Americans' appetite for life-prolonging therapies has led to unsustainable growth in health care costs. It is tempting to target older people for health care rationing based on their disproportionate use of health care resources and lifespan already lived, but aged-based rationing is unacceptable to many. Systems reforms can improve the efficiency of health care and may lessen pressure to ration services, but difficult choices still must be made to limit expensive, marginally beneficial interventions. In the absence of agreement on principles (...) to govern health care resource allocation, a fair, open priority-setting process should be created to allow for reasonable disagreement on principles while being seen as legitimate by all stakeholders. At the patient-care level, careful discussions about the benefits and burdens of medical intervention and support for slow medicine — a gentle, family-centered care approach for frail elders — can do much to avoid harming these patients with aggressive yet unwanted medical care while reducing wasteful spending. Mr. M. was a 77-year-old decisionally incapacitated long-term nursing home resident with chronic schizophrenia who was admitted to the hospital with a bacterial pneumonia. His past medical history was notable for deteriorating functional status over the past 2-3 years, urinary retention requiring chronic indwelling bladder catheterization, and two recent hospitalizations for urinary tract infections leading to sepsis. He developed respiratory failure soon after admission and was intubated and placed on mechanical ventilation. According to an editor of The Economist, the world produced, in the years since World War II, seven times more goods than throughout all history. This is well appreciated by lay people, but has hardly affected social scientists. They do not have the conceptual apparatus for understanding accelerated material-technical change and its meaning for people's personal lives, for their ways of relating to them-selves and to the outside world. Of course, a great deal of speculation about emerging life forms in (...) industrialised societies exists and social scientists with a futuristic bend have projected their diverse visions upon public debates, ranging from thc Efficient Hedonism of "post-industrialist' society a la Daniel Bell to the “Responsible Convivialism” of 'post-materialist" critics such as Fritz Schumacher. Competing images of the coming "services society" or "self-service society" share a central concern: the ongoing relation between tile spheres of large organisations and personal lifestyles, between salaried work arid private consumption. They also share a eel-tau' implausibility: few people recognize themselves in either projection. And they sham ubiquitous reference to "technology", without accounting for it in real terms. A good diagnostic of what is actually happening seems to me to be Jonathan Gershuny, who sees a drift toward a particular type of self-service economy: a quite radical shift in the mode of provision of social services, as he calls it, based on new kinds of consumer technologies. Industrialization used to be partial, but is becoming total fast. This process obviously has many facets, the one I am interested in here is the intrusion of modern technology into spheres of life which in the past have been relatively little dependent on it. Die im Reader versammelten Beiträge verstehen sich als Versuche zu einer Soziologie des Visuellen. Sie untersuchen am Beispiel des Mediums Stadtfilm, welche Rolle die dorterzeugten Bilder großer Städte bei der Produktion urbanistischer Repräsentanten spielen. Aus diesem Grund werden insbesondere Übergänge analysiert, die Spielfilme einerseits und urbanistische Diskurse andererseits miteinander verknüpfen. Gemeinsamer Ausgangspunkt ist die These, daß es vor allem Bilder sind, die solche Verknüpfungen gewährleisten. Es wird unterstellt, daß es das Medium Film erlaubt, gerade über den Einsatz von Bildern "näher" (...) an soziale Wirklichkeiten heranzukommen, als dies textzentrierter Praxis empirischer Sozialforschung möglich ist. Stadtfilme erzeugen wirkmächtige Bilder, auf die dann unter bestimmten Bedingungen in urbanistischen Kontexten zurückgegriffen wird. Die Autoren präsentieren konzeptionelle Überlegungen und Filminterpretationen zu Moebius (1991, Regie: Matti Geschonneck), Der Himmel über Berlin (1987, Regie: Wim Wenders), In weiter Ferne, so nah! (1993, Regie: Wim Wenders), Der Bauch des Architekten (1987, Regie: Peter Greenaway) und Hass (1995, Regie: Mathieu Kassovitz). Sie bedienen sich dabei sehr unterschiedlicher Verfahren. Gemeinsam sind ihnen allerdings die Anliegen, die größere Nähe der Filme zu sozialen Welten hervorzuheben und die Übergänge zwischen Stadtfilmen und Urbanistik herauszuarbeiten. -/- City and Film. Attempts at a 'Visual Sociology' Summary The contributions collected in this reader present attempts at a Visual sociology'. Based on a study on the medium 'city film' the author s determine which role the images of big cities evoked in such films can play for urbanistic representations. Therefore the analysis is, in particular, focussed on points of transition marking a connection of movies, on the one hand, and urbanistic discourse, on the othe r. The common point of departure is the hypothesis that images, more than anything else, do guarantee such connections. The authors assume that the medium film may lead 'closer' to social realities than the text- centered practices of empirical social research. City films produce powerful images which, under certain cond itions, can be utilized in urbanistic contexts. The authors present conceptual considerations and interpretations of the following films: Moebius (1991, director: Matti Geschonneck); Der Himmel über Berlin (1987, director: Wim Wenders); In weiter Ferne, so nah! (1993, director: Wim Wenders); The Belly of the Architect (1987, director: Peter Greenaway); and quite different methods they aim at the same goals: emphasizing the greater closeness of films to social worlds and bringing out the points of transition between city films and urbanistics. This article makes the case that place-based and environmental education theory and practice must be responsive to, while attempting to transform, the institutional dynamics of schooling. In the present climate of education in the USA two dynamics of schooling deserve particular attention with respect to the possibilities for place-based and environmental education: the discourse of accountability and the discourse of collaboration. Drawing especially on Foucault's analyses of disciplinary power and governmentality, I show how practices associated with accountability and collaboration limit (...) or preclude the deepening and spreading of place-based and environmental education. However, I also argue that even given the serious limitations imposed by current trends of collaboration and accountability, these trends offer place-based and environmental educators powerful entry points through which to initiate change within the formal school setting. The conclusion of the paper outlines three models of collaboration for place-conscious education through which accountability is redefined in the interest of places. (shrink)

The World Professional Association for Transgender Health’s Standards of Care, now in its seventh edition, is a frequently cited, internationally recognized, evidence-based document that details a comprehensive framework for gender-related care of trans people. However, the WPATH SOC still relies heavily in some cases on gatekeeping practices, dubbed “triadic therapy,” or a process where a trans patient is encouraged to seek out psychotherapy, and hormone therapy, and only then be able to engage in surgical options for transitioning. I use G. (...) Alan Marlatt’s harm reduction framework to argue that the triadic process creates its own set of harms that trans people have to contend with, especially insofar as it focuses on resolving gender dysphoria in a demanding, moralizing, and top-down way as opposed to enriching trans lives by reducing harms that prevent us from flourishing. Using Marlatt’s criterion that harm reduction ought to be bottom-up, low threshold, and not moralizing, I develop a list of suggestions for what ought to be centrally considered in treating trans patients. Scientific creativity, i.e. the production of new and socially effective empirical knowledge, can itself be subjected to empirical analysis. Research on the determinants of creative work in science suggests that the internal and external world of creative scientists is characterised by a series of tensions or "oppositions of forces" which "normally" cannot be integrated. Conversely, creative work in science can be understood as the provision of new answers to scientific problems where conventional answers are no longer sufficient to reduce incongruities (...) between cognitive, affective and social standards of satisfactory problem solution. In this paper I examine the question of whether ethicists are moral experts. I call people moral experts if their moral judgments are correct with high probability and for the right reasons. I defend three theses, while developing a version of the coherence theory of moral justification based on the differences between moral and nonmoral experience: The answer to the question of whether there are moral experts depends on the answer to the question of how to justify moral judgments. Deductivism (...) and the coherence theory both provide some support for the opinion that moral experts exist in some way. I maintain – within the framework of a certain kind of coherence theory – that moral philosophers are 'semi-experts'. (shrink)

When an individual’s action is only one among a large number of similar actions and does not seem to make any difference to the bad collective outcome, can it nonetheless be condemned by act utilitarianism? This question has currently regained interest with papers, e.g., by Shelly Kagan, Julia Nefsky, and Felix Pinkert. Christopher Morgan-Knapp and Charles Goodman answer the question in the affirmative for miniscule emissions in the context of climate change. They use expected utility analysis as Kagan did in (...) consumer ethics. The assumptions about the impact of emissions vary according to some underlying empirical scenarios, all of which are possible. Individual actions might be relevant in the sense of contributing to a mere linear accumulation of emissions; or they might be relevant by leading to an accumulation in the form of crossing thresholds, be it one or several, Finally, such actions might not be relevant at all. To give an answer that solves the problem and that is based solely on expected utility analysis is impossible. Therefore, the view of Morgan-Knapp and Goodman must be rejected. (shrink)

The question of the moral relevance of the individual demand is fundamental to many purchase decisions of daily consumer life. Can a single purchase make a difference for the better or worse? Each individual consumer could argue that companies are unlikely to adjust their production due to one single item more or less being sold. He might therefore decide not to change his consumption behavior but instead to rely on the effort of others, a pattern commonly referred to as collective (...) action problem. In this article, we study collective action problems with regard to everyday purchase situations. We base our discussion on Shelly Kagan’s famous article “Do I make a difference?” and critically discuss a central assumption of his model: the symmetric relationship between supply and demand. We find that Kagan’s solution to collective action problems is not true a priori but has to be evaluated in certain empirical surroundings. We therefore discuss the approach in the context of the European meat market and demonstrate that Kagan’s argument does not provide a universal solution to cases of meat purchasing. We conclude with an outlook regarding the role of consumer ethics. (shrink)

Dabei handelt es sich um die traditionelle und die Kantische Theorie sowie um die Demütigungs- und die Achtungstheorie. Zudem werden zwei Ansätze präsentiert, die Menschenwürde über Relationen zu Menschenrechten erklären. Es soll ermittelt werden, ob einer der Kandidaten das Potenzial hat, einen begründbaren, gehaltvollen, präzisen und gegenüber dem Common-Sense Gebrauch nicht völlig revisionären Begriff der Menschenwürde zu liefern. Die hier (...) verteidigte These wird lauten, dass Menschenwürde als Bezeichnung dafür verwendet werden kann, dass die fundamentalen Menschenrechte einer menschlichen Person in substantieller Weise erfüllt sind.This essay discusses the most debated theories of the concept of human dignity in contemporary ethical discourses. The essay will explore if one of these theories is capable of delivering a justifiable, substantial and precise concept of human dignity that does not demand an entire revision of the concept in respect of its ordinary usage. Die Forschung sucht etwa nach Präparaten, die das Gedächtnis verbessern, das Leben verlängern und Gefühle verändern können. Tierversuche sind bereits erfolgreich. Führt die Befreiung von allen Grenzen von Körper und Geist in eine Katastrophe oder erwarten uns neue Chancen? Das Buch soll die Perfektionierung des Menschen leicht verständlich und philosophisch anspruchsvoll hinterfragen. Dabei werden soziale Folgen von "Enhancement" ebenso thematisiert wie die Gefahren für das Individuum. Droht uns (...) eine Zwei-Klassen-Gesellschaft ungeahnten Ausmaßes oder kann Enhancement gerade dazu dienen, Ungleichheiten der natürlichen Lotterie auszugleichen? Können autonome Individuen durch die Wahl von Verbesserungen ihre Interessen realisieren oder muss man sie paternalistisch schützen? Neben diesen Fragen widmet sich das Buch der Untersuchung der menschlichen Natur: Worin besteht sie und kann sie Grenzen für die Technik diktieren? Abschließend werden die im Buch behandelten Aspekte in der Frage fokussiert, ob es wünschenswert ist, die maximale Lebensspanne des Menschen zu vergrößern. (shrink)

Ist es moralisch verantwortbar, Menschen mit technischen Eingriffen zu verbessern? Der Aufsatz versucht diese Frage zu beantworten, indem zwei Gefahren für Verbesserungswillige beleuchtet werden: der Verlust der Menschlichkeit und der unerwünschte Wandel der individuellen Persönlichkeit. Sodann wird ein „Liberalismus mit Auffangnetz“ als Lösung des Problems vorgestellt, die eine unterschiedliche Bewertung von reversiblen und irreversiblen Eingriffen vornimmt. Im letzten Schritt wird überprüft, wie weit diese Konzeption auch anwendbar ist, wenn Eltern ihre Kinder verbessern lassen wollen, also ein „informed consent“ nicht vorausgesetzt (...) werden kann. Dabei werden Argumente zurückgewiesen, die Habermas gegen eine Verbesserung von Kindern vorgebracht hat. (shrink)

This essay sketches out what a utilitarian should support when considering global warming along with what measures can be recommended to political leaders for utilitarian reasons. If we estimate the utility of the great advantages that any ambitious climate policy might create in the name of poverty reduction in the present, I will show how a decision can be made in favor of a vigorous climate policy based on such estimates. My argument is independent of the truth of the claims (...) of climate sceptics. Until now, the debate has neglected the double effects of a vigorous climate policy that not only avoids risks of damages but also creates utility. In conclusion, three strategies of a climate policy legitimized by utilitarianism will be introduced: (1) Utilitarianism favors global emissions trading in comparison with a global CO2 tax. (2) Utilitarianism calls for this trade to be introduced while, at the same time, investments in renewable energies should be increased. (3) Furthermore, utilitarianism favors a policy that aims to slow down the rate of population growth. (shrink)

Zusammenfassung. Um die normative Frage nach der Zulässigkeit von aktiver und passiver Sterbehilfe diskutieren zu können, muss man zuerst klären, worin genau aktive und passive Sterbehilfe bestehen. Dass man hier eine nicht moralisch motivierte Unterscheidung auf der Basis eines handlungstheoretischen Unterschieds von Tun und Unterlassen treffen kann, wird vielfach bezweifelt. In diesem Aufsatz soll eine solche rein deskriptive Unterscheidbarkeit verteidigt werden. Dazu wird die von Dieter Birnbacher in seinem Buch „Tun und Unterlassen” entwickelte Theorie vorgestellt und verfeinert, was in einer (...) neuen Definition aktiver und passiver Sterbehilfe mündet.
