Large-scale sequencing tests, including whole-exome and whole-genome sequencing, are rapidly moving into clinical use. Sequencing is already being used clinically to identify therapeutic opportunities for cancer patients who have run out of conventional treatment options, to help diagnose children with puzzling neurodevelopmental conditions, and to clarify appropriate drug choices and dosing in individuals. To evaluate and support clinical applications of these technologies, the National Human Genome Research Institute and National Cancer Institute have funded studies on clinical and research sequencing under (...) the Clinical Sequencing Exploratory Research program as well as studies on return of results. Most of these studies use sequencing in real-world clinical settings and collect data on both the application of sequencing and the impact of receiving genomic findings on study participants. They are occurring in the context of controversy over how to obtain consent for exome and genome sequencing. (shrink)

The Protestant ethic — a moral code stressing hard work, rigorous self-discipline, and the organization of one's life in the service of God — was made famous by sociologist and political economist Max Weber. In this brilliant study (his best-known and most controversial), he opposes the Marxist concept of dialectical materialism and its view that change takes place through "the struggle of opposites." Instead, he relates the rise of a capitalist economy to the Puritan determination to work out anxiety over (...) salvation or damnation by performing good deeds — an effort that ultimately discouraged belief in predestination and encouraged capitalism. In late January of 1987, the State Treasurer of Pennsylvania, R. Budd Dwyer, shot himself to death in front of a dozen reporters and camera crews during a news conference in his office. Much was subsequently made in the popular press, and within the profession, about the difficult ethical decision television journalists were faced with in determining how much of the very graphic suicide tape to air. A review of the literature in this area suggests, however, that journalists have established (...) a set of relatively detailed conventions for dealing with events involving graphic depictions of death. Analysis of the Dwyer tape and interviews conducted with Pennsylvania television news directors show that eighteen of the twenty stations in the state that carry news used basically the same type and amount of footage in their evening newscasts. One decided to use no tape. One showed the moment of death. When the story broke around noon, two additional stations showed the moment of suicide, but they revised their story for the evening program. In addition, the wide majority of news directors interviewed said they had little difficulty in deciding how to edit the tape. The processing of the Dwyer story suggests that any ethical dilemmas faced by journalists during decision making were put aside for later consideration. The material was edited quickly and according to similar patterns, or conventions, around the state. The study suggests greater attention be given to the definition and interaction of personal professional values, in the ethical sense, and norms of news processing, in the sociological sense. Rapid individual cognitive phenotyping holds the potential to revolutionize domains as wide‐ranging as personalized learning, employment practices, and precision psychiatry. Going beyond limitations imposed by traditional lab‐based experiments, new efforts have been underway toward greater ecological validity and participant diversity to capture the full range of individual differences in cognitive abilities and behaviors across the general population. Building on this, we developed Skill Lab, a novel game‐based tool that simultaneously assesses a broad suite of cognitive abilities while providing an engaging (...) narrative. Skill Lab consists of six mini‐games as well as 14 established cognitive ability tasks. Using a popular citizen science platform (N = 10,725), we conducted a comprehensive validation in the wild of a game‐based cognitive assessment suite. Based on the game and validation task data, we constructed reliable models to simultaneously predict eight cognitive abilities based on the users’ in‐game behavior. Follow‐up validation tests revealed that the models can discriminate nuances contained within each separate cognitive ability as well as capture a shared main factor of generalized cognitive ability. Our game‐based measures are five times faster to complete than the equivalent task‐based measures and replicate previous findings on the decline of certain cognitive abilities with age in our large cross‐sectional population sample (N = 6369). Taken together, our results demonstrate the feasibility of rapid in‐the‐wild systematic assessment of cognitive abilities as a promising first step toward population‐scale benchmarking and individualized mental health diagnostics. BackgroundChronic kidney disease is a significant cause of global deaths. Those who progress to end-stage kidney disease often commence dialysis as a life-extending treatment. For cognitively impaired patients, the decision as to whether they commence dialysis will fall to someone else. This scoping review was conducted to map existing literature pertaining to how decisions about dialysis are and should be made with, for, and on behalf of adult patients who lack decision-making capacity. In doing so, it forms the basis of (...) a larger body of work that is exploring how these decisions ought to be made.MethodsTo identify relevant papers, searches were conducted on Ovid MEDLINE(R), Embase, PsychINFO, The Cochrane Library, and Web of Science. Inclusion criteria were then applied, requiring that papers: report on empirical studies about how decisions about dialysis are made and/or discuss how decisions about dialysis should be made with, for, and on behalf of adult patients who lack decision-making capacity; be published from 1961 onwards; and be published in English. This resulted in 27 papers eligible for inclusion.ResultsOf note, the majority of papers originated in the United States. There was wide variation across the included papers. Extracted data were grouped under the following themes: involving various parties (patient involvement, family dominance, and wider communication); objectivity about care options (including difficulties with family detachment); cultural sensitivity; medical versus non-medical factors; managing nonadherent patients; and the role and prevalence of substituted judgement. The literature shows that there is inconsistency in the principles and processes surrounding decisions made about dialysis with, for, and on behalf of adult patients who lack decision-making capacity.ConclusionsThis scoping review demonstrates that there is significant variation in both the practice and theory of dialysis decision making with, for, and on behalf of cognitively impaired adult patients. Complexity arises in considering who should get a say, how influential their say should be in a decision, and what factors are most relevant to the decision. A lack of up-to-date literature exploring this issue is highlighted, with this scoping review providing a useful groundwork from which further research can be undertaken. (shrink)

In this paper I revisit R. M. Martin’s logic of belief. As with much of Martin’s work, his formal studies into belief and belief reports have gone largely unnoticed. However, in my article I suggest reasons for thinking that these studies warrant revisiting. One reason is that Martin adopted an account of the notion of belief which was more comprehensive than that employed by most rival theorists. Another reason is that Martin couched his theory in a formal pragmatics which utilised (...) a pragmatical meta-language. This method resulted in a rather novel approach which may turn out to be more enlightening than many popular alternative accounts. Martin was a constructivistic nominalist whose approach to philosophical analysis was characterised by a commitment to first-order extensional systems. Yet in the statement of his theory of belief he employed a platonistic syntax simply for the reason that this kind of syntax is a little easier to implement than one which is nominalistic. However Martin also supposed that a fully nominalised version of his theory could be developed. In my article I investigate whether it can. I do this by sketching an inscriptional pragmatics, where this pragmatical system presupposes an inscriptional semantics of the kind developed by Martin himself. While further work is required, I think that a nominalisation of Martin’s logic of belief may be developed. (shrink)

This book offers a comprehensive primer for the study of intensionality. It explores and assesses those key theories of intensionality which have been developed in the twentieth and early twenty-first centuries. Each of the examined theories is tested as to whether it can account for the problems associated with the intersubstitution salva veritate of co-extensional expressions, and existential generalisation. All of these theories are subsequently compared so as to determine which of them comes closest to successfully solving these problems. The (...) book examines four kinds of intensionalist approaches: the Fregean approach ; the possible-worlds approaches of Carnap, Montague and Cresswell; the theory of properties relations and propositions devised by Bealer; and the Meinongian approaches put forward by Zalta and Priest. The book also proposes an alternative to intensionalism: sententialism. Sententialists argue that the problems of intensionality could be solved by appealing to linguistic items rather than intensional entities. Drawing on the works of Quine, Davidson, Scheffler and R. M. Martin, it explores the viability and value of sententialism as an alternative to intensionalism. (shrink)

Traditionally, analytic philosophers writing on aesthetics have given short shrift to nature. The last thirty years, however, have seen a steady growth of interest in this area. The essays and books now available cover central philosophical issues concerning the nature of the aesthetic and the existence of norms for aesthetic judgement. They also intersect with important issues in environmental philosophy. More recent contributions have opened up new topics, such as the relationship between natural sound and music, the beauty of animals, (...) and the aesthetics of gardens. In this paper we attempt to formulate logical foundations for a theory of actions or performance. Human beings act in various ways, and their actions are intimately interrelated with their use of language. But precisely how actions and the use of language are interrelated is not very clear. One of the reasons is perhaps that we have no precise vocabulary in terms of which such interrelations may be handled. There is need for developing a systematic theory in which different kinds (...) of actions may be discussed, contrasted, and compared. Then the various interrelations between actions and linguistic usage may perhaps be discussed rather more carefully and thoroughly than heretofore. Although much important preliminary work has been done in the analysis of actions, no one it seems has attempted to develop a strict logical theory for such analysis. A few tentative and programmatic steps were taken in the author's Toward a Systematic Pragmatics. Let us attempt here to improve those and take a few more. Any first attempts of this kind are of course fraught with difficulties. There inevitably will be some oversimplification or some overelaboration here or there. Ultimately of course we are interested in interrelating performance with various notions from syntax, semantics, and quantitative pragmatics. But this is not easy and only a few tentative suggestions toward such a development can be given here. In section 1 the distinction between action-kinds and action-events is drawn, and the character of the primitive or primitives needed for the theory of performance is discussed. In section 2 some further notions are then defined. Certain Rules of Performance are suggested in section 3. In section 4 there is discussion of the somewhat tenuous notion of acceptance as a basis for action. In section 5 the fundamental notions required for Parsons and Shils' theory of social action is discussed briefly. In section 6 we attempt to define the two basic notions required in Leonard's recent papers concerning authorship and purpose. Finally, in section 7, the theory of performance is interrelated with von Wright's deontic logic, in which such notions as permission and obligation are considered. (shrink)

Saul Kripke’s analysis of the concept of the natural numbers that we are taught in school yields a novel and axiomatically economical way of representing arithmetic in standard set theory—one that helps to answer Benacerraf’s objection from extraneous content as well as Wittgenstein’s objection from unsurveyability. After describing Kripke’s proposal in some detail, we examine it in the light of work by Quine, Steiner, Parsons, Boolos and Burgess. Although the primary aim of this paper is to present and explicate (...) Kripke’s view, we conclude by discussing some of the issues that are faced by Kripke’s proposal, so that the reader can get a sense of the geography of these issues. (shrink)

This chapter outlines the theoretical deep structure that is common to Durkheim's social psychology and the general theory of action. It first demonstrates the limits of the intellectual-historicist approach to classic sociology (Jones, 1977). It then induces the generative theoretical paradigm of Suicide from a textual analysis. It concludes by demonstrating the formal and substantive equivalence of this paradigm to the four-function general action system of Talcott Parsons.

Parsons's epistemology of "analytical realism" could be developed only by first displacing Weber's alternative epistemology within the social action perspective. Reconsideration of Parsons's epistemological moves shows that he came to conclusions unsupportable within the social action perspective. Reassertion of the postulate of Verstehen retrieves his achievements from the pure functionalism and positivism he opposed, by establishing a comprehensive action scheme centered on ideal-type analysis.

Hao Wang, a prolific philosopher and researcher, was known for his close intellectual friendship with Kurt Gödel, his advocacy for the study of logic at Oxford, and for his path-breaking contributions to logic and automated theorem proving. Yet in a memorial volume published sixteen years after Wang’s passing, the editors Charles Parsons and Montgomery Link noted “rather little has been published on Wang’s considerable body of work or on the man’s personality and unusual personal history.” The goal of this (...) article is to see what light Wang’s logical journey in life sheds on Chinese philosophy and the Western analytic tradition. The dissertation explores certain puzzles about fiction and existence. Some historical discussion of Brentano, Meinong and Russell sets the stage for an extended account of three neo-Meinongian semantic theories: those of Terence Parsons, Richard Routley , and Edward Zalta. It is argued that these authors rely on a false understanding of fiction. A distinction between setting out linguistic precedents in storytelling and following such precedents helps allow for the notion of being true in a story. However, fictional truth is (...) not a special case of truth nor is there any set of objects called 'fictional objects'. (shrink)

Abstract Though it has been claimed that Frege's commitment to expressions in indirect contexts not having their customary senses commits him to an infinite number of semantic primitives, Terrence Parsons has argued that Frege's explicit commitments are compatible with a two-level theory of senses. In this paper, we argue Frege is committed to some principles Parsons has overlooked, and, from these and other principles to which Frege is committed, give a proof that he is indeed committed to an (...) infinite number of semantic primitives?an intolerable result. Ever since Plato's Republic, a persistent problem and dilemma in Western thought has been the relation of the love of wisdom and political power, especially the role that the intellectual does or ought to play in the world of action. This volume includes both theoretical studies and case studies of modern intellectuals. Most of the articles have been published before but several, including T. Parson's "'The Intellectual': A Social Role Category" and J. Netl's "Ideas, Intellectuals, and Structures of Dissent" were (...) written for this volume. Other contributors are Shils, Dahrendorf, Berlin, Bushnell, Samuels, Comte, Nisbet, and Rieff. The most eloquent and moving essay is Isaiah Berlin's sympathetic study of Moses Hess. Through the great diversity of approaches and issues discussed, a common theme emerges--the fragility of the role of the intellectual, who is frequently duped, sometimes subtly corrupted or persecuted, but who, on occasion can shape and humanize the vision of his fellowmen, even though he may appear as the "fool" to his contemporaries. One striking lack here is a "case study" of any representative intellectual of the "New Left," but the total effect of this intelligently chosen collection is to provide us with a needed perspective for assessing the possibilities and dangers open to intellectuals in our time.--R. The essays collected in this volume to honor Ernest Nagel reflect his wide interest in all topics relating philosophy to the natural and social sciences. The essays, written by distinguished philosophers and scientists form a mixed bag, but most of them are very good. The first part, "Science and Inquiry" begins with notes taken by Patrick Suppes of Nagel's lectures on Dewey's logic delivered in 1947. It follows with essays on knowledge by Stuart Hampshire, on intensions and the law of (...) inverse variation by R. M. Martin, and four essays on problems of induction and confirmation which contribute markedly to this well-worn field. The eleven essays of the second part, "Structure of Science" range from discussions of physics and ontology, mechanism and evolution to essays on functionalism in anthropology and ethics and legal theory. In between there are very good essays by C. Hempel on reduction, S. Morgenbesser on the realist-instrumentalist controversy, essays on the identity thesis, extensive measurement, causation and action, philosophy of language, and the differences between the natural and social sciences. Part three contains six essays on the role of values in various settings, e.g., ethical theory, the social sciences, legal theory and existentialism. The fourth and final part contains a variety of historical studies: Clagett on the quadrature of lunes in medieval science, I. Bernard Cohen on Newton, A. Koslow on the law of inertia, Charles Parsons on Kant's philosophy of arithmetic, Philip Wiener on a Soviet view of Peirce's pragmatism and S. Diamond on John D. Rockefeller and the historians.--R. I want to join Dummett in saying that the reality of the past (and, by analogy, the reality of the future) is an issue of realism versus anti-realism: (Dummett 1969) If you affirm the reality of the past, you are a realist about the past. If you deny the reality of the past, you are an anti-realist about the past. (And likewise, in each case, for the future). It makes sense to think of these issues by analogy with realism about (...) the external world, unobservable objects, mathematical objects, universals, and so on. These are all properly described as ontological issues. (shrink)
