Como Fazer Sentido de Coisas tão Díspares?

Abstract

Este ensaio abre um estudo de Medicina e Cuidados Paliativos (CP) que inclui algumas reflexões propedêuticas, testemunhos profissionais e investigação empírica. Procurou-se realizar um dossier médico multidisciplinar sobre o morrer e a morte na perspectiva de uma Medicina que se confronta, entre o absoluto e a acédia, com as coisas mais dispares. Aqui revisitamos os admiráveis textos inaugurais da Medicina de Cuidados Paliativos (MCP) legados por Cicely Saunders. Proporemos, contra a intuiçào clínica, uma hipótese sobre as fundações e fundamentos da Medicina que faz sistema e aparenta ter fortes esteios na realidade antropológica e sociológica. Para contextualizar examinamos aspectos pontuais de história da medicina e, muito esquemáticamente, contrastamos a busca de sentido, condição da leitura clínica do síntoma, com o reducionismo e reificação da doença. Assim, identificando áreas de saberes olvidados, restituímos a problemática da Condição Humana as Ciencias Biomédicas. Aplicamos estas ideias à reconstrução do acompanhamento médico, lato senso, dos doentes com doença terminal. This essay opens up a study on Medicine and Palliative Care which includes some basic reflections, professional accounts and empirical research. Our aim is to organize a small file about dying and death as seen through medicine which, between the absolute and the acedia, confronts itself with conflictingly dissimilar events. Here we revisit the first texts of palliative medicine, a legacy from Dame Cicely Saunders. We will formulate, against clinical intuition, a hypothesis about the foundations of Medicine which is a systemic set of ideas and seems to have broad support in the anthropological and sociological spheres. To contextualize it we explore incidental aspects of the history of western medicine, and only in a schematic way we contrast the search for meaning, a requirement for the clinical reading of the symptom, with reductionism and disease's reification. So, by recalling buried areas of knowledge we hand back the issue of the Human Condition to the Biomedical Sciences. We apply these ideas to the clinical caring for terminally ill patients
