The informed consent process provides protection by ensuring that potential research subjects understand the goals of the research project they are being asked to voluntarily partake in as well as the risks associated with the study. We examined subjects' comprehension and ability to identify issues explicitly raised during the consent process that was conducted as part of their participation in a genetic family study (GFS). We employed cross-sectional design by providing a short, self-administrative questionnaire to 246 participants recruited from families (...) enrolled in the Extended Family Investigation of Nephropathy and Diabetes (EFIND) study conducted at the University of Texas Health Science Center. Participants responded to the questionnaire directly after their enrollment in the EFIND study. The questionnaire consisted of multiple-choice questions and focused on the understanding of the purposes, procedures, and risks associated with their participation in the EFIND study. These questions were formulated to reflect basic information presented to subjects through the consent process. Responses to questions were expressed as percentages, placing equal weight on each response. Participants were Mexican-American, 62.3% female and averaged 35.2 ± 12.7 years old (range: 18-76). Our findings showed that the average comprehension score was 58. About 30% of the participants did not know the name of the study, and 70% did not identify all elements related to the study procedures. The most striking finding was the lack of understanding concerning the social risks associated with participation in EFIND. While 35.1% of participants identified all potential physical risks, only 1.3% could identify all of the social risks. Our findings showed that participants comprehension score was significantly associated with their level of education and income. We conclude that using the informed consent process to communicate research social risks to subjects participating in GFS has some limitations. Future research directed at improving risk communications to subjects of low socioeconomic levels participating in genetic research is justified. (shrink)

Developmental research suggests that young children tend to value dominant individuals over subordinates. This research, however, has nearly exclusively been carried out in Western cultures, and cross-cultural research among adults has revealed cultural differences in the valuing of dominance. In particular, it seems that Japanese culture, relative to many Western cultures, values dominance less. We conducted two experiments to test whether this difference would be observed in preschoolers. In Experiment 1, preschoolers in France and in Japan were asked to identify (...) with either a dominant or a subordinate. French preschoolers identified with the dominant, but Japanese preschoolers were at chance. Experiment 2 revealed that Japanese preschoolers were more likely to believe a subordinate than a dominant individual, both compared to chance and compared to previous findings among French preschoolers. The convergent results from both experiments thus reveal an early emerging cross-cultural difference in the valuing of dominance. (shrink)

Background Hip fracture repair surgery carries a certain mortality risk, yet evidence suggests that orthopedic surgeons often refrain from discussing this issue with patients prior to surgery. Aim This study aims to examine whether orthopedic surgeons raise the issue of one-year post-surgery mortality before hip fracture repair surgery and to explore factors influencing this decision. Method The study employs a cross-sectional design, administering validated digital questionnaires to 150 orthopedic surgeons. Results A minority of orthopedic surgeons reported always informing patients about (...) the risk of mortality in the year following hip fracture surgery. The main reasons for not discussing this risk were a desire to avoid frightening patients, time constraints, and concerns about undermining patient hope. Orthopedic surgeons reported a medium-high level of perceived self-efficacy, with higher self-efficacy associated with a reduced likelihood of discussing one-year mortality risk. Conversely, older age and holding a specialist status in orthopedic surgery were associated with an increased likelihood of discussing this risk with patients. Conclusions These findings suggest a need for interventions to address communication barriers and ensure consistent provision of essential information to patients undergoing hip fracture surgery. Additionally, they highlight the importance of considering individual factors such as self-efficacy, age, and expertise in designing strategies to improve patient-provider communication in orthopedic care settings. Automatic synonym extraction plays an important role in many natural language processing systems, such as those involving information retrieval and question answering. Recently, research has focused on extracting semantic relations from word embeddings since they capture relatedness and similarity between words. However, using word embeddings alone poses problems for synonym extraction because it cannot determine whether the relation between words is synonymy or some other semantic relation. In this paper, we present a novel solution for this problem by proposing the (...) SynoExtractor pipeline, which can be used to filter similar word embeddings to retain synonyms based on specified linguistic rules. Our experiments were conducted using KSUCCA and Gigaword embeddings and trained with CBOW and SG models. We evaluated automatically extracted synonyms by comparing them with Alma’any Arabic synonym thesauri. We also arranged for a manual evaluation by two Arabic linguists. The results of experiments we conducted show that using the SynoExtractor pipeline enhances the precision of synonym extraction compared to using the cosine similarity measure alone. SynoExtractor obtained a 0.605 mean average precision for the King Saud University Corpus of Classical Arabic with 21% improvement over the baseline and a 0.748 MAP for the Gigaword corpus with 25% improvement. SynoExtractor outperformed the Sketch Engine thesaurus for synonym extraction by 32% in terms of MAP. Our work shows promising results for synonym extraction suggesting that our method can also be used with other languages. (shrink)

Biobanks and biospecimen collections are becoming a primary means of delivering personalized diagnostics and tailoring individualized therapeutics. This shift towards precision medicine requires interactions among a variety of stakeholders, including the public, patients, healthcare providers, government, and donors. Very few studies have investigated the role of healthcare students in biobanking and biospecimen donations. The main aims of this study were to evaluate the knowledge of senior healthcare students about biobanks and to assess the students’ willingness to donate biospecimens and the (...) factors influencing their attitudes. A cross-sectional study was conducted among senior healthcare students at King Abdulaziz University, Saudi Arabia. The data were obtained using a self-administered questionnaire in English. In addition to the respondents’ biographical data section, the questionnaire assessed the respondents’ general knowledge about biobanking, the factors influencing their willingness to donate biospecimens to biobanks and their general attitudes towards biomedical research. A total of 597 senior healthcare students were included in the study. The general knowledge score was 3.2 out of 7. Only approximately 44% and 27% of students were aware of the terms “Human Genome Project” and “biobank,” respectively. The majority of the students were willing to donate biospecimens to biobanks. Multiple factors were significantly associated with their willingness to donate, including their perceived general health, past experience with both tissue testing and tissue donation, biobanking knowledge score and biomedical research attitude score. The main reasons for students’ willingness to donate were advancement of medical research and societal benefits, whereas misuse of biospecimens and confidentiality breaches were the main reasons for a reluctance to donate. Despite their strong willingness to donate biospecimens, students exhibited a notable lack of knowledge about biobanking and the HGP. To expedite the transition towards PM, it is highly recommended to enhance healthcare curricula by including more educational and awareness programmes to familiarize students with OMICs technologies in addition to the scope of research and clinical applications. Given the adverse consequences of destructive leadership at work, we examine leader favoritism prevalent in contemporary organizations. Our study builds on previous research on unethical leadership behaviors and extends social exchange theory by assessing whether leader favoritism contributes to employee psychological withdrawal behavior at work and whether perceived unfairness explains this link, addressing a gap in the literature on this topic. In addition, we investigate the condition of perceived employability to seek whether the influence of perceived unfairness due to leader (...) favoritism on psychological withdrawal behavior at work is strengthened when employees think they can secure alternative employment elsewhere. The study utilizes a two-wave data collection approach to gather responses from 206 front end customer service employees working in the hospitality and tourism firms in the United Arab Emirates (UAE). The study findings offer empirical support for the proposed mediated moderation model. Specifically, the results demonstrate that leader favoritism directly, indirectly (via perceived unfairness), and positively influence employee withdrawal behaviors. This influence is stronger among employees who believe they can easily find alternative employment options. Our findings highlight the relevance of incorporating social exchange theory into the unethical leadership behaviors and withdrawal behaviors literature and provide valuable insights for managers into how to mitigate the practice of leader favoritism because of its adverse consequences on employees' work attitudes and behavior. As Medicine shifts from a paternalistic practice to a patient-centered approach, the concept of medical informed consent (IC) has evolved to safeguard patient autonomy. However, its current implementation still presents many challenges in clinical practice. We assessed the knowledge and attitudes of the general Lebanese population regarding the IC process as well as their sociodemographic and medical correlates. An anonymous online survey was distributed to the Lebanese population using social media channels. A sample of 500 adults with an average age (...) of 36.2 ± 13.5 years, including 319 females and 181 males, was recruited. Most of the respondents had a university degree (85.8%), reported previous hospital admissions (75.9%) and had signed an IC for surgical procedures (40.7%). Few participants were knowledgeable about IC Lebanese law. Variability in knowledge level was significantly related to gender and a previous hospitalization history. Positive attitudes toward patient autonomy (53.1%) and shared decision-making (57.5%) correlated with older age, female gender, graduate education, and a previous history of signing an IC document. Males were more likely to believe that IC has positive effects on health than females. This is the first study that provides novel findings regarding Lebanese peoples’ awareness of the ethico-legal components of medical IC. (shrink)

This article examines the post-9/11 policing of points of entry and transfer at US airports and the ways these points become “forbidden places” to those deemed undesirable, in order to expose the ambiguity of forbiddenness with respect to place. It uses Michel Foucault’s theory of biopolitics to argue that the War on Terror has created a class of expendable non-persons whose legal identities (citizenships) are not acknowledged and Giorgio Agamben’s analysis of “the camp” as a metaphor for the spaces in (...) airports that are neither entirely inside nor outside a national jurisdiction. This discussion takes place, in part, through the case study of suspected terrorist Maher Arar, arguing that his case shows the displacement of our sense of prohibition, away from spaces and onto persons. Kant, 1770 yılında Duyulur ve Anlaşılır Dünyanın Form ve İlkeleri eserini kaleme aldıktan sonra on yıl boyunca sessiz kalıp ardından Saf Aklın Eleştirisi’ni yazar. Bu bakımdan 1770 tezi Kant’ın eleştirel döneme geçiş eseri olarak değerlendirilir. Eser Kant’ın bitmez uğraşı metafiziği sağlam bir eksene yerleştirmek için giriş önerisidir. Bu bağlamda Kant duyusallığın konusu olan dünya ile anlayışın konusu olan dünyayı ayırarak metafizik için bir yol arar. Duyusallığa konu edilen dünya uzay ve zaman formları yoluyla kavranabilir. Anlaşılır dünya ise duyuların sızıntılarından (...) uzak yalnızca anlayış kavramları yoluyla belirlendiğinde metafizik yanılsamalardan korunabilir. 1770 tezi, Saf Aklın Eleştirisi’nin özellikle transendental estetik bölümü göz önünde bulundurulduğunda eleştirel dönemle büyük oranda örtüşse de anlaşılır dünyada noumenanın bilinebileceğini varsayması açısından eleştirel dönemden uzaklaşır. Çalışmada 1770 tezinin temel varsayımlarını ortaya koyarak bu varsayımları eleştirel dönemle ilgileri açısından ele alacağız. Özellikle, Saf Aklın Eleştirisi’nin A ve B baskılarında iki kez yazılan, deneyim ile anlama yetisi kavramları arasındaki uyuşmayı ele alan transendental türetim sorununun başlangıcının 1770 tezinde olduğunu göstereceğiz. Böylelikle 1770 tezinin eleştirel felsefede sürdürülen ve yadsınan varsayımlarını belirgin kılacağız. Çalışmadaki bir başka çabamız ise 1770’ten sonraki mektuplaşmaları yoluyla Kant’ın felsefi tutumunun nasıl farklılaştığını ortaya koymak olacak. 1772 yılında Herz’e yazdığı ünlü mektuptan önce Kant’ın Lambert, Herz ve Mendelssohn’la yazışmalarında yer alan 1770 tezi değerlendirmelerinin eleştirel döneme geçiş sürecini nasıl etkilediğini konu edeceğiz. Subsequent to the crystallization of the legal schools, Muslim jurists felt the need to consolidate the massive corpus of legal opinion in order to aid students and practitioners of the law. The result was legal maxims, concise theoretical statements that captured the objectives of the Sharia. An example is al-ḍarar yuzāl, which is based on the hadith lā ḍarar wa-lā ḍirār. This article analyzes the role of legal maxims in Yūsuf al-Qaraḍāwī’s jurisprudence and fatwas, as found in his numerous books (...) and articles. Its preliminary assumption is that Qaraḍāwī uses legal maxims to control and systematize the use of considerations of public welfare, especially in the field of “the jurisprudence of reality”. Because this fiqh deals mainly with political topics on which there are hardly any guidelines in scripture, and stems therefore from mostly nontextual benefits, it is an area vulnerable to undisciplined use of utilitarian considerations by jurists. Legal maxims then come in handy when weighing the relevant benefit and harm related to each topic. (shrink)
