In controlled human infection studies (CHIs), participants are deliberately exposed to infectious agents in order to better understand the mechanism of infection or disease and test therapies or vaccines. While most CHIs have been conducted in high‐income countries, CHIs have recently been expanding into low‐ and middle‐income countries (LMICs). One potential ethical concern about this expansion is the challenge of obtaining the voluntary informed consent of participants, especially those who may not be literate or have limited education. In some CHIs (...) in LMICs, researchers have attempted to address this potential concern by limiting access to literate or educated populations. In this paper, we argue that this practice is unjustified, as it does not increase the chances of obtaining valid informed consent and therefore unfairly excludes illiterate populations and populations with lower education. Instead, we recommend that investigators improve the informed consent process by drawing on existing data on obtaining informed consent in these populations and interventions aimed at improving their understanding. Based on a literature review, we provide concrete suggestions for how to follow this recommendation and ensure that populations with lower literacy or education are given a fair opportunity to protect their rights and interests in the informed consent process. Social norm transgressions are assumed to be at the root of numerous substantial negative outcomes for transgressors. There is a prevailing notion among lay people and scholars that transgressing social norms can negatively impact one’s mental health. The present research aimed to examine this assumption, focusing on clinically relevant outcomes such as anxiety and depression. The present research further aimed to examine a social cognitive process for these outcomes in the form of fear of negative evaluations as a result of (...) one’s norm transgressing behavior. Specifically, it examined whether it is negative evaluations about ourselves or about those close to us that mediates the effect of social norm transgressions, and whether those may vary as a function of culture. Results of the present research, including a study with a community sample, suggest a positive association between social norm transgressions and psychological distress. Results also suggest that increased fear of negative evaluation mediates that association but does so differently for people from more collectivistic cultures and people from less collectivistic cultures. For people from more collectivistic cultures increased fear of negative evaluation of close others may mediate the association between social norm transgressions and psychological distress. However, for people from less collectivistic cultures that association may be mediated by increased fear of negative evaluation of oneself. Implications for research on consequences of social norm transgressions and cross-cultural differences in perceptions of such consequences are discussed as are practical implications for motivating social norm adherence and the maintenance of constructive social norms. Conventional medical training offers little help to students to resolve the ethical dilemmas they face as healthcare professionals. Public awareness of the ethical behavior of medical practitioners has been growing. Aim of this study was to assess knowledge of, practice in and attitudes of healthcare ethics among medical, dental and physiotherapy postgraduate students. A cross-sectional analysis based on a questionnaire was performed at a hospital and dental institution of the medical college. The present study included 60 postgraduate students. The questionnaire (...) on health care ethics is a 44-point self-administered standardized questionnaire that poses ethical dilemmas in 5 vignettes with closed-ended questions to test awareness, attitude and experience in the area of healthcare ethics. In the first half of the questionnaire, knowledge and attitude were assessed using 5 case scenarios representing ethical dilemmas and the participants answered the questions using the 5 point likert scale. The second half assessed the practice of Ethics in research and clinical practice. The expected time to fill each questionnaire was about 10–15 min. 62 questionnaires were distributed. 60 completed questionnaires were returned. The response rate was 96%. The mean age of the postgraduates was 26.23 ± 2.33. Among the participants 28 were male and 32 female. 20 completed questionnaires from each, medical, dental and physiotherapy postgraduate students were collected. The multiple group comparison showed that the difference in the knowledge scores were statistically significant between dental and physiotherapy and between medical and physiotherapy but no significant difference between medical and dental. Study suggests that abstract knowledge cannot be converted into practical knowledge automatically. There is a major lack of knowledge of the ethics of health care among graduate students. The postgraduate curriculum must include a good mix of both theoretical concepts and practical application of multidisciplinary healthcare ethics ethical principles and guidelines. (shrink)

Review of 'Peace or Perish' by J P Vaswani published by Gita Publishing House.

In recent years, the National Crime Records Bureau recommendation is that the growth rate of crime against women has skyrocketed in India, even higher than the population growth rate. According to lawyer, Kamlesh Vaswani, the commercial exploitation of coital activity paramount in pornography is the result of crimes against women, and fills perverse traits in the roots of society. Following that, he filed a petition (2013) in the Honourable Supreme Court to blanket ban pornography with the aim of diluting (...) the subordination of women and the crimes against women in society. Taking this into consideration, I argue that the Vaswani petition draws back the issue of twentieth century Western political and philosophical debate on pornography which triggers one of the moot problems of moral legislation—whether the state should be neutral or not towards individuals’ preferences, merits, desires, and the status to live well. This article focuses on whether this discourse has any relevance in the Indian value system. In this article, through the comparison between Indian and Western values, I make an attempt to analyse the moot problem of moral legislation which is bridging the gap between public and private morality for the well-being of women as well as our society. Oxford and New York: Oxford University Press, 2002. xv + 303 pp.At the Parliament of World Religions held in Cape Town in 1999, Dada Vaswani, a leading spiritual voice within India, proclaimed that the nineteenth century brought the liberation of slaves, that the (...) twentieth century brought the liberation of women, and that the twenty-first century will bring the liberation of animals. For Paul Waldau, a key piece in this transformation will result from the insights of ethology, the specialized branch of biology that offers new information regarding the consciousness of animals. Scientific research has revealed that animals occupy a complex world, replete with desires and emotions. Based on this scientific research, Waldau asserts that a new morality will arise that acknowledges the inherent moral standing of animals, and will overturn the long-held view that animals serve only one purpose: to serve humanity. Otherwise, he asserts, humans will remain enmeshed in the tyranny of speciesism.In this meticulous study, Paul Waldau assesses the origins of the term "speciesism," coined by Ryder in 1970. He explores the link between speciesism and slavery, colonization, and the oppression of women and children. Just as ethical discourse has been altered through history regarding these issues, Waldau suggests that as more information becomes available, human attitudes regarding animals will evolve. However, due to entrenched religious views, this change will be gradual. This book consists of a sustained argument for expanding our categories of worth beyond purely anthropocentric values. Waldau suggests that although humans hold a power advantage over animals, "[d]ifferences in abilities, and even hierarchy, to the extent they exist, do not automatically imply the propriety of dominance, let alone tyranny" (p.39).To develop his argument, Waldau writes about three higher-order mammals that merit consideration: the great apes, elephants, and cetaceans (whales and dolphins). Each of these groups of animals display complex abilities and have, in proportion to the size of their bodies, large brains. Genetically, chimpanzees and humans share 98.4% identical DNA. Chimpanzees can develop high language functioning and use [End Page 293] tools, subverting the assumptions that these two capabilities are uniquely human. In regard to elephants, Waldau writes that "[b]oth Aristotle and Pliny designated elephants as the animals closest to humans in intelligence" (p. 79). He particularly notes their highly developed matriarchal social structure and their ability to communicate over long distances. Similarly, Waldau summarizes recent research on cetaceans' ability to protect one another through echolocation and a highly intricate communication system based on touch and soundings. Because of this scientific resarch, Waldau asserts that Descartes was wrong to proclaim that "dumb animals do not think" (p. 89).Waldau's underlying argument asks that humans not "lump" all animals together under one umbrella category, but assess each group of animals according to its own gifts. He asks that we not regard humans as utterly unique (and hence better), but pay attention to current scientific research and ethical insights that value animals for their inherent worth. Though Waldau does not attempt to establish a new hierarchy of more-valued creatures, he does argue persuasively that great apes, elephants, and the cetaceans deserve greater respect, and that we need to re-examine and hopefully deconstruct the inherited cultural understanding of all animals as simply "other."This brings us to the primary focus of the book: an investigation of the place of animals in Buddhist and Christian traditions. In Buddhism, animals suffer their fate because of negative karma, and, at best, can hope to be reborn as a human being capable of receiving the Buddha's teachings. Waldau cites various texts, from the Jataka tales to the Mahayana sutras, that accord conscience and agency to specific animals. He claims, however, that Buddhist cultures have generally not gone out of their way to cultivate a culture of animal benevolence, citing the example of elephant trainers in Southeast Asia.
