intensity, type of exercise, participants' age, and type of control group were examined.Results: Twenty-two studies were included into analysis. All aggregated effect sizes ranged from small to moderate. Overall, a small significant beneficial effect was revealed. Heterogeneity of included effect sizes was moderate and significant. Moderator analyses revealed a larger average effect in older adults than for studies examining younger adults. Light exercise led to larger effects than moderate or vigorous exercise. Studies testing acute exercise against active control groups showed a noticeably smaller average effect than studies that used passive or cognitive engaging control groups. Interestingly, application of resistance or aerobic exercise led to no different average effect sizes. However, none of the tested covariates reached statistical significance.Conclusion: Acute exercise improves subsequent set shifting performance. However, effect sizes are small, making the relevance for everyday life questionable. The results indicate that older adults benefit more from acute exercise than younger adults do. Light intensity exercise seems most effective while the type of exercise does not seem to influence the magnitude of effects. The umbrella-term ‘executive functions’ includes various domain-general, goal-directed cognitive abilities responsible for behavioral self-regulation. The influential unity and diversity model of EF posits the existence of three correlated yet separable executive domains: inhibition, shifting and updating. These domains may be influenced by factors such as socioeconomic status and culture, possibly due to the way EF tasks are devised and to biased choice of stimuli, focusing on first-world testees. Here, we propose a FREE test battery that includes two open-access (...) tasks for each of the three abovementioned executive domains to allow latent variables to be obtained. The tasks were selected from those that have been shown to be representative of each domain, that are not copyrighted and do not require special hardware/software to be administered. These tasks were adapted for use in populations with varying SES/schooling levels by simplifying tasks/instructions and using easily recognized stimuli such as pictures. Items are answered verbally and tasks are self-paced to minimize interference from individual differences in psychomotor and perceptual speed, to better isolate executive from other cognitive abilities. We tested these tasks on 146 early adolescents of both sexes and varying SES, because this is the age group in which the executive domains of interest become distinguishable and in order to confirm that SES effects were minimized. Performance was determined by Rate Correct Scores, which consider speed-accuracy trade-offs. Scores were sensitive to the expected improvement in performance with age and rarely/inconsistently affected by sex and SES, as expected, with no floor or ceiling effects, or skewed distribution, thus suggesting their adequacy for diverse populations in these respects. Using structural equation modeling, evidence based on internal structure was obtained by replicating the three correlated-factor solution proposed by the authors of the model. We conclude that the FREE test battery, which is open access and described in detail, holds promise as a tool for research that can be adapted for a wide range of populations, as well as altered and/or complemented in coming studies. We carried out a replication study of the simulation experiments by McClelland (1989) and McClelland and Jenkins (1991) in which a PDP network learns to solve balance scale problems. In objective tests motivated from catastrophe theory, a mathematical theory of transitions in epigenetical systems, no evidence for stage transitions in network performance was found. It is concluded (...) that PDP networks lack the ability to recover the positive outcomes of analogous catastrophe analyses of real cognitive developmental data. In an attempt to further characterize the learning behaviour of PDP networks, we carried out a second simulation study using the discrimination‐shift paradigm. The results thus obtained indicate that PDP learning is compatible with the learning of stimulus‐response relationships, not with the acquisition of mediating rules such as conceived in (neo‐)Piagetian theory. Purpose With the rapid improvement in digital infrastructure, the popularity of digital devices and smartphones in every pocket, the yearning to stay connected with others has increased manifold, especially in youngsters. This has raised multiple concerns primarily related to the problematic usage of the internet (PUI). The current research study aims to scrutinize the association between PUI, psychological and mental health (PMH), social media fatigue (SMF), fear of missing out (FOMO), desire to disconnect (DD) and its relation with a novel (...) phenomenon of joy of missing out (JOMO). Design/methodology/approach The present research study embraces the empirical research method through quantitative analysis. The proposed theoretical model was empirically tested using primary data, collected through a self-designed structured questionnaire. The study sample included individuals between 16 and 39 years of age as these are the most active demographics on social media. The model is empirically tested with the help of structural equation modeling applied using software IBM AMOS 20.0 and SPSS 22.0. Initially, first-order confirmatory factor analysis was conducted, to measure and test the fit indices of the proposed model. Secondly, path analysis using structural equation modeling was carried out for the model. Findings Empirical synthesis of this research shows that PUI significantly and positively impacts mental and psychological health, FOMO and SMF. Also, SMF significantly and positively affects the DD which significantly and positively affects the JOMO. However, as depicted by the results of this study, FOMO have no considerable impact on SMF. Originality/value A study that connects the PUI with PMH, SMF and FOMO is rare to find. Second, this study uses data collected from social media users of India in the age group of 16–39 years. This slice of the population is most active in internet, and internet-enabled platform and are scantly studied, especially in the Indian context. This makes the study more exciting and crucial. A hypothesis about sensorimotor integration (the λ model) is described and applied to movement control and kinesthesia. The central idea is that the nervous system organizes positional frames of reference for the sensorimotor apparatus and produces active movements by shifting the frames in terms of spatial coordinates. Kinematic and electromyographic patterns are not programmed, but emerge from the dynamic interaction among the system s components, including external forces within the designated frame of reference. Motoneuronal threshold properties and proprioceptive inputs (...) to motoneurons may be cardinal components of the physiological mechanism that produces positional frames of reference. The hypothesis that intentional movements are produced by shifting the frame of reference is extended to multi-muscle and multi-degrees-of-freedom systems with a solution of the redundancy problem that allows the control of a joint alone or in combination with other joints to produce any desired limb configuration and movement trajectory. The model also implies that for each motor behavior, the nervous system uses a strategy that minimizes the number of changeable control variables and keeps the parameters of these changes invariant. Examples are provided of simulated kinematic and electromyographic signals from single- and multi-joint arm movements produced by suggested patterns of control variables. Empirical support is provided and additional tests of the model are suggested. The model is contrasted with others based on the ideas of programming of motoneuronal activity, muscle forces, stiffness, or movement kinematics. (shrink)

In William Angoff's recent article in Theoretical and Philosophical Psychology, he presented several thought-provoking philosophical issues of concern to measurement-oriented psychologists. The first of these issues, the notion of test and item bias, is the topic of the present paper. In his article, he reports that average score differences in the performance on many cognitive tests have appeared among racial or ethnic groups. Twenty to thirty years ago, differential psychologists, summarizing and critically evaluating studies demonstrating performance differences on tests (...) and in other behavior among cultural, ethnic and racial groups, agreed that such group differences indeed existed, but that causal interpretations differed markedly among the investigators at the time. More recently, the focus has shifted to what has been termed test fairness, with the focus falling more upon tests and their inadequacies than the behavior which underlies the tests. Similar findings and conclusions were frequently advanced in physiological tests of strength and endurance which are often used for civil service positions such as those in the fire fighting service and which frequently disadvantage women. The question raised is simply whether the discrimination is fair or not; that is, do given test score differences reflect variation in performance between groups which are reflective of true behavioral differences or are these differences artifactual and embedded in the assessment device itself, whether intentional or not. 2012 APA, all rights reserved). Somatic cell count is a diagnostic test of milk for mastitis in cows. Its specificity and sensitivity are less than 1.0, making test results uncertain. I discuss epistemological problems of the test such as underdetermination, undercalibration and underdiscrimination, in the solution of which biomedical and economic factors may play a role. Diagnostics of the SCC should be considered as an epistemological network, functioning in a network in which farmers, veterinarians, epidemiologists and milk industry shift their position (...) following biomedical, technological and economic changes, which is described as an actor-network to understand how human and non-human agents in mastitis may be associated. Shifting the focus from fear, guilt, and indignation related to deteriorating environmental quality, the authors hypothesized that people who see greater potential for restorative experiences in natural environments also do more to protect them by behaving ecologically, as with recycling or reduced driving. University students rated a familiar freshwater marsh in terms of being away, fascination, coherence, and compatibility, qualities of restorative person-environment transactions described in attention restoration theory. They also reported on their performance of various ecological behaviors. The (...) authors tested a structural equation model with data from a randomly drawn subset of participants and then confirmed it with the data from a second subset. For the combined subsets, perceptions of the restorative qualities predicted 23% of the variance in general ecological behavior. Community initiatives to create more localized food systems ofteninclude the strategy of import substitution, i.e., increasing local foodproduction for local consumption. The purpose of this policy iseffectively to supplant some level of imported food into the region. Weargue that such action can carry social and environmental risks as wellas benefits and we have developed research parameters to measure theimpact of such strategies. Harriet Friedmann's seminal work (1991) onthe employment of import substitution by transnational corporationsprovides a framework to identify possible advantages (...) and disadvantagesof the same approach locally. We propose local autonomy and sustainabledevelopment as positive indicators of a more localized food system.Three units of analysis are proposed to measure changes in localautonomy and sustainable development as a result of import substitutionschemes: fair labor trade, equity and democracy, and environmentalstewardship. We propose that this flexible framework of analysisincreases our ability to describe the shifting and integrated balancebetween more local and more global food systems. (shrink)

Background: In prevalence studies of sexually transmitted infections (STIs), investigators often provide syndromic management for symptomatic participants, but may not provide specific treatment for asymptomatic individuals with positive laboratory test results due to the delays between sample collection and availability of results as well as logistical constraints in recontacting study participants. Methods: To characterize the extent of this issue, 80 prevalence studies from the World Health Organization’s Report on global sexually transmitted infection surveillance, 2018, were reviewed. Studies were classified (...) as to whether clinically relevant positive results were returned or if this was not specified. Results: More than half (56%) of the cited studies did not specify if participants were notified of clinically relevant positive STI test results. The percentages were similar for low- and middle-income country populations (57%) and high-income country populations (53%). Conclusions: The absence of documentation of the provision of test results raises the possibility that in some instances, results may not have been communicated, with potential negative effects for participants, their sexual partners, and newborns. From an ethical perspective, clinically relevant results should be returned to study participants and treating clinicians in a timely fashion to ensure appropriate management of identified infections. Study authors should document if they returned test results to study participants and report on numbers lost to follow-up. (shrink)

Since the 1970s, prenatal testing has been integrated into many health care systems on the basis of two competing and largely irreconcilable rationales. The reproductive autonomy rationale focuses on nondirective counseling and consent as ways to ensure that women's decisions about testing and subsequent care are informed and free of undue pressures. It also represents an easily understandable and ethically convincing basis for widespread access to prenatal testing, since the value of autonomy is well established in Western bioethics and widely (...) recognized by funders of health care. In contrast, the public health rationale approaches prenatal testing as designed to reduce the incidence of certain conditions in the population to reduce the burden of disease. This rationale emphasizes the societal consequences of reproduction and the aggregate impact of women's individual reproductive decisions on the overall health of future populations. In this essay, I argue that, despite what could be seen as a persistent failure to meet the ideals of reproductive autonomy, resisting the public health rationale as a basis for prenatal screening is ethically and pragmatically crucial. I recommend policy mechanisms that can enhance reproductive autonomy at a societal level to support choice at the individual level. (shrink)

Two main types of philosophical arguments have been given in support of the claim that the citizens of affluent societies have stringent moral duties to aid the global poor: “positive duty” arguments based on the notion of beneficence and “negative duty” arguments based on noninterference. Peter Singer’s positive duty argument (Singer 1972) and Thomas Pogge’s negative duty argument (Pogge 2002) are among the most prominent examples. Philosophers have made speculative claims about the relative effectiveness of these arguments in promoting attitudes (...) and behaviors that could lead to the alleviation of poverty. In this article we present the results of two empirical studies that evaluate these claims, and suggest that both arguments have a modest effect on people’s attitudes and behaviors regarding global poverty. In a replication of the second study, the negative duty argument, in particular, had a statistically significant effect on donations. We discuss the theoretical and practical significance of these results and suggest directions for further research on the role that philosophical arguments can play in engendering concern and action on pressing moral problems. According to Positioning Theory, participants in narrative interaction can position themselves on a representational level concerning the autobiographical, told self, and a performative level concerning the interactive and emotional self of the tellers. The performative self usually is much harder to pin down, because it is a non-propositional, enacted self. In contrast to everyday interaction, psychotherapists regularly topicalize the performative self explicitly. In our paper, we study how therapists respond to clients' narratives by interpretations of the client’s conduct, (...) class='Hi'>shifting from the autobiographical identity of the told self, which is the focus of the client’s story, to the present performative self of the client. Drawing on video-recordings from three psychodynamic therapies (tiefenpsychologisch fundierte Psychotherapie) with 25 sessions each, we will analyze in detail five extracts of therapists‘ shifts from the representational to the performative self. We highlight four findings: • Whereas clients’ narratives often serve to support identity-claims in terms of personal psychological and moral characteristics, therapists rather tend to focus on clients’ feelings, motives, current behavior and ways of interacting. • In response to clients’ stories, therapists first show empathy and confirm clients’ accounts, before shifting to clients’ performative self. • Therapists ground the shift to clients’ performative self by references to clients’ observable behavior. • Therapists do not simply expect affiliation with their views on clients’ performative self. Rather they use such shifts to promote the clients’ self-exploration. Yet, if clients resists to explore their selves in more detail, therapists more explicitly ascribe motives and feelings that clients do not seem to be aware of. The shift in positioning levels thus seems to have a preparatory function for engendering therapeutic insights. (shrink)

To decrease shift impact on agricultural tractors with HMCVT and find the best working point, the following methods are proposed in this paper: firstly, conducting all-factor method and response surface method to design and carry out bench test of shift impact; secondly, using the basic linear fitting, RSM, and stepwise regression analysis to establish comprehensive (...) mathematical models and selecting models with high prediction accuracy; thirdly, putting forward the method of determining the weight of variance to match parameters of three quality evaluation indexes with inconsistent correlation; finally, establishing the total shift quality evaluation index and obtaining the optimal shift working point. It is found that oil filling pressure has a significant effect on speed drop, while the engine speed has a significance on the dynamic load and sliding friction. To make the transmission section of the best quality, the oil pressure at this time is 1 MPa and the engine speed is 1040 r/min. The results show that the proposed method can determine the optimal shift point and provide a reference for developing the control strategy. (shrink)

I explore how many within modern industrial societies currently understand, manage, and respond to their emotional suffering. I argue that this understanding and management of suffering has radically altered in the last 30 years, creating a new model of suffering, “the negative model” (suffering is purposeless), which has largely replaced the “positive model” (suffering is purposeful) that prevailed in the 18th and 19th centuries. This shift has been hastened by what I call the “rationalization of suffering”—namely, the process by which (...) suffering is predominantly now understood in biological and psychiatric terms rather than in religious, philosophical, or moral. This process accompanied the great forces of secularization, medicalization unleashed at the enlightenment—forces gaining their modern expression through psychiatry, the behavioral psychotherapies, the happiness industries, as well as through aggressive market, individualistic, medical, and positivistic ideas and movements. This shift has entailed many socio/cultural implications important for anthropologists to explore further. (shrink)

It is widely accepted that our initial intuitions regarding knowledge attributions in stakes-shifting cases (e.g., Cohen’s Airport) are best explained by standards variantism, the view that the standards for knowledge may vary with contexts in an epistemically interesting way. Against standards variantism, I argue that no prominent account of the standards for knowledge can explain our intuitions regarding stakes-shifting cases. I argue that the only way to preserve our initial intuitions regarding such cases is to endorse position (...) variantism, the view that one’s epistemic position may vary with contexts in an epistemically interesting way. Some had argued that epistemic position is incompatible with intellectualism. In reply, I point out that position variantism and intellectualism are compatible, if one’s truth-relevant factors with respect to p can vary with contexts in an epistemically interesting way. In this paper I am going to raise a problem for recent attempts to derive positive duties from Kant’s universalizability tests. In particular, I argue that these recent attempts are subject to reductio and that the most obvious way of patching them renders them impracticable. I begin by explaining the motivation for these attempts. Then I describe how they work and begin my attack. I conclude by considering some patches.

In the context of deceased organ donation, donors are routinely tested for HIV, to check for suitability for organ donation. This article examines whether a donor’s HIV status should be disclosed to the donor’s next of kin.On the one hand, confidentiality requires that sensitive information not be disclosed, and a duty to respect confidentiality may persist after death. On the other hand, breaching confidentiality may benefit third parties at risk of having been infected by the organ donor, as it may (...) permit them to be tested for HIV and seek treatment in case of positive results.We conclude that the duty to warn third parties surpasses the duty to respect confidentiality. However, in order to minimize risks linked to the breach of confidentiality, information should be restrained to only concerned third parties, that is, those susceptible to having been infected by the donor. We investigate the epistemological consequences of a positive polymerase chain reaction SARS-CoV test for two relevant hypotheses: V is the hypothesis that an individual has been infected with SARS-CoV-2; C is the hypothesis that SARS-CoV-2 is the cause of flu-like symptoms in a given patient. We ask two fundamental epistemological questions regarding each hypothesis: First, how much confirmation does a positive test lend to each hypothesis? Second, how much evidence does a positive test provide for each hypothesis (...) against its negation? We respond to each question within a formal Bayesian framework. We construe degree of confirmation as the difference between the posterior probability of the hypothesis and its prior, and the strength of evidence for a hypothesis against its alternative in terms of their likelihood ratio. We find that test specificity—and coinfection probabilities when making inferences about C—were key determinants of confirmation and evidence. Tests with 8) for V against ¬V regardless of sensitivity. Accordingly, low specificity tests could not provide strong evidence in favor of C in all plausible scenarios modeled. We also show how a positive influenza A test disconfirms C and provides weak evidence against C in dependence on the probability that the patient is influenza A infected given that his/her symptoms are not caused by SARS-CoV-2. Our analysis points out some caveats that should be considered when attributing symptoms or death of a positively tested patient to SARS-CoV-2. (shrink)
