Background Healthcare systems are increasingly struggling with resource constraints, given demographic changes, technological development, and citizen expectations. The aim of this article is to normatively analyze different suggestions regarding how publicly financed plastic surgery should be delineated in order to identify a well-considered, normative rationale. The scope of the article is to discuss general principles and not define specific conditions or domains of plastic surgery that should be treated within the publicly financed system. Methods This analysis uses a reflective equilibrium (...) approach, according to which considered normative judgements in one area should be logically and argumentatively coherent with considered normative judgements and background theories at large within a system. Results and conclusions In exploring functional versus non-function conditions, we argue that it is difficult to find a principled reason for an absolute priority of functional conditions over non-functional conditions. Nevertheless, functional conditions are relatively easier to establish objectively, and surgical intervention has a clear causal effect on treating a functional condition. Considering non-functional conditions that require plastic surgery [i.e., those related to appearance or symptomatic conditions ], we argue that the patient needs to experience some degree of suffering, which must be ‘validated’ in some form by the healthcare system. This validation is required for both functional and non-functional conditions. Functional conditions are validated by distinguishing between statistically normal and abnormal functioning. Similarly, for non-functional conditions, statistical normality represents a potential method for distinguishing between what should and should not be publicly funded. However, we acknowledge that such a concept requires further development. (shrink)

Cost–benefit analysis (CBA) is much more philosophically interesting than has in general been recognized. Since it is the only well-developed form of applied consequentialism, it is a testing-ground for consequentialism and for the counterfactual analysis that it requires. Ten classes of philosophical problems that affect the practical performance of cost–benefit analysis are investigated: topic selection, dependence on the decision perspective, dangers of super synopticism and undue centralization, prediction problems, the indeterminateness of our control over future decisions, the need to exclude (...) certain consequences for moral reasons, bias in the delimitation of consequences, incommensurability of consequences, difficulties in defending the essential requirement of transferability across contexts, and the normatively questionable but equally essential assumption of interpersonal compensability. The AGM (Alchourrón-Gärdenfors-Makinson) model of belief change is extended to cover changes on sets of beliefs that are not closed under logical consequence (belief bases). Three major types of change operations, namely contraction, internal revision, and external revision are axiomatically characterized, and their interrelations are studied. In external revision, the Levi identity is reversed in the sense that one first adds the new belief to the belief base, and afterwards contracts its negation. It is argued that external revision represents an (...) intuitively plausible way of revising one's beliefs. Since it typically involves the temporary acceptance of an inconsistent set of beliefs, it can only be used in belief representations that distinguish between different inconsistent sets of belief. In non-technical contexts, the word “risk” refers, often rather vaguely, to situations in which it is possible but not certain that some undesirable event will occur. In technical contexts, the word has many uses and specialized meanings. The most common ones are the following.

In order to explore the quantifiability and formalizability of uncertainty a wide range of uncertainties are investigated. They are summarized under eight main categories: factual, possibilistic, metadoxastic, agential, interactive, value, structural, and linguistic uncertainty. This includes both classical uncertainty and the uncertainties commonly called great, deep, or radical. For five of the eight types of uncertainty, both quantitative and non-quantitative formalizations are meaningful and available. For one of them (interactive uncertainty), only non-quantitative formalizations seem to be meaningful, and for two (...) (agential and structural uncertainty) neither quantitative nor non-quantitative formalization seems to be a useful approach. (shrink)

ABSTRACT Semi-revision is a mode of belief change that differs from revision in that the input sentence is not always accepted. A constructive approach to semi-revision is proposed. It requires an efficient treatment of local inconsistencies, which is more easily obtainable in belief base models than in belief set models. Axiomatic characterizations of two semi-revision operators are reported.

Over the last few decades, philosophers and social scientists have applied the so-called powers ontology to the social domain. I argue that this application is highly problematic: many of the alleged powers in the social realm violate the intrinsicality condition, and those that can be coherently taken to be intrinsic to their bearers are arguably causally redundant. I end the paper by offering a diagnosis of why philosophers and social scientists have been tempted to think that there are powers in (...) social realm. (shrink)

A definition of pseudoscience is proposed, according to which a statement is pseudoscientific if and only if it (1) pertains to an issue within the domains of science, (2) is not epistemically warranted, and (3) is part of a doctrine whose major proponents try to create the impression that it is epistemically warranted. This approach has the advantage of separating the definition of pseudoscience from the justification of the claim that science represents the most epistemically warranted statements. The definition is (...) used to explain why proponents of widely divergent criteria for the demarcation between science and pseudoscience tend to be in almost complete agreement on the particular demarcations that should presumably be based on these general criteria. This article is an attempt at a systematic account of decision making under greater uncertainty than what traditional, mathematically oriented decision theory can cope with. Four components of great uncertainty are distinguished: (1) the identity of the options is not well determined (uncertainty of demarcation) ; (2) the consequences of at least some option are unknown (uncertainty of consequences); (3) it is not clear whether information obtained from others, such as experts, can be relied on (uncertainty of reliance); and (4) (...) the values relevant for the decision are not determined with sufficient precision (uncertainty of values). Some possible strategy types are proposed for each of these components. Decisions related to environmental issues are used to illustrate the proposals. (shrink)

It has been much debated whether epistemic relativism in academia, for instance in the form of social constructivism, the strong programme, deconstructionism, and postmodernism, has paved the way for the recent upsurge in science denial, in particular climate science denial. In order to provide an empirical basis for this discussion, an extensive search of the social science literature was performed. It showed that in the 1990s, climate science was a popular target among academic epistemic relativists. In particular, many STS scholars (...) used it as an allegedly clear example of claims by natural scientists that should be treated as mere social constructions, rather than as reports on the actual state of the natural world. A few connections between social constructivists and corporate science denialism were also uncovered, but the extent of such connections could not be determined. With few exceptions, the stream of criticism of climate science from academic relativists has dwindled since the 1990s. One reason for this seems to be that the contrarian position lost its attraction when it became associated with corporate and right-wing propagandists. (shrink)

Specified meet contraction is the operation defined by the identity where ∼ is full meet contraction and f is a sentential selector, a function from sentences to sentences. With suitable conditions on the sentential selector, specified meet contraction coincides with the partial meet contractions that yield a finite-based contraction outcome if the original belief set is finite-based. In terms of cognitive realism, specified meet contraction has an advantage over partial meet contraction in that the selection mechanism operates on sentences rather (...) than on temporary infinite structures (remainders) that are cognitively inaccessible. Specified meet contraction provides a versatile framework in which other types of contraction, such as severe withdrawal and base-generated contraction, can be expressed with suitably chosen properties of the sentential selector. A new formal model of belief dynamics is proposed, in which the epistemic agent has both probabilistic beliefs and full beliefs. The agent has full belief in a proposition if and only if she considers the probability that it is false to be so close to zero that she chooses to disregard that probability. She treats such a proposition as having the probability 1, but, importantly, she is still willing and able to revise that probability assignment if she receives information (...) that gives her sufficient reasons to do so. Such a proposition is undoubted, but not undoubtable. In the formal model it is assigned a probability 1 − δ, where δ is an infinitesimal number. The proposed model employs probabilistic belief states that contain several underlying probability functions representing alternative probabilistic states of the world. Furthermore, a distinction is made between update and revision, in the same way as in the literature on belief change. The formal properties of the model are investigated, including properties relevant for learning from experience. The set of propositions whose probabilities are infinitesimally close to 1 forms a belief set. Operations that change the probabilistic belief state give rise to changes in this belief set, which have much in common with traditional operations of belief change. (shrink)

An operator of contraction for a belief set (a theory) can be obtained by assigning to it a belief base and an operator of partial meet contraction for that base. It is argued that closure of the base under disjunction is an intuitively reasonable condition. Axiomatic characterizations are given of the contractions of belief sets that can be generated by (various types of) partial meet contraction on disjunctively closed bases. Different similarity relations on sets are introduced, and their logical properties are investigated. Close relationships are shown to hold between similarity relations that are based on symmetrical difference and operators of belief contraction that are based on relational selection functions. Two new rationality criteria for minimal belief contraction, the maximizing property and the reducing property, are proposed.

In Elder-Vass’s response to my it is maintained: that a social object is not identical with but is merely composed of its suitably interrelated parts; that a social object is necessarily indistinguishable in terms of its causal capacities from its interrelated parts; and that ontological individualism lacks an adequate ontological justification. In this reply, I argue that in view of the so-called redescription principle defended by Elder-Vass ought to be reformulated and renamed; that the conjunction of and renders social objects (...) causally redundant; and that ontological individualism can be coherently formulated and theoretically justified within Elder-Vass’s own metaphysics of objects with causal powers. An alternative to the standard endurance/perdurance accounts of persistence has recently been developed: the stage theory (Sider, T. Four-Dimensionalism: an Ontology of Persistence and Time. Oxford: Oxford University Press, 2001; Hawley, K. How Things Persist. Oxford: Oxford University Press, 2001). According to this theory, a persisting object is identical with an instantaneous stage (temporal part). On the basis of Leibniz's Law, I argue that stage theorists either have to deny the alleged identity (i.e., give up their central thesis) or hold (...) that stages are both instantaneous and continuants. I subsequently show that, although stage theory is flexible enough to accommodate the latter claim, the cost for accommodating it is an excessive proliferation of persistence concepts. (shrink)

Available methods such as technology assessment and risk analysis have failed to predict the effects of technological choices. We need to give up the futile predictive ambitions of previous approaches and instead base decisions on systematic studies of alternative future developments. It will then be necessary to cope with mere possibility arguments, i.e., arguments in which a conclusion is drawn from a mere possibility that a course of action may have certain consequences. A five-step procedure is proposed for the assessment (...) of such arguments. It includes a search for mere possibility arguments pointing in different directions, a scientific evaluation that may lead to the specification or refutation of some of these arguments, two symmetry tests, evaluation of the seriousness of the arguments in terms of novelty, spatio-temporal unlimitedness and interference with complex systems, and hypothetical retrospection that aims at finding a course of action that will be defensible in retrospect. Setting off from a familiar distinction in the philosophy of properties, this paper introduces a tripartite distinction between sparse causation, abundant causation and mere abundant causation. It is argued that the contrast between sparse and mere abundant causation allows us to resolve notorious philosophical issues having to do with negative causation, causation involving institutional properties and physical macro-causation in a way that is unified, intuitive and in line with scientific doctrines and practices.

In 1974, a wonderful little book came out entitled Formal Philosophy: Selected Papers of Richard Montague, edited by Richmond H. Thomason. The book was a beautiful testimony to the fact that formal methods may indeed clarify, sharpen and solve philosophical problems, defusing airy philosophical intuitions in clear, crisp and concise ways while at the same time turning philosophical wonder into scientific inquiry.

It was deemed important that the donating woman or couple made the choice to donate embryos voluntarily. A general theory of coherence is proposed, in which systemic and relational coherence are shown to be interdefinable. When this theory is applied to sets of sentences, it turns out that logical closure obscures the distinctions that are needed for a meaningful analysis of coherence. It is concluded that references to “all beliefs” in coherentist phrases such as “all beliefs support each other” have to be modified so that merely derived beliefs are excluded. Therefore, in order to avoid absurd conclusions, (...) coherentists have to accept a weak version of epistemic priority, that sorts out merely derived beliefs. Furthermore, it is shown that in belief revision theory, coherence cannot be adequately represented by logical closure, but has to be represented separately. (shrink)

Two recent defences of the recovery postulate for contraction of belief sets are analyzed. It is concluded that recovery is defensible as a by-product of a formalization that is idealized in the sense of being simplified for the sake of clarity. However, recovery does not seem to be a required feature of the doxastic behaviour of ideal (perfectly rational) agents. It is reasonable to expect that there should be epistemic residues (remnants of rejected beliefs), but not that these should always (...) suffice to recover all the original beliefs if the discarded belief is reinstated. By replacement is meant an operation that replaces one sentence by another in a belief set. Replacement can be used as a kind of Sheffer stroke for belief change, since contraction, revision, and expansion can all be defined in terms of it. Replacement can also be defined either in terms of contraction or in terms of revision. Close connections are shown to hold between axioms for replacement and axioms for contraction and revision. Partial meet replacement is axiomatically characterized. Our uncertainty about matters of fact can often be adequately represented by probabilities, but there are also cases in which we, intuitively speaking, know too little even to assign meaningful probabilities. In many of these cases, other formal representations can be used to capture some of the prominent features of our uncertainty. This is a non-technical overview of some of these representations, including probability intervals, belief functions, fuzzy sets, credal sets, weighted credal sets, and second order probabilities.

A value statement such as “she is a good teacher” is categoryspecified, i.e., the criteria of evaluation are specified as those that are applicable to a given category, in this case the category of teachers. In this study of categoryspecified value statements, certain categories are identified that cannot be used to specify value aspects. Special attention is paid to categories that are constituted by functional characteristics. The logical properties of value statements that refer to such categories are shown to differ (...) significantly from the corresponding properties in social choice theory. (shrink)

Science is praxis relevant to the extent that it guides goal-directed action by telling us how to act in order to achieve the goals. Investigations aiming at high praxis relevance are performed in various disciplines under names such as clinical trials, evaluation research, intervention research and social experiments. In this contribution, the notion of (direct) praxis relevance is delineated, and it is distinguished from related properties of science such as those of being applied and being practically useful in a wider (...) sense. Recommendations for the achievement of praxis relevance are offered in the form of five principles: the prerogative of direct experiments, minimized theory-induced uncertainty, multiple approximations, causal chain decomposition and successive improvements. Safety is an essential ethical requirement in engineering design. Strategies for safe design are used not only to reduce estimated probabilities of injuries but also to cope with hazards and eventualities that cannot be assigned meaningful probabilities. The notion of safe design has important ethical dimensions, such as that of determining the responsibility that a designer has for future uses (and misuses) of the designed object.

The basic assumption of repertoire contraction is that only some of the logically closed subsets of the original belief set are viable as contraction outcomes. Contraction takes the form of choosing directly among these viable outcomes, rather than among cognitively more far-fetched objects such as possible worlds or maximal consistent subsets of the original belief set. In this first investigation of repertoire contraction, postulates for various variants of the operation are introduced. Necessary and sufficient conditions are given for when repertoire (...) contraction coincides with AGM contraction or with operations generated by AGM-style contraction on a belief base. A close connection is shown to hold between repertoire contraction and specified meet contraction. (shrink)

This paper asks whether persistence can be a matter of convention. It argues that in a rather unexciting de dicto sense persistence is indeed a matter of convention, but it rejects the notion that persistence can be a matter of convention in a more substantial de re sense. However, scenarios can be imagined that appear to involve conventional persistence of the latter kind. Since there are strong reasons for thinking that such conventionality is impossible, it is desirable that our metaphysical-cum-semantic (...) theories of persistence be able to account for such scenarios in terms of conventions of the first kind. Later parts of the article therefore investigate whether three of the currently most influential metaphysical-cum-semantic theories of persistence—the endurance theory, the stage theory, and the perdurance theory—can do this. Fortunately, for them, it turns out that all can, though some philosophers have disputed this. However, when we ask how they account for a typical case of “conventional persistence” some problematic features of the theories—having to do with reference, persistence conditions, how they relate, and the epistemology of persistence—are revealed. Situationist deontic logic is a model of that fraction of normative discourse which refers to only one situation and one set of alternatives. As we can see from a whole series of well-known paradoxes, standard deontic logic (SDL) is seriously mistaken even at the situationist level. In this paper it is shown how a more realistic deontic logic can be based on the assumption that prescriptive predicates satisfy the property of contranegativity. A satisfactory account of situation-specific norms is a necessary (...) prerequisite for a successful treatment of more complex normative structures. (shrink)

The ideal world semantics of standard deontic logic identifies our obligations with how we would act in an ideal world. However, to act as if one lived in an ideal world is bad moral advice, associated with wishful thinking rather than well-considered moral deliberation. Ideal world semantics gives rise to implausible logical principles, and the metaphysical arguments that have been put forward in its favour turn out to be based on a too limited view of truth-functional representation. It is argued (...) that ideal world semantics should be given up in favour of other, more plausible uses of possible worlds for modelling normative subject-matter. (shrink)

The remainder set A⟂B of a set of sentences A modulo a set of sentences B is the set of all maximal subsets of A not implying any element of B. A remainder equation is an expression containing remainder sets, such as {A} = B⟂X, in which at least one set is unknown. Solutions to some classes of remainder equations are reported, and some unsolved problems are listed.

Real-life decision-making often begins with a disorderly decision problem that has to be clarified and systematized before a decision can be made. This is the process of decision structuring that has largely been ignored both in decision theory and applied decision analysis. In this contribution, ten major components of decision structuring are identified, namely the determination of its scope, subdivision, agency, timing, options, control ascriptions, framing, horizon, criteria and restructuring. The typical function of goals is to regulate action in a way that furthers goal achievement. Goals are typically set on the assumption that they will help bring the agent closer to the desired state of affairs. However, sometimes endorsement of a goal, or the processes by which the goal is set, can obstruct its achievement. When this happens, the goal is self-defeating. Self-defeating goals are common in both private and social decision-making but have not received much attention by decision (...) theorists. In this paper, we investigate different variants of three major types of self-defeating mechanisms: The goal can be an obstacle to its own fulfilment, goal-setting activities can impede goal achievement, and disclosure of the goal can interfere with its attainment. Different strategies against self-defeasance are tentatively explored, and their efficiency against different types of self-defeasance is investigated. (shrink)
