Background:Nurse managers are placed in a unique position within the healthcare system where they greatly impact upon the nursing work environment. Ethical dilemmas and moral distress have been reported for staff nurses but not for nurse middle managers.Objective:To describe ethical dilemmas and moral distress among nurse middle managers arising from situations of ethical conflict.Methods:The Ethical Dilemmas in Nursing–Middle Manager Questionnaire and a personal characteristics questionnaire were administered to a convenience sample of middle managers from four hospitals in Israel.Results:Middle managers report (...) low to moderate levels of frequency and intensity of ethical dilemmas and moral distress. Highest scores were for administrative dilemmas.Conclusion:Middle managers experience lower levels of ethical dilemmas and moral distress than staff nurses, which are irrespective of their personal characteristics. Background:Cardio-pulmonary resuscitation is the default procedure during cardio-pulmonary arrest. If a patient does not want cardio-pulmonary resuscitation, then a do not attempt resuscitation order must be documented. Often, this order is not given; even if thought to be appropriate. This situation can lead to a slow code, defined as an ineffective resuscitation, where all resuscitation procedures are not performed or done slowly.Research objectives:To describe the perceptions of nurses working on internal medicine wards of slow codes, including the factors associated with (...) its implementation.Research design:This was a cross-sectional, descriptive study. Participants completed a personal characteristics questionnaire and the Perceptions and Factors of Slow Codes questionnaire designed for this study.Participants and research context:The sample was a convenience sample of nurses working on internal medicine wards in two Israeli hospitals.Ethical considerations:The study received ethical approval from both institutions, where data were collected and stored according to institutional policy.Findings:Most reported that resuscitations were conducted according to protocol. Some took their time calling the code, or waited by the bedside and did not perform cardio-pulmonary resuscitation. Factors most associated with slow codes were poor patient prognosis and a low chance of patient survival. Two-thirds reported that slow codes were done on their unit and the majority perceived slow codes as ethical.Discussion:This study confirms that slow codes are part of medical care on internal medicine wards, where most nurses perceive them as an ethical alternative. These perceptions are in contrast to most legal and ethical opinions expressed in the literature.Conclusion:Nurses should be educated about the legal and ethical implications of slow codes, and qualitative and quantitative studies should be conducted that further investigate its implementation. Background and aims. In pediatrics receiving a diagnosis of a chronic condition is a matter that involves caregivers at first. Beyond the basic issues of caring for the physical body of the ill child, caregivers’ manners of facing and making sense of the disease orient and co-construct their children’s sensemaking processes of the disease itself. The aim of this article is to explore the experience of a rare chronic illness, Hereditary Angioedema (HAE), in pediatrics, from the caregivers’ perspective. Hereditary Angioedema (...) is characterized by subcutaneous swellings that can involve the mucosal tissues of external as well as internal parts of the body, manifesting in a highly variable and unpredictable way in terms of localization, severity, and frequency. Materials and methods. Within a qualitatively-driven research design, we conducted a qualitative narrative semiotic analysis of n.28 mothers’ narratives on their children’s disease experience. Narratives were collected by an ad hoc interview on three domains of the disease experience: A. interpretation of the disease variability; B. dialogical processes; C. management of the disease. Subsequently, we executed a TwoStep cluster analysis for categorical data to detect cross-sectional profiles of the maternal sensemaking processes of the disease. Results. The coding grid was built analyzing the characteristic of the narrative links that orient the connection between the elements of the experience within each domain: A. the connection among events, for the domain of the interpretation of the disease variability; B. the connection between self and other, for the domain of the dialogue; C. the connection among sensemaking and actions, for the domain of the management of the disease. Results from cluster analysis show three narrative profiles: 1. adempitive; 2. reactive; 3. dynamic. Discussion. Profiles will be discussed in the light of the general conceptual framework of the Sense of Grip on the Disease (SoGoD) highlighting the importance of those sensemaking processes which, instead of relying on a coherent and close interpretation of the disease, are characterized by a degree of tolerance for the uncertainty and the unknowingly. Denis McManus presents a novel account of Martin Heidegger's early vision of our subjectivity and the world we inhabit. He explores key elements of Heidegger's philosophy, and argues that Heidegger's central claims identify genuine demands that must be met if we are to achieve the feat of thinking determinate thoughts about the world around us.

The authoress questions the historiographic dogma that, tracing back to F.W. Maitland, gave the pure «Englishry» of English law for granted, thus building its history on its exceptionality with respect to European tradition, from which it would not have been contaminated at all. The Anglo-Saxon historiographical analysis of the most important English constitutional document aims at answering to the provocative question: how English is Magna Carta? What role did jus commune play in its drafting? What is its relationship with contemporary (...) Roman Canon law? Through the examination of its cultural roots, the essay shows not only the substantial influence of Roman Canon law on Magna Carta, but also, more generally, the affinities and the common elements that characterize in principle the relationship between English juridical tradition and the continental one. Recently the attempt has been made to demonstrate Heidegger's relevance to the concerns of analytic philosophers. A focus for this effort has been the criticism in his early work of Cartesian ontology. While a number of important works have mapped out this area of Heidegger's thought, a crucial task has not been carried out, namely that of assessing how Heidegger can accommodate those phenomena which motivate the Cartesian to adopt his highly counter-intuitive ontology. As long as we fail to examine (...) how Heidegger's early ontology copes with the possibilities of error and of hallucination, the suspicion will remain that Heidegger is simply insensitive to those phenomena on which the Cartesian focuses. Neither Heidegger nor the Cartesian have been done any favours by commentators showing little inclination to bring the opponents into closer combat. This paper attempts to correct that omission. Even though the meaning of an utterance appears to depend on where and when we use it, on what we use it for and on what we expect in response, we do not examine such circumstances when asked what we mean. Instead we simply say what we mean. Similarly, our having grasped (...) a rule is something shown by how we perform certain tasks and respond to certain requests. But we frequently declare that we have indeed grasped a rule without paying any attention to those overt performances and, despite this, we are normally correct. These facts seem puzzling and impel us towards a certain philosophical picture of meaning and understanding. This picture identifies the meaning of a subject’s utterances, and his understanding of the rules that he follows, with some kind of structure of which he has immediate knowledge. By virtue of their connection with these private, meaning-constituting phenomena, the public manifestations of meaning and understanding are invaluable as clues to the meaning of an utterance or a subject’s understanding of a rule. But the latter are, nevertheless, ultimately fixed by the inner structures to which only the subject in question has immediate access and upon which he or she therefore has authority. This picture prompts a host of perplexing questions. “What are these immediately-knowable structures?” “What does it mean to say that we have immediate acquaintance with them?” “How are these private structures connected with those public performances?” These questions are notoriously difficult to answer. Creating what sort of professional? Master's level nurse education as a professionalising strategy This paper reports on a detailed analysis of selected findings from a larger study of master's level nurse education. It locates some features of such education within the contemporary situation of nursing as a profession and interprets the role of master's level nurse education as a professionalising strategy. In‐depth interviews were undertaken with a purposive sample of 18 nurse lecturers drawn from eight universities in the United Kingdom. (...) The interview agenda explored participants’ perspectives of the characteristics of master's level performance in practice. Interview transcripts were interpreted by drawing upon hermeneutic methodology.The following themes emerged. (a) The credibility of the master's level nurse was of central importance. In terms of the literature of professionalisation, this may be interpreted as a factor in enhancing the legitimacy of nursing as an occupation. (b) The clinical capability attributed to the nurse is interpreted as leading to an increase in the authority commanded by the expert professional. Thus, the individual capability of the master's level nurse enhances the attribution of autonomous skill to the occupation as a whole. (c) The master's level nurse is seen to exercise influence and leadership and this strengthens the power and status of nursing.Nursing does not have the appearance of a ‘traditional’ profession, neither has it a clear stance as a ‘new profession’. Rather it appears to be especially responsive to the tide of public opinion manifest through government edicts. While nursing is employing rhetoric that espouses both positions, the direction of master's level education is anomalous. (shrink)

Development of a vaccine for schistosomiasis, a parasitic disease currently affecting over 200 million people worldwide, has been targeted as a priority by the World Health Organisation. Research demonstrating the ability of humans to acquire natural immunity to schistosome infection, together with the successful use of attenuated vaccines in animals both under laboratory and field conditions, suggest that development of a human vaccine is feasible. Attenuated vaccines for schistosomiasis are considered neither safe nor practicable for human use, however, and therefore (...) other approaches must be considered. This review examines progress currently being undertaken in a number of different areas towards achieving the goal of a safe and effective human vaccine for schistosomiasis. (shrink)

Many business schools profess a commitment to ethics in their mission statements and focus a spotlight on the intersection between the university’s mission and attention to business ethics. To explore this trend, we analyze a sample of students’ values from two universities with an explicit religious foundation and recognized commitment to ethics against students from another university where this attention is not as explicit. This study identifies the personal values orientations (PVOs) for these students, born between 1980 and 2000, thus (...) millennials, and considers if differences can be attributed to the university’s culture and mission or business school curriculum. We utilize Social Identity Theory as a promising framework for illuminating these considerations. Our results confirm that a religious‐orientation characterizing a university’s mission may influence the business students’ (or millennials’) PVOs. Students at the two religious institutions manifested a stronger social, rather than personal, and moral, rather than competence, value orientation. Only marginal differences were discovered when assessing within group differences in the value orientations among our sample of business students. (shrink)

Wittgenstein is arguably the greatest philosopher of the last hundred years and scepticism is one of the central problems that modern philosophy faces. This collection is the first to be devoted to an examination of how that great philosopher's work bears on this fundamental philosophical problem. Wittgenstein's reaction to scepticism is complex, articulating both a sense that sceptical problems are ultimately unreal and a sense that scepticism teaches us something about the fundamental character of the human predicament. William James is usually associated with the philosophy of pragmatism and with his interest in religious experiences. But James also developed a methodology which has received far less attention. James called this methodology "radical empiricism," an approach that requires that (1) all of the ideas and theories in science be grounded in direct experience, and (2) no experience be excluded from scientific purview. This paper describes James' thoughts about radical empiricism, and discusses some of the strengths of, and problems with (...) his view in light of more contemporary science. A biogenetic structural elaboration of James' notion of relations and pure experience is offered, and is used to counter the so called post‐structuralist critique of Jacques Derrida. The relevance of James' views to the anthropology of consciousness is explored, with emphasis upon the necessity ‐‐ anticipated by James ‐‐ of merging a phenomenology with structuralism. (shrink)

Although generational differences have been studied in developed countires, not much information is available about generational cohorts and how they differ in terms of work ethics in developing countries. A cross-sectional study was conducted with a sample of 301 respondents from South Africa. Work ethics of three generational cohorts were measured, namely the Baby Boomers, Generation X, and Generation Y. The main finding of this research was that statistically significant differences and similarities were found between the various generational cohorts in (...) terms of work ethics facets. In This Paper, I Identify a Problem, which the project that I will refer to as the ‘Being and Time Project’ (or ‘BTP’ for short) aimed to solve; this is the project within which Heidegger reinterpreted his early thought—and which he unsuccessfully attempted to bring to fruition—in, roughly speaking, the years 1925–28. The problem in question presents several faces: viewed from one angle, it concerns the unity of the concept of “Being in general,” from another, the integrity of the notion (...) of “Dasein,” and from another, the possibility of the perspective from which the philosopher does her work. McManus presents an intellectual history of the conservative and reactionary tradition, stretching from Aristotle and Confucius to Ayn Rand and Patrick Deneen. Providing a comprehensive critical genealogy of the intellectual political right, McManus traces its core to a nostalgia for the hierarchical cosmos of antiquarian and scholastic thinking. The yearning for a shared vision of the universe where each part of reality has its place maps onto the conservative admiration for orderly political and social stratification. It stamps even (...) the more moderate forms of liberal conservatism which emerged in the aftermath of the revolutionary 18th century, as the political right struggled to accept and later master first the politics of liberal capitalism and later universal suffrage. In its most radical forms this nostalgia for an orderly and hierarchical existence can harden into a resentment at the perceived shallowness of liberal modernity. McManus argues for those who support the project of modernity to commit themselves to better understanding the depth of the political right's critiques, many of which expose uncomfortable but solvable problems with the quest for equality and freedom. This paper explores two models using which we might understand Heidegger's notion of ‘Eigentlichkeit’. Although typically translated as ‘authenticity’, a more literal construal of this term would be ‘ownness’ or ‘ownedness’; and in addition to the paper's exegetical value, it also develops two interestingly different understandings of what it is to have a judgment of one's own. The first model understands Heideggerian authenticity as the owning of what I call a ‘standpoint’. Although this model provides an understanding of a number (...) of key features of authenticity, it also invites an important objection—which I call ‘the closure objection’—that can be found in, for example, the work of Steven Galt Crowell and Tony Fisher. Although I argue that this objection can be met, the response for which it calls reveals that the feat of authenticity as understood through the standpoint model rests upon a further feat, and one which may itself have a stronger claim to be identified with Heideggerian authenticity. I develop this thought, introducing what I call the ‘all-things-considered judgment model’ of authenticity, the basis of which lies in, among other sources, Heidegger's appropriation of themes from Aristotle's discussion of phronesis. I explain the exegetical benefits of adopting this model and consider some objections that it invites, before closing with a discussion of how the two models understand the notion of ‘a judgment of one's own’.
