Previous work has shown that concerns of breast cancer patients after finishing radiotherapy are responsive to conversations with radiographers during the treatment period. This study seeks to further understand radiographer and patient experiences, determine shared priorities for improvement in clinical interaction and develop communication guidelines and training to help radiographers support patients.Methods: Using the principles of Experience-Based Co-Design, semi-structured interviews were held with N = 4 patients and N = 4 radiographers, followed by feedback events to validate findings. Patients and (...) radiographers exchanged experiences in a joint co-design session, agreed with shared priorities and generated ideas for further support. A survey was conducted for process evaluation. To scale up findings, UK-wide representatives from patient networks and radiographers and managerial staff provided consultative input utilizing an iterative, adaptive procedure.Results: Radiographers expressed a need for support with “difficult conversations,” especially those on Fear of Cancer Recurrence, and their appropriate management. Important pointers for reassuring communication were identified, including: being treated like a person, knowing what to expect, and space to ask questions. The co-design process was rated positively by both staff and patients. Thematic collation of findings and mapping these on literature evidence resulted in the “KEW” communication guidelines for radiographers: Know, Encourage, Warmth. National stakeholder consultations validated and helped fine-tune the training model. The resulting training package, included: trigger videos, a simulated patient scenario and interactive handouts on fears of cancer recurrence and the patient pathway.Conclusions: The co-design process captured good practice to help standardize quality in empathic communication in the radiotherapy service. The resulting KEW: Know, Encourage, Warmth guidelines, and training package are user-centered as well as evidence-based. Supplementing single-site co-design with national consultative feedback allows for the development of interventions that are relevant to the clinical practice, even in detail, and helps to generate appropriate buy-in for roll out on a wider scale after evaluation.Trial Registration:www.ClinicalTrials.gov ID: NCT03468881. 'Mass terms', words like water, rice and traffic, have proved very difficult to accommodate in any theory of meaning since, unlike count nouns such as house or dog, they cannot be viewed as part of a logical set and differ in their grammatical properties. In this study, motivated by the need to design a computer program for understanding natural language utterances incorporating mass terms, Harry Bunt provides a thorough analysis of the problem and offers an original and detailed solution. An extension of classical set theory, Ensemble Theory, is defined, and this provides the conceptual basis of a framework for the analysis of natural language meaning which Dr Bunt calls Two-level model-theoretic semantics. The validity of the framework is convincingly demonstrated by the formal analysis of a fragment of English including sentences with quantified and modified mass terms. Separate chapters of the book are devoted to an axiomatic definition of Ensemble Theory and a detailed discussion of its status as a mathematical formalism. (shrink)

Quassim Cassam has argued that psychological and epistemological analyses of conspiracy theories threaten to overlook the political nature of such theories. According to Cassam, conspiracy theories are a form of political propaganda. I develop a limited critique of Cassam's analysis.This paper advances two core theses. First, acceptance of conspiracy theories requires a rejection of epistemic authority that renders conspiracy theorists susceptible to co-option by certain political programs while insulating such programs from criticism. I argue that the contrarian nature of conspiracy (...) theories partially accounts for the prominence of such theories in populist movements.Second, the contrarian nature of conspiracy theories partially accounts for their attractiveness, especially among those to whom populism already appeals. I argue that for those who resent what appears from their perspective as the shaping of the epistemic landscape by alien perspectives, conspiracy theorizing may facilitate the reassertion of epistemic autonomy. This paper describes the ways in which trolls and bots impede the acquisition of knowledge online. I distinguish between three ways in which trolls and bots can impede knowledge acquisition, namely, by deceiving, by encouraging misplaced skepticism, and by interfering with the acquisition of warrant concerning persons and content encountered online. I argue that these threats are difficult to resist simultaneously. I argue, further, that the threat that trolls and bots pose to knowledge acquisition goes beyond the mere threat of (...) online misinformation, or the more familiar threat posed by liars offline. Trolls and bots are, in effect, fake persons. Consequently, trolls and bots can systemically interfere with knowledge acquisition by manipulating the signals whereby individuals acquire knowledge from one another online. I conclude with a brief discussion of some possible remedies for the problem of fake persons. (shrink)

An approach to emotion is described in which emotions are defined as states elicited by instrumental reinforcers, that is, by stimuli that are the goals for action. This leads to a theory of the evolutionary adaptive value of emotions, which is that different genes specify different goals in their own self-interest, and any actions can then be learned and performed by instrumental learning to obtain the goals. The brain mechanisms for emotion in brain regions such as the orbitofrontal and anterior (...) cingulate cortex provide a foundation for understanding the neural basis of emotion. Classically conditioned effects may modulate the actions initiated by this system. In addition to this instrumental learning system, some stimuli may elicit responses, for example approach, withdrawal, or fixed action patterns, but intervening states are not required for this type of adaptive response. In addition, a rational thought system involved in multistep planning can allow gene-specified goals to be deferred or avoided in order to achieve longer-term types of goals that may be more advantageous to the individual than to the genes. Distinguishing “business” concerns from “ethical” values is not only an unfruitful and meaningless task, it is also an impossible endeavor. Nevertheless, fruitless attempts to separate facts from values produce detrimental second-order effects, both for theory and practice, and should therefore be abandoned. We highlight examples of exemplary research that integrate economic and moral considerations, and point the way to a business ethics discipline that breaks new ground by putting ideas and narratives about businesstogetherwith ideas and narratives about ethics.

Several social metaphysicians have argued that groups are constituted by, but not identical to, their members. While the constitution view is promising, there are significant difficulties with existing versions of that view. Fortunately, lessons may be extracted from more traditional metaphysics and applied to the case of group agents. Drawing on such lessons, I present a novel account of the constitution relation holding between individuals and group agents. According to the resulting structural-constitution view, when individuals constitute a group of a (...) certain kind, they do so in virtue of exhibiting the structure characteristic of groups of that kind. A philosophical exploration of the origin and limits of the modern world.

A life of the mind can be lived only by creatures who know that they have minds. We call these creatures “persons,” and currently, all such persons THAT we know OF are “alive” in the biological sense. But are there, or could there be, either in the future or elsewhere in the universe, creatures with “a life of the mind” that are not “alive” in the sense that we humans usually understand this term today?

The epistemic virtues and vices are typically defined in terms of effects or motivations related to the epistemic states of their possessors. However, philosophers have recently begun to consider _other-regarding_ epistemic virtues, traits oriented toward the epistemic flourishing of others. In a similar vein, this paper discusses _outward-facing_ epistemic vices, properties oriented toward the epistemic languishing of others. I argue for the existence of both reliabilist and responsibilist outward-facing vices, and illustrate how such vices negatively bear on the epistemic prospects (...) of others. I pay special attention to how outward-facing epistemic vices may manifest in online activities that promote the epistemic languishing of others by negatively influencing the online epistemic environment. In this book, Carlos Montemayor and Harry Haladjian consider the relationship between consciousness and attention. The cognitive mechanism of attention has often been compared to consciousness, because attention and consciousness appear to share similar qualities. But, Montemayor and Haladjian point out, attention is defined functionally, whereas consciousness is generally defined in terms of its phenomenal character without a clear functional purpose. They offer new insights and proposals about how best to understand and study the relationship between consciousness and attention (...) by examining their functional aspects. The book's ultimate conclusion is that consciousness and attention are largely dissociated. -/- Undertaking a rigorous analysis of current empirical and theoretical work on attention and consciousness, Montemayor and Haladjian propose a spectrum of dissociation—a framework that identifies the levels of dissociation between consciousness and attention—ranging from identity to full dissociation. They argue that conscious attention, the focusing of attention on the contents of awareness, is constituted by overlapping but distinct processes of consciousness and attention. Conscious attention, they claim, evolved after the basic forms of attention, increasing access to the richest kinds of cognitive contents. -/- Montemayor and Haladjian's goal is to help unify the study of consciousness and attention across the disciplines. The family is hotly contested ideological terrain. Some defend the traditional two-parent heterosexual family while others welcome its demise. Opinions vary about how much control parents should have over their children's upbringing. The relation between mental states and brain states is important in computational neuroscience, and in psychiatry in which interventions with medication are made on brain states to alter mental states. The relation between the brain and the mind has puzzled philosophers for centuries. Here a neuroscience approach is proposed in which events at the sub-neuronal, neuronal, and neuronal network levels take place simultaneously to perform a computation that can be described at a high level as a mental state, with content (...) about the world. It is argued that as the processes at the different levels of explanation take place at the same time, they are linked by a non-causal supervenient relationship: causality can best be described in brains as operating within but not between levels. This allows the supervenient properties to be emergent, though once understood at the mechanistic levels they may seem less emergent, and expected. This mind-brain theory allows mental events to be different in kind from the mechanistic events that underlie them; but does not lead one to argue that mental events cause brain events, or vice versa: they are different levels of explanation of the operation of the computational system. This approach may provide a way of thinking about brains and minds that is different from dualism and from reductive physicalism, and which is rooted in the computational processes that are fundamental to understanding brain and mental events, and that mean that the mental and mechanistic levels are linked by the computational process being performed. Explanations at the different levels of operation may be useful in different ways. For example, if we wish to understand how arithmetic is performed in the brain, description at the mental level of the algorithm being computed will be useful. But if the brain operates to result in mental disorders, then understanding the mechanism at the neural processing level may be more useful, in for example, the treatment of psychiatric disorders.
