Objectives: To study the attitudes of both medical and non-medical students towards the do-not-resuscitate decision in a university in Hong Kong, and the factors affecting their attitudes.Methods: A questionnaire-based survey conducted in the campus of a university in Hong Kong. Preferences and priorities of participants on cardiopulmonary resuscitation in various situations and case scenarios, experience of death and dying, prior knowledge of DNR and basic demographic data were evaluated.Results: A total of 766 students participated in the study. There were statistically (...) significant differences in their DNR decisions in various situations between medical and non-medical students, clinical and preclinical students, and between students who had previously experienced death and dying and those who had not. A prior knowledge of DNR significantly affected DNR decision, although 66.4% of non-medical students and 18.7% of medical students had never heard of DNR. 74% of participants from both medical and non-medical fields considered the patient’s own wish as the most important factor that the healthcare team should consider when making DNR decisions. Family wishes might not be decisive on the choice of DNR.Conclusions: Students in medical and non-medical fields held different views on DNR. A majority of participants considered the patient’s own wish as most important in DNR decisions. Family wishes were considered less important than the patient’s own wishes. (shrink)

Sham surgery is a controversial and rarely used component of randomised clinical trials evaluating surgical interventions. The recent use of sham surgery in trials evaluating efficacy of intracerebral fetal tissue grafts in Parkinson’s disease has highlighted the ethical concerns associated with sham surgery controls. Macklin, and Dekkers and Boer argue vigorously against use of sham surgery controls. Macklin presents a broad argument against sham surgery controls while Dekkers and Boer present a narrower argument that sham surgery is unnecessary in the (...) specific setting of fetal tissue engraftment for Parkinson’s disease. I defend sham surgery controls against both these criticisms. Appropriate clinical trial design, sometimes including sham surgery, is needed to ensure that false positive trial results do not occur and endanger public safety. Results of a completed trial of fetal tissue grafting for Parkinson’s disease are used to illustrate the potential benefits of, and problems associated with, sham surgery controls. Sham surgery controls, however, should be employed only when absolutely necessary. I suggest criteria for appropriate use of sham surgery controls. (shrink)

Next SectionThere has been considerable debate surrounding the ethics of sham-controlled trials of procedures and interventions. Critics argue that these trials are unethical because participants assigned to the control group have no prospect of benefit from the trial, yet they are exposed to all the risks of the sham intervention. However, the placebo effect associated with sham procedures can often be substantial and has been well documented in the scientific literature. We argue that, in light of the scientific evidence supporting (...) the benefits of sham interventions for pain and Parkinson's disease that stem from the placebo effect, these sham-controlled trials should be considered as offering potential direct benefit to participants. If scientific evidence demonstrates the positive effect of placebo from sham interventions on other conditions, sham-controlled trials of interventions for the treatment of these conditions should be considered to have prospects of benefit as well. This potential benefit should be taken into account by research ethics committees in risk-benefit analyses, and be included in informed consent documents. (shrink)

Debate continues about the ethics of sham surgery controls. The most powerful argument for sham surgery controls is that rigorous experiments are needed to demonstrate safety and efficacy of surgical procedures. Without such experiments, there is danger of adopting worthless procedures in clinical practice. Opponents of sham surgery controls argue that sham surgery constitutes unacceptable violation of the rights of research subjects. Recent philosophical discussion has used two thought experiments—the transplant case and the trolley problem—to explore the circumstances under which (...) individuals may be harmed to benefit a larger group. The transplant case is felt to exemplify circumstances that forbid harming some to benefit a larger group while the trolley problem exemplifies circumstances that permit harming some to benefit others. I argue that sham surgery controls satisfy criteria derived from the trolley problem and are morally permissible. (shrink)

The ipsilesional arm of stroke patients often has functionally limiting deficits in motor control and dexterity that depend on the side of the brain that is lesioned and that increase with the severity of paretic arm impairment. However, remediation of the ipsilesional arm has yet to be integrated into the usual standard of care for upper limb rehabilitation in stroke, largely due to a lack of translational research examining the effects of ipsilesional-arm intervention. We now ask whether ipsilesional-arm training, tailored (...) to the hemisphere-specific nature of ipsilesional-arm motor deficits in participants with moderate to severe contralesional paresis, improves ipsilesional arm performance and generalizes to improve functional independence. We assessed the effects of this intervention on ipsilesional arm unilateral performance [Jebsen–Taylor Hand Function Test ], ipsilesional grip strength, contralesional arm impairment level [Fugl–Meyer Assessment ], and functional independence [Functional independence measure ]. Intervention occurred over a 3 week period for 1.5 h/session, three times each week. All sessions included virtual reality tasks that targeted the specific motor control deficits associated with either left or right hemisphere damage, followed by graded dexterity training in real-world tasks. We also exposed participants to 3 weeks of sham training to control for the non-specific effects of therapy visits and interactions. We conducted five test-sessions: two pre-tests and three post-tests. Our results indicate substantial improvements in the less-impaired arm performance, without detriment to the paretic arm that transferred to improved functional independence in all three posttests, indicating durability of training effects for at least 3 weeks. We provide evidence for establishing the basis of a rehabilitation approach that includes evaluation and remediation of the ipsilesional arm in moderately to severely impaired stroke survivors. This study was originally a crossover design; however, we were unable to complete the second arm of the study due to the COVID-19 pandemic. We report the results from the first arm of the planned design as a longitudinal study. (shrink)

Obstructive sleep apnea (OSA) severely impacts sleep and has long-term health consequences. Treating sleep apnea with continuous positive airway pressure (CPAP) not only relieves obstructed breathing, but also improves sleep. CPAP improves sleep by reducing apnea-induced awakenings. CPAP may also improve sleep by enhancing features of sleep architecture assessed with electroencephalography (EEG) that maximize sleep depth and neuronal homeostasis, such as the slow oscillation and spindle EEG activity, and by reducing neurophysiological arousal during sleep (i.e., beta EEG activity). We examined (...) cross-sectional differences in quantitative EEG characteristics of sleep, assessed with power spectral analysis, in 29 adults with type 2 diabetes treated with CPAP and 24 adults undergoing SHAM CPAP treatment (total n = 53). We then examined changes in spectral characteristics of sleep as the SHAM group crossed over to active CPAP treatment (n = 19). Polysomnography (PSG) from the CPAP titration night was used for the current analyses. Analyses focused on EEG frequencies associated with sleep maintenance and arousal. These included the slow oscillation (0.5–1 Hz), sigma activity (12–16 Hz, spindle activity), and beta activity (16–20 Hz) in F3, F4, C3, and C4 EEG channels. Whole night non-rapid eye movement (NREM) sleep and the first period of NREM spectral activity were examined. Age and sex were included as covariates. There were no group differences between CPAP and SHAM in spectral characteristics of sleep architecture. However, SHAM cross-over to active CPAP was associated with an increase in relative 12–16 Hz sigma activity across the whole night and a decrease in average beta activity across the whole night. Relative slow oscillation power within the first NREM period decreased with CPAP, particularly for frontal channels. Sigma and beta activity effects did not differ by channel. These findings suggest that CPAP may preferentially enhance spindle activity and mitigate neurophysiological arousal. These findings inform the neurophysiological mechanisms of improved sleep with CPAP and the utility of quantitative EEG measures of sleep as a treatment probe of improvements in neurological and physical health with CPAP. In this study, we examine this puzzle by explicating the discordant space between the prevailing biomedical paradigm, which focuses on a technical understanding of diagnosis and treatment, (...) and a broader understanding of illness and healing as relational and embodied. Although biomedical achievements are impressive, the knowledge resulting from this paradigm is limited by its ontological and epistemological assumptions. When the body and world are objectified, illness meanings, therapeutic relationships, and healing practices are dismissed or distorted. In spite of a robust critique of the tenets of biomedicine for guiding practice, the biomedical paradigm retains a tenacious hold on evidence-based medicine and nursing, downplaying our clinical understanding of the sentient body, patients’ life-worlds, and illness and healing. In reality, skilled nurses rely on multiple forms of knowledge in providing high-quality care to particular patients. Clinically wise nurses integrate their experience and knowledge of patients’ priorities, fears, and illness trajectories along with biomedical findings to make astute judgments and promote health and healing. Common symptoms of multiple sclerosis include motor impairments of the lower extremities, particularly gait disturbances. Loss of balance and muscle weakness, representing some peripheral effects, have been shown to influence these symptoms, however, the individual role of cortical and subcortical structures in the central nervous system is still to be understood. Assessing [18F]fluorodeoxyglucose uptake in the CNS can assess brain activity and is directly associated with regional neuronal activity. One potential modality to increase cortical excitability and improve motor function in (...) patients with MS is transcranial direct current stimulation. However, tDCS group outcomes may not mirror individual subject responses, which impedes our knowledge of the pathophysiology and management of diseases like MS. Three PwMS randomly received both 3 mA tDCS and SHAM targeting the motor cortex that controls the more-affected leg for 20 min on separate days before walking on a treadmill. The radiotracer, FDG, was injected at minute two of the 20 min walk and the subjects underwent a Positron emission tomography scan immediately after the task. Differences in relative regional metabolism of areas under the tDCS anode and the basal ganglia were calculated and investigated. The results indicated diverse and individualized responses in regions under the anode and consistent increases in some basal ganglia areas. Thus, anodal tDCS targeting the M1 that controls the more-affected leg of PwMS might be capable of affecting remote subcortical regions and modulating the activity of the circuitry connected to these regions. 2. Fī al-quṭb wa-al-aqṭāb wa-al-ghawth wa-dhikr al-abdāl -al-awtād wa-al-nuqabāʼ wa-al-nujabāʼ wa-ghayrihim --.

The post-classical genre of the “protocols for dialectical inquiry and disputation” has its more proximate origins in the famed Risāla of Shams al-Dīn al-Samarqandī. The greater part of his conceptions and methodology, however, consists in a streamlining and universalizing of the more strictly juristic dialectic of his teacher Burhān al-Dīn al-Nasafī ; and this in turn draws on the highly logicized dialectic of Rukn al-Dīn al-ʿAmīdī and his teacher Raḍī al-Dīn al-Nīsābūrī. At the heart of methods in this lineage, (...) and carried forward by al-Samarqandī into the universal ādāb al-baḥth, are three truth-preserving logical relationships critical to the truth-seeking enterprise of dialectic: entailment, mutual negation or exclusion, and causal concomitance. The practical elaboration of these relations reveals a logic in action—a premodern dialogical logic for living disputation praxis. In fact, so critical were these to the dialectical enterprise that al-Samarqandī devoted a specialized treatise entirely to summarizing their defining features and rules, aptly naming it the ʿAyn al-Naẓar, or “Wellspring of Rational Investigation.” In this article, and drawing upon a recently published digital critical edition, I will present an analytical outline of these core logical relations as presented in the ʿAyn al-Naẓar. Then I will address a number of points of interest in this text, grouped under six themes: the potential for cross-disciplinary advancement; notions in discursive development; significant or uniquely contributive formulations; peculiarities of content; signs of an evolving, universalist agenda; and evidence that the ʿAyn al-Naẓar was designed as an aide-mémoire for the active disputant. Fibre suspension has garnered considerable attention in turbulent flows that are used in many industries. Solid particles, such as dust particles, notably affect the turbulent flow field in a rotational frame. In assessing their impacts, the dusty turbulent flow for fibre suspensions needs to be studied in a frame of rotation that can be substantially applied in many industries. This study, therefore, aims to build a theoretical model for the energy motion of dusty turbulent flow of fibre suspensions in a (...) rotational frame. The turbulence momentum equation was considered to formulate the model in presence of dusty fluid rotating flow of fibre suspensions. The newly derived equation was derived in second-order correlation tensors Fi,j, Wi,j, Gi,j, Si,j, Xi,j, Yi,j, Qi,j, and Ri,j at any two points in the flow domain, where the tensors were expressed as space, time, and distance functions. The developed model is a considerable improvement because it takes into account all of the potential influential parameters that could affect the motion of turbulent energy, such as dust particles, suspending particles, and rotating frame. However, the impact of these parameters on turbulence energy motion must be evaluated in order to assess the performance of turbulence systems utilized in a variety of industries, such as paper manufacturing. The present theoretical development will contribute to open up experimental and numerical research opportunities for the advancement of the industry, science, and technology. (shrink)

This book provides the first annotated English translation of Physics and Metaphysics and edits the original Arabic text on which the translation is based where it is corrupt or incomprehensible.

_Al-Isharat wal-Tanbihat_ is one of the most mature and comprehensive philosophical works by Ibn Sina. Grounded in an exploration of logic and happiness, the text illuminates the divine, the human being, and the nature of things through a wide-ranging discussion of topics. The sections of _Physics and Metaphysics_ deal with the nature of bodies and souls as well as existence, creation, and knowledge. Especially important are Ibn Sina's views of God's knowledge of particulars, which generated much controversy in medieval Islamic (...) and Christian philosophical and theological circles and provoked a strong rejection by eleventh-century philosopher al-Ghazali. This book provides the first annotated English translation of _Physics and Metaphysics_ and edits the original Arabic text on which the translation is based. It begins with a detailed analysis of the text, followed by a translation of the three classes or groups of ideas in the Physics and the four in the Metaphysics (On Existence and Its Causes, Creation Ex Nihilo and Immediate Creation, On Ends, on Their Principles, and on the Arrangement [of Existence], and Onion. _The Metaphysics_ closes with a significant discussion of the concepts of providence, good, and evil, which Ibn Sina uses to introduce a theodicy. Researchers, faculty, and students in philosophy, theology, religion, and intellectual history will find in this work a useful and necessary source for understanding Ibn Sina's philosophical thought and, more generally, the medieval Islamic and Christian study of nature, the world beyond, psychology, God, and the concept of evil. Few figures have been of such enduring importance as Ibn Sina, also known as Avicenna (980-1037 AD), the great Persian philosopher and physician of the Abassid period. This work is a study of the fourth part of Ibn Sina's late and most comprehensive book al-Isharat wat-Tanbihat, Remarks and Admonitions, a book which Ibn Sine describes as 'the cream of the truth', containing 'the best pieces of wisdom' expressed 'in sensitive words'. The present volume includes an introduction, discussing the nature of (...) the fourth part of al-Isharat and the procedures followed in analyzing and translating it; a lengthy analysis of Ibn Sina's ideas on the three main topics in this part of al-Isharat (the nature of happiness, the stations of the knowers, and the signs manifested by the knowers); and a translation of this part. The work shows that, to Ibn Sina, knowledge of the eternal aspects of the universe, primarily of God, is the highest human objective and the only thing that secures human happiness or heavenly existence. While no permanent knowledge of this kind is possible for a human being on earth, signs of its temporary acquisition are evidenced in many ways, one of which is possession of powers that a person who does not know the secrets of the universe may consider extraordinary. Although the focus of this volume is on the fourth part of al-Isharat, many references are made to some of Ibn Sina's other writings by way of clarifying certain points and placing this part of al-Isharat in the context of Ibn Sina's general philosophy. A weighted total of 630 pupils aged 1440. Among 1440-year-olds, especially females. Among 3015-year-old Glasgow Asians 86% were so born, indicating that they are the children of migrants. Generational differences in these comparisons cannot be due to positive selection of the migrant generation for height, and are attributed to improved environment, including nutrition and public health measures. This suggests the possibility of corresponding improvements in coronary and diabetic risk.

Open Shop Scheduling Problem is one of the most important scheduling problems in the field of engineering and industry. This kind of problem includes m machines and n jobs, each job contains a certain number of operations, and each operation has a predetermined processing time on its corresponding machine. The order of processing of these operations affects the completion times of all jobs. Therefore, the purpose of OSSP is to achieve a proper order of processing of jobs using specified machines, (...) so that the completion time of all jobs is minimized. In this paper, the strengths and limitations of three methods are evaluated by comparing the results of solving the OSSP in large-scale and small-scale benchmarks. In this case, the minimized completion time and total tardiness are considered the objective functions of the adapted methods. To solve small-scale problems, we adapt a mathematical model called Multiobjective Mixed Linear Programming. To solve large-scale problems, two metaheuristic algorithms including Multiobjective Parallel Genetic Algorithm and Multiobjective Parallel Simulated Annealing are adapted. In experimental results, we randomly generated small-scale problems to compare MOMILP with the Genetic Algorithm and Simulate Annealing. To compare MOPSA and MOPGA with the state of the art and show their strengths and limitations, we use a standard large-scale benchmark. The simulation results of the proposed algorithms show that although the MOPSA algorithm is faster, the MOPGA algorithm is more efficient in achieving optimal solutions for large-scale problems compared with other methods. The ambiguity of August Strindberg’s approach to women has engendered varying interpretations, including accusations of misogyny. Among his allegedly misogynistic plays is the 1887 naturalistic masterpiece, The Father. Chronologically coinciding with the rise of the women’s movement in Sweden, The Father, rather than endorsing a misogynistic culture, allows for an alternative reading that contributes to the destabilisation of gender binaries and an understanding of gender identities as relational and performative. In its portrayal of a fierce struggle between a seemingly diabolic (...) wife and a supposedly tyrannical husband, the play delves deeply into the dynamics of gender and the subversion of normatively established orders. This article analyses Strindberg’s play in relation to Slavoj Žižek’s conception of the ‘femme fatale’ and Judith Butler’s account of gender performativity to illustrate how the play’s central characters performatively subvert the hegemonic norms by which they are constituted. Nearly all fractions within the political, economic, and social spheres gave responses to the recent financial crisis. In broad terms, both left-leaning and right-leaning scholars and commentators presented their explanations for the crisis, with confident agendas defining ‘what is wrong’ and how to deal with it. However, as intellectual history shows us, most of those explanations were no less fascinating precisely because they shared more that they acknowledged. A major divide has been over the role of governments in coordinating markets, (...) or, in the case of Roberts’sThe Logic of Discipline, the role of markets in coordinating governments. In this essay, I offer an overview of Alasdair Roberts’s arguments inThe Logic of Discipline. Gradually, I extend the examined issues beyond the state-market dichotomy, arguing that only in understanding the interactive force of the two can a systemic analysis of capitalism hope to be plausible. Open Innovation and Sustainability-Oriented Innovation are undoubtedly two of the most debated topics of the last decades, gaining the interest of policymakers, practitioners, and scholars all over the world. Even if they have been usually described as two independent research fields, there are some emblematic examples presenting interplay and synergy between these topics, represented either by the hybrid perspectives of Open Sustainable Innovation, that is, the Open Innovation approach acting as an enabler of Sustainability-Oriented Innovation, and Sustainable Open Innovation, which (...) instead analyzes how firms developing Sustainability-Oriented Innovation also adopt the Open Innovation approach. On the basis of these two perspectives and through a systematic literature review, this paper investigates the relationships between the Open Innovation and Sustainability-Oriented Innovation approaches and frames these relationships by developing an innovative framework, which highlights the main aspects characterizing the hybrid perspectives of Open Sustainable Innovation and Sustainable Open Innovation. The proposed framework highlights the Open Innovation practices and strategies enabling Sustainability-Oriented Innovation, as well as the contextual factors enabling both practices and strategies for Sustainability-Oriented Innovation. In addition, it shows how firms developing Sustainability-Oriented Innovation have a similar orientation toward the adoption of the Open Innovation approach both in terms of how they engage stakeholders, and the innovation capabilities they develop. Finally, a research agenda identifying the central issues and the key research gaps is offered for further development in future studies. (shrink)
