ObjectiveThis pilot study aimed to evaluate the efficacy of a digital cognitive behavioral therapy in patients with cancer experiencing sleep problems.MethodsA total of 57 participants aged 25–65 years were randomly assigned to three groups—21 participants to a dCBT program, 20 participants to an app-based attentional control program, and 16 participants to a waitlist control group—and evaluated offline before and after the program completion. The (...) dependent variables were sleep quality scores, measured by the Pittsburgh Sleep Quality Index and health-related quality of life scores, measured using the Short-Form 36, and attentional bias scores from a dot-probe computer task.ResultsFor both the intention-to-treat and study-completers analyses, a significant increase supported by a large effect size was found in the quality of sleep score of the HARUToday Sleep group compared to both the app-based attentional control and the waitlist control group. However, no significant changes were found in the quality of life and attentional bias scores.ConclusionOur results suggest that the HARUToday Sleep app has the potential to serve as an intervention module to enhance the sleep quality of patients with cancer experiencing sleep problems. This paper analyzes the ‘Theory of Inverse Numbers’ that appears in Chapter 3 of the “Shuogua zhuan” 說卦傳 in the I Ching and seeks to apply the findings to the Yi studies of Cheng Yi. The Yi studies of Shao Yong, Zhu Xi, and Cheng Yi are generally explained in the categories of Xiangshu(Image-Number) Yi studies and Yili(Meaning-Principle) Yi studies. However, upon analyzing the ‘Theory of Inverse Numbers’ as a theory representing numbers of principles, it is evident that the framework (...) of this theory is also reflected in Cheng Yi’s Yi studies. The analysis of the ‘Theory of Inverse Numbers’ was conducted through the interpretations of Shao Yong and Zhu Xi, proposing the concept of the ‘Theory of Inverse Numbers’ as a Yi interpretation framework that accounts for what has already occurred [yisheng 已生] and what has not yet occurred [weisheng 未生]. By applying the Yi interpretation framework of what has arisen and what has not, which was developed through the explanations of Shao Yong and Zhu Xi, to Cheng Yi’s Yi studies, it is revealed that even though Cheng Yi does not directly mention Chapter 3 of the “Shuogua zhuan,” the narrative framework of Yichuan Yizhuan 伊川易傳 can be well elucidated by the ‘Theory of Inverse Numbers.’ Accordingly, it suggests that viewing Cheng Yi’s Yi studies merely as a dichotomy of meaning-principle and image-number is limited, and that the concept of Xiangshu Yi is inherent in Cheng Yi’s Yi studies. In other words, the ‘Theory of Inverse Numbers’ as a theory of principle-representing numbers originating from Shao Yong is inherent throughout Cheng Yi’s interpretation of the I Ching as a framework of Yi interpretation for what has arisen and what has not, and this was systematized as the ‘Theory of Inverse Numbers’ by Zhu Xi. Cancer chemotherapy has been the most common cancer treatment. However, it has side effects that kill both tumor cells and immune cells, which can ravage the patient’s immune system. Chemotherapy should be administered depending on the patient’s immunity as well as the level of cancer cells. Thus, we need to design an efficient treatment protocol. In this work, we study a feedback control problem of tumor-immune system to design an optimal chemotherapy strategy. For this, we first propose a mathematical model (...) of tumor-immune interactions and conduct stability analysis of two equilibria. Next, the feedback control is found by solving the Hamilton–Jacobi–Bellman equation. Here, we use an upwind finite-difference method for a numerical approximate solution of the HJB equation. Numerical simulations show that the feedback control can help determine the treatment protocol of chemotherapy for tumor and immune cells depending on the side effects. (shrink)

Spanning the 19th and 20th centuries and identifying multiple waves of modernization, this book illustrates how principles originating in Chinese Confucianism have impacted the modernization of East Asia, especially in Korea. It also analyzes how such principles are exercised at personal, interpersonal and organizational levels. As modernization unfolds in East Asia, there is a rising interest in tradition of Confucianism and reconsider the relevance of Confucianism to global development. This book considers the actual historical significance of Confucianism in the modernization (...) of the three nations in this region, China, Korea, and Japan through the nineteenth century and early twentieth century to the aftermath of the end of World War II. Examining the existing literature dealing with how Confucianism has been viewed in connection with modernization, it provides insight into western attitudes towards Confucianism and the changes in perceptions relative to Asia in the very process of modernization itself. Group contingency (GC) is an effective and cost-efficient strategy that can be successfully applied to technology-based interventions. This study examined the relative effectiveness and cost efficiency of three types of technology-based group contingencies on walking among adults. Seventy two students were divided into teams of three. Each team was randomly assigned to one of three GC conditions (independent, interdependent, or dependent) and underwent 66 days of technology-based group contingency intervention. Sixty five participants completed the intervention and 61 completed the follow-up (...) assessment 2 months later. Step counts and self-reported walking activity increased after the intervention under all three conditions. The proportion of participants that met the target step counts was significantly higher under the dependent group contingency condition. However, 2 months later, intervention effects were not maintained under any condition. For cost efficiency, the increase in step count per point was significantly higher under the interdependent group contingency condition. Group cohesion and social validity (point satisfaction and point utility) were significantly higher under the dependent group contingency condition. Finally, the clinical implications and limitations of this study are discussed. (shrink)

This paper argues that Foucault’s genealogy of racism deserves appreciation due to the highly original concept of racism as biopolitical government. Modern racism, according to Foucault, is not merely an irrational prejudice, a form of socio-political discrimination, or an ideological motive in a political doctrine; rather, it is a form of government that is designed to manage a population. The paper seeks to advance this argument by reconstructing Foucault’s unfinished project of a genealogy of racism. Initially, the paper situates the (...) genealogy of racism within the context of Foucault’s work. It belongs to a period of transition between the mature and the late part of Foucault’s work, more specifically a period of transition from discipline to governmentality. The paper proceeds by reading closely key passages from the 1976 lectures at Collège de France in which Foucault proposes to rethink racism as a form of biopolitical government. While Foucault’s genealogy of racism remains an incomplete project, lacking for example any substantial treatment of European colonialism, the paper proposes to expand the Foucauldian analysis by linking it to the pan-German discourse between 1890 and 1914. Finally, the paper reflects on some of the implications of the Foucauldian analysis, in particular attempts to understand and counter contemporary forms of racism. Foucault’s genealogy of racism, in short, shows us the constructedness of our racialized world and challenges us to develop new and more effective strategies to change it. (shrink)

이 논문의 목적은 주체성의 생산으로서의 규율권력 개념을 강조하면서 ????감시와 처벌????(Discipline and Punish)의 주요 논증을 재구성하는 데 있다. 그것은 감옥의 역사에 대한 푸코의 풍부하면서 세밀한 연구의 일부를 재구성하는 것이다. 이러한 재구성은 푸코의 권력 개념에 대한 스티븐 루크스의 비판적 논의를 배경으로 한다. 이 글은 전반적으로 루크스의 비판적 논점들에 동의하고 있을지라도, 규율권력에 대한 푸코의 개념화와 주체성에 대한 규율권력의 형성적 영향은 현대 미학에서 매우 중요하게 남아 있다. 푸코의 주체화로서의 권력 개념은 저항, 자율성, 효과적인 자기 함양에 관해 의문을 제기하는 것이다.

BackgroundThis study aimed to identify the healthcare providers’ experience and perspectives toward end-of-life care decisions focusing on end-of-life discussion and physician’s order of life-sustaining treatment documentation in Korea which are major parts of the Life-Sustaining Treatment Act.MethodsA cross-sectional survey was conducted using a questionnaire developed by the authors. A total of 474 subjects—94 attending physicians, 87 resident physicians, and 293 nurses—participated in the survey, and the data analysis was performed in terms of frequency, percentage, mean and standard deviation using the (...) SPSS 24.0 program.ResultsStudy results showed that respondents were aware of terminal illness and physician’s order of life-sustaining treatment in Korea well enough except for some details. Physicians reported uncertainty in terminal state diagnosis and disease trajectory as the most challenging. Study participants regarded factors (related to relationships and communications) on the healthcare providers’ side as the major impediment to end-of-life discussion. Study respondents suggested that simplification of the process and more staff are required to facilitate end-of-life discussion and documentation.ConclusionBased on the study results, adequate education and training for better end-of-life discussion are required for future practice. Also, a simple and clear procedure for completing a physician’s order of life-sustaining treatment in Korea should be prepared and legal and ethical advice would be required. Since the enactment of the Life-Sustaining Treatment Act, several revisions already have been made including disease categories, thus continuous education to update and support clinicians is also called for. The aim of this study was to investigate factors associated with seriously ill patients’ preferences for their family and physicians making resuscitation decisions on their behalf. Using SUPPORT II data, the study revealed that, among 362 seriously ill patients who were experiencing pain, 277 (77%) answered that they would want their family and physicians to make resuscitation decisions for them instead of their own wishes being followed if they were to lose decision-making capacity. Even after controlling for other variables, patients (...) who preferred the option of undergoing cardiopulmonary resuscitation (CPR) in the future were twice as likely, and those who had had ventilator treatment were four-fifths less likely, to rely on their family and physicians than those who did not want CPR (odds ratio (OR)-2.28; 95% confidence interval (CI) 1.18-4.38) or those who had not received ventilator treatment (OR-0.23; 95% CI 0.06-0.90). Psychological variables (anxiety, quality of life, and depression), symptomatic variables (severity of pain and activities of daily living) and the existence of surrogates were not significantly associated with patients’ preferences for having their family and physicians make resuscitation decisions for them. Age was not a significant factor for predicting the decision-making role after controlling for other variables. (shrink)
