An effectiveness assessment on ASCT in locally advanced and metastatic breast cancer identified serious ethical issues associated with this intervention. Our objective was to systematically review these aspects by means of a literature analysis. We chose the reflexive Socratic approach as the review method using Hofmann's question list, conducted a comprehensive literature search in biomedical, psychological and ethics bibliographic databases and screened the resulting hits in a 2-step selection process. The most important ethical issues were the implementation of ASCT in clinical practice on the basis of phase-II trials in the 1990s and the publication of falsified data in the first randomized controlled trials (Bezwoda fraud), which caused significant negative effects on recruiting patients for further clinical trials and the doctor-patient relationship. Recent meta-analyses report a marginal effect in prolonging disease-free survival, accompanied by severe harms, including death. ASCT in breast cancer remains a stigmatized technology. Reported health-related-quality-of-life data are often at high risk of bias in favor of the survivors. Furthermore little attention has been paid to those patients who were dying. The questions were addressed in different degrees of completeness. All arguments were assignable to the questions. That interest has been the traditional stock in the trade of philosophers since the ancient Greeks, and most crucially since the subjectivization of the aesthetic sphere in the speculative thought of Kant and German idealism. But the point is that there is a gap between those philosophers who speculated about perception, aesthetic pleasure, and the enjoyment of the senses, and who tried to justify those speculations through wide-ranging inquiries into the order of the world and its understanding through our ego and its potentialities--betweenthem and Dilthey, who knows that this age-old enthusiasm for metaphysical and epistemological inquiries has become obsolete. The earlier display of erudition may be vast and awe inspiring. But in Dilthey's mind an interest in those inquiries is no longer foremost. Commendable as all of them may be, they approach the world of the work of art and its experience from the outside, in an independent, self-sufficient, and almost autonomous manner, without facing up to the horizon of the world in which an artistic work is rooted. This horizon of the world emerges as the leitmotif of Heinen's book. Heinen reminds the reader that those who create are immersed in this horizon. The conditions under which they work, the tools they have, the methods they use, the materials that are available to them, the ideas they have in mind--all this can be traced back to the world which surrounds them. Moreover, they belong to an historical world. They are oriented toward a past they remember, a future they anticipate; and all they think, or feel, or will, stands in an historical context, long before they are conscious of themselves in this respect. Heinen rightly points out that this creates a problem not only for the analysis of the artistic expression but for the field of hermeneutics, for the method of understanding this expression. It is not so much something that is understood in its meaning by referring to an ego, or a system of cognitive faculties. It emerges from the socio-historical world; and we must place it back in the same horizon from which it emerges. Heinen agrees with Dilthey that we always find ourselves within the whole of life; and when understanding something--a work of art or any other historical object--we have to do it within the full context of life to which we belong. It is an objectification of life that we have to understand from life itself. This may help to explain why a work of art is more than an object created by an artist through his inspiration and the unconscious production of his genius. From his Hegelian perspective Heinen tells us that whenever an artist is involved in the creation of something he is dependent upon the world in which he lives and upon a dialectic between himself and the horizon of life. Heinen expressly defends the probability of this dialectic in a chapter on the presuppositions of Dilthey's aesthetics (pp. 134-162). He studies Dilthey's attempt to discover a methodological basis for the human sciences. Recent years have witnessed intensive efforts to specify which requirements ethical artificial intelligence (AI) must meet. General guidelines for ethical AI consider a varying number of principles important. A frequent novel element in these guidelines, that we have bundled together under the term explicability, aims to reduce the black-box character of machine learning algorithms. The centrality of this element invites reflection on the conceptual relation between explicability and the four bioethical principles. This is important because the application of general ethical (...) frameworks to clinical decision-making entails conceptual questions: Is explicability a free-standing principle? Is it already covered by the well-established four bioethical principles? Or is it an independent value that needs to be recognized as such in medical practice? We discuss these questions in a conceptual-ethical analysis, which builds upon the findings of an empirical document analysis. On the example of the medical specialty of radiology, we analyze the position of radiological associations on the ethical use of medical AI. The Metaphysical Club, a gathering of intellectuals in the 1870s associated with Harvard, is widely recognized as the crucible where pragmatism, America's distinctively original philosophy, was refined and proclaimed. Biomarker-based predictive tests for subjectively asymptomatic Alzheimer’s disease (AD) are utilized in research today. Novel applications of artificial intelligence (AI) promise to predict the onset of AD several years in advance without determining biomarker thresholds. Until now, little attention has been paid to the new ethical challenges that AI brings to the early diagnosis in asymptomatic individuals, beyond contributing to research purposes, when we still lack adequate treatment. The aim of this paper is to explore the ethical arguments put forward (...) for AI aided AD prediction in subjectively asymptomatic individuals and their ethical implications. The ethical assessment is based on a systematic literature search. Thematic analysis was conducted inductively of 18 included publications. The ethical framework includes the principles of autonomy, beneficence, non-maleficence, and justice. Reasons for offering predictive tests to asymptomatic individuals are the right to know, a positive balance of the risk-benefit assessment, and the opportunity for future planning. Reasons against are the lack of disease modifying treatment, the accuracy and explicability of AI aided prediction, the right not to know, and threats to social rights. We conclude that there are serious ethical concerns in offering early diagnosis to asymptomatic individuals and the issues raised by the application of AI add to the already known issues. Nevertheless, pre-symptomatic testing should only be offered on request to avoid inflicted harm. We recommend developing training for physicians in communicating AI aided prediction. (shrink)

In this article, I argue that Hegel’s concept of recognition and Levinas’ concept of responsibility complement each other and lead to the idea of an asymmetrical reciprocity in which the origin of our social relations is not mutual equality, but rather mutual inequality. I will unfold this argument in three steps. I will first work out a fundamental asymmetry of recognition in Hegel by means of the figure of the bondsman before elucidating in a second step the asymmetry of responsibility (...) in Levinas by means of the figure of the hostage. In the last and third step, I will correlate both asymmetries and show how far the asymmetry of recognition and the asymmetry of responsibility constantly develop from and transition into one another in our social relationships. The present article represents a further critical use of this material that we deem worthy of publication.in this vital and splendidly crafted work, Bethany Henning recovers a philosophy of aesthetic wisdom far richer (...) than the narrow epistemological lens dominant today. From its start, American philosophy looked beyond Europe's atomistic empiricism toward a continuity between our thought and nature's aesthetic splendor. Jonathan Edwards found beauty in the structure of being reflected in relations among ideas. For Emerson, each of us is a "transparent eyeball" in which nature's beauty fortifies our moral and spiritual agency. At the apex of this tradition is John Dewey, for whom "aesthetic experience [is] the highest mode of human experience" (Henning 55).Although aesthetic theory is usually associated with the cognitive evaluation of art, Dewey explores a broader non-reflective or unconscious dimension. Henning's appropriation of "unconscious"—the cradle of Freud's infantile id—is intentionally provocative. American culture, reflected in academic thought, has largely resisted appeals to subterranean urges and drives. In doing so, however, we perpetuate a myth of rational and moral exceptionalism masking a history of colonialism, oppression, and crass commercialism. In accepting the unconscious, European thought has taken strides to acknowledge and address such ills. Henning wonders whether America might accept "a dynamic unconscious, were it presented with an alternative mythology" (10) and finds a viable candidate in Dewey's underappreciated notion of qualitative immediacy: an "aesthetic (directly felt) connection between ourselves and [End Page 77] the world" (Henning 5). In affirming the unconscious depth of experience, this American reconfiguration welcomes "aesthetic experience back into our philosophical lives" (Henning 5).When embodied meaning supplements cognitive reflection, we come to champion "the sensuous, lived body" as "the primary reality" of qualitative immediacy—a felt aesthetic unity punctuated by "impulsions" arising from an organism's needs (Henning 69). As "a site of adaptive adjustments," Henning continues, "the body is responsive to the brink of coalescence at the point of contact with the world, in which things that are 'outside of it' belong to it" (71). "To move adeptly into aesthetic experience means to be skilled at loosening focal consciousness" so as to cultivate "the essential porousness of the world" (130).With this, the body-mind relation becomes a problem of normative aesthetics, not epistemology. This paper argues that the passages on practical spirit within Hegel's 'Psychology' are able to enrich the picture of Hegel's account of intentional action by providing us with a genuine discussion of 'subjective action.' This kind of intentional activity is not yet part of moral or legal philosophy, and it is neutral as regards the question how an action becomes actually manifest in the world as a 'deed', potentially causing unintended consequences. Instead, subjective action consists in the teleological, end-pursuing action (...) of an agent acting on drives and wilfully choosing ends, which are practical as the causa finalis of an agent's doing. Thus, the 'Psychology' investigates intentional action from the first-personal perspective of the mental activity of the subject. It is legitimately part of subjective rather than objective spirit and its socially mediated normativity is psychological rather than moral. (shrink)

Are public officials morally justified in threatening violence, engaging in deception, or forcing citizens to act for their own good? Can individual officials be held morally accountable for the wrongs that governments commit? Dennis Thompson addresses these questions by developing a conception of political ethics that respects the demands of both morality and politics. He criticizes conventional conceptions for failing to appreciate the difference democracy makes, and for ascribing responsibility only to isolated leaders or to impersonal organizations.
