RESUMEN Introducción: El proceso de cuidar es el resultado de una construcción propia de cada situación, se origina con la identificación de los problemas de salud y las necesidades reales o potenciales de las personas, familia y comunidad que demandan cuidado. Objetivo: Evaluar el impacto del Diplomado Prevención del Síndrome de Burnout, en el autocuidado del personal de enfermería de la Atención Secundaria de Salud de la provincia Camagüey. Métodos: Se realizó un estudio descriptivo observacional, en el Hospital Amalia Simoni (...) Argilagos de Camagüey, durante el Diplomado Prevención del Síndrome de Burnout dirigido al personal de enfermería de la Atención Secundaria de Salud. Para ello se aplicó una encuesta y el Test Psicológico de Maslach y Jackson al inicio y al final de la aplicación del Diplomado. Los datos aportados por estos instrumentos se complementaron con los provenientes de la aplicación de la técnica del PNI. Resultados: Predominó el sexo femenino. El total de los cursistas son licenciados en Enfermería. Con la ejecución del diplomado se constatan cambios importantes en cuanto a la autopreparación, el rendimiento, los hábitos de descanso, satisfacción personal y realización de ejercicios físicos. Discusión: La aplicación del diplomado logró transformar estilos de trabajo, afrontamientos al estrés, así como una mejor satisfacción personal y colectiva. Objective: To evaluate the impact of Postgraduate Course Prevention of the Syndrome of Burnout, in the self-care of the personnel of infirmary of the Secondary Attention of Health of the Camagüey province. Methods: a descriptive, observational study was carried out in the Hospital Amalia Simoni Argilagos of Camagüey, during Postgraduate Course Prevention of the Syndrome of Burnout directed to the personnel of infirmary of the Secondary Attention of Health. For that reason, a survey and the Psychological Test of Maslach and Jackson was applied at the beginning and at the end of the application of the course. The information contributed by these instruments complemented itself with the originated ones from the application of the technique of the PNI. Results: The sex that predominated was the feminine one. All of the students are licensed in Infirmary. With the execution of the course important changes are stated as for the self-preparation, the yield, the habits of rest, personal satisfaction and achievement of physical exercises. Discussion: The application of the course managed to transform styles of work, confrontations to the stress, as well as a better personal and collective satisfaction. (shrink)

Speaking of the public response to the deaths of children at the Bristol Royal Infirmary before 2001, the BMJ commented that the NHS would be ‘all changed, changed utterly’. Today, two inquiries into the Mid Staffordshire Foundation Trust suggest nothing changed at all. Many patients died as a result of their care and the stories of indifference and neglect there are harrowing. Yet Bristol and Mid Staffordshire are not isolated reports. In 2011, the Health Services Ombudsman reported on the (...) care of elderly and frail patients in the NHS and found a failure to recognise their humanity and individuality and to respond to them with sensitivity, compassion and professionalism. Likewise, the Care Quality Commission and Healthcare Commission received complaints from patients and relatives about the quality of nursing care. These included patients not being fed, patients left in soiled bedding, poor hygiene practices, and general disregard for privacy and dignity. Why is there such tolerance of poor clinical standards? We need a better understanding of the circumstances that can lead to these outcomes and how best to respond to them. We discuss the findings of these and other reports and consider whether attention should be devoted to managing individual behaviour, or focus on the systemic influences which predispose hospital staff to behave in this way. Lastly, we consider whether we should look further afield to cognitive psychology to better understand how clinicians and managers make decisions? (shrink)

‘Organisational failure’ is central to medical ethics. In the National Health Service (NHS), we usually examine failures at hospital level. We have had around 100 hospital inquiries since the first in 1969, into Ely Hospital, Cardiff. This year, we had the Ockenden Report into Shrewsbury and Telford Hospital. Last year, we had the Outram Inquiry into West Suffolk Hospital. In 2020, the James Inquiry into Ian Paterson. And, before that, Morecombe Bay, Gosport War Memorial, Mid Staffordshire, Liverpool Community Health, Winterbourne (...) View, Anthony Ledward, Stoke Mandeville, Bristol Royal Infirmary and so on. As Taylor says, these reports teach us clinical failure usually has an institutional setting and that blaming individuals overlooks working ‘culture’. They have also led to: (a) new regulatory bodies to manage, investigate and improve NHS standards, (b) new codes of conduct to mandate reporting of serious accidents—backed up by the Whistle-blower’s Act and explicit statutory duties of disclosure, (c) root cause analysis to encourage consistent investigations so we can learn from reliable data and (d) new ‘Speaking Up Guardians’ and a Patient Safety Commissioner to encourage informal discussion about patient safety. This article examines the St. James Infirmary, a nonprofit occupational health and safety clinic for sex workers in San Francisco, to consider how particular organizational spaces and practices may challenge gender ideologies in the United States—in this case, of women sex workers as “victim-criminals.” Drawing empirically from multimethod qualitative research and theoretically from feminist institutionalism, I indicate how the SJI’s broader institutional context has produced a victim-criminal ideology of women in prostitution. Next, I consider the SJI’s organizational emergence and (...) operations to argue that, by deploying particular spatial-discursive practices and operational procedures, nonprofits with legacies of activism may draw from these to challenge dominant gender ideologies, even as they work alongside the broader institutional structures that promote them. Although single case studies like the SJI cannot establish broad theoretical generalizations and propositions, I use it to build knowledge and highlight important lessons about nonprofits, gender, and institutional change. In 1736 Colin MacLaurin, Professor of Mathematics in the University of Edinburgh petitioned the Town Council for permission to erect an astronomical observatory in the College to broaden the research and teaching base of the University. After MacLaurin's death, the Town Council and University Senate, more concerned with the promotion of the Infirmary and associated medical teaching, took no further action. The funds raised by MacLaurin were lent to his successor, and largely dissipated. In 1776 the balance was transferred (...) to the proposed Calton Hill Observatory. This private scheme was to be financially self-sufficient, generating income from the casual visitor and fees charged to students. It too failed. Funds were wasted on unnecessary architectural elaboration, the Town Council failed to meet their financial obligations, the University acquiesced in the total inactivity of the Regius Professor of Astronomy, whilst the commercial management-base of the Observatory vitiated any serious scientific activity. Na busca da cura do corpo, a oração opera milagres: uma discussão sobre eficácia simbólica, perspectivismo, cura e religião (In the search for the healing of the body, prayer works miracles: a discussion on symbolic efficacy, perspectivism) - DOI: 10.5752/P.2175-5841.2013v11n31p965 O artigo busca descrever, compreender e interpretar o caso observado de uma jovem portadora de teratoma ovariano maligno, internada em hospital do câncer na cidade de Belém, estado do Pará. A jovem foi desenganada diante do diagnóstico da doença, mas sua (...) mãe agarrou-se na “fé em Deus”, pois “Ele opera milagres”, dizendo que dela retiraram “coisa feia”, cheia de dentes, cabelos e gorduras, que não sabia inteiramente explicar, mas que “Jesus cura”. A mãe, evangélica, mantinha a Bíblia na enfermaria, orando em voz alta diante do leito da menina. Aos poucos, amenizou-se a gravidade do caso, deixando perplexo o saber biomédico, diante do reconhecimento de outras buscas de cura, como através da oração, que levou aquela jovem a sair de alta hospitalar. Esse episódio, observado atentamente pela coautora deste artigo, permite uma discussão que transcende o âmbito da biomedicina, do ponto de vista antropológico, especialmente no campo da chamada “eficácia simbólica”, em que xamanismo e psicanálise podem ser chamados para conferir sentido a esse drama, cujo desfecho mostrou-se favorável para a jovem e sua mãe, até o ponto que foi possível observar. Eficácia Simbólica.This article aims to describe, understand and interpret the observed case of a young woman with ovarian malignant teratoma, hospitalized in the cancer hospital in the city of Belém, State of Pará, Brazil. The young woman was thought as incurable on the diagnosis of the disease, but her mother clung in “faith in God”, because “He works miracles”, saying that “ugly things”, full of teeth, hair and grease, were removed from her; she also said that she could not understand it all, or full explain it, but she is convinced that “Jesus heals”. The mother, an Evangelical woman, held the Bible in the infirmary, praying out loud in front of the girl’s bed. Gradually it was alleviated the severity of the case, leaving bewildered the biomedical knowledge, on the recognition of other results, such as healing through prayer, which led that young woman to leave the hospital. This episode, closely observed by one of the authors of this article, allows a discussion that transcends the scope of biomedicine, from the anthropological point of view, especially in the field of the so-called “symbolic efficacy” (Lévi-Strauss), in which shamanism and psychoanalysis can be called to give meaning to this drama, whose outcome was favorable to the young woman and to her mother, until the point that it was possible to observe. RESUMEN Se aplicó una intervención socio sanitaria en el asentamiento rural El León de Camagüey basada en los resultados de un estudio anterior, desde la comunicación social, educativa, así como aspectos socioculturales evaluados que permitieron la realización de este estudio. Su objetivo fue implementar una estrategia de intervención basada en acciones sociales y educativas colectadas en un manual que organizó contenidos de antropología socio cultural, psicología, sociología de la salud y trabajo social comunitario, la que fue conducida por profesionales de (...) enfermería como prestadores, cuyo encargo social les asigna una mayor permanencia e intercambio con los pobladores, potenciándose el trabajo comunitario a partir de febrero de 2016,como etapa de sostenibilidad. La investigación constituyó un diseño mixto, con un estudio cuasi experimental sin grupo control combinado con métodos cualitativos. La evaluación final efectuada en el periodo 2017-2018 exhibió modificaciones positivas en indicadores de las historias clínicas familiares de los pobladores, repercutiendo en el Análisis de la Situación de Salud, realizado en abril de 2018 y el entorno comunitario. Se demostró la importancia de la aplicación de la estrategia de intervención comunitaria desde las disciplinas de las ciencias sociales, por el personal de enfermería como agente de cambio de desarrollo local y protagonista de sostenibilidad. ABSTRACT A social sanitary intervention was applied in the rural establishment El León of Camagüey, based on social and educational actions collected in a manual that organized social cultural anthropology contents, psychology, sociology of the health and community social work, which was led by infirmary professionals like lenders, whose social order assigns a major permanence and exchange of them with the settlers. The investigation constituted a mixed design, with a study quasi experimentally without control group combined with qualitative methods. The final evaluation carried out in the period 2017-2018 exhibited positive modifications in indicators of the familiar clinical history of the settlers, what reverberated in the Analysis of the Situation of Health, carried in April, 2018 and the community environment. It was demonstrated the importance of the application of the strategy of community intervention from the disciplines of the social sciences, by the infirmary personnel as change agent of local development and protagonist of sustainability. This article seeks to identify a 'problem' in the interaction between medical law and ethics, which is that neither fully appreciates how the other works. In particular, it argues that medical law has not only failed to formulate a consistent conception of the role that medical ethics performs, but it does not adequately differentiate between categories of medical ethics discourse. Consequently, the ethical content of a case, if identified at all, will not be dealt with in a consistent manner. The (...) article further argues that medical ethics does not recognize the power that law gives to it, and that the result is a regulatory vacuum. It then uses the findings of the Bristol Royal Infirmary Report to demonstrate, by analogy, how this may lead to problems. (shrink)

Three medical practitioners were accused of serious professional misconduct relating to 29 deaths in 53 paediatric cardiac operations undertaken at the Bristol Royal Infirmary between 1988 and (...) 1995. All three denied the charges but, after 65 days of evidence over eight months , all three were found guilty." "The doctors concerned are Mr James Wisheart, a paediatric and adult cardiac surgeon and the former Medical Director of the United Bristol Healthcare Trust; Mr Janardan Dhasmana, paediatric and adult cardiac surgeon; and Dr John Roylance, a former radiologist, and Chief Executive of the Trust." "The central allegations were that the Chief Executive and the Medical Director allowed to be carried out, and the two paediatric cardiac surgeons carried out, operations on children knowing that the mortality rates for these operations, in the hands of these surgeons, were high. Furthermore, the surgeons were accused of not communicating to the parents the correct risk of death for these operations in their hands.1"Mr Wisheart and Dr Roylance were subsequently struck off the medical register. Mr Dhasmana was disqualified from practising paediatric cardiac surgery for three years. The article presents, analyzes and systematizes the sources on constituent elements of educational Jesuit Colleges of the East Slavic region in the late 16th - early 17th centuries. The author tabulates the data about the period of creation or functioning of Jesuit educational facilities and their components (missions, residences, schools, collegiums, "bursa", music "bursa", "konvikty", seminaries, houses of the third probation, theaters, student and philistine fraternities, churches, chapels, pharmacies, infirmaries, hospitals, orphanages, libraries, printing houses) in the twenty-seven cities of region (...) mentioned in the research. The author analyzes the purposes of "bursa" and "music bursa", devoted to music teaching and singing, where students performed during religious holidays and other events. But the main purpose was the support of the poor youth. The article deals with the specific facilities - "konvikty" for gentry with student cohabitation in the flats of locals or individual facilities. The paper provides information about seminaries of order of Jesuits in the East Slavic region, which were an important component of Jesuit educational system of priest training, those who spread Catholicism among different stratums. The author analyzes "Sodalicii Marian" (Marian Congregations) of collegiums' students (organization, methods of skills and knowledge improvements, individual live events of fraternity members). The article shows that Marian Congregations had similar organizational elements to the order of Jesuits. For example, Society of Jesus prepared not only faithful Catholics, but also its own new members. The author mentions the theaters of Jesuit collegiums, their specific purposes as an effective way of spreading Catholicism among Orthodox population. The article also deals with educational and training purposes of churches, libraries, orphanages and hospitals of Society of Jesus. The author states the ways of implementing and funding certain components of the aforementioned colleges and examines their role in the educational system of the Order. (shrink)

RESUMEN La discapacidad que afecta a la población infantil constituye un problema emergente; diagnosticarla y tratarla de manera integral es parte del trabajo diario del profesional de Enfermería en la atención primaria de salud. Durante el estudio se identificaron carencias en las habilidades profesionales de los licenciados en Enfermería respecto a este saber. Se aplicó un entrenamiento encaminado al desarrollo de habilidades profesionales especializadas para la atención integral a la discapacidad infantil en este personal. Por ello el objetivo del presente (...) trabajo está dirigido a exponer los resultados del entrenamiento. Los métodos y técnicas empleados incluyeron la observación científica, encuesta, entrevista, la técnica de grupo focal y el criterio de expertos. El entrenamiento se sometió al criterio de expertos. Se corroboró su impacto positivo como una vía importante para la preparación y desarrollo de habilidades profesionales en la muestra seleccionada. ABSTRACT The disability that affects the children population constitutes an emergent problem; to diagnose it and to treat it as an integral way is a part of the daily work of the professional of Infirmary in the primary health care. During the study, lacks in the professional skills of the Bachelors of Infirmary with regard to this know ledge were identified. There was applied a training directed to the development of specialized professional skills for the integral attention to the infant disability in this personnel. That is why the objective of the present work is directed to exhibit the results of the training. The methods and techniques used included the scientific observation, polls, interviews, the technique of focal group and the experts' criteria. The training was surrendered to the experts' criteria. It was corroborated its positive impact as an important route for the preparation and development of professional skills in the chosen sample. (shrink)

“Big Data” in health and medicine in the 21st century differs from “Big Data” used in health and medicine in the 1700s and 1800s. However, the old data sets share one key component: large numbers. The term “Big Data” is not synonymous with large numbers. Large numbers are a key component of Big Data in health and medicine, both for understanding the full range of how a disease presents in a human for diagnosis, and for understanding if one treatment of (...) a disease is better than another treatment or better than just leaving the patient on his or her own without therapy. In this paper, we examine the first considerations of Big Data in medicine in Paris in the early 1800s when urologic surgeon Jean Civiale collected the first large numbers. Civiale collected the large numbers to defend the efficacy of his urologic instrument, the lithotrite, and the surgical procedure he developed, lithotrity, for the removal of bladder stones compared with earlier, more invasive surgical approaches. We examine how large numbers were adjudicated in social decision-making in the Académie des sciences, Paris, when a dispute arose among French urologic surgeons about the importance of large numbers in surgical science. After Civiale’s successful defense of his instrument and procedure in Paris, we examine how his approach to Big Data impacted data collection by George Buchanan in his use of the procedure at the Royal Hospital Infirmary in Glasgow. Leeds Teaching Hospitals NHS Trust v Mr & Mrs A & othersThe decision of Dame Butler-Sloss in the case of Leeds Teaching Hospitals NHS Trust v Mr & Mrs A & Others1 has already achieved notoriety. The disturbing facts have received widespread media coverage and fuelled further legal and political debate surrounding the “reproductive revolution”.2 This short paper assesses the context and potential implications of the recent decision and, in particular, the nature and meaning of parenthood and the family.THE CASETwo (...) couples, referred to Mr and Mrs A and Mr and Mrs B, were attending the assisted conception unit at Leeds General Infirmary to receive infertility treatment. Both couples understood that they were participating in a technique called intracytoplasmic sperm injection , a technique which would use the eggs of the wife and the sperm of the husband in each case. Mrs A had consented to her eggs being mixed with her husband’s sperm to create an embryo which would then be implanted in her, hopefully resulting in pregnancy. Mr A had consented to his wife’s treatment. Mr and Mrs B gave their consent to similar IVF procedures, although Mr B had expressly refused to consent to his sperm being used in treating others.Following a mix up at the unit, which Butler-Sloss referred to as “a ‘mistake’ … despite its inadequacy as a description of what occurred”, Mr B’s sperm was injected into Mrs A’s eggs. She became pregnant and later gave birth to healthy twins. The mistake became apparent only on Mrs A’s delivery of twins of mixed race; Mr and Mrs A are both white, Mr and Mrs B both black. McCullough (bio)Those of us who work in the field of bioethics tend to think that, because the word “bioethics” is new, so too the field is new in all respects, but we are not the first to do bioethics. John Gregory (1724–1773) did bioethics just as we do it, at least two centuries before (...) we thought to do it (Gregory 1772). He deployed philosophical methods as sophisticated as our own. Indeed, Gregory took up the very best moral philosophy available to thinkers of the Scottish Enlightenment, namely, David Hume’s moral philosophy and its core concept of sympathy. Gregory also responded in a conceptually powerful and clinically applicable way to the problems of his time, just as we do. I want here to outline Gregory’s accomplishment and to identify some aspects of its importance for bioethics in the twenty-first century.Gregory’s “Problem List”Gregory wrote his medical ethics as a response to the then current state of medicine in Britain. We now follow Gregory in our writing of bioethics and clinical ethics in response to the problems of our time. A great deal that we now take for granted did not then exist. 1There was not, as there is now, any uniform pathway into medicine. Nor did there exist universal licensure. The Royal Colleges did attempt to assert, but failed to achieve, monopoly control of medical practice. The concepts of health and disease were themselves contested and therefore competed for their success in the market place. There was a marked oversupply of practitioners who competed fiercely—very fiercely indeed—in the medical market place for their concepts of health and disease, their treatments, and therefore their livelihoods. Patients had their own concepts of health and disease, engaged in “self-physicking” or self-care, and often traded physician’s prescriptions.Medicine exhibited little scientific discipline in its accounts of disease and in determining the efficacy of treatments, a fact that Gregory (1743) laments as a [End Page 329] medical student. There was no marked improvement when he began to give his medical ethics lectures nearly a quarter of a century later. Treatments failed as often, perhaps more often, than they succeeded in benefiting patients. The sick usually sought out the help of a physician after trying self-physicking.Physicians, Gregory taught, could not hope to control human biology, though they could aim to manage its processes well. When nature underresponds to disease the physician should assist her processes; when nature overresponds, the physician should tamp down nature’s responses, to lessen their “violence”—in both cases always attentive to the limits of medicine’s capacities in treating disease.Physicians attended the well-to-do sick at home, and the sick person summoned and dismissed physicians, surgeons, or apothecaries at will. A physician therefore might find himself—no women had yet been admitted to the ranks of university-trained physicians—summoned before, after, or simultaneously with a competitor, with his concepts and diagnosis and treatment put to the acid test. In this setting, there existed only a patient-physician relationship, not a physician-patient relationship.Physicians left off the care of dying patients, a practice that was made a matter of duty by Friederich Hoffmann (1749). One would suffer punishing economic consequences if one had a high mortality rate. Better, then, to label the patient incurable, withdraw, and turn matters over to clergy. Whistleblowing in the NHS engages the fundamental right to free speech and as the paper makes clear is a means of ensuring individual patient safety. The discourse around the whistleblower is not simply about individuals being safeguarded if they blow the whistle on poor standards of patient care but that they may indeed be obliged as healthcare professionals to positively make the decision to blow the whistle to raise concerns. As noted in the paper, the NHS is an organisation in (...) which many—though not all—individuals who work there are bound by their own professional codes of practice in addition to broader employment requirements in relation to ethical and safety issues. But having an obligation to raise concerns and actually doing so are very different things. Individuals who blow the whistle have often found that following their conscience comes with consequences. They may ultimately be vindicated, as, for example in the 1990s, Stephen Bolsin the anaesthetist who blew the whistle in relation to Bristol Royal Infirmary in the 1990s.1 But getting such vindication can in many instances be over a long and very painful period of time and be exceedingly costly in both financial and emotional terms. Speech is a primary means of human communication and one of the most basic features of human conduct. Voice is an important part of its subsystems. A speech disorder is a condition that affects the ability of a person to speak normally, which occasionally results in voice impairment with psychological and emotional consequences. This study highlights the (...) following issues: recent studies, methods of voice pathology detection, machine learning and deep learning (DL) methods used in data classification, main datasets utilized, and the role of Internet of things (IoT) systems employed in voice pathology diagnosis. Moreover, this study presents different applications, open challenges, and recommendations for future directions of IoT systems and artificial intelligence (AI) approaches in the voice pathology diagnosis. Finally, this study highlights some limitations of voice pathology datasets in comparison with the role of IoT in the healthcare sector, which shows the urgent need to provide efficient approaches and easy and ideal medical diagnostic procedures and treatments of disease identification for doctors and patients. This review covered voice pathology taxonomy, detection techniques, open challenges, limitations, and recommendations for future directions to provide a clear background for doctors and patients. Standard databases, including the Massachusetts Eye and Ear Infirmary, Saarbruecken Voice Database, and the Arabic Voice Pathology Database, were used in most articles reviewed in this article. The classes, features, and main purpose for voice pathology identification are also highlighted. This study focuses on the extraction of voice pathology features, especially speech analysis, extends feature vectors comprising static and dynamic features, and converts these extended feature vectors into solid vectors before passing them to the recognizer. У статті проаналізовано та узагальнено відомості про діяльність аптек, лікарень та інфірмерій ордену єзуїтів у східнослов'янському регіоні в останній третині XVI - першій половині ХVІІ ст. Представлений, проаналізований і систематизований матеріал щодо закладів лікувального призначення Товариства Ісуса, які обслуговували мешканців єзуїтських домів, а також населення краю. За допомогою архівних та опублікованих історичних джерел відмічені способи впровадження й фінансування певних аптек, інфірмерій і лікарень, розглянуто їхнє місце в системі домів ордену. З'ясовано, що ці заклади мали не лише внутрішньоорденське значення, виконували певні (...) функції в системі домів, але й відігравали певну роль у громадському розвитку регіону, поширювали благодійну справу й соціальні інститути.
