Abstract

Following publication of the influential Flexner Report on medical education in 1910, the US built a health care system on a foundation of science that, by the end of the 20th century, provided some of the best medical care in the world. Now, at the start of the 21st century, we are in real danger of destroying those impressive achievements. The primary reason is the failure over many years to change our increasingly dysfunctional health insurance system. Chief among its problems is reduced access to medical services as growing numbers of Americans lose health insurance altogether, and coverage for the rest is eroding.
