Si l’appellation de consultation conjugale a été retenue, malgré ses inconvénients, c’est qu’elle permet de toucher un très large public, en particulier de nombreuses personnes insuffisamment individuées pour qui l’appartenance au « Nous-couple » est constitutive. Ce texte est une petite histoire de la consultation conjugale durant ces cinquante dernières années, avec l’idéologie de ses débuts, les recherches théoriques, cliniques et méthodologique du conseil conjugal. La pression de la demande et son évolution est mise en parallèle avec les recherches cliniques ayant amené à proposer des thérapies de couple. Celles-ci à leur tour nous renseignent sur les difficultés des couples actuels, reflétant l’évolution des valeurs de la société.

Comment aborder la souffrance conjugale dans la consultation de conseil conjugal et familial? Le couple qui pousse la porte du bureau d’une conseillère conjugale n’a pas d’a priori, il vient chercher une écoute, de l’aide. À travers quelques vignettes cliniques, l’auteur expose la spécificité de l’écoute et du soutien du conseiller conjugal.

Bien que les violences conjugales soient un véritable problème de santé publique, force est de constater qu’elles restent souvent indifférenciées. Les auteurs discutent dans cette contribution des enjeux de la violence causée par des vulnérabilités spécifiques, inhérentes souvent à la conjugalité mais se répercutant sur la parentalité et l’enfant, et qui nécessitent l’intervention de la justice. Ils se basent sur le dispositif « Protection médiation prévention » ( pmp ) pour illustrer que la justice peut présenter un vecteur thérapeutique utile (...) à appeler à la (juste) place de chaque membre de la famille dès l’annonce d’un conflit en amont de la violence et l’installation de troubles psychologiques pérennes, notamment pour l’enfant. La confrontation avec la justice peut être l’occasion de sensibiliser aux souffrances engendrées par les violences et créer une amorce thérapeutique. Pour cela, les professionnels doivent pouvoir discriminer ce qui relève du registre du conflit et de la violence tout comme de la dysparentalité et de la conjugopathie. Ces réflexions et expériences de terrain ont débouché sur une recherche-action à l’université de Caen qui vise à proposer des interventions spécifiques et différenciées dans des situations de conflits et/ou de violence dans le cadre de l’assistance éducative. For decades a debate has played out in the literature about who bioethicists are, what they do, whether they can be considered professionals qua bioethicists, and, if so, what professional responsibilities they are called to uphold. Health care ethics consultants are bioethicists who work in health care settings. They have been seeking guidance documents that speak to their special relationships/duties toward those they serve. By approving a Code of Ethics and Professional Responsibilities for Health Care Ethics Consultants, the American Society (...) for Bioethics and Humanities (ASBH) has moved the professionalization debate forward in a significant way. This first code of ethics focuses on individuals who provide health care ethics consultation (HCEC) in clinical settings. The evolution of the code's development, implications for the field of HCEC and bioethics, and considerations for future directions are presented here. (shrink)

This paper focuses on the topic of evaluation of clinical ethics consultation. The concept of evaluation seems to contain an internal tension: On the one hand, evaluation is seen as distorting the conceptual and normative content of the case under scrutiny and, on the other, the evaluative act is the most important use of judgment and an inescapable part of everyday life. As such, we maintain that evaluation is essential.

Cet article interroge l’espace thérapeutique mouvant expérimenté dans une téléconsultation avec un couple. Il décrit les frontières polytopiques d’une séance online. Désormais, l’espace thérapeutique réunit trois lieux qui se superposent : chez le thérapeute, chez le patient et l’interface de rencontre, la plateforme numérique. L’auteure constate l’émergence d’une illusion partagée entre le thérapeute et son patient, favorisée par la superposition de deux mondes : virtuel et réel, dans l’écran et hors de l’écran. Le passage à une thérapie conjugale à (...) distance déplace les symptômes sur l’aspect territorial du cadre, tout en permettant de déployer un nouveau processus thérapeutique. Puisque les patients font entrer le regard du thérapeute dans leur habitat intérieur, la question du privé et de l’intime se pose : qu’est-ce qui est dévoilé? La problématique de la séparation est inhérente aux violences conjugales. Le couple se fonde sur le fantasme : être tout pour l’autre et que l’autre soit tout pour soi ou tout à soi. Le choix du conjoint est massivement narcissique et s’associe à des angoisses d’effondrement du Moi. L’hypothèse présentée dans l’article est que l’objet premier fut probablement insuffisamment sécurisant et constitué pour que le sujet puisse s’en séparer sans crainte d’un effondrement psychique. Cette fragilité expliquerait l’absence d’intériorisation de l’objet (...) absent et donc un défaut de symbolisation. Dans ce contexte, l’emprise serait une tentative de négociation et d’apaisement des angoisses de perte de l’objet premier. The nature, possibility, and implications of ethics expertise in general and of bioethics expertise in particular has been the focus of extensive debate for over thirty years. What is ethics expertise and what does it enable experts to do? Knowing what ethics expertise is can help answer another important question: What, if anything, makes a claim of expertise legitimate? In other words, how does someone earn the appellation “ethics expert?” There remains deep disagreement on whether ethics expertise is possible, and (...) if so, what constitutes such expertise and what it entails and legitimates. Discussion of bioethics expertise has become particularly important given the growing presence of bioethicists in the clinical setting as well as efforts to professionalize bioethics through codes of ethics and certification efforts. Unlike in the law or in engineering, where there may be a body of knowledge that professional organizations or others have articulated as important for education and training of experts, ethics expertise admits of no such body of knowledge or required experience. Nor is there an entity seen as having the authority to articulate the necessary scope of knowledge. Questions about whether there is such a body of knowledge for particular areas within bioethics have emerged and played a central role in professionalization efforts in recent years, especially in the area of clinical ethics. Guiding our response in this essay is our view that current efforts to demarcate the role of the clinical ethicist risk reducing its complex network of authorizations to sites of power and payment. In turn, the role becomes susceptible to various ideologies—individualisms, proceduralisms, secularisms—that further divide the body from the web of significances that matter to that body, where only she, the patient, is located. The security of policy, standards, and employment will pull against and eventually sever the authorization secured (...) by authentic moral inquiry. In the clinical setting, questions of medical ethics raise a host of perplexing problems, often complicated by conflicting perspectives and the need to make immediate decisions. In this volume, bioethicists and physicians provide a nuanced, in-depth approach to the difficult issues involved in bioethics consultation. The carbon emissions of global health care activities make up 4–5% of total world emissions, placing it on par with the food sector. Carbon emissions are particularly relevant for health care because of climate change health hazards. Doctors and health care professionals must connect their health care delivery with carbon emissions and minimize resource use when possible as a part of their obligation to do no harm. Given that reducing carbon is a global ethical priority, the informed consent process in (...) health care delivery must change. I argue that the expanded role of bioethicists in this climate crisis is to promote and support “green informed consent:” the sharing of climate information with patients, offering options for lower-carbon health care, and accepting the patient’s right to decline treatments which are deemed too carbon intensive for their values. The shared values, beliefs, and behaviors by which people interpret life events are what we call culture. More than medical science, culture determines how people react to illness and death. Science may determine which drug or surgery best treats the disease, but culture often determines how the health professional best treats a patient with the disease. Culture influences when the patient believes he is ill, which treatments he accepts, and which results he prefers.
