BackgroundLow literacy of study participants in Sub - Saharan Africa has been associated with poor comprehension during the consenting process in research participation. The concerns in comprehension are far greater when consenting to participate in genomic studies due to the complexity of the science involved. While efforts are made to explore possibilities of applying genomic technologies in diseases prevalent in Sub Saharan Africa, we ought to develop methods to improve participants’ comprehension for genomic studies. The purpose of this study was (...) to understand different approaches that can be used to seek consent from individuals with low literacy in Sub-Saharan African countries in genomic research to improve comprehension.MethodsUsing qualitative study design, we conducted focus-group discussions, in-depth interviews and participant observations as data collection methods. This study was embedded in a hospital based genomic study on Sickle Cell Disease at Muhimbili National Hospital in Tanzania. Thematic content analysis was used to analyse the transcripts and field notes.ResultsFindings from this study show that literacy level has little influence on understanding the research details. According to the participants of this study, the methods used to provide information, the language, and time spent with the study participants were the key factors influencing understanding. The availability of group sessions held before individual consent to allow for a detailed questions and answers format was agreed to be the best method to facilitate the comprehension.ConclusionThe quality of the consenting process of participants will be influence by a number of factors. The type of research consented for, where the research will be implemented and who are the potential study participants are amongst the factors that need to be assessed during the consenting. Measures to improve participants’ comprehension need to be developed when consenting participants with low literacy level in genomic studies. (shrink)

Several key issues newly discussed in these proposed revisions include transgender policies, the question of referrals for unethical clinical interventions, and triage and limited-resource allocation protocols for crisis situations. In Evangelium vitae, Pope St. John Paul II addresses euthanasia and physician-assisted suicide by striking a balance—maintaining the inherent dignity of all persons while considering the lived experience of those struggling to see dignity amidst suffering. Subsequently, a debate about the word dignity has led to clarifications from the President’s Council on Bioethics regarding different uses of the word. This essay relies on the work of the council, especially an essay by Edmund Pellegrino, to provide a basis for reflecting on (...) Ars moriendi, or The Art of Dying, was a highly influential fifteenth-century text designed to guide dying persons and their loved ones in Catholic religious practices at a time when access to priests and the sacraments was limited. Given recent challenges related to the coronavirus pandemic, there is a heightened need to offer additional forms of guidance related to death and dying. This essay examines the content of the Ars moriendi and considers how key principles from the work apply to (...) the current context. We present a cross-cultural analysis showing that the presence of an active or moral High God in societies varies generally along a continuum from lesser to greater technological complexity and subsistence productivity. Foragers are least likely to have High Gods. Horticulturalists and agriculturalists are more likely. Pastoralists are most likely, though they are less easily positioned along the productivity continuum. We suggest that belief in moral High Gods was fostered by emerging leaders in societies dependent on resources that were difficult (...) to manage and defend without group cooperation. These leaders used the concept of a supernatural moral enforcer to manipulate others into cooperating, which resulted in greater productivity. Reproductive success would accrue most to such leaders, but the average reproductive success of all individuals in the society would also increase with greater productivity. Supernatural enforcement of moral codes maintained social cohesion and allowed for further population growth, giving one society an advantage in competition with others. (shrink)

It is my contention that what is generally construed as the Euthyphro Dilemma as a reason to deny that moral facts are based on theological facts is one of the worst arguments proposed in philosophy of religion or ethical theory, and that Socrates, the character of the dialogue who poses the dilemma, was both morally bankrupt in his challenge to Euthyphro, but more importantly here, ought to have lost the argument hands down. But in any dialogue, the author controls what (...) people say. Plato was able to easily give Socrates the victory by writing the ending of the story himself, where Euthyphro, believing that piety is what the gods approve of, loses the argument abysmally. The version of events presented here is different. This time, Euthyphro is permitted to offer a reasonable defence of his position, and he has the benefit of having been able to read all that has been said on the Euthyphro dilemma over the last couple of millennia, and especially the last fifty years. Southeast Asia has numerous religions and diverse forms of state-governance, so the populations largely have the freedom to express themselves within the context of their society. Expressing oneself can occur within the context of their religion, using the language they have been cultured within, if they remain in their cultural-context. This paper explores the context of Buddhist nations using militarized-language, seen as problematic by Dr. Matthew Kosuta, who professes in his masters-thesis that it is a contradiction. A portion of my (...) article refutes his position, on the grounds that Buddhism developed within the Hindu-Brahmanism context and caste was normalized in regions influenced by such culture. Equally, it could be noted, the Buddha also used a language that was suitable for agriculturalists, merchants, Brahmins, so a militarized-language was just one way to speak to some people within their own social context, which was understandable not just to them, but to anyone interested in attaining Nibbāna. William Hasker thinks that his emergent dualism provides a plausible account of the mind’s survival of bodily death, giving it a crucial advantage over physicalism. I do not share this appraisal. Emergentism by its very nature works against the (immediate) survival of death. The analogies that Hasker employs to overcome this initial implausibility fail due to factual errors, and his position ends up in no less a difficult position than the physicalism that Hasker rejects. Rare copy number variants have a prominent role in the aetiology of schizophrenia and other neuropsychiatric disorders. Substantial risk for schizophrenia is conferred by large CNVs at several loci, including microdeletions at 1q21.1, 3q29, 15q13.3 and 22q11.2 and microduplication at 16p11.2. However, these CNVs collectively account for a small fraction of cases, and the relevant genes and neurobiological mechanisms are not well understood. Here we performed a large two-stage genome-wide scan of rare CNVs and report the significant association of copy (...) number gains at chromosome 7q36.3 with schizophrenia. Microduplications with variable breakpoints occurred within a 362-kilobase region and were detected in 29 of 8,290 patients versus 2 of 7,431 controls in the combined sample. All duplications overlapped or were located within 89 kilobases upstream of the vasoactive intestinal peptide receptor gene VIPR2. VIPR2 transcription and cyclic-AMP signalling were significantly increased in cultured lymphocytes from patients with microduplications of 7q36.3. These findings implicate altered vasoactive intestinal peptide signalling in the pathogenesis of schizophrenia and indicate the VPAC2 receptor as a potential target for the development of new antipsychotic drugs. © 2011 Macmillan Publishers Limited. ‘Fake news’ has become an increasingly common refrain in public discourse, though the term itself has several uses, at least one of which constitutes Frankfurtian bullshit. After examining what sorts of fake news appeals do and do not count as bullshit, I discuss strategies for overcoming our openness to such bullshit. I do so by drawing a parallel between openness to bullshit and naïve skepticism—one’s willingness to reject the concept of truth on unsupported or ill-considered grounds—and suggest that this parallel (...) indicates three principles for how we ought to combat our openness to fake news and other bullshit. First, the root causes of bullshit openness are not monolithic; we should adopt anti-bullshit strategies in recognition of this fact. Second, our efforts to overcome bullshit openness should be collaborative efforts to create an environment that allows for sustained interrogation of our bullshit openness, rather than a confrontational provision of contrary evidence, despite the fact that such strategies are more time-intensive. Third, social media is unlikely to be a fertile ground on which we will make meaningful progress in the fight against bullshit because of the inherent nature of social media platforms as spaces for short, declarative, confrontational claims. In about 700, a century after his death, the Life of Columba was written by Adomnán, ninth abbot of Iona. It has long been valued as the major primary source on the subject, for the light it (...) throws on early medieval Scotland and Ireland, and as an important work of literature. The 1961 edition of the Life, by Alan and Marjorie Anderson, has long been unavailable. Marjorie Anderson has now revised both the Latin text and the English translation, provided new historical notes, and rewritten the Introduction to take account of recent work in the area. Kant limits cosmopolitan right to a universal right of hospitality, condemning European imperial practices towards indigenous peoples, while allowing a right to visit foreign countries for the purpose of offering to engage in commerce. I argue that attempts by contemporary theorists such as Jeremy Waldron to expand and update Kant’s juridical category of cosmopolitan right would blunt or erase Kant’s own anti-colonial doctrine. Waldron’s use of Kant’s category of cosmopolitan right to criticize contemporary identity politics relies on premises that (...) upset Kant’s balanced right to hospitality. An over-extensive right to visit can invoke “Kantian” principles that Kant himself could not have consistently held, without weakening his condemnation of European settlement. I construct an alternative spirit of cosmopolitan right more favorable to the contemporary claims of indigenous peoples. Kant’s analysis suggests there are circumstances when indigenous peoples may choose whether to engage in extensive cultural interaction, and reasonably refuse the risks of subjecting their claims to debate in democratic politics in a unitary public. Cosmopolitan right accorded respect to peoples; any “domestic” adaptation of cosmopolitan right should respect indigenous peoples as peoples, absent a serious public explanation by a democratic state for why it has now become appropriate to treat indigenous peoples merely as individual citizens. When people think about how a situation might have turned out differently, they tend to imagine counterfactual alternatives to their actions. We report the results of three experiments which show that people imagine alternatives to actions differently when they know about a reason for the action. The first experiment ( n = 36) compared reason - action sequences to cause - effect sequences. It showed that people do not imagine alternatives to reasons in the way they imagine alternatives to causes: (...) they imagine an alternative to an action more than an effect, and to a cause more than a reason. The second experiment ( n = 214) and the third experiment ( n = 190) both show that different sorts of reasons have different sorts of effects on how people imagine alternatives to actions. People imagine an alternative to an action (the protagonist went to a ball) less often when they know the reason for the action was an obligation (he had to participate in fundraising) compared to when they know about a weaker reason (he wanted to meet a famous violinist) or no reason. The second experiment shows the effect for a social obligation and the third experiment replicates and extends it to a health obligation. We interpret the results in terms of the possibilities that people keep in mind about actions and their reasons. (shrink)

Background The Patient Right to Autonomy Act (PRAA), implemented in Taiwan in 2019, enables the creation of advance decisions (AD) through advance care planning (ACP). This legal framework allows for the withholding and withdrawal of life-sustaining treatment (LST) or artificial nutrition and hydration (ANH) in situations like irreversible coma, vegetative state, severe dementia, or unbearable pain. This study aims to investigate preferences for LST or ANH across various clinical conditions, variations in participant preferences, and factors influencing these preferences among urban (...) residents. Methods Employing a survey of legally structured AD documents and convenience sampling for data collection, individuals were enlisted from Taipei City Hospital, serving as the primary trial and demonstration facility for ACP in Taiwan since the commencement of the PRAA in its inaugural year. The study examined ADs and ACP consultation records, documenting gender, age, welfare entitlement, disease conditions, family caregiving experience, location of ACP consultation, participation of second-degree relatives, and the intention to participate in ACP. Results Data from 2337 participants were extracted from electronic records. There was high consistency in the willingness to refuse LST and ANH, with significant differences noted between terminal diseases and extremely severe dementia. Additionally, ANH was widely accepted as a time-limited treatment, and there was a prevalent trend of authorizing a health care agent (HCA) to make decisions on behalf of participants. Gender differences were observed, with females more inclined to decline LST and ANH, while males tended towards accepting full or time-limited treatment. Age also played a role, with younger participants more open to treatment and authorizing HCA, and older participants more prone to refusal. Conclusion Diverse preferences in LST and ANH were shaped by the public’s current understanding of different clinical states, gender, age, and cultural factors. Our study reveals nuanced end-of-life preferences, evolving ADs, and socio-demographic influences. Further research could explore evolving preferences over time and healthcare professionals’ perspectives on LST and ANH decisions for neurological patients.. (shrink)

Objective: To explore the experiences of people with a “terminal illness”, focusing on the patients’ perspective of euthanasia and assisted suicide.Method: A qualitative study using narrative interviews was conducted throughout the UK. The views of the 18 people who discussed euthanasia and assisted suicide were explored. These were drawn from a maximum variation sample, who said that they had a “terminal” illness, malignant or non-malignant.Results: That UK law should be changed to allow assisted suicide or voluntary euthanasia was felt strongly (...) by most people. Those who had seen others die were particularly convinced that this should be a right. Some had multiple reasons, including pain and anticipated pain, fear of indignity, loss of control and cognitive impairment. Those who did not want to be a burden also had other reasons for wanting euthanasia. Suicide was contemplated by a few, who would have preferred a change in the law to allow them to end their lives with medical help and in the company of family or friends. The few who opposed a change in UK law, or who felt ambivalent, focused on involuntary euthanasia, cited religious reasons or worried that new legislation might be open to abuse.Conclusion: Qualitative research conducted on people who know they are nearing death is an important addition to the international debate on euthanasia and assisted suicide. Tim Mulgan develops a new theory of our obligations to future generations, based on a new rule-consequentialist account of the morality of individual reproduction. His aim is to produce a coherent, intuitively plausible moral theory that is not unreasonably demanding, even when extended to cover future people. Most people are skeptical of the claim that the expectation that a person would have a life that would be well worth living provides a reason to cause that person to exist. In this essay I argue that to cause such a person to exist would be to confer a benefit of a noncomparative kind and that there is a moral reason to bestow benefits of this kind. But this conclusion raises many problems, among which is that it must be (...) determined how the benefits conferred on people by causing them to exist weigh against comparable benefits conferred on existing people. In particular, might the reason to cause people to exist ever outweigh the reason to save the lives of existing people? (shrink)

This chapter explores policy questions that arise from accepting our moral arguments for happy‐people‐pills. It looks at arguments from the liberty and justice point of view which support the policy prescription that society should permit the development of happy‐people‐pills. The crucial difference is that the justice argument is compatible with paternalism in a way that the liberty argument is not. The chapter then turns to objections to such a policy based on adverse effects on health and society at large. Finally, (...) it addresses the question of how happy‐people‐pills ought to be justly distributed. The chapter examines at length the question of what role, if any, governments ought to have in the creation and distribution of happy‐people‐pills. It argues that happy‐people‐pills are important for the wellbeing of many, but this in itself does not answer the question of just distribution of this good. (shrink)

Suppose we discover how we could live for a thousand years, but in a way that made us unable to have children. Everyone chooses to live these long lives. After we all die, human history ends, since there would be no future people. Would that be bad? Would we have acted wrongly? Some pessimists would answer No. These people are saddened by the suffering in most people’s lives, and they believe it would be wrong to inflict such suffering on others (...) by having children. In earlier centuries, this bleak view was fairly plausible. But our successors would be able to prevent most human suffering. Some optimists would also answer No . . . These [views] are, I believe, deeply mistaken. Given what our successors could achieve in the next million or billion years, here and elsewhere in our galaxy, it would be likely to be very much worse if there were no future people. (shrink)

In a recent paper in this journal, David Chalmers introduced the meta-problem of consciousness as “the problem of explaining why we think consciousness poses a hard problem” (Chalmers, 2018, p. 6). A solution to the meta-problem could shed light on the hard problem of consciousness. In particular, it would be relevant to elucidate whether people’s problem intuitions (i.e. intuitions holding that conscious experience cannot be reduced to physical processes) are driven by factors related to the nature of consciousness, or rather (...) by factors that are independent of consciousness. Whether people hold problem intuitions, and what factors drive those intuitions, are largely empirical questions. However, there is a lack of empirical research on these issues to date. The results of four studies will show that (1) problem intuitions are not widespread, and (2) when they arise, they do so because of factors that are unrelated to the nature of consciousness. This suggests that consciousness is, after all, not so problematic. (shrink)
