In this paper, we consider ethical issues related to the treatment of posttraumatic stress disorder (PTSD) in combat zones, via exposure therapy. Exposure-oriented interventions are the most well-researched behavioral treatments for PTSD, and rigorous studies across contexts, populations, and research groups provide robust evidence that exposure therapy for PTSD is effective and can be widely disseminated. Clinical procedures for Prolonged Exposure therapy, a manualized exposure-oriented protocol for PTSD, are reviewed, and we illustrate the potential benefits, as well as the potential (...) difficulties, associated with providing this treatment in combat zones. Several ethical considerations are identified: (1) Assuming successful treatment, is it ethical to send individuals with a known risk of developing PTSD back into combat? (2) If treatment is unsuccessful in theater (perhaps due to the confounding factor of ongoing danger), could that impact treatment effectiveness for soldiers who attempt therapy again post-deployment? (3) If the military finds combat-zone treatment effective and useful in maintaining an efficient work force, will treatment become mandatory for those diagnosed with PTSD? (4) What unintended consequences might be associated with large-scale dissemination of exposure therapy in or near combat, outside of mental health care infrastructures? (5) How would genetic variations known to be associated with PTSD risk influence decisions regarding who receives treatment or returns to combat? We conclude with a review of the personal and societal costs associated with not providing evidence-based PTSD treatments wherever possible. (shrink)

Treatment of military-related posttraumatic stress disorder is a major public health care concern. Since 2001 over 2.5 million troops have been deployed to Iraq or Afghanistan, many of whom have experienced direct combat and sustained threat. Estimates of PTSD rates related to these wars range from 8% to over 20%, or 192,000 to 480,000 individuals. Already, nearly 250,000 service members of Operations Enduring Freedom, Iraqi Freedom, and New Dawn have sought VA health care services for PTSD. This recent increased need (...) for mental health services comes in addition to the ongoing needs of Vietnam-era and other veterans who continue to suffer from PTSD. PTSD is related to high co-morbidities of other mental health difficulties, poorer physical health status, and increased medical care utilization. Such high demand for services is an important contributor to the large cost associated with combat-related PTSD. Accordingly, promoting successful, cost-effective treatment strategies for PTSD is a chief public health care priority. (shrink)

Post-Traumatic Stress Disorder is a major public health concern in both civilian and military populations, across race, age, gender, and socio-economic status. While PTSD has been around for centuries by some name or another, its definition and description also continue to evolve. Within the last few years, the American Psychological Association has published the 5th edition of the Diagnostic and Statistical Manual of Mental Disorders, which includes some major changes in the diagnostic criteria for PTSD. Recent data on epidemiology, etiological (...) theories, and empirically supported methods of treatment, as well as implications for legal processes and criminal justice system personnel, are discussed. New coverage of the recent wars in Afghanistan and Iraq, and the ensuing public education campaigns by the Department of Veterans Affairs and private veterans advocacy groups combine to call the public's attention to the many potential mental health problems associated with traumatic event exposure. Indeed, since 2001, Operation Iraqi Freedom and Operation Enduring Freedom combat and peacekeeping missions have been characterized by high levels of exposure to acts of extreme violence, with often gruesome effects. Less publically discussed is the (...) fact that a surprisingly large number of United States civilians also report exposure to traumatic events, such as severe interpersonal violence, natural disasters, and serious automobile accidents. In fact, approximately 70% of randomly sampled respondents indicated that they have experienced an incident characterized by significant perceived life threat at some point during their lives. Küresel güç mücadelesinde ağırlık merkezinin son yıllarda Asya-Pasifik’e kaymasıyla birlikte, 21. yüzyılın yükselen iki gücü olan Çin Halk Cumhuriyeti (ÇHC) ile Hindistan arasındaki rekabet de önemli ölçüde arttı. Hindistan’ın 1947’de İngiltere’den bağımsızlığını kazanması ile Çin’in Komünist bir Halk Cumhuriyeti olarak 1949’da kurulmasından günümüze dek aralarında süregelen sınır anlaşmazlıkları, iki ülke arasındaki ilişkiyi belirleyen en önemli konuların başında gelmektedir. İki ülkenin de geçmişinde Batı’nın aşağılayıcı müdahalelerine maruz kalmış olmasının yanında bu dönemde uluslararası siyasete hâkim olan iki kutupluluğun dünya barışına (...) hizmet etmeyeceğine olan ortak inançları dünya barışı için alternatif bir oluşum olan Bağlantısızlık Hareketine katılmalarını sağladı. Dolayısıyla bu iki neden ve her iki ülkenin de ulus-devlet oluşumunun erken aşamalarında olması, kuruluşlarının ilk on yılında birbirlerine karşı idealist yaklaşımlar benimsemelerini sağladı. Ancak çok geçmeden her iki ülkenin de materyal kapasitelerini arttırmasıyla birlikte çatışmaya başlayan ulusal çıkarları, Asya idealizminin altını oymaya başladı. Netice itibariyle 1962 gibi erken bir dönemde sıcak çatışmaya girerek Asya idealizmini bir kenara bırakıp ulus devlet realizmini benimsemiş oldular. Bu bağlamda, bu çalışma 21. yüzyılın yükselen iki gücü arasındaki en sorunlu alanlardan biri olan sınır anlaşmazlıklarının süreç içerisindeki evrimini ve bunun iki ülkenin dış politika vizyonlarının değişimleri üzerindeki etkisini ortaya koymaya çalışmaktadır. Bu analizde neoklasik realizm teorik bir çerçeve olarak rehberlik ederken ÇHC ve Hindistan arasındaki sınır sorunlarının yerel, bölgesel ve küresel yönlerini ve bunların iki ülkenin dış politikası üzerindeki etkilerini anlamak için süreç izleme yöntemi kullanılacaktır. Bu makalede Marmara Denizi’nin Kuzeybatısında yer alan Rodosçuk/Tekirdağ kazasında yaşayan halkın beslenme kültürü ele alınmıştır. Hangi ürünlerin yaygın olarak tüketildiğini belirlemek için başvurulan temel kaynak narh defterleridir. Bilindiği üzere narh, mal ve hizmet fiyatlarının esnaf örgütlerinin görüşü alınarak devlet tarafından belirlenen üst sınırını ifade etmektedir. Piyasada bulunan malların listelerini barındırdığı için ayrı bir önem taşıyan narh defterleri ilgili kazaya ait şer’iyye sicillerinde yer almaktadır. 1638-1730 yıllarına ait narh listelerinde bulunan ürünler türlerine göre sınıflandırılarak başlıklar halinde değerlendirilmiştir. Bu araştırmada ayrıca narh (...) defterlerine yansıdığı kadarıyla Rodosçuk piyasasında bulunan yiyecek maddelerinin çeşitliliği, yakınlığı nedeniyle özellikle İstanbul’da ve Saray’da tüketilen ürünlerle karşılaştırılarak zaman içinde meydan gelen değişimler belirlenmiş ve incelenen dönemin başından sonuna kadar önemli bir dönüşümün yaşanmadığı ortaya çıkmıştır. Varılan sonuçlardan bir diğeri de Amerikan menşeli yiyeceklerin henüz Rodosçuk’ta sahneye çıkmamış olmasıdır. (shrink)

This study focuses on the concept of people developed by Ernesto Laclau and Chantal Mouffe in the context of radical theories of democracy and populism. People is defined as a subjectivity established as a contingency in the conflictual environment of politics. The construction of the people is a condition of the existence of populist politics as a form of subject that enables the division of politics and social into two camps in the form of friend/enemy and the formation of antagonisms. The article seeks to answer the question of how to create a political order in Laclau and Mouffe's conception of the people in a pluralistic society, in order to cleanse politics from antagonistic (struggle between enemies) relations and transform it into agonistic (struggle between adversaries) relations, despite the danger of eliminating each other through a friend-enemy relationship. In this study, it will be discussed whether the antagonism that constitutes the foundation of politics and society is indestructible and whether it is a subject figure in which the people is organized as a counter-hegemonic subject against certain kinds of antagonisms and whether those who are partners can come together agonistically.
