This article examines instances of the pu relative construction in Modern Greek in which the semantic role of the head is underspecified by the syntax. Such cases include sentences whose nominal head corresponds to some complement of the relative clause predicate and sentences in which the head does not have any sort of syntactic relationship with the relative. The latter, which are characteristic of oral, informal discourse, have been completely ignored in the previous literature, which has defined relatives on (...) the basis of exclusively structural criteria. It is argued that a unified account of the pu-construction (including gapped and gapless relatives) can be achieved if we analyze it as a conventional instruction for a particular kind of conceptual integration. Semantic and pragmatic factors influencing successful construal (one which leads to the construction of a unique blend) are systematically examined. The lack of a clear cut-off point in acceptability for such utterances tallies with the conclusion reached here, namely that the constraints governing such uses are constraints on interpretability. In almost all opt-in systems of postmortal organ procurement, if the deceased has not made a decision about donation, his relatives will be asked to make it. Can this decision power be justified? I consider three possible justifications. (1) We could presume the deceased to have delegated this power to his relatives. (2) It could be argued that, if the deceased has not made a decision, a proxy decision has to be made in his best interests. (3) The (...) relatives could have a standing of their own because they are singled out from the parties whose interests are being affected by the decision by the special relation they had to the deceased. To date, few Norwegian clinical ethics committees (CECs) have included patients or next of kin in case discussions. In 2008, Rikshospitalet's (The National Hospital's) CEC began to routinely invite patients and relatives into case discussions. In this paper, we describe seven cases discussed by this committee in 2008. Six involved life and death decision-making in collaboration with the next of kin, while one related case did not include relatives. In our opinion, representing the patient's perspective was advantageous to (...) the discussion itself, to the conclusion made and to the next of kin's acceptance of the resolution. We believe that if the patient had been represented in the last case, the outcome might have been different. We conclude that successful patient involvement will rely on well-structured case discussions, an open atmosphere and good preparation and follow-up. (shrink)

Genomic research results and incidental findings with health implications for a research participant are of potential interest not only to the participant, but also to the participant's family. Yet investigators lack guidance on return of results to relatives, including after the participant's death. In this paper, a national working group offers consensus analysis and recommendations, including an ethical framework to guide investigators in managing this challenging issue, before and after the participant's death.

It has been proposed that an old and ill person may have a ‘duty to die’, i.e. to refuse life-saving treatment or to end her own life, when she is dependent on the care of intimates and the burdens of care are becoming too heavy for them. In this paper I argue for three contentions: You cannot have a strict duty to die, correlating to a claim-right of your relatives, because if they reach the point at which the burdens (...) of care are larger than you can reasonably expect them to take, the natural conclusion is that their duty ends. They may be prepared, however, to go on caring for you beyond that point. In that case your responsibility for their wellbeing may require you to refuse this care, even if this results in a situation for you in which death will be preferable to continued life. If this is the correct understanding of your responsibilities, the objection that in the context of family life the burdens of care attached to one family member’s valued existence can never be ‘too heavy’, fails. It postulates unlimited concern on one side and a total lack of concern on the other. (shrink)

This article provides an empirically based, interdisciplinary approach to the following two questions: Do animals possess behavioral and cognitive characteristics such as culture, language, and a theory of mind? And if so, what are the implications, when long-standing criteria used to justify differences in moral consideration between humans and animals are no longer considered indisputable? One basic implication is that the psychological needs of captive animals should be adequately catered for. However, for species such as great apes and dolphins with (...) whom we share major characteristics of personhood, welfare considerations alone may not suffice, and consideration of basic rights may be morally warranted—as for humans. Although characteristics supporting the status of personhood are present to differing degrees among the diverse array of animal species, this is a barrier to moral consideration only if anthropocentric, exclusive, and monolithic viewpoints about the necessary prerequisites for personhood are applied. We examine the flaws inherent within such positions and argue for inalienable species-appropriate rights. (shrink)

The essay considers two sets of interrelated difficulties that follow from our kinship to animals: those that arise chronically from our individual psychologically complex and often ambivalent relations to animals, and those that reflect the intellectually and ideologically criss-crossed connections among the various discourses currently concerned with those relations, including the movement for animal rights, ecological ethics, posthumanist theory, and such fields as primatology and evolutionary psychology. When considering the principle of medical confidentiality, disclosure of genetic information constitutes a special case because of the impact that this information can have on the health and the lives of relatives. The aim of this study is to explore the attitudes of Turkish physicians and patients about sharing information obtained from genetic tests.

In the Physics, Aristotle says that there is no change associated with the category of relatives. In this paper, I examine a widespread but neglected strategy that medieval thinkers use to understand Aristotle’s claim. According to this strategy, which I label initial presence, if there is no change in the category of relatives, it is because the relation-properties are already present in their subject as soon as the properties on which relation-properties are founded exist. Appreciating the importance of (...) this strategy is crucial not only for understanding medieval theories of relation but also for assessing the credibility of arguments used in the secondary literature to interpret medieval texts, in particular a well-known passage from Thomas Aquinas’s commentary on the Physics. Background: When a genetic mutation is identified in a family member, internationally, it is usually the proband’s or another responsible family member’s role to disclose the information to at-risk relatives. However, both active and passive non-disclosure in families occurs: choosing not to communicate the information or failing to communicate the information despite intention to do so, respectively. The ethical obligations to prevent harm to at-risk relatives and promote the duty of care by genetic health professionals is in conflict (...) with Privacy laws and professional regulations that prohibits disclosure of information to a third party without the consent of the proband. In New South Wales, Australia, amendments to Privacy legislation permits such disclosure to living genetic relatives with the process defined under guidelines although there is no legal duty to warn. This study assessed NSW GHP’s awareness and experience of the legislation and guidelines. Methods: An online survey collected demographics; theoretical knowledge; clinical scenarios to assess application knowledge; attitudes; confidence; experience with active non-disclosure. A link to correct answers was provided after completion. Knowledge scores above the median for non-parametric data or above the mean for parametric data were classified as ‘good’ or ‘poor’. Chi square tests assessed associations between confidence and knowledge scores. Results: While many of the 37 participants reported reading the guidelines, there was limited awareness of their scope and clinical application; that there is no legal duty to warn; and that the threat does not need to be imminent to warrant disclosure. No association between confidence and ‘good’ theoretical or applied clinical knowledge was identified. Uncertainty of their professional responsibility was identified and in the several case examples of active non-disclosure that were reported this uncertainty reflected the need for further understanding of the guidelines in regard to the processes required before disclosure was initiated. Conclusions: There is a need for further education and training about the guidelines associated with the legislation that would be relevant to support disclosure. The findings may inform future strategies to support introduction of policy changes in other jurisdictions where similar regulatory regimes are introduced. (shrink)

In this paper we devise non-distributive relatives of Exactly true logic by Pietz and Riveccio and its dual Non-falsity logic by Shramko, Zaitsev and Belikov. We consider two pre-orders which are algebraic counterparts of the ETL’s and NFL’s entailment relations on the de Morgan lattice 4. We generalise these pre-orders and determine which distributive properties that hold on 4 are not forced by either of the pre-orders. We then construct relatives of ETL and NFL but lack such distributive (...) properties. For these logics we also devise a truth table semantics which uses non-distributive lattice M3 as their lattice of truth values. We also provide analytic tableaux systems which work with sequents of the form \. We then prove correctness and completeness results for these proof systems and provide a neat generalisation for non-distributive ETL- and NFL-like logics built over a certain family of non-distributive modular lattices. In Categories 7, Aristotle gives two different accounts of relatives, and presents the principle of cognitive symmetry, which seems to help distinguish between relatives and some secondary substances. I suggest that the long-disputed difference between the two accounts lies in a difference in the determination of the categorial status of the object in question, and I formulate the principle of cognitive symmetry such that it plays a crucial role in making explicit how one conceptualizes the categorial status of (...) the object. The process of ‘logical differentiation’ was introduced by Peirce in 1870. Directly analogous to mathematical differentiation, it uses logical terms instead of mathematical variables. Here, this mysterious process receives new interpretations which serve to clarify Peirce’s use of logical terms. I introduce the logical terms, the operation of multiplication, the logical analogy to the binomial theorem, infinitesimal relatives, the concepts of numerical coefficients and the number associated with each term. BackgroundDecision-making in end-stage dementia is a complex process involving medical, social, legal and ethical issues. In ESD, the person suffers from severe cognitive problems leading to a loss of capacity to decide matters regarding health and end-of-life issues. The decisional responsibility is usually passed to clinicians and relatives who can face significant difficulty in making moral decisions, particularly in the presence of life-threatening swallowing problems.AimThis study aimed to understand the decision-making processes of clinical teams and relatives in addressing (...) life-threatening swallowing difficulties in ESD in long-term care in Malta.MethodThe study followed a qualitative approach where six case studies, involving six different teams and relatives of six different patients, were interviewed retrospectively to understand their decision-making in connection with the management of swallowing difficulties in ESD. Data were collected through semistructured interviews with each stakeholder. All data were transcribed and subjected to thematic analysis.ResultsFour themes were identified: the vulnerability of patients in dementia decision-making; the difficult role of relatives in decision-making; the decisional conflict between aggressive care through tube feeding versus oral comfort feeding; a consensus-building decision-making process as ideal to facilitate agreement and respect for patient’s dignity.ConclusionDecision-making to manage swallowing difficulties in ESD is a challenging process, which involves an interpretation of personal values, beliefs, patient preferences, care needs and clinical practice. Better communication between clinicians and relatives in dementia helps promote agreement between stakeholders leading to a care plan that respects the dignity of patients at their end of life. In today's societies relationships between near relatives and friends appear to be somewhat paradoxical. Some accounts present them as the social ideal, exalting the solidarity and altruism represented by proximal relationships. By contrast, others point to the social dangers in such relationships when they are conducted in the public sphere. In order to grasp the coexistence of these opposite views, this article attempts to place proximal relationships in the explanatory context of a gift economy, a concept with a long (...) history in anthropology and which has lately been the focus of interest of a significant group of social thinkers. Physicians often struggle with ethical issues surrounding intervention in their relatives’ health care. Many editorials, letters, and surveys have been written on this topic, but there is no systematic review of its prevalence. An Ovid Medline search was conducted for articles in English, written between January 1950 and December 2010, using the key words family member, relatives, treatment, prescribing, physician, and ethics. The search identified 41 articles (editorials, letters, and surveys). Surveys were reviewed to explore demographics of these (...) treating physicians and reasons for and against intervention. Physicians often intervene directly or indirectly in the health care of relatives. The most common reasons were convenience, cost savings, and the perception of having greater knowledge or concern than colleagues. Lost objectivity, fear of misdiagnosis, and inability to provide complete care were the main considerations against intervention. The characteristics of treating doctors were nonspecific. Most surveys recommend against this practice except for emergencies or minor ailments. This review included only a few surveys with small sample size and only assessed scientific literature written in English after 1950. Survey data may be biased by physicians’ self-reporting. In conclusion, most doctors occasionally intervene in their relatives’ care. The decision to do so is determined by multiple factors. Physicians should treat only short-term or minor illnesses within their scope of practice. Future research should evaluate doctors’ attitudes toward their relatives, medical student feelings about treating family, and intervention frequencies of medical and nonmedical professionals. This study seeks to understand, in the context of intersectional theory, the roles of family members in nursing home care. The unique social locus at which each person sits is the result of the intersection of gender, status, ethnicity and class; it is situational, shifting with the context of every encounter. A content analysis of 15 qualitative interviews with relatives of nursing home residents in Sweden was used to gain a perspective on the relationships between relatives and residents, (...) relatives and the nursing home as an institution, and relatives and the nursing home staff. We sought to understand these relationships in terms of gendered notions of the family and the residents, which are handed down from generation to generation and thus condition who and how relatives should be involved in care, and the ways in which relationships change as care moves from home to nursing home. It requires knowledge and awareness that the nursing home culture is based on intersectional power structures in order for relatives to be involved in nursing home care in alternative and individual ways.
