Health care systems throughout the developed world face ‘crises’ of quality, financing and sustainability. These pressures have led governments to look for more efficient and equitable ways to allocate public resources. Prioritisation of health care services for public funding has been one of the strategies used by decision makers to reconcile growing health care demands with limited resources. Priority setting at the macro level has yet to demonstrate real successes. This paper describes international approaches to explicit prioritisation at the macro-governmental (...) level in the six experiences most published in the English literature; analyzes the ways in which values, principles and other normative concepts were presented in these international priority setting experiences; and identifies key elements of a more robust framework for ethical analysis which could promote meaningful and effective health priority setting. This article brings together and compares my own artistic practice of drawing/painting and the eighteenth-century novel Tristram Shandy. In both cases, there is a free play of lines, textual or graphic, which sets ‘all things out of rule’. A whole typology of lines is woven throughout Sterne’s text and reappears, alter-inscribed, in the artworks. The article presents an account of these lines: rectilinear, hylomorphic, fractal and nomadic, as well as the line of incision (or the cut). Each is explored as (...) a specific mode of line with differing effects. To follow these lines is to enter a world of material expressivity, to be exposed to an ontology of becoming and change, of flows and transformations, that overturns the traditional ontology of being and stable identity. Sterne’s use of wild digression, doodles and graphisms, establishes certain proximity between writing and drawing. Both may be seen as types of ‘figured metaphysics’ and as ways of bringing something new into the world. The article concludes with some philosophical observations on the strange event of drawing a line, with reference to the work of Heidegger and Deleuze and Guattari. (shrink)

This paper analyzes and discusses related passages of Euripides’ „Electra“ and Aristophanes’ „Wasps“ in order to suggest a date for the performance of the „Electra“. Particularly interesting are: Ar. „Wasps“ 615 and Eur. „Electra“ 985; „Wasps“ 1490, 1492, 1530 and „Electra“ 860-861, which seem to reveal more complex dynamics of dependence in the Aristophanic attempt to mock the language excessively hyperbolic of Euripides. In my opinion these puns and innuendos may endorse the dating of Euripides’ „Electra“ at the Great Dionysia (...) of 423 b. C. one year before the performance of Aristophanes’ „Wasps“. (shrink)

Drawing on Jacques Derrida’s work, I argue that neither of the two standard accounts of forgiveness offer an adequate understanding of forgiveness. Conditional accounts insist on specifying the conditions an offender needs to satisfy in order to count as deserving of forgiveness. I argue that such accounts not only render forgiveness unintelligible (since forgiveness is intelligibly offered only to the offender qua offender), but also dissolve the ethical decision forgiveness demands of us. Unconditional accounts promise to do justice to both (...) by insisting that forgiveness is a freely granted gift offered to the guilty as guilty. But I argue that when pressed to justify why one should forgive unconditionally and how one avoids the threat of condoning, they typically fall back onto the conditionalist’s ground and lose the electivity of forgiving. I conclude by arguing that genuine forgiveness would have to be purely unconditional but could never appear as such. (shrink)

Ethics guidance and ethical frameworks are becoming more explicit and prevalent in health policy proposals. However, little attention has been given to evaluating their roles and impacts in the policy arena. Before this can be investigated, fundamental questions must be asked about the nature of ethics in relation to policy, and about the nexus of the fields of applied ethical analysis and health policy analysis. This paper examines the interdisciplinary stretch between bioethics and health policy analysis. In particular, it highlights (...) areas of scholarship where a health policy ethicsspecialization—as distinctive from bioethics—might develop to address health policy concerns. If policy and ethics both ask the same question, that question is: “What is the good, and how do we achieve (create, protect, cultivate) it?” To answer this question, the new field of “health policy ethics” requires development. First, we should develop a full set of ethical principles and complementary ethical theories germane to public policy per se. Second, we must understand better how explicit attention to ethical concerns affects policy dynamics. Third, we require new policy and ethical analytic approaches that contribute to constructive (not obstructive) policy making. Finally, we need indicators of robust, high quality ethical analysis for the purpose public policy making. (shrink)

It is common for these plans to state the importance of drawing upon a well-developed (...) ethics framework and we argue that this framework should reflect the values and insights of feminist relational theory. More specifically, we argue that pandemic planning must be squarely situated in the larger realm of public health and that an ethics framework for public health will be one that recognizes the need to pay particular attention to the vulnerability of subpopulations lacking in social and economic power. We propose an ethics framework for public health that builds on the notions of relational personhood (including relational autonomy and social justice) and relational solidarity. Physicians and nurses need to sustain their unique strengths and work in true collaboration, recognizing their interdependence and the complementarity of their knowledge, skills and perspectives, as well as their common moral commitments. In this article, challenges often faced by both nurses and physicians in working collaboratively are explored with a focus on the ways in which each profession's preparation for practice has differed over time, including shifts in knowledge development and codes of ethics guiding their practice. A call for (...) envisioning their practice as shared moral work as well as practical strategies to begin that work are offered as a basis for reflection towards enhanced nurse-physician relationships. During the 1920s and 1930s geneticist L. C. Dunn of Columbia University cautioned Americans against endorsing eugenic policies and called attention to eugenicists' less than rigorous practices. Then, from the mid-1940s to early 1950s he attacked scientific racism and Nazi Rassenhygiene by co-authoring Heredity, Race and Society with Theodosius Dobzhansky and collaborating with members of UNESCO on their international campaign against racism. Even though shaking the foundations of scientific discrimination was Dunn's primary concern during the interwar and post-World War II (...) years, his campaigns had ancillary consequences for the discipline. He contributed to the professionalization of genetics during the 1920s and 1930s and sought respectability for human genetics in the 1940s and 1950s. My article aims to elucidate the activist scientist's role in undermining scientific discrimination by exploring aspects of Dunn's scientific work and political activism from the 1920s to 1950s. Worldwide, there is renewed public and political attention focused on public health fueled by the globally explosive H1N1 pandemic. Pandemic planning emerged as a major area for public action in the absence of an overarching ethics framework appropriate for the community and population focus of public health. Baylis, Sherwin, and Kenny propose relational personhood and relational solidarity as core values for a public health ethics. The Catholic faith tradition makes three useful contributions in support of a relational ethic: first, a (...) religious ontology that aligns with the view that human persons are inherently relational; second, a coherent account of the requisite social and communal ideals and structures that this belief demands; and third, inspiration and motivation for the attitudes and actions that are required in response to a relational ontology. In a recent article, Director makes the case that many individuals with bipolar disorder have the capacity to consent to many decisions while acutely manic, even when those decisions are out of character and cause harm. Referring to recent qualitative evidence, I argue that Director overlooks a key mechanism of manic incapacity, an inflexible experience of the future that impairs one’s ability to value. Without attention to the illness-specific experience of decision-making, capacity assessments risk false negatives in people with mania.

Theories of deliberative democracy have faced the charge of leading to the unjust exclusion of voices from public deliberation. The recent rhetorical turn in deliberative theory aims to respond to this charge. I distinguish between two variants of this response: the supplementing approach and the systemic approach. On the supplementing approach, rhetorical modes of political speech may legitimately supplement the deliberative process, for the sake of those excluded from the latter. On the systemic approach, rhetorical modes of political speech are (...) legitimate within public deliberation, just so long as they result in net benefits to the deliberative system. I argue that neither of these two approaches adequately meets the unjust exclusion charge. Whereas the supplementing approach does not go far enough to incorporate rhetorical speech into public deliberation, the systemic approach goes too far by legitimizing forms of rhetoric that risk only exacerbating the problem of unjust exclusion. More constructively, I draw on Aristotle’s conception of rhetoric, as an art that is a counterpart to dialectic, to argue for a constitutive approach to rhetoric. I show how this approach provides a more expansive notion of deliberation that remains normatively orientated. (shrink)

The aim of this paper is to bring into view an ethical task that we face when grieving the loss of a loved one. That task is to see the independent reality of the lost other. I shall do so through a reading of C. S. Lewis’s A Grief Observed. I shall try to show that Lewis’s struggle to see the independent reality of his wife, Joy, provides an important, and troubling, insight into what it means for us to grieve (...) well. Lewis’s account forces us to reflect on a key, but largely overlooked, assumption in contemporary philosophical accounts of grief, namely, that we do indeed see the independent reality of the lost other. Lewis’s account reveals that the struggle to see the lost other is at the same time a struggle to escape deeply-rooted aspects of the self. (shrink)

Throughout history Christians have responded to the need for direct care for the sick in imitation of the healing ministry of Jesus and in the creation of hospitals as signs of God’s love. The COVID-19 pandemic has created a global, unprecedented modern experience of vulnerability. It has resulted in moral distress for doctors and health care workers in overwhelmed facilities. It has also revealed profound inequity in access to health care, the tragic consequences of the neglect of public health and (...) its focus on communities, and the effects of poverty and marginalization on the risk of illness and death. This paper proposes insights from Catholic social teaching that can guide a renewal of public health by rebalancing duties to individuals and communities, and renewing medicine as a public trust. Our political climate is increasingly characterised by hostility towards constructed others. Steven Gormley answers the question: what does it mean to do justice to others? He pursues this question by developing a critical, but productive, dialogue between deliberative theory and deconstruction. Two key claims emerge from this. First: doing justice to the other demands that we maintain an ethos of interruption. And secondly: Such an ethos requires a democratic form of politics. In _The __Critical Advantage_, noted scholar and early childhood expert William T. Gormley, Jr. takes a wide-ranging look at the important role of critical thinking in preparing students for college, careers, and civic life. Drawing on research from psychology, philosophy, business, political science, neuroscience, and other disciplines, he offers a contemporary definition of critical thinking and its relationship to other forms of thinking, including creative thinking and problem solving. When defined broadly and taught early, he argues, critical thinking is (...) a “potential cure for some of the biggest problems we face as a nation,” including education deficits, employment deficits, and the recent surge of partisanship in democratic politics. While there are encouraging signs—the Common Core State Standards have drawn attention to the importance of critical thinking—recent efforts have been too narrowly focused on improving textual analysis in high school. Those who might benefit the most from curricula prioritizing critical thinking, including disadvantaged students, are less likely to be represented in courses and other activities that encourage this skill. Gormley argues for prioritizing critical thinking skills in PreK–12. He takes readers into innovative classrooms around the country, including schools in Pennsylvania, Oklahoma, and Virginia, and offers specific recommendations for promoting critical thinking and embedding it across the curriculum. _The Critical Advantage_ is an insightful and fascinating account of an intellectual journey culminating in a fresh vision for the future of US schooling. The wide breadth of the African elements fit Black Panther well within Afrofuturism, a genre defined by its use and placement of people of African descent in the past, present, and future of society. Beyond these cultural elements, Black Panther 's Afrofuturism employs water imagery and spinal cord injury as potent symbols of disconnection and reconnection. Black Panther draws from a long tradition of Afrofuturist literature that is influenced by a desire to remedy (...) the injuries of the past and reconnect people of African descent with the continent from which they've been severed. Black Panther 's Afrofuturism reimagines antebellum slavery society's flight fantasy as a powerful science fiction, T'Challa's Royal Talon Flyer. The narratives of T'Challa, Shuri, and Wakanda have been continuously remolded and expanded upon since the 1960s by artists who have been inspired by other Afrofuturist works. (shrink)

Abstract A principle aim of this paper is to convince friends and critics of deconstruction that they have overlooked two crucial aspects of Derrida's work, namely, his rearticulation of the concept of experience and his account of the experience of undecidability as an ordeal. This is important because sensitivity to Derrida's emphasis on the ordeal of undecidability and his rearticulation of the concept of experience-a rearticulation that is already under way in his early engagement with Husserl and continued in later (...) work-necessitates a rethinking of what the `experience of undecidability' entails. Rather than signaling a withdrawal from politics or a normatively impotent ethics of `mere openness to the other,' Derrida's account of the experience of undecidability not only points to a fundamental aspect of our basic ethical experience but also leads to a number of ethico-political demands, which I summarize as the demand to maintain an ethos of interruption. The present commentary addresses two issues arising from Memmert’s paper. First, can the ‘misdirection’ and ‘inattentional blindness’ paradigms provide important insights into the relationship between ‘overt’ and ‘covert’ attentional processes? Second, what are the most fruitful directions for research that seeks to combine these attentional paradigms in ecologically valid settings? We argue that although Memmert’s paper postulates several important differences between the misdirection and inattentional blindness paradigms, it may not emphasise sufficiently strongly the significant insights into attention that have been (...) yielded by the former approach. To illustrate, we discuss the utility of the misdirection paradigm in providing an ecologically valid method to investigate the relationship between overt and covert attentional processes. Such naturalistic methods are required to ensure optimal integration of the misdirection and inattentional blindness paradigms within a general theory of attention. (shrink)

Special moral, regulatory, and scientific questions surround the inclusion of children in health-related research. These questions arise from a fundamental moral tension between the obligation to expose children to research participation to ensure that they share in the benefits that arise from it and the obligation to protect them from the harms associated with their inappropriate involvement in research.
