Engagement with genomic medicine and research has increased globally during the past few decades, including rapid developments in Sri Lanka. Genomic research is carried out in Sri Lanka on a variety of scales and with different aims and perspectives. However, there are concerns about participants' understanding of genomic research, including the validity of informed consent. This article reports a qualitative study aiming to explore the understanding, knowledge, and attitudes of the Sri Lankan public towards genomic medicine and to inform the (...) development of an effective and appropriate process for informed consent in that setting. Purposive sampling was employed. Participants were recruited from a sub-group of the public in Colombo, Sri Lanka who had either consented or refused to donate genetic material for a biobank. Data were collected using face-to-face semi-structured interviews. Interview data were transcribed verbatim and translated into English. Conventional content analysis was used. The analysis developed three key themes: a) ‘Scientific literacy’ describes an apparent lack of scientific knowledge that seems to affect a participant’s ability to understand the research, b) ‘Motivation’ describes narratives about why participants chose (not) to take part in the research, despite not understanding it, and c) ‘Trust’ describes how trust served to mitigate the apparent ethical deficit created by not being fully informed. In this article, we argue that informed trust is likely an acceptable basis for consent, particularly in settings where scientific literacy might be low. However, researchers must work to be worthy of that trust and ensure that misconceptions are actively addressed. (shrink)

The subject matter of Ward’s book is Kant’s view of the nature of morality. Its object is to show that there is a development in Kant’s view, not only from the precritical to the critical stage of his thought, but also within the critical stage of his thought. The occasion for writing this book, it should be noted, is the prevalence of "verdicts" regarding Kant’s view of the nature of morality formed by British scholars on the basis of an acquaintance (...) of only the major critical works of Kant. According to Ward, an adequate understanding of Kant’s view of the nature of morality can come only from a reading of all of Kant’s works, most especially from a reading of his final, posthumously published, notes. In these notes, Kant equates God with practical reason. The significance of this equation is that it finally provides a stable solution to the problem of the foundations of morality, a problem which had occupied Kant since the beginning of his philosophical career. Kant’s precritical works, as well as the critiques, provide only unstable solutions. In his pre-critical works, Kant oscillates between seeking the foundations in knowledge and feeling. In his critiques, Kant seems to find a stable solution by conceiving reason as the source of principles, both theoretical and practical. The instability of this solution lies in the fact that Kant retains, along with the formal principles, a morality the source of which is religion. In the critiques, however, Kant is unable to conceive religion as anything more than a supplement to morality. Not until his final notes, Ward argues, does Kant recognize this instability and rectify it by giving religion an integral place in his conception of morality.—W. A. S. (shrink)

The fulfillment of the pedagogical intention of this work takes the form of developing a specific conception of metaphysics. The author’s conception is not the original one in terms of the object of metaphysics, but one that encompasses as well the various philosophical developments that affect the original conception of it, including critiques of it. The author conceives metaphysics in terms of "motives" of thought, originally articulated by Plato, namely, 1) the knowledge of principles of being; 2) the standard of (...) true knowledge; and 3) the way in which this knowledge is attained. According to the author, the first definitive treatment of the first of these themes is given by Aristotle, in the conception of the essences of things as the principles of being. In subsequent treatments, these themes undergo various transformations, which are presented by the author as being the result of critical reflections on prior treatments. In fact, they are the result of shifts in perspective, the most fundamental of which is, as the author himself notes, the introduction of perspectivity as a category in the sense of a condition of knowledge. The author traces the function of this category in the thought of various philosophers from Augustine to Nietzsche, suggesting, though not explicitly establishing, its theological origin. (shrink)

The subject of the first two essays is Hegel the man: the essays are contributions to Hegel’s biography and psychography, and are of little philosophical interest. The subject of the remaining essays is Hegel’s work, at least that part of it which is judged by the contributors (...) to this volume to be of more than historical interest. This is primarily the practical-philosophical part of Hegel’s work. Specifically, the essays deal with Hegel’s philosophy of history ; his philosophy of religion ; and his philosophy of law. Objective: To review the principles and practice of the use of conscious sedation for IVF.Design: The pertinent literature was reviewed and recommendations are provided.Result(s): Conscious sedation appears to be the most commonly used method of pain relief for transvaginal retrieval of oocytes. Conscious sedation does not require the presence of an anesthesiologist and can be done in freestanding clinics. Agents commonly used include opioids in combination with benzodiazepines. This combination minimizes pain, decreases anxiety, and provides sedation and some amnesia Adjuvants (...) such as promethazine and hydroxyzine can also be used but often are not needed. Conscious sedation is well tolerated by patients and does not require highly specialized equipment. However, there are specific safeguards that should be followed. Only a few toxicity studies have been performed, but they are reassuring because they have not found significant effects on fertilization or cleavage.Conclusion(s): Conscious sedation appears to be a safe and cost-effective method of providing analgesia and anesthesia for transvaginal retrieval of oocytes. Stress is one of the most powerful experiences to influence health and disease. Through epigenetic mechanisms, stress may generate a footprint that propagates to subsequent generations. Programming by prenatal stress or adverse experience in parents, grandparents, or earlier generations may thus be a critical determinant of lifetime health trajectories. Changes in regulation of microRNAs (miRNAs) by stress may enhance the vulnerability to certain pathogenic factors. This review explores the hypothesis that miRNAs represent stress‐responsive elements in epigenetic regulation that are potentially (...) heritable. Recent findings suggest that miRNAs are key players linking adverse early environments or ancestral stress with disease risk, thus they represent useful predictive disease biomarkers. Since miRNA signatures of disease are potentially heritable, big data management platforms will be vital to harness multi‐generational information and capture succinct yet potent biomarkers capable of directing preventative treatments. This feature would offer a unique window of opportunity to advance personalized medicine. (shrink)

This work, a reworked doctoral thesis written for Roland Bainton at the Yale Divinity School, begins with an announcement of a specific scholarly purpose: "To clarify the relationship between the clergy and the magistracy which grew out of Zwingli's reforming work at Zurich... the main focus of the study is upon the early stages of Zwingli's career at Zurich.... The ensuing study accepts the assumption that Zwingli believed in a Christian society ruled by two God-ordained officers, the magistrate and the (...) pastor, and asks 'What place did Zwingli assign to the magistrate and the clergy in order to realize the rule of God?'" Using as his model the German habilitations schrift, the author exhausts every possible aspect of Zwingli's theological and political thought. All of Zwinglian scholarship is summarized here, and problems in Zwingli's thought are dealt with emphasizing its development and its contradictions. Basically Walton argues that most of the scholarship about Zwingli is wrong because it assumes that Zwingli advocated domination of the government by the clergy. Zwingli was a defender of a theory of government which envisaged a co-operation between the spiritual and secular authority in a Christian society in order to realize the will of God. Walton finds that Zwingli's success in reforming Zurich was based upon the fact that his formulation of a Church-State detente conformed to pre-existing conditions in that city. Zurich was dominated by a corporate form of government which had already permitted the magistracy to take on a semi-sacral character. Zwingli, Walton argues, fit his theory of government to the conditions as they existed at Zurich. This is a book for Zwingli aficionados. It is an example of extremely well-done Continental scholarship: precise, insightful, and dry. No doubt the work has already become a definitive work on Zwingli's notion of theocracy.--W. A. J. In his paper I found many aspects of Hinduism that directly engage the mimetic theory, and I hope that I can articulate some of these in such a way as to initiate discussion during the next hour or (...) so this afternoon. As one of the so-called "Girardians," my first impression in response to Professor Clooney's paper is that the complexity of Hindu attitudes toward violence and sacrifice seem to indicate an awareness of the ritualization and control of violence, as developed over a very long period of time. I. Language of Avoidance/Denial: The Horse I would like to start with a small detail, but one which seems to me key to many larger issues in light of the mimetic theory: the ritual directions and prayers [1.1] in which the words "to kill" and "to die" are carefully avoided through the substitutions of such words as "he takes hold of and "he makes consent." I might add here also, that the avoidance ofusing "real" words for what is "really" being doneDkilling the animalDreminds me to some extent of the use of language by contemporary torturers as analyzed in the book The Body in Pain: The Making and Unmaking ofthe World by Elaine Scarry. Scarry argues that the language used by torturers avoids any acknowledg- Julia W. Shinnick141 ment of the real actions they are committing, as well as any acknowledgment of the very real pain they are causing their victims; furthermore, it is a dehumanizing language, referring to the victims as, for example, submarines, in one particular type of torture. This discrepancy between language and actual action is intriguing in both cases. Granted, the sacrificers in the Rg Veda do not deny that they are sacrificing the horse, and the "apology" (the words to the horseD"Do not let the axe linger in yourbody...") is not part ofan act oftorture, but the words have in common with Scarry's report a certain denial or avoidance of the reality of both the actions ofkilling and/orharming and the result ofthese actions, pain and/or death. Furthermore, the avoidance of the words "to kill" and "to die" would fit in with the mimetic theory's idea ofprohibiting the ritual from bringing actual facts of the founding murder to consciousness. II.Sacrificial Instructions The words addressed to the horse in the Rg Veda [I.l]and the instructions for the treatment of animals to be sacrificedO'The axe penetrates the thirty-four ribs of the swift horse...so that the limbs may be unperforated, and recapitulating, joint by joint "Dseem to me to intersect with the mimetic theory in terms of ritual sacrifice as a reenactment of the founding conflict. As I understand it, in the mimetic theory, one reason for careful, detailed ritual instructions and the sense ofgreatdangerin sacrifice (the sense that if the ritual is not carried out exactly and correctly, something horrible might happen) has to do with a non-conscious fear of unleashing the mimetic crisis all over again. I see aspects of this in the minutely detailed instructions to the priest such as the importance of leading the horse but not holding on to it, and the desire to pacify the animal to be sacrificed. These kinds of details are of course also present in other sacrifice rituals. For example, in Perpetua's Passion: The Death and Memory of a Young Roman Woman, Joyce Salisbury speaks of the simultaneous sacrifice of hundreds of children in ancient Carthage during the Punic Wars. The children had to be (appear) "willing" to be sacrificed. Ifthey cried out, the sacrifice was considered invalid. The parents also had to be (appear) "willing;" ifthe mother shed a tear, the sacrifice was invalid. Thus, immediately before the fatal knife thrust, the sacrificing parents would caress their children and tickle them to make them laugh.
