The Perceptions of Conscience Questionnaire (PCQ) and the Stress of Conscience Questionnaire (SCQ) have previously been developed and validated within the ‘Stress of Conscience Study’. The aim was to revalidate these two questionnaires, including two additional, theoretically and empirically significant items, on a sample of healthcare personnel working in direct contact with patients. The sample consisted of 503 healthcare personnel. To test variation and distribution among the answers, descriptive statistics, item analysis and principal component analysis (PCA) were used to examine (...) the underlying factor structure of the questionnaires. Support for adding the new item to the PCQ was found. No support was found for adding the new item to the SCQ. Both questionnaires can be regarded as valid for Swedish settings but can be improved by rephrasing some of the PCQ items and by adding items about private life to the SCQ. (shrink)

This paper is a revalidation of Oliver Sacks's role in the development of medicine's narrative turn and, as such, a reinterpretation of the history of narrative in medicine. It suggests that, from the late 1960s, Sacks pioneered in his ‘Romantic Science’ a new medical mode that reunited the seemingly incommensurable art and science of medicine while also offering a way for medical humanities to shape clinical reasoning more effectively.

Not everyone finds a in suffering. Indeed, even those who do subscribe to this interpretation recognize the responsibility of each individual to show not only sensitivity and compassion but render assistance to those in distress. Pharmacologic hypnosis, morphine intoxication, and terminal sedation provide their own type of medical to the terminally ill patient suffering unremitting pain. More and more states are enacting legislation that recognizes this need of the dying to receive relief through regulated administration of controlled substances. Wider legislative (...) recognition of this need would go far toward allowing physicians, in the exercise of their reasonable medical judgment, to administer a range of narcotics and barbiturates to the terminally ill without fear of legal sanctions. Sadly, social attitudes and governmental concerns about the spread of drug addiction provide an undeniable policy nexus that impedes unduly a rational approach or exception for the treatment of pain experienced by the dying. Background Traditional top-down national regulation of internationally mobile doctors and nurses is fast being rendered obsolete by the speed of globalisation and digitisation. Here we propose a bottom-up system in which responsibility for hiring and accrediting overseas staff begins to be shared by medical employers, managers, and insurers. Discussion In this model, professional Boards would retain authority for disciplinary proceedings in response to local complaints, but would lose their present power of veto over foreign practitioners recruited by employers who have (...) independently evaluated and approved such candidates' ability. Evaluations of this kind could be facilitated by globally accessible National Registers of professional work and conduct. A decentralised system of this kind could also dispense with time-consuming national oversight of continuing professional education and license revalidation, which tasks could be replaced over time by tighter institutional audit supported by stronger powers to terminate underperforming employees. Summary Market forces based on the reputation (and, hence, financial and political viability) of employers and institutions could continue to ensure patient safety in the future, while at the same time improving both national system efficiency and international professional mobility. Nursing codes of ethics and conduct are features of professional practice across the world, and in the UK, the regulator has recently consulted on and published a new code. Initially part of a professionalising agenda, nursing codes have recently come to represent a managerialist and disciplinary agenda and nursing can no longer be regarded as a self‐regulating profession. This paper argues that codes of ethics and codes of conduct are significantly different in form and function similar to the difference between (...) ethics and law in everyday life. Some codes successfully integrate these two functions within the same document, while others, principally the UK Code, conflate them resulting in an ambiguous document unable to fulfil its functions effectively. The paper analyses the differences between ethical‐codes and conduct‐codes by discussing titles, authorship, level, scope for disagreement, consequences of transgression, language and finally and possibly most importantly agent‐centeredness. It is argued that conduct‐codes cannot require nurses to be compassionate because compassion involves an emotional response. The concept of kindness provides a plausible alternative for conduct‐codes as it is possible to understand it solely in terms of acts. But if kindness is required in conduct‐codes, investigation and possible censure follows from its absence. Using examples it is argued that there are at last five possible accounts of the absence of kindness. As well as being potentially problematic for disciplinary panels, difficulty in understanding the features of blameworthy absence of kindness may challenge UK nurses who, following a recently introduced revalidation procedure, are required to reflect on their practice in relation to The Code. It is concluded that closer attention to metaethical concerns by code writers will better support the functions of their issuing organisations. (shrink)

The article presents a conceptual framework for distinguishing different sorts of heterogeneous digital materials. The hypothesis is that a wide range of heterogeneous data resources can be characterized and classified due to their particular configurations of hypertext features such as scripts, links, interactive processes, and time scalings, and that the hypertext configuration is a major but not sole source of the messiness of big data. The notion of hypertext will be revalidated, placed at the center of the interpretation of networked (...) digital media, and used in the analysis of the fast-growing amounts of heterogeneous digital collections, assemblages, and corpora. Wittgenstein took the _Tractatus Logico-Philosophicus_ to be eventually invalidated by logical atomism. Our main thesis is that it can be revalidated, provided that we subtract the thesis 2.02 (“The object is simple.”) from it: atoms are not simple objects but, rather, bits of information the objects are made of. Starting from an introductory discussion about what is meant by a ‘logic of colors’, an explanatory framework is then proposed in the form of a partition semantics. The philosophical problem of Wittgenstein’s (...) color-statements is reformulated accordingly, thus overcoming the trouble with logical atomism. A Boolean treatment is advanced to solve the initial difficulty and leads to two final results: Wittgenstein’s logic of colors requires a mereological understanding of the relations between colors in order to make sense; the analysis of psychophysical concepts like colors casts some additional doubt about the analytic–synthetic distinction, to the point of treating relations between colors as analytic a posteriori judgments. This paper examines the figure of silence in the works of Michel Serres and Simone Weil. It argues that, in the spirit of Serres and Weil, our time of crisis calls not for a short-term response, but for long-term engagement in a dialectics of silence: the dialogical movement between the silencing of institutions and the attentive silence of visionary insights. Such dialectics can revalidate the value of institutional silencing if based on solid rational proof while simultaneously showing the value of (...) visionary ideas that rightfully combat problematic institutional silencing. Especially in this current moment, in which science and scientific propositions are relentlessly questioned, there is a need to lean into silence so as to promote a productive dialogue that regains trust in proven scientific ideas and institutions while allowing visionary insights their place as well, provided that we are willing to test them. However, we think that the richness of Luhmann’s work allows a dialogue with the mainstream of contemporary theoretical practice. Three rules about what “should not be done” are revalidated from this analysis to philosophically base a theory in sociology and a key suggestion to overcome the problem of explanation in Luhmann’s sociology: social science seeks and directs its activity towards discovery and the use of substantive, real distinctions that also enrich the factual content of statements about social reality. Since the 1990’s the rise of China provokes heated debates in the West. Numerous politicians and scholars, who study contemporary political affairs, pose the question, which will be the new role of China in international affairs? Many Western observers presume that China will act as the Western powers did in the past, promoting policy of domination, enslavement and gaining profits at all costs. The Chinese declarations on peace, co-operation, mutual interests, and harmony are often considered empty words, a certain decorum (...) of “real politics”, as it had been often practised in the West. An inquiry into the Chinese political and intellectual traditions, almost unknown in the West, challenges such widespread opinions and fears. Of course, it is an open question to which degree the traditional Chinese concepts will determine contemporary policy, since China had been westernised in enormous degree.Nevertheless, attitude to war, struggle, and competition constitutes one of the principal characteristics of each culture and appears closely related to its structure of values. Such attitudes determine political concepts and systems, foreign relations, norms of social life, etc. The analysis of Western and Chinese civilisations demonstrates that in this respect they represented divergent orientations. In Erich Fromm’s typology of societies, the Chinese culture could be considered non-destructive but aggressive, whereas Western civilisation could be called aggressive and destructiveOf course, there were great differences both within Chinese and within Western civilisations. Local/regional cultures belonging to each of them had their own characteristics in respect to struggle, and one could also notice great changes in the course of their histories. In general, at the dawn of these two civilisations, both of them were more or less bellicose, although in the cultures of the Chinese Central Plain “avoiding struggle” traits could be detected quite early. Appreciation of peace and harmony eventually prevailed with the predomination of the Confucian ideology, although Taoism and other schools of thought also condemned struggle and war. On the other hand, in the Mediterranean the bellicose cultures and war-mongering states prevailed. The beginning of this “great divergence” could approximately be dated for the middle of the 1st millennium BC, and the civilisations of East Asia and Europe took the opposite courses in theirsocial and political development. Struggle constituted a fundamental principle in the Western vision of the world-order, and its crucial archetype constituted the eternal struggle of Evil against Good, with the eventual triumph of God, who embodies Good and forces of Light. The Chinese tradition promoted instead the vision of the universe as one giant organism based on the principle of harmony, where Yin complements Yang. Hence both civilisations adopted the dualistic concepts, but one was antagonistic, and the second—complementary.In the 19th century, when Asian nations faced the colonial conquests, many of them tried to protect their independence and modernise themselves. For this end they tried to adopt not only Western armaments and military skills, but also Western bellicose ideology and values. Japan was obviously the champion of such a militarist course, but there were similar tendencies in China as well. Such bellicose ideology reached its apex there during the famous Maoist Cultural Revolution, when class struggle and bellicosity was exalted. The triumph of the Western bellicose approach and rejection of the traditional native heritage marked the century 1860’s–1960’s in China. However, after the unimaginable sufferings of the World War I and II, of Nazism and of communism, the West started to revalidate its traditions condemning war and violence. People started to appreciate peace and co-operation within and among nations. The United Nations and the European Union had been founded for such purposes: to promote peace and co-operation. However, the condemnation of wars did not halt military confrontation of the states, and the West still cultivates political ideologies, which propagate the “proper order” based on competition and struggle. Hence the concepts of multi-party democracy and of free market could serve as hallmarks of the West. In China, since the end of the 1970’s step by step the ancient ideological traditions have been appreciated again and the new concepts of peaceful and harmonious development evolved. At the beginning of the 21st century the Chinese leaders elaborated a new ideology of both harmonious domestic and international order aiming at peaceful development based on co-operation. Of course, there are still vivid the remnants of the previous Western-type approach of the brutal competition and of aggressive foreign policy.Notwithstanding divergent tendencies inside each of these civilisations, in general they both approach each other again, but this time in a common search for peace, cooperation and harmonious development. Their interpretations obviously differ, since in China such concepts are traditional, but rather new in the West, and in various respects both sides have different values and ideals. (shrink)

Analytical philosophy abounds in tours de force [[sic]], but these are usually directed against other genres of philosophy, particularly the brand which passes under the various titles of "speculative," "systematic," or "substantive" philosophy. What distinguishes Fain's tour de force is that he turns the cutting edge of analytical philosophy on itself and, in so doing, seeks to revalidate speculative philosophy on analytical grounds. The main attack is against the stereotypes of a dichotomy between history and the philosophy of history, of (...) analytical philosophy presented as the true basis of philosophy of history, and of speculative philosophy of history as some kind of pseudo-discipline which is neither philosophy nor history. At the same time, the author rejects the standard division of philosophy of history into analytical and speculative approaches. His positive arguments center around the crucial role of narration in history. It is not the historical facts which give intelligibility to history, but the story-line or plot which combines them into a coherent narrative. Narration is counterposed to the analytical philosopher's emphasis on descriptive explanation of the facts. Fain argues that narration not only does not preclude consideration of history as a science, but that science itself often requires narration for purposes of intelligibility. He downgrades the importance of the debate which has raged for the last three decades over the nature of historical explanation. Absorption with the pros and cons of the covering law model of historical explanation has tended to preempt the whole field of philosophy of history and to obscure other more basic issues such as the relationship of history to science. On the other hand, Fain could not resist the temptation to append an epilogue on Hempel's covering law model, thus denying in practice what he proclaimed in principle. The examples of speculative philosophy are drawn mainly from the works of Hegel, Marx, and Collingwood. They are expounded and criticized within the context of Fain's definition of philosophy of history as "the formulation and the critique of criteria of intelligibility of historical concepts." Although he finds shortcomings in the story-lines of all three from the standpoint of intelligibility, the author concludes with a call for historians and philosophers to debate the story-line that a history of mankind should adopt.
