Este estudio de relaciones médico-paciente en Mexico involucró entrevistas realizadas a 1700 pacientes inmediatamente de.\pués de salir de su consulta clínica. Se llevó a cabo entre los años 2004-05 en León, More ha, Guanajuato y en Charleston, West Virginia, USA. Los propósitos fueron: 1) establecer un base de datos como punto de referencia para investigaciones futuras en instituciones del cuidado de la salud con referencia a la comunicación médico-paciente, a la satisfacción de los pacientes y al consentimiento infórmado y, 2) (...) hacer una comparación entre las relaciones medico-paciente en Mexico y los Estados Unidos de America. Hicimos varias conclusiones sobre la comunicación médico-paciente, participación del paciente en decisiones con respecto a su cuidado de salud y el ambiente social de la consulta médica. Ofi-ecemos los datos y los cuestionarios completos por internet a cualquier investigador interesado para estudios futuros de acuerdo con sus propios intereses. Además, encontramos que no huho ninguna diferencia importante en las relaciones médico-pacientes con respecto al consentimiento infórmado y la satisfáccián del paciente entre México y los EEUU. This article analyzes the evolution of best practices in the maquiladora industry in Mexico. Since the mid-1960s, the maquiladora has been understood as a simple assembly activity based on cheap labor, with low added value, and limited linkage with local suppliers. However, the maquiladora industry has evolved since the early 1980s as a consequence of the adoption of best practices in the productive processes and industrial organization. The best practices examined in this article are increases or improvements in complex activities, (...) capabilities, just-in-time, continuous improvement, environmental performance, and job safety. The data come from three surveys administered in major U.S./Mexico border cities in 1990, 2002, and 2006. Based on an analysis of these surveys, there has been a broad diffusion of the best organizational practices since the 1960s. (shrink)

The concept of “site” is at the center of current debates in theories of social practices as well as in cultural anthropology. It is unclear, however, how to assess the associated methodological assumption that overriding social structures or cultural formations can manifest themselves in sites. The article draws on the conception of social practices and introduces the notion of “publicness” in order to explicate how and why sociality and social structures can be accessed through “siting”. Sites as well as social (...) practices, it is claimed, have to be conceptualized as essentially public and thus principally observable phenomena. This assumption of publicness implicit to both site ontology and theories of social practices is unfolded on the basis of a praxeological reformulation of the paradigm of joint attention elaborated in developmental psychology. To avoid presentist misinterpretations, we then conceptualize sociality as chains of practices across time and space, drawing on the works of Theodore Schatzki, Ludwig Wittgenstein, Anthony Giddens and Bruno Latour. The public carriers of practices (artifacts, symbols, media, bodies) and the translocal structures they establish acquire particular significance in these approaches. In a further step, we present some methodological consequences of the “publicness assumption” and support our arguments by referring to Pierre Bourdieu's study on Distinction. (shrink)

In March 2020, the rapid increase in severe COVID‐19 cases overwhelmed the healthcare systems in several European countries. The capacities for artificial ventilation in intensive care units were too scarce to care for patients with acute respiratory disorder connected to the disease. Several professional associations published COVID‐19 triage recommendations in an extremely short time: in 21 days between March 6 and March 27. In this article, we compare recommendations from five European countries, which combine medical and ethical reflections on this (...) situation in some detail. Our aim is to provide a detailed overview on the ethical elements of the recommendations, the differences between them and their coherence. Previous studies examining EEG and LORETA in patients with chronic pain discovered an overactivation of high theta and low beta power in central regions. MEG studies with healthy subjects correlating evoked nociception ratings and source localization described delta and gamma changes according to two music interventions. Using similar music conditions with chronic pain patients, we examined EEG in response to two different music interventions for pain. To study this process in-depth we conducted a mixed-methods case study approach, based on three (...) clinical cases. Effectiveness of personalized music therapy improvisations versus preferred music on chronic pain was examined with 16 participants. Three patients were randomly selected for follow-up EEG sessions three months post-intervention, where they listened to recordings of the music from the interventions provided during the research. To test the difference of EM versus preferred music, recordings were presented in a block design: silence, their own composed EM, preferred music, and a non-participant’s EM as a control. Participants rated their pain before and after the EEG on a 1–10 scale. We conducted a detailed single case analysis to compare all conditions, as well as a group comparison of entrainment-healing condition versus preferred music condition. Power spectrum and according LORETA distributions focused on expected changes in delta, theta, beta, and gamma frequencies, particularly in sensory-motor and central regions. Intentional moment-by-moment attention on the sounds/music rather than on pain and decreased awareness of pain was experienced from one participant. Corresponding EEG analysis showed accompanying power changes in sensory-motor regions and LORETA projection pointed to insula-related changes during entrainment-pain music. LORETA also indicated involvement of visual-spatial, motor, and language/music improvisation processing in response to his personalized EM which may reflect active recollection of creating the EM. Group-wide analysis showed common brain responses to personalized entrainment-healing music in theta and low beta range in right pre- and post-central gyrus. We observed somatosensory changes consistent with processing pain during entrainment-healing music that were not seen during preferred music. These results may depict top–down neural processes associated with active coping for pain. (shrink)

Was ist Kunst? Was leistet Kunst? Warum bewegt uns Kunst? Und warum kommt ästhetischen Fragestellungen gerade heute wieder eine besondere Relevanz zu? Diesen Fragen geht der seit 2019 von der DFG eingerichtete Tübinger Sonderforschungsbereich 1391 Andere Ästhetik nach. Dabei möchte er die historische Tiefenschicht eines Perspektivwechsels aufarbeiten, der sich darauf richtet, Kunst und Künste nicht in autonomen Sonderräumen zu situieren. Hierfür bringt das Forschungsprojekt die 2000-jährige europäische Kunst- und Kulturgeschichte vor dem 18. Jahrhundert neu in Anschlag. Der Band erläutert, worin (...) der Gewinn der Konzentration auf die Vormoderne besteht, welche Neujustierung der Analyseinstrumentarien damit verbunden ist und welche grundsätzlichen Konsequenzen für Fragen der Ästhetik sich hieraus ergeben. Die Beiträge, die von den Philologien über die Kunstgeschichte, Archäologie und Musikwissenschaft bis hin zur Geschichtswissenschaft, Theologie und den Digital Humanities reichen, bieten hierfür konkrete Beispiele. Ziel ist es, von den ästhetischen Reflexionen der Vormoderne aus Impulse auch und gerade für die Diskussionen in der Gegenwart zu gewinnen. The current assessment of behaviors in the inventories to diagnose autism spectrum disorders (ASD) focus on observation and discrete categorizations. Behaviors require movements, yet measurements of physical movements are seldom included. Their inclusion however, could provide an objective characterization of behavior to help unveil interactions between the peripheral and the central nervous systems. Such interactions are critical for the development and maintenance of spontaneous autonomy, self-regulation and voluntary control. At present, current approaches cannot deal with the heterogeneous, dynamic and stochastic (...) nature of development. Accordingly, they leave no avenues for real-time or longitudinal assessments of change in a coping system continuously adapting and developing compensatory mechanisms. We offer a new unifying statistical framework to reveal re-afferent kinesthetic features of the individual with ASD. The new methodology is based on the non-stationary stochastic patterns of minute fluctuations (micro-movements) inherent to our natural actions. Such patterns of behavioral variability provide re-entrant sensory feedback contributing to the autonomous regulation and coordination of the motor output. From an early age, this feedback supports centrally driven volitional control and fluid, flexible transitions between intentional and spontaneous behaviors. We show that in ASD there is a disruption in the maturation of this form of proprioception. Despite this disturbance, each individual has unique adaptive compensatory capabilities that we can unveil and exploit to evoke faster and more accurate decisions. Measuring the kinesthetic re-afference in tandem with stimuli variations we can detect changes in their micro-movements indicative of a more predictive and reliable kinesthetic percept. Our methods address the heterogeneity of ASD with a personalized approach grounded in the inherent sensory-motor abilities that the individual has already developed. (shrink)

Symptom Checker Applications (SCA) are mobile applications often designed for the end-user to assist with symptom assessment and self-triage. SCA are meant to provide the user with easily accessible information about their own health conditions. However, SCA raise questions regarding ethical, legal, and social aspects (ELSA), for example, regarding fair access to this new technology. The aim of this scoping review is to identify the ELSA of SCA in the scientific literature. A scoping review was conducted to identify the ELSA (...) of SCA. Ten databases (e.g., Web of Science and PubMed) were used. Studies on SCA that address ELSA, written in English or German, were included in the review. The ELSA of SCA were extracted and synthesized using qualitative content analysis. A total of 25,061 references were identified, of which 39 were included in the analysis. The identified aspects were allotted to three main categories: (1) Technology; (2) Individual Level; and (3) Healthcare system. The results show that there are controversial debates in the literature on the ethical and social challenges of SCA usage. Furthermore, the debates are characterised by a lack of a specific legal perspective and empirical data. The review provides an overview on the spectrum of ELSA regarding SCA. It offers guidance to stakeholders in the healthcare system, for example, patients, healthcare professionals, and insurance providers and could be used in future empirical research to investigate the perspectives of those affected, such as users. Background Symptom checker apps (SCAs) are mobile or online applications for lay people that usually have two main functions: symptom analysis and recommendations. SCAs ask users questions about their symptoms via a chatbot, give a list with possible causes, and provide a recommendation, such as seeing a physician. However, it is unclear whether the actual performance of a SCA corresponds to the users’ experiences. This qualitative study investigates the subjective perspectives of SCA users to close the empirical gap identified in (...) the literature and answers the following main research question: How do individuals (healthy users and patients) experience the usage of SCA, including their attitudes, expectations, motivations, and concerns regarding their SCA use? Methods A qualitative interview study was chosen to clarify the relatively unknown experience of SCA use. Semi-structured qualitative interviews with SCA users were carried out by two researchers in tandem via video call. Qualitative content analysis was selected as methodology for the data analysis. Results Fifteen interviews with SCA users were conducted and seven main categories identified: (1) Attitudes towards findings and recommendations, (2) Communication, (3) Contact with physicians, (4) Expectations (prior to use), (5) Motivations, (6) Risks, and (7) SCA-use for others. Conclusions The aspects identified in the analysis emphasise the specific perspective of SCA users and, at the same time, the immense scope of different experiences. Moreover, the study reveals ethical issues, such as relational aspects, that are often overlooked in debates on mHealth. Both empirical and ethical research is more needed, as the awareness of the subjective experience of those affected is an essential component in the responsible development and implementation of health apps such as SCA. Unfounded—conspiracy and health—beliefs about COVID-19 have accompanied the pandemic worldwide. Here, we examined cross-nationally the structure and correlates of these beliefs with an 8-item scale, using a multigroup confirmatory factor analysis. We obtained a two-factor model of unfounded (conspiracy and health) beliefs with good internal structure (average CFI = 0.98, RMSEA = 0.05, SRMR = 0.04), but a high correlation between the two factors (average latent factor correlation = 0.57). This model was replicable across 50 countries (total N = 13,579), (...) as evidenced by metric invariance between countries (CFI = 0.96, RMSEA = 0.06, SRMS = 0.07) as well as scalar invariance across genders (CFI = 0.98, RMSEA = 0.04, SRMS = 0.03) and educational levels (CFI = 0.98, RMSEA = 0.04, SRMS = 0.03). Also, lower levels of education, more fear of COVID-19, and more cynicism were weakly associated with stronger conspiracy and health beliefs. The study contributes to knowledge about the structure of unfounded beliefs, and reveals the potential relevance of affective (i.e., fear of COVID-19) and cognitive (i.e., cynicism) factors along with demographics, in endorsing such beliefs. Over the past 50 years, Jorge J.E. Gracia has been a seminal figure in Latin American philosophy, philosophy of race and ethnicity, metaphysics and ontology, medieval philosophy, and the theory of interpretation. This book commemorates Gracia’s legacy with a critical investigation of his deep and wide-ranging impact.

In this chapter, I will address the philosophical ambivalence of the concept of Willkür in and after Kant. The aim of my chapter is to defend it against the charge of irrationality and mere chance, and to rehabilitate it from a historical and analytic point of view. I will analyze Kant’s use of the word “Willkür”, and chronologically follow the semantic and systematic changes in his philosophical work. Finally, I address recent attempts to revitalize the concept of Willkür in the (...) analytic debate by referring to the work of Harry Frankfurt and Robert Kane. I shall argue that we need to distinguish between two kinds of libera arbitria. Whereas the liberum arbitrium indifferentiae can be interpreted in terms of the pejorative sense of “Willkür”—as mere chance—the liberum arbitrium voluntatis equals a reflective kind of Willkür—literal Will-kür —which involves freedom of the will. This dissertation addresses the question of whether externalism and privileged access are compatible. I defend the view according to which they are, indeed, incompatible. However, this follows unproblematically from a distinction between two notions of mental content that I introduce and develop. My main suggestion is that a confusion between two notions of mental content is responsible for the view that the incompatibility of privileged access and externalism is problematic, since these are views about, strictly speaking, different varieties of mental (...) content. ;The discussion is divided into an introduction and four chapters. In the second chapter, I discuss the main argument for the position according to which externalism and privileged access are compatible. In the third chapter, I distinguish two kinds of mental content by using the two-dimensional semantics framework developed by David Kaplan, Robert Stalnaker and others. I then move to argue that externalism and privileged access are views about different sorts of content. Externalism is correct only when it is understood as a view about "objective content", essentially constituted by truth-conditions, whereas privileged access is correct only when it is understood as a view about "subjective content", a kind of "narrow" content essentially constituted by Stalnaker's "diagonal proposition". In the fourth chapter, I focus on privileged access and I offer an explanation of it. The main tenet of my account of privileged access is that our grounds for our first-order beliefs are the same as our grounds for the appropriate second-order beliefs. In the fifth chapter, I discuss the two main arguments for the position according to which externalism and privileged access are incompatible. By using the distinction regarding mental content drawn on chapter three, I argue that neither argument shows the incompatibility of externalism about objective content and privileged access about subjective content. Thus, I conclude that the incompatibility of privileged access and externalism is not problematic. (shrink)

Arguably the most famous heterotopia that appears in Foucault’s work is the Chinese encyclopedia, which originates in the fiction of Jorge Luis Borges. Drawing on this citation of Borges, this article examines Foucault’s notion of the heterotopia as it relates to order and knowledge production. Frequently, heterotopias are understood as sites of resistance. This article argues that shifting the focus from resistance to order and knowledge production reveals how heterotopias make the spatiality of order legible. By juxtaposing and combining many (...) spaces in one site, heterotopias problematize received knowledge by destabilizing the ground on which knowledge is built. Yet heterotopias always remain connected to the dominant order; thus as heterotopias clash with dominant orders, they simultaneously produce new ways of knowing. This article first explores the tensions between Foucault’s two definitions of heterotopias before connecting these definitions to Foucault’s distinctly spatial understanding of knowledge as emerging from a clash of forces. For example, at the conclusion of his Histoire de la folie (Madness and Civilization, 1961)—a book which brought him extensive intellectual recognition in (...) France—Foucault drew his readers' attention to José de Goya y Lucientes's (1746-1828) well-known etching, El Sueño de la Razón Produce Monstruos (The Sleep of Reason Brings Forth Monsters, Plate 43 of Los Caprichos, 1797). In Foucault's estimation, by revealing a "night which is doubtless that of classical unreason," Goya's work represented a socially removed condition within which people encounter "what is most solitary" and experience what is "deepest" within themselves. With his reference to Goya's imagery, Foucault illustrated a major theme of his history of madness, namely, how people referred to as "mentally ill" experienced a gradual, physical exclusion from participation in public society, and how they were eventually isolated in a more frightening way: within their physical confinement, they were subject to techniques of psychological and spiritual alienation that reduced them to a condition of utter solitude.In a comparable, if not more central manner, Foucault used as thematic motifs for the preface, first and second chapters of a book for which he is widely known, Les Mots et les choses (The Order of Things, 1966), the work of three major writers and artists from the Spanish-speaking world: Jorges Luis Borges (1899-1986), Diego Velázquez (1599-1660), and Miguel de Cervantes (1547-1616). Inclusion of Gracia’s responses to his critics makes this book a useful companion to Gracia’s _Metaphysics and its Task: The Search for the Categorial Foundation of Knowledge_.

In 2008 Robert Lovato coined the phrase Juan Crow. Juan Crow is a type of policy or enforcement of immigration laws that discriminate against Latino/as in the United States. This essay looks at the implications this phenomenon has for an ethics of immigration. It argues that Juan Crow, like its predecessor Jim Crow, is not merely a condemnation of federalism, but of any immigration reform that has stricter enforcement as one of its key components. Instead of advocating for increased (...) enforcement, I want to suggest that just immigration reform must adhere to two standards, equality of burdens and universal protections, and that only by doing so can the potential for Juan Crow be adequately avoided. In the realm of the philosophy of sounds and auditory experience there is an ongoing discussion concerned with the nature of sounds. One of the contestant views within this ontology of sound is that of the Property View, which holds that sounds are properties of the sounding objects. A way of developing this view is through the idea of dispositionalism, namely, by sustaining the theory according to which sounds are dispositional properties (Pasnau 1999; Kulvicki 2008; Roberts 2017). That portrayal, however, (...) is not sufficient, as it has not inquired the metaphysical debates about dispositions beyond the conditional analysis. In this paper, I try to advance this view by including recent developments (for instance Bird 2007; Vetter 2015) in the field of dispositionalism and I analyse whether this new version can sort out known and new objections to Property View. El italiano Roberto Esposito ha propuesto leerla como formando e informando el paradigma de la biopolítica. Se discuten cuatro lecturas de esa biopolítica: como formadora del paradigma de la inmunidad, como tanatopolítica, como liberal y neoliberal, y como biopolítica afirmativa. Twentieth-century readers wondered if Nietzschean philosophy was apolitical, (...) impolitic, or anti-political; that is, if it could be assimilated by democracy or if it was antimodern, elitist, and reactionary. The Italian philosopher Robert Esposito has proposed reading Nietzsche's philosophy as forming and informing the biopolitical paradigm. This collection of essays is a sequel to the editors' 1976 volume Studies in Nietzsche and the Classical Tradition. Philosophers, theologians, and literary historians discuss important aspects of Nietzsche's attack on Judaism and Christianity. The book contains studies of his view of biblical figures, Luther and Pascal as well as comparisons of his thought with that of Spinoza, Lessing, Heine, and Kierkegaard. Nietzsche's critique of the Old Testament, the Jewish religion of the diaspora, and historical Christianity are also investigated. The philosopher Jorge J. E. Gracia engages fifteen prominent scholars on race, ethnicity, nationality, and Hispanic/Latino identity in the United States. Their discussion joins two distinct traditions: the philosophy of race begun by African Americans in the nineteenth century, and the search for an understanding of identity initiated by Latin American philosophers in the sixteenth century. Participants include Linda M. Alcoff, K. Anthony Appiah, Richard J. Bernstein, Lawrence Blum, Robert Gooding-Williams, Eduardo Mendieta, and Lucius T. Outlaw Jr., and their (...) dialogue reflects the analytic, Aristotelian, Continental, literary, Marxist, and pragmatic schools of thought. These intellectuals start with the philosophy of Hispanics/Latinos in the United States and then move to the philosophy of African Americans and Anglo Americans in the United States and the philosophy of Latin Americans in Latin America. Gracia and his interlocutors debate the nature of race and ethnicity and their relation to nationality, linguistic rights, matters of identity, and Affirmative Action, binding the concepts of race and ethnicity together in ways that open new paths of inquiry. Gracia's Familial-Historical View of ethnic and Hispanic/Latino identity operates at the center of each of these discussions, providing vivid access to the philosopher's provocative arguments while adding unique depth to issues that each of us struggles to understand. El italiano Roberto Esposito ha propuesto leerla como formando e informando el paradigma de la biopolítica. Se discuten cuatro lecturas de esa biopolítica: como formadora del paradigma de la inmunidad, como tanatopolítica, como liberal y neoliberal, y como biopolítica afirmativa. Twentieth-century readers wondered if Nietzschean philosophy was apolitical, (...) impolitic, or anti-political; that is, if it could be assimilated by democracy or if it was antimodern, elitist, and reactionary. The Italian philosopher Robert Esposito has proposed reading Nietzsche's philosophy as forming and informing the biopolitical paradigm. This article examines the relativistic view according to which the interpretation of texts is a matter of opinion and one interpretation is as good as any other. It clarifies the question by establishing precise understandings of texts and interpretations and by introducing various distinctions between different kinds of interpretations based on their function. It argues that not all kinds of interpretations are relativistic, although all interpretations are relative, and that even those interpretations that are relativistic are not so in the (...) same ways. Utilitarianism, the great reforming philosophy of the nineteenth century, has today acquired the reputation for being a crassly calculating, impersonal philosophy unfit to serve as a guide to moral conduct. Yet what may disqualify utilitarianism as a personal philosophy makes it an eminently suitable guide for public officials in the pursuit of their professional responsibilities. Robert E. Goodin, a philosopher with many books on political theory, public policy and applied ethics to his credit, defends utilitarianism against its critics and (...) shows how it can be applied most effectively over a wide range of public policies.
