General practitioners (GPs) have to negotiate a range of challenges when they suspect child abuse or neglect. This article details findings from a Delphi exercise that was part of a larger study exploring the conflicts of interest that arise for UK GPs in safeguarding children. The specific objectives of the Delphi exercise were to understand how these conflicts of interest are seen from the perspectives of an expert panel, and to identify best practice for GPs. The Delphi exercise involved four (...) iterative rounds with questionnaires completed by an expert panel. Results from each round were distilled and findings sent to panel members until consensus was reached. Panel members shared insights regarding their understanding of conflicts of interest in relation to GPs and safeguarding children and responses when conflicts of interests arise. Findings suggested a broader understanding of conflicts of interest (intrapersonal, interpersonal, interprofessional and interagency), the importance of professional judgement in uncertain situations when both action and inaction have potentially negative consequences and the importance of trust. The Delphi exercise was an effective means to bring together a wide range of professional and disciplinary perspectives on a complex topic. Findings caution against the oversimplification of the conceptual and practical issues, emphasise the importance of professional judgement, and support the development of open and trusting relationships with families and among professionals in health and social care agencies. He also deals with the question of whether or not reference to these abstract entities is really indispensible in logic and whether it is necessary in physical science in general.

A frequent feature of pharmaceutical research is the open label extension study, in which patients participating in double blind placebo controlled trials of new medications are invited, on completion of the initial trial, to take the study drug for some further period. Patients are openly given the active substance at this stage, regardless of their assignment in the initial trial. Investigators are typically reluctant to unblind the patients’ assignment at the point of entry into the open label phase, on the (...) grounds that this may introduce ascertainment bias in the main study.It is argued that patients invited to participate in open label extension studies cannot give a proper consent to such research unless they know to which arm of the main trial they were recruited. It is further argued that to recruit certain groups of patients from placebo controlled trials into open label extension studies may also be unethical for clinical reasons. (shrink)

Can we reconcile the idea that we are free and responsible agents with the idea that what we do is determined according to natural laws? For centuries, philosophers have tried in different ways to show that we can. Hilary Bok takes a fresh approach here, as she seeks to show that the two ideas are compatible by drawing on the distinction between practical and theoretical reasoning.Bok argues that when we engage in practical reasoning--the kind that involves asking "what should (...) I do?" and sifting through alternatives to find the most justifiable course of action--we have reason to hold ourselves responsible for what we do. But when we engage in theoretical reasoning--searching for causal explanations of events--we have no reason to apply concepts like freedom and responsibility. Bok contends that libertarians' arguments against "compatibilist" justifications of moral responsibility fail because they describe human actions only from the standpoint of theoretical reasoning. To establish this claim, she examines which conceptions of freedom of the will and moral responsibility are relevant to practical reasoning and shows that these conceptions are not vulnerable to many objections that libertarians have directed against compatibilists. Bok concludes that the truth or falsity of the claim that we are free and responsible agents in the sense those conceptions spell out is ultimately independent of deterministic accounts of the causes of human actions.Clearly written and powerfully argued, Freedom and Responsibility is a major addition to current debate about some of philosophy's oldest and deepest questions. Local review of research by ethics committees in the UK has long been held to be an important right of the local research ethics committee and, even with the introduction of the European Clinical Trials Directive, the governance arrangements for research ethics committees continue to allow for local review of multicentre studies. There is no requirement for local review in either the European Union directive or in the guidelines on good clinical practice, and there is little evidence of it anywhere (...) else in Europe. The idea that there can be “local”, as opposed to “central” ethical issues in research is an interesting one, which raises important issues about the nature of research ethics and ethical review. The aim of this paper is to argue that there are no such things as local issues in research ethics, and suggest that those questions currently addressed as local issues properly belong within the research governance framework. Dignity appears to be an important concept in nursing philosophy and more widely in health care policy and provision. Recent events in the UK have generated much interest in the subject. However, there appears to be some confusion about the precise meaning and application of the concept. An influential contribution to the debate has come from Nordenfelt, who, as part of a European project investigating dignity and the care of older people, has proposed a four‐part typology of dignity. In this (...) article, we will explore some of the background to the dignity debate in UK nursing and health care, give a brief overview of Nordenfelt's position, offer some criticisms of his work and propose some modifications to his view. He also rejects the idea that knowledge can be demarcated from non-knowledge by the fact that the former alone adheres to 'the scientific method'. The first part of the book consists of Professor Putnam's John Locke (...) lectures, delivered at the University of Oxford in 1976, offering a detailed examination of a 'physicalist' theory of reference against a background of the works of Tarski, Carnap, Popper, Hempel and Kant. The analysis then extends to notions of truth, the character of linguistic enquiry and social scientific enquiry in general, interconnecting with the great metaphysical problem of realism, the nature of language and reference, and the character of ourselves. Hilary Kornblith argues for a naturalistic approach to investigating knowledge. Knowledge, he explains, is a feature of the natural world, and so should be investigated using scientific methods. He offers an account of knowledge derived from the science of animal behavior, and defends this against its philosophical rivals. This controversial and refreshingly original book offers philosophers a new way to do epistemology.

Naturalism in philosophy has a long and distinguished heritage. This is no less true in epistemology than it is in other areas of philosophy. At the same time, epistemology in the English speaking world in the first half of die twentieth century was dominated by an approach quite hostile to naturalism. Now, at the close of the twentieth century, naturalism is resurgent.

This book is unified by three broad concerns: the rationality of belief in God, the relation between religion and morality, and the explication of the concept of God. The essays are, however, marked by diversity. Some focus on historical figures, such as Aquinas and Locke; others bring recent epistemological and metaphysical developments to bear on problems of religious belief. Some of the papers explore neglected issues central to religious practice, such as the question of how total devotion to God can (...) permit other deep commitments; others apply philosophical distinctions from within a religious tradition, for example, in setting out a Christian approach to the problem of evil. I explore the prospects for modelling epistemic rationality (in the probabilist setting) via an epistemic decision theory, in a consequentialist spirit. Previous work has focused on cases in which the truth-values of the propositions in which the agent is selecting credences do not depend, either causally or merely evidentially, on the agent’s choice of credences. Relaxing that restriction leads to a proliferation of puzzle cases and theories to deal with them, including epistemic analogues of evidential and causal decision theory, and (...) of the Newcomb Problem and ‘Psychopath Button’ Problem. A variant of causal epistemic decision theory deals well with most cases. However, there is a recalcitrant class of problem cases for which no epistemic decision theory seems able to match our intuitive judgements of epistemic rationality. This lends both precision and credence to the view that there is a fundamental mismatch between epistemic consequentialism and the intuitive notion of epistemic rationality; the implications for understanding the latter are briefly discussed. Decisions, whether moral or prudential, should be guided at least in part by considerations of the consequences that would result from the various available actions. For any given action, however, the majority of its consequences are unpredictable at the time of decision. Many have worried that this leaves us, in some important sense, clueless. In this paper, I distinguish between ‘simple’ and ‘complex’ possible sources of cluelessness. In terms of this taxonomy, the majority of the existing literature on cluelessness focusses (...) on the simple sources. I argue, contra James Lenman in particular, that these would-be sources of cluelessness are unproblematic, on the grounds that indifference-based reasoning is far less problematic than Lenman (along with many others) supposes. However, there does seem to be a genuine phenomenon of cluelessness associated with the ‘complex’ sources; here, indifference-based reasoning is inapplicable by anyone’s lights. This ‘complex problem of cluelessness’ is vivid and pressing, in particular, in the context of Effective Altruism. This motivates a more thorough examination of the precise nature of cluelessness, and the precise source of the associated phenomenology of discomfort in forced-choice situations. True benevolence is, most fundamentally, a desire that the world be better. It is natural and common, however, to frame thinking about benevolence indirectly, in terms of a desire to make a difference to how good the world is. This would be an innocuous shift if desires to make a difference were extensionally equivalent to desires that the world be better. This paper shows that at least on some common ways of making a “desire to make a difference” precise, this (...) extensional equivalence fails. Where it fails, “difference-making preferences” run counter to the ideals of benevolence. In particular, in the context of decision making under uncertainty, coupling a “difference-making” framing in a natural way with risk aversion leads to preferences that violate stochastic dominance, and that lead to a strong form of collective defeat, from the point of view of betterness. Difference-making framings and true benevolence are not strictly mutually inconsistent, but agents seeking to implement true benevolence must take care to avoid the various pitfalls that we outline. According to Bayesian epistemology, the epistemically rational agent updates her beliefs by conditionalization: that is, her posterior subjective probability after taking account of evidence X, pnew, is to be set equal to her prior conditional probability pold(·|X). Bayesians can be challenged to provide a justification for their claim that conditionalization is recommended by rationality—whence the normative force of the injunction to conditionalize? There are several existing justifications for conditionalization, but none directly addresses the idea that conditionalization will be epistemically rational (...) if and only if it can reasonably be expected to lead to epistemically good outcomes. We apply the approach of cognitive decision theory to provide a justification for conditionalization using precisely that idea. We assign epistemic utility functions to epistemically rational agents; an agent’s epistemic utility is to depend both upon the actual state of the world and on the agent’s credence distribution over possible states. We prove that, under independently motivated conditions, conditionalization is the unique updating rule that maximizes expected epistemic utility. (shrink)

In this paper we set out to explore the enactments of risk by clinicians involved in the development of stem cell therapy for liver disease. In the process, we contribute to a performative re-thinking of how ‘risk’ can be analytically treated in relation to health. The bulk of the paper, drawing on interview data, is concerned with how clinicians’ accounts about the risks entailed in their research-oriented work performatively ‘make’ clinicians themselves, but also various other ‘constituencies’ – notably, publics and (...) regulatory actors, and also such diverse ‘entities’ as scientific rigour and ethics. We trace the ways in which they enact the complexities of taking risks in the context of public demands, regulatory constraints, and resource limitation and explore how clinicians tend to represent themselves as caught between the ‘rock of courage’ and the ‘hard place of caution’. Hilary Kornblith presents a new account of mental reflection, and its importance for knowledge, reasoning, freedom, and normativity. He argues that reflection cannot solve the philosophical problems it has traditionally been thought to, and offers a more realistic, demystified view of its nature which draws on dual process approaches to cognition.

Population axiology is the study of the conditions under which one state of affairs is better than another, when the states of affairs in ques- tion may differ over the numbers and the identities of the persons who ever live. Extant theories include totalism, averagism, variable value theories, critical level theories, and “person-affecting” theories. Each of these the- ories is open to objections that are at least prima facie serious. A series of impossibility theorems shows that this is no coincidence: (...) it can be proved, for various sets of prima facie intuitively compelling desiderata, that no axiology can simultaneously satisfy all the desiderata on the list. One’s choice of population axiology appears to be a choice of which intuition one is least unwilling to give up. (shrink)

This article is a critical survey of the debate over the value of the social discount rate, with a particular focus on climate change. The ma- jority of the material surveyed is from the economics rather than from the philosophy literature, but the emphasis of the survey itself is on founda- tions in ethical and other normative theory rather than highly technical details. I begin by locating the standard approach to discounting within the overall landscape of ethical theory, and explaining (...) the assumptions and simplifications that are needed in order to arrive at the model that is standard in the discounting literature. The article then covers the general theory of the Ramsey equation and its relationship to observed interest rates, arguments for and against a positive rate of pure time preference, the consumption elasticity of utility, and the effect of various sorts of uncertainty on the discount rate. Finally, it turns specifically to the ap- plication of this debate to the case of climate change, focussing on the recent controversy over the low discount rate used in the Stern Review of the Economics of Climate Change. The antibiotic streptomycin was discovered soon after penicillin was introduced into medicine. Selman Waksman, who was awarded the Nobel Prize for the discovery, has since generally been credited as streptomycin's sole discoverer. However, one of Waksman's graduate students, Albert Schatz, was legally recognized as streptomycin's co-discoverer and received a share of the royalties from the drug. The aim of this essay is to discuss the streptomycin story, largely using previously unquoted archival material, and in particular to provide further evidence for (...) the important role which Schatz played in the discovery. In response to growing concerns of bias, discrimination, and unfairness perpetuated by algorithmic systems, the datasets used to train and evaluate machine learning models have come under increased scrutiny. Many of these examinations have focused on the contents of machine learning datasets, finding glaring underrepresentation of minoritized groups. In contrast, relatively little work has been done to examine the norms, values, and assumptions embedded in these datasets. In this work, we conceptualize machine learning datasets as a type of informational infrastructure, (...) and motivate a genealogy as method in examining the histories and modes of constitution at play in their creation. We present a critical history of ImageNet as an exemplar, utilizing critical discourse analysis of major texts around ImageNet’s creation and impact. We find that assumptions around ImageNet and other large computer vision datasets more generally rely on three themes: the aggregation and accumulation of more data, the computational construction of meaning, and making certain types of data labor invisible. By tracing the discourses that surround this influential benchmark, we contribute to the ongoing development of the standards and norms around data development in machine learning and artificial intelligence research.
