Following the widespread occurrence of birth defects from the use of thalidomide to treat nausea during pregnancy in the 1960s and 1970s, the Food and Drug Administration became particularly vigilant about the use of therapeutic agents during pregnancy and in women of childbearing potential. The FDA developed a list that categorizes an agent according to the known risks to a fetus. The drug thalidomide falls into Category X: agents that have demonstrated clear risk of fetal abnormalities and whose risks outweigh (...) the benefits of use during pregnancy.Another Category X drug is isotretinoin, commonly known by its trade name Accutane. Isotretinoin is an oral medication prescribed for severe recalcitrant, nodular acne in patients who are unresponsive to conventional therapy. Isotretinoin has been available since 1982, and it remains the only effective treatment for this condition. (shrink)

The United States Food and Drug Administration , in collaboration with pharmaceutical manufacturers, have recently implemented a heavily revised risk-management program for patients on isotretinoin , a drug with known and pronounced teratogenic effects. This revised risk management plan places significant burdens on both providers and patients in the hopes of achieving its goal of reducing fetal exposure to isotretinoin. The main focus of this paper is to discuss the burdens of various aspects of the program in relationship to potential (...) corresponding benefits. In particular, we evaluate the pregnancy rates of women on isotretinoin therapy compared with that of the general population and the rate changes based on the risk management programs. Additionally, we investigate whether the benefits of the program for women have increased as the benefits have risen. We devote special attention to the ethical implications of the intent of the program and to an analysis of the ethical justification of the restrictions placed on women of childbearing potential as it compares to the risk-benefit relationship of using isotretinoin. The author argues the following hypothesis: the medical clinic is an experimental practice, in the sense given to this term by Claude Bernard, and the clinic is its specific laboratory. Its object is not the disease, but the patient. Careful examination of the clinic attests to its very close proximity to the experimental method, and the comparison also raises a number of difficulties. The main obstacle arises from the specificity of medicine, which involves treating individual human subjects whose words cannot (...) be suppressed. In their article in the September-October 2010 issue of IRB: Ethics & Human Research, Chris Kaposy and Françoise Baylis argue that the contraception policy of the University of Nebraska Medical Center’s institutional review board fails to meet an ethically ideal standard in establishing the criteria investigators should follow when including women of childbearing potential in clinical research. In this response, we argue that the UNMC IRB’s policy is evidence-based and pragmatically oriented and that it therefore better safeguards women’s autonomy than (...) an ideal policy could. Y a-t-il une psychanalyse chrétienne? Cette question a-t-elle un sens? Un croyant souffrant doit-il ou non s'assurer que son psychanalyste est lui-même croyant pour protéger sa foi? Autrement dit, comment l'analyse intègre-t-elle ou modifie-t-elle une donnée religieuse? Faire une analyse, c'est accepter de traverser le tragique de sa propre vie. En raison du dévoilement de la vérité que ce processus met en œuvre, le patient voit se dégager devant lui la perspective d'un désir dont la nouvelle mesure est infinie. Ce (...) désir avait été comblé par des objets qui, jusque-là, semblaient le satisfaire. Puis, le symptôme surgit, déstabilise, perturbe les petits arrangements que le patient s'était autorisés. Première ou dernière scène de son tragique? Dans le huis clos théâtral qu'est la séance analytique, le patient-acteur reprend et parle toute son histoire. Comme à son insu, il se voit contraint de redéfinir ce qui donne sens à sa vie : sens religieux? sens éthique? Bref, tout ce qui échappe à l'analyse et que, paradoxe imprévisible, elle évoque et convoque... Pourtant, un point restera toujours aveugle pour lui, à l'instar d'Œdipe qui, les yeux crevés, poursuit son chemin. C'est peut-être en ce même point - tragique-aveugle - que l'homme reconnaît sous une lumière nouvelle que son histoire personnelle rejoint la détresse commune et ordinaire de toute l'humanité. L’élargissement de l’Union à l’Ukraine se pose en des termes particuliers. Il est devenu le fondement politique d’une solidarité européenne et d’un soutien inconditionnel face à l’agression de Poutine. Bien que le pays se conforme à « l’acquis communautaire » en termes d’État de droit, le principe de l’élargissement se heurte à de nombreuses difficultés et oppositions des États membres : faible soutien militaire et insuffisance des industries de défense, concurrence des produits agricoles ukrainiens en cas d’intégration dans un grand (...) marché libre exempté de droits de douane. On est face à un paradoxe qu’il faudra dépasser. L’élargissement est à la fois indispensable, inévitable pour la sécurité de tout le continent, et complexe du point de vue économique. (shrink)

" Fort de cette certitude, le philosophe et historien des sciences français Jacques Merleau-Ponty a cherché toute sa vie à donner un sens à l'extraordinaire histoire que nous racontent les astronomes contemporains. Or le visage de l'univers actuel est une véritable énigme pour l'homme moderne : origine inaccessible, histoire chaotique, espace infini, temps relatif, avenir imprévisible... A l'accumulation des connaissances, il fallait répondre par le courage de (...) la pensée. Contemporain de toutes les découvertes cosmologiques du XXe siècle, Jacques Merleau-Ponty a été un des très rares philosophes à y réfléchir pour redonner une place à l'homme dans cet ensemble démesuré. Car si la science cosmologique appartient aux spécialistes, la description du ciel est un bien commun et la pensée du big bang une nouvelle nécessité. Ce livre expose pour la première fois cette pensée qui prend le risque de poser la plus universelle et intime des questions : quel est le sens de l'univers? People are motivated by shared social values that, when held with moral conviction, can serve as compelling mandates capable of facilitating support for ideological violence. The current study examined this dark side of morality by identifying specific cognitive and neural mechanisms associated with beliefs about the appropriateness of sociopolitical violence, and determining the extent to which the engagement of these mechanisms was predicted by moral convictions. Participants reported their moral convictions about a variety of sociopolitical issues prior to undergoing functional (...) MRI scanning. During scanning, they were asked to evaluate the appropriateness of violent protests that were ostensibly congruent or incongruent with their views about sociopolitical issues. Complementary univariate and multivariate analytical strategies comparing neural responses to congruent and incongruent violence identified neural mechanisms implicated in processing salience and in the encoding of subjective value. As predicted, neuro-hemodynamic response was modulated parametrically by individuals’ beliefs about the appropriateness of congruent relative to incongruent sociopolitical violence in ventromedial prefrontal cortex, and by moral conviction in ventral striatum. Overall moral conviction was predicted by neural response to congruent relative to incongruent violence in amygdala. Together, these findings indicate that moral conviction about sociopolitical issues serves to increase their subjective value, overriding natural aversion to interpersonal harm. (shrink)

One of the most perceptive and ambidextrous social commentators of our day, Augustinian scholar Jean Bethke Elshtain furnishes in ever fresh ways through her writings a bridge between the ancient and the modern, between politics and ethics, between timeless moral wisdom and cultural sensitivity. To read Elshtain seriously is to take the study of culture as well as the "permanent things" seriously. But Elshtain is no mere moralist. Neither is she content solely to dwell in the domain of the (...) theoretical. For it is Elshtain the citizen - the creatively engaged and contributing citizen - whom the reader encounters on virtually every page of her writings. But reader beware: Elshtain does not shy away from controversy. At the same time, she is anything but a controversialist. In the essay that follows, several prominent themes that emerge from Elstain's writings - civic responsibility, justice, gender, and war - are considered afresh. Whether one agrees with her positions or not, one is forced to confess in the end that she cares deeply about the common good. Jean Starobinski, one of Europe's foremost literary critics, examines the life that led Rousseau, who so passionately sought open, transparent communication with others, to accept and even foster obstacles that permitted him to withdraw into himself. First published in France in 1958, Jean-Jacques Rousseau remains Starobinski's most important achievement and, arguably, the most comprehensive book ever written on Rousseau. The text has been extensively revised for this edition and is published here along with seven essays on Rousseau that (...) appeared between 1962 and 1970. (shrink)

In this paper, we examine recent critiques of the debate about defining disease, which claim that its use of conceptual analysis embeds the problematic assumption that the concept is classically structured. These critiques suggest, instead, developing plural stipulative definitions. Although we substantially agree with these critiques, we resist their implication that no general definition of “disease” is possible. We offer an alternative, inductive argument that disease cannot be classically defined and that the best explanation for this is that the concept (...) is structured as a cluster. We further argue that we do not need to reject the possibility of defining the general concept “disease” to legitimate developing stipulative definitions of disease that are relative to particular purposes. A cluster definition of disease is compatible with contextually motivated definitions, which may be considered précisifications of the more general cluster concept. (shrink)

Alors qu’il était précepteur en Suisse, le jeune Hegel traduisit minutieusement un libelle révolutionnaire dénonçant l’oppression que les autorités bernoises exerçaient sur le Pays de Vaud. Il publia ces Lettres de Jean-Jacques Cart à son retour en Allemagne, en 1798, sous le couvert d’un anonymat qu’il ne leva jamais. Derrière le caractère anecdotique de cette première publication, on peut déceler des enjeux qui conduisirent à l’instauration de l’idéalisme spéculatif. Le motif de la lutte pour la reconnaissance, en particulier, semble (...) y trouver sa première apparition. Plus fondamentalement cette première publication éclaire le travail d’élaboration stylistique conduisant progressivement au système, avec, à l’horizon, la question de la portée pratique de la philosophie.
