What provoked the fierce and systematic 'will to experiment' that was Modernism? Paranoia--thought especially to afflict those whose identities were founded on professional expertise--was described in the contemporary psychiatric literature as the violent imposition of system onto life's randomness. Modernism's great writers--Conrad, Ford, Lewis, Lawrence--both lived and wrote about these psychopathies of expertise.

The interview with economist William Baumol, published in 2003 in weekly Respekt, deals with alternative activities for talented individuals. If they can't pursue productive activities (technological innovations) they go for rent-seeking activities or activities with negligible social return (e.g. chess). I also present a thesis, that if there is no sophisticated alternative activities, the talent may be redirected into pathological ones: psychopathy, neurosis, paranoia, psychosis... Freud's 1911 case study based on Schreber's Memoirs of My Nervous Illness provides the investigator with the opportunity to reexamine Freud's interpretation through a return to the original data Freud used. This study reveals both the insights and limitations of Freud's theory of paranoia. An alternative interpretation of the case is overed from an existential-phenomenological perspective which aims both to expand upon and transform Freud's study without negating its value. Freud draws on the mythologies of the sun to argue for (...) his hypothesis that the "father-complex" lies behind Schreber's God. By following some of the many other mythological themes in Schreber's memoirs, Freud's interpretation is opened to a larger, socially and historically situated context. An examination of cross-cultural and historical studies of mystical experience shows how Schreber's psychosis is simultaneously a form of madness and spiritual breakthrough. Schreber's is viewed as a narcissistic experience of the infant-child in which the imaginal has been exiled from rational, modern adulthood and is inaccessible to Cartesian language. Instead of recognizing the soul-full world of "miracles," Schreber envisions the absorption of the entire world into himself and he thus becomes an inflated caricature of the "heroic ego" at its extreme. Multiple measures of decision-making under uncertainty (e.g. jumping to conclusions (JTC), bias against disconfirmatory evidence (BADE), win-switch behavior, random exploration) have been associated with delusional thinking in independent studies. Yet, it is unknown whether these variables explain shared or unique variance in delusional thinking, and whether these relationships are specific to paranoia or delusional ideation more broadly. Additionally, the underlying computational mechanisms require further investigation. To investigate these questions, task and self-report data were collected in 88 individuals (46 healthy controls, (...) 42 schizophrenia-spectrum) and included measures of cognitive biases and behavior on probabilistic reversal learning and explore/exploit tasks. Of those, only win-switch rate significantly differed between groups. In regression, reversal learning performance, random exploration, and poor evidence integration during BADE showed significant, independent associations with paranoia. Only self-reported JTC was associated with delusional ideation, controlling for paranoia. Computational parameters increased the proportion of variance explained in paranoia. Overall, decision-making influenced by strong volatility and variability is specifically associated with paranoia, whereas self-reported hasty decision-making is specifically associated with other themes of delusional ideation. These aspects of decision-making under uncertainty may therefore represent distinct cognitive processes that, together, have the potential to worsen delusional thinking across the psychosis spectrum. _Covert treatment raises a number of ethical and practical issues in psychiatry. Viewpoints differ from the standpoint of psychiatrists, caregivers, ethicists, lawyers, neighbours, human rights activists and patients. There is little systematic research data on its use but it is quite certain that there is relatively widespread use. The veil of secrecy around the procedure is due to fear of professional censure. Whenever there is a veil of secrecy around anything, which is aided and abetted by vociferous opposition from some (...) sections of society, the result is one of two: 1) either the activity goes underground or 2) it is reluctantly discarded, although most of those who used it earlier knew it was needed. Covert treatment has the dubious distinction of suffering both such secrecy and disapproval. Covert treatment has a number of advantages and disadvantages in psychotic disorders. The advantages are that it helps solve practical clinical problems; prevents delays in starting treatment, which is associated with clinical risks and substantial costs; prevents risk of self-destructive behaviour and/or physical assault by patient; prevents relapse; and prevents demoralization of staff. The disadvantages are that it maybe used with malafide intent by caregivers with or without the complicity of psychiatrists; it may be used to force conformity in dissenters; and the clinician may land himself in legal tangles even with its legitimate use. In addition, it may prevent insight, encourage denial, promote unhealthy practices in the treating staff and prevent understanding of why noncompliance occurs in the first place. Some support its use in dementia and learning disorders but oppose it in schizophrenia. The main reason is that uncooperative patients of schizophrenia (and related psychoses) are considered to be those who refuse treatment but retain capacity; while in dementia and severe learning disorder, uncooperative patients are those who lack capacity. This paper disputes this contention by arguing that although uncooperative patients of schizophrenia (and related psychoses) apparently retain capacity, it is limited, in fact distorted, since they lack insight. It presents the concept of insight-unconsciousness in a patient of psychosis. Just as an unconscious patient has to be given covert medical/surgical treatment, similarly an insight-unconscious patient with one of the different psychoses (in the acute phase or otherwise) may also have to be given covert treatment till he regains at least partial insight. It helps control psychotic symptoms and assists the patient in regaining enough insight to realize he needs treatment. Another argument against covert treatment is that people with schizophrenia have the capacity to learn and therefore can learn that they are required to take medications, but if medications are given covertly it may well fuel their paranoia. However, it should be noted that the patient who has lack of insight cannot learn unless he regains that insight, and he may need covert treatment to facilitate this process. Covert treatment can fuel the paranoia, true, but it can also control the psychotic symptoms sufficiently so that regular treatment can be initiated. In a patient who refuses to accept that he is sick and when involuntary commitment is not an option to be considered, covert treatment is the only option, apart from physical restraint. Ultimately, a choice has to be made between a larger beneficence (control of symptoms and start of therapy) and a smaller malevolence (necessary therapy, but without the patient's knowledge and consent). A number of practical clinical scenarios are outlined wherein the psychiatrist should adopt covert treatment in the best interests of the patient. Ethical issues of autonomy, power, secrecy and malafide intent arise; each of these can be countered only by non-malfeasance (above all, do no harm) under the overarch of beneficence (even above that, dare to care). An advance directive with health care proxy that sanctions covert treatment is presented. Questions raised by the practical clinical scenarios are then answered. The conclusions are as follows: covert treatment, i.e, temporary treatment without knowledge and consent, is seldom needed or justified. But, where needed, it remains an essential weapon in the psychiatrist's armamentarium: to be used cautiously but without guilt or fear of censure. However, the psychiatrist must use it very judiciously, in the rarest of rare cases, provided: i) he is firmly convinced that it is needed for the welfare of the patient; ii) it is the only option available to tide over a crisis; iii) continuing efforts are made to try and get the patient into regular psychiatric care; iv) the psychiatrist makes it clear that its use is only as a stop-gap; v) he is always alert to the chances of malevolence inherent in such a process and keeps away from conniving or associating with anything even remotely suspicious; and vi) he takes due precautions to ensure that he does not land into legal tangles later. The need of the hour is to explore in greater detail the need and justification for covert treatment, to lay out clear and firm parameters for its legitimate use, follow it up with standard literature and, finally, to establish clinical practice guidelines by unconflicted authors. The term "covert treatment" is preferable to "surreptitious prescribing"; they should not be used synonymously, the latter term being reserved for those cases where there is malafide intent._. The benzodiazepines can produce a wide variety of abnormal mental responses and hazardous behavioral abnormalities, including rebound anxiety and insomnia, mania and other forms of psychosis, paranoia, violence, antisocial acts, depression, and suicide. These drugs can impair cognition, especially memory, and can result in confusion. They can induce dependence and addiction. Severe withdrawal syndromes with psychosis, seizures, and death can develop. The short-acting benzodiazepines, alprazolam and triazolam , are especially prone to cause psychological and behavioral abnormalities. In his previous book, La philosophie et les experiences naturelles, A. De Waelhens claims that great philosophers in the past have not only been in dialogue with their predecessors but also that each great philosopher benefited from a confrontation with a non-philosophical experience. This previous book forms the theoretical justification for the present one. Here the author studies the problem of psychosis, with the hope and the intention of contributing to the further development of philosophy. Insofar as philosophy is (...) fascinated with the obligation "Learn to know yourself" or the search for an answer for the question "Who am I," insofar so claims the author, contemporary philosophy cannot afford to bypass psychoanalysis. The book has six chapters and a conclusion, but could easily be divided in three parts. The first part is a survey of the general approach to psychosis and schizophrenia by a selected group of European psychiatrists. The author indicates the crucial shortcomings of each approach and shows how there is among them a gradual turning away from a physiological interpretation of schizophrenia and psychosis towards a psychological one. In Part II, the author uses mainly the theory of Lacan and his disciples to present a coherent theory of psychosis. The crucial concepts are 1) the Oedipus-complex, 2) the mirror stage, 3) the foreclosure of a reference to the father. The Oedipus-complex is well known from Freud’s works although it receives here a more structural interpretation. The mirror stage refers to the events around 6-8 months when the child has to appropriate himself as a unified body, not just a collection of partial elements. This task can be noticed easily when one observes the attitude of the child towards the mirror. Hence the name. In order to perform this task successfully the child needs the support of the mother. The function of the mother is still more important for the third problem: a reference to the father. A healthy development of the child requires that the mother is capable of recognizing and of acknowledging the part played by the father in the creation of the child. This is done in our culture by giving the child the name of the father. Mothers of psychotics often are emotionally unable to tolerate that. They want to situate their child exclusively in relation to themselves: they foreclose any reference to the father. Psychosis is therefore related to the foreclosure of the name-of-the-father. These three crucial concepts allow the author to explain the crucial symptoms of schizophrenia, and to compare it with paranoia and paraphrenia. The third part of the book is a philosophical discussion of the problem of psychosis. The author offers the continental philosophical tradition as a better intellectual framework to study psychosis than the physical framework that Freud used in his Project for a Scientific Psychology and that he never completely abandoned. In the conclusion A. De Waelhens summarizes the direct philosophical consequences of the study of psychosis, the most fascinating being that one has to recognize the priority of language in the constitution of the subject, and that there is a meaningful relation between the way I relate to myself, the things and other people. Two improvements could be made in the book. The first is pedagogical: i.e., a critical bibliography of the crucial works of Lacan and his school and of the phenomenological tradition would have been very helpful for any reader not familiar with one of the two streams of thought. A second improvement would be the inclusion in Chapter One of a discussion of the development which has taken place in the U.S.A. in the explanation of schizophrenia. It is parallel with the development in Europe. A useful reference is E. G. Mishler and N. E. Walker: "Family Interaction Processes and Schizophrenia: A Review of Current Theories," in International Journal of Psychiatry, July 1966, Vol. 2, Number 4. Although the book is difficult, it is very insightful and provocative and it certainly merits translation.—W. V. E. (shrink)
