Background:Cardio-pulmonary resuscitation is the default procedure during cardio-pulmonary arrest. If a patient does not want cardio-pulmonary resuscitation, then a do not attempt resuscitation order must be documented. Often, this order is not given; even if thought to be appropriate. This situation can lead to a slow code, defined as an ineffective resuscitation, where all resuscitation procedures are not performed or done slowly.Research objectives:To describe the perceptions of nurses working on internal medicine wards of slow codes, including the factors associated with (...) its implementation.Research design:This was a cross-sectional, descriptive study. Participants completed a personal characteristics questionnaire and the Perceptions and Factors of Slow Codes questionnaire designed for this study.Participants and research context:The sample was a convenience sample of nurses working on internal medicine wards in two Israeli hospitals.Ethical considerations:The study received ethical approval from both institutions, where data were collected and stored according to institutional policy.Findings:Most reported that resuscitations were conducted according to protocol. Some took their time calling the code, or waited by the bedside and did not perform cardio-pulmonary resuscitation. Factors most associated with slow codes were poor patient prognosis and a low chance of patient survival. Two-thirds reported that slow codes were done on their unit and the majority perceived slow codes as ethical.Discussion:This study confirms that slow codes are part of medical care on internal medicine wards, where most nurses perceive them as an ethical alternative. These perceptions are in contrast to most legal and ethical opinions expressed in the literature.Conclusion:Nurses should be educated about the legal and ethical implications of slow codes, and qualitative and quantitative studies should be conducted that further investigate its implementation. The doctrine of karma and rebirth is often praised for its ability to offer a successful solution to the Problem of Evil. This essay evaluates such a claim by considering whether the doctrine can function as a systematic theodicy, as an explanation of all human suffering in terms of wrongs done in either this or past lives. This purported answer to the Problem of Evil must face a series of objections, including the problem of anylackofmemoryofpastlives,the lack of proportionality between wrongdoing (...) and the observed suffering in the world, the problem of infinite regress of explanation, and the problem of compatibility of free will with karmic determinism. These objections, either separately or taken together, provide sufficient reason to doubt whether the doctrine of karma and rebirth can in fact provide a satisfactory theodicy. (shrink)

Negotiating the relation between divine transcendence and divine immanence lies at the heart of Maimonides' Guide of the Perplexed and of Kierkegaard's philosophical works. The purpose of the paper is to explore the manners in which they do so. I argue that despite various differences between them, both engage with the tension between divine transcendence and immanence by turning away from objectivity to subjectivity and, moreover, by placing paradox, riddle and secret at the heart of their philosophical works. In other (...) words, I argue that they do not attempt to solve or dissolve the great paradox of God's immanent transcendence but to present it in its most acute forms as the paradox within which the religious life is lived. For much of its history, philosophy was not merely a theoretical discipline but a way of life, an "art of living." This practical aspect of philosophy has been much less dominant in modernity than it was in ancient Greece and Rome, when philosophers of all stripes kept returning to Socrates as a model for living. The idea of philosophy as an art of living has survived in the works of such major modern authors as Montaigne, Nietzsche, and Foucault. Each of (...) these writers has used philosophical discussion as a means of establishing what a person is and how a worthwhile life is to be lived. In this wide-ranging, brilliantly written account, Alexander Nehamas provides an incisive reevaluation of Socrates' place in the Western philosophical tradition and shows the importance of Socrates for Montaigne, Nietzsche, and Foucault. This book compares two competing theories of human nature: the more traditional theory espoused in different forms by centuries of western philosophy and the newer, Darwinian model. In the traditional view, the human being is a hybrid being, with a lower, animal nature and a higher, rational or “spiritual” component. The competing Darwinian account does away with the idea of a higher nature and attempts to provide a complete reduction of human nature to the evolutionary goals of survival and reproduction. (...) The National Institutes of Health and other federal health agencies are considering establishing a national biobank to study the roles of genes and environment in human health. A preliminary public engagement study was conducted to assess public attitudes and concerns about the proposed biobank, including the expectations for return of individual research results. A total of 141 adults of different ages, incomes, genders, ethnicities, and races participated in 16 focus groups in six locations across the country. Focus group participants voiced (...) a strong desire to be able to access individual research results. Recognizing the wide range of possible research results from a large cohort study, they repeatedly and spontaneously suggested that cohort study participants be given ongoing choices as to which results they received. The aim of interpretation is to capture the past in the future: to capture, not to recapture, first, because the iterative prefix suggests that meaning, which was once manifest, must now be found again. But the postulated author dispenses with this assumption. Literary texts are produced by very complicated actions, while the significance of even our simplest acts is often far from clear. Do people have free will, or this universal belief an illusion? If free will is more than an illusion, what kind of free will do people have? How can free will influence behavior? Can free will be studied, verified, and understood scientifically? How and why might a sense of free will have evolved? These are a few of the questions this book attempts to answer. People generally act as though they believe in their own free will: they don't feel like (...) automatons, and they don't treat one another as they might treat robots. While acknowledging many constraints and influences on behavior, people nonetheless act as if they (and their neighbors) are largely in control of many if not most of the decisions they make. Belief in free will also underpins the sense that people are responsible for their actions. Psychological explanations of behavior rarely mention free will as a factor, however. Can psychological science find room for free will? How do leading psychologists conceptualize free will, and what role do they believe free will plays in shaping behavior? In recent years a number of psychologists have tried to solve one or more of the puzzles surrounding free will. This book looks both at recent experimental and theoretical work directly related to free will and at ways leading psychologists from all branches of psychology deal with the philosophical problems long associated with the question of free will, such as the relationship between determinism and free will and the importance of consciousness in free will. Nietzsche often gives the impression that all human beliefs are false. Some scholars, like Maudemarie Clark, believe that such a “falsification thesis” is unacceptable and try to limit Nietzsche's commitment to it, claiming that he abandons it in his very last works. Others, like Lanier Anderson and Nadeem Hussain, take it in ways that make it true and locate it in all. I argue that the view that is common to both approaches—that Nietzsche held that thesis in the first place—is (...) unjustified. To that end, I interpret the texts where these scholars claim to find the thesis in a way that shows that they do not commit Nietzsche to it, and I offer some reasons for thinking that this is a more fruitful way of interpreting his views on the importance of truth—and falsehood—and their function in the economy of human life. Problems with representing friendship in painting and the novel and its more successful displays in drama reflect the fact that friends seldom act as inspiringly as traditional images of the relationship suggest: friends' activities are often trivial, commonplace and boring, sometimes even criminal. Despite all that, the philosophical tradition has generally considered friendship a moral good. I argue that it is not a moral good, but a good nonetheless. BackgroundThe Arab population in Israel is a minority ethnic group with its own distinct cultural subgroups. Minority populations are known to underutilize genetic tests and counseling services, thereby undermining the effectiveness of these services among such populations. However, the general and culture-specific reasons for this underutilization are not well defined. Moreover, Arab populations and their key cultural-religious subsets (Muslims, Christians, and Druze) do not reside exclusively in Israel, but are rather found as a minority group in many European and North (...) American countries. Therefore, focusing on the Arab population in Israel allows for the examination of attitudes regarding genetic testing and counseling among this globally important ethnic minority population.MethodsWe used a qualitative research method, employing individual interviews with 18 women of childbearing age from three religious subgroups (i.e., Druze, Muslim, and Christian) who reside in the Acre district, along with focus group discussions with healthcare providers (HCPs; 9 nurses and 7 genetic counselors) working in the same geographical district.ResultsA general lack of knowledge regarding the goals and practice of genetic counseling resulting in negative preconceptions of genetic testing was identified amongst all counselees. Counselors’ objective of respecting patient autonomy in decision-making, together with counselees’ misunderstanding of genetic risk data, caused uncertainty, frustration, and distrust. In addition, certain interesting variations were found between the different religious subgroups regarding their attitudes to genetic counseling.ConclusionsThe study highlights the miscommunications between HCPs, particularly counselors from the majority ethno-cultural group, and counselees from a minority ethno-cultural group. The need for nuanced understanding of the complex perspectives of minority ethno-cultural groups is also emphasized. Such an understanding may enhance the effectiveness of genetic testing and counseling among the Arab minority group while also genuinely empowering the personal autonomy of counselees from this minority group in Israel and other countries. This study explored parental mentalization processes as they unfolded during a sculpting task administered to fathers of toddlers. Parental mentalization—the parent’s ability to understand behavior based on its underlying mental states —is considered crucial within parent–child relationships and child development. Eleven Israeli first-time fathers of children aged 2–3 were asked to sculpt a representation of themselves with their child using clay. Following the task, the fathers were interviewed while observing the sculpture they had created. Qualitative thematic analysis integrated three types (...) of data—video footage of the sculpting processes, the sculptures themselves, and the transcripts of the post-sculpting interviews. By focusing on data extracts relating to mentalization processes, three main aspects of the clay-sculpting task and interview were identified as processes that either preceded controlled mentalization instances and/or related to their underlying dynamics: discussing the sculpting process elicited the father’s curiosity and wonder; observing the sculpture/sculpting process revealed gaps in paternal representations; and the preplanning of the sculptures sparked non-verbal exploration of metaphors and symbolism. Special attention was given, in the analysis, to the interplay between verbal and non-verbal aspects of mentalization as they appeared in the metaphorical representations that arose through the sculpting process. Comparing this sample to a previous sample of mothers who were given the same task, similarities and differences were explored, with specific reference to topics of embodiment, gender roles, paternity leave, and an active approach in art therapy. The discussion indicates that clay sculpting may offer unique insight into implicit parental mentalization. Possible clinical applications are discussed, with reference to attachment theory and clinical art therapy approaches. (shrink)

A LARGE NUMBER OF PEOPLE, ancient and modern alike, have always found in Socrates what seemed to them a suspicious, if not actually repugnant, aspect. This aspect, to put the point first in crude terms, is his devotion to philosophy, which presupposes an apparently unshakable faith in reason, in the power of understanding to secure goodness, and in the power of goodness to provide us with happiness.

Nietzsche sometimes writes as if we are not in control—at least not in conscious control—of our actions. He seems to suggest that what we actually do is independent of our intentions. It turns out, though, that his understanding of both intention and action differs radically from most contemporary treatments of the issue. In particular, he denies that our actions are caused by their intentions, whose role is hermeneutical in a sense that this essay develops. How then is responsibility to be (...) assigned, since its moral variety, at least, depends, on several views, on the intention with which an action is performed? Nietzsche, of course, is not interested in making attributions of moral responsibility. Still, his views on the relationship between an individual action, its intention, other actions by its agent, and the agent's character, as this essay presents them, provide a reasonable account of action generally and a different, broader account of responsibility for oneself. They address the philosophy of Schopenhauer, the nature of tragedy, the relationship of language to music, the importance of Classical Greek culture for modern life, and the value of the unfettered pursuit of truth and knowledge which (...) Nietzsche thought was a central feature of western culture since it was first introduced by Plato. They contain startling and original answers to the questions which were to occupy Nietzsche throughout his life and demonstrate the remarkable stability and consistency of his fundamental concerns. More strongly, It is suggested that "only" self-Predications can be accepted as clearly true by plato. This is to deny that plato had at his disposal an articulated notion of predication, And his middle theory of forms, Primarily the relation of (...) participation, Is seen as his attempt to arrive at that notion. It is argued that his metaphysical and semantical views are heavily influenced by eleatic monism. But this monism is incompatible with the very idea of predication. It is only in his later works, Where he attacks parmenides directly, That plato manages to lay the groundwork for what can be considered as the discovery of predication. (shrink)
