In this analysis we discuss the change in criteria for triage of patients during three different phases of a pandemic like COVID-19, seen from the critical care point of view. Availability of critical care beds has become a hot topic, and in many countries, we have seen a huge increase in the provision of temporary intensive care bed capacity. However, there is a limit where the hospitals may run out of resources to provide critical care, which is heavily dependent on (...) trained staff, just-in-time supply chains for clinical consumables and drugs and advanced equipment. In the first phase, we can still do clinical prioritisation and decision-making as usual, based on the need for intensive care and prognostication: what are the odds for a good result with regard to survival and quality of life. In the next, the resources are mostly available, but the system is stressed by many patients arriving over a short time period and auxiliary beds in different places in the hospital being used. We may have to abandon admittance of patients with doubtful prognosis. In the last phase, usual medical triage and priority setting may not be sufficient to decrease inflow and there may not be enough intensive care unit beds available. In this phase different criteria must be applied using a utilitarian approach for triage. We argue that this is an important transition where society, and not physicians, must provide guidance to support triage that is no longer based on medical priorities alone. (shrink)

The multilevel model of meme diffusion conceptualizes how mediated messages diffuse over time and space. As a pilot application of implementing the meme diffusion, we developed the social media analytics and research testbed to monitor Twitter messages and track the diffusion of information in and across different cities and geographic regions. Social media analytics and research testbed is an online geo-targeted search and analytics tool, including an automatic data processing procedure at the backend and an interactive frontend user interface. Social (...) media analytics and research testbed is initially designed to facilitate searching and geo-locating tweet topics and terms in different cities and geographic regions; filtering noise from raw data ; analyzing social media data from a spatiotemporal perspective; and visualizing social media data in diagnostic ways. Social media analytics and research testbed provides researchers and domain experts with a tool that can efficiently facilitate the refinement, formalization, and testing of research hypotheses or questions. Three case studies are introduced to illustrate how the predictions of meme diffusion can be examined and to demonstrate the potentials and key functions of social media analytics and research testbed. Ethical approval must be obtained before medical research can start. We describe the differences in EA for an pseudonymous, non-interventional, observational European study. Sixteen European national coordinators of the international study on very old intensive care patients answered an online questionnaire concerning their experience getting EA. N = 8/16 of the NCs could apply at one single national ethical committee, while the others had to apply to various regional ECs and/or individual hospital institutional research boards. The time between applying for (...) EA and the first decision varied between 7 days and 300 days. In 9/16 informed consent from the patient was not deemed necessary; in 7/16 informed consent was required from the patient or relatives. The upload of coded data to a central database required additional information in 14/16. In 4/16 the NCs had to ask separate approval to keep a subject identification code list to de-pseudonymize the patients if questions would occur. Only 2/16 of the NCs agreed that informed consent was necessary for this observational study. Overall, 6/16 of the NCs were satisfied with the entire process and 8/16 were unsatisfied. 11/16 would welcome a European central EC that would judge observational studies for all European countries. Variations in the process and prolonged time needed to get EA for observational studies hampers inclusion of patients in some European countries. This might have a negative influence on the external validity. Further harmonization of ethical approval process across Europe is welcomed for low-risk observational studies. Getting ethical approval for low-risk, non-interventional, observational studies varies enormously across European countries. (shrink)

This longitudinal study examined the development of moral judgement in 37 nursing students attending a university in Suwon, Korea. The participants completed the Korean version of the Defining Issues Test to allow analysis of their level of moral judgement. The development of moral judgement was quantified using ‘the moral development score’ at each stage (i.e. the six stages detailed by Kohlberg) and the ‘P(%) score’ (a measure of the overall moral judgement level). The results were as follows: (1) the moral (...) development score for stage 5A was consistently the highest across the four years of the students’ course, showing significant differences in some sociodemographic factors including home, birth order and monthly income; and (2) the P(%) score was higher in fourth-year (47.47 ± 11.21) than in first-year (46.13±9.73) students. There was no significant difference in the P(%) score according to sociodemographic factors. Further studies will examine in detail the correlation between curriculum and moral judgement development. We suggest that courses in ethics education should be made more relevant. (shrink)

Living organisms are the ultimate survivalists, having evolved phenotypes with unprecedented adaptability, ingenuity, resourcefulness, and versatility compared to human technology. To harness these properties, functional descriptions and design principles from all sources of biodiversity information must be collated − including the hundreds of thousands of possible survival features manifest in natural history museum collections, which represent 12% of total global biodiversity. This requires a consortium of expert biologists from a range of disciplines to convert the observations, data, and hypotheses into (...) the language of engineering. We hope to unite multidisciplinary biologists and natural history museum scientists to maximize the coverage of observations, descriptions, and hypotheses relating to adaptation and function across biodiversity, to make it technologically useful. This is to be achieved by developments in meta‐ taxonomic classification, phylogenetics, systematics, biological materials research, structure and morphological characterizations, and ecological data gathering from the collections − the aim being to identify and catalogue features essential for good biomimetic design. The great academic disputation of Ho-rak had been made for about 200 years since it began in 1709. An argument that the human nature and the nature of things are same or different was one of the main subjects for the great academic disputation. Gan Lee (李柬, 1677-1727) and Wonjin Han (韓元震, 1682-1751) were leading discussants of the argument. After Lee and Han, Neo-Confucian scholars in Joseon dynasty attempted comprehensively to synthesize their two views. But such Scholars as Seong-ju Lim (...) (任聖周, 1711-1788) and Jeongjin Ki (奇正鎭, 1798-1879) criticized a reformulation of One Principle and Many Differentiations. Lim argued that there was not only One Principle but also one vital breath as well as Many Differentiations of vital breath as well as One Principle. His idea of One Principle is relevant for a similarity between the natures of humans and things, and his idea of Many Differentiations is relevant for a difference between the natures of humans and things. Lim grasped that all the natures of things had two aspects of similarity and difference. But Gi criticized that their discussants were too narrowly specific in arguing this issue. He argued that One principle and Many Differences entailed each other. Like Lim's idea, his idea confirmed the sides of both, too. The nature of original substance is equivalent to the nature of existence. It seems to me that such a fruitful result of Korean philosophy, which an argument between the natures of humans and things had been distinctly made, for the future, will be an area of being made a deeper exploration from various aspects. Rudolf Carnap hat nur im Vorwort zum „Logischen Aufbau der Welt“ Bemerkungen zum Verhältnis seiner wissenschaftlichen Arbeit zur Kunst und Architektur seiner Zeit veröffentlicht. Aber sein emphatisches Bekenntnis zur Moderne der 20er-Jahre hat bisher nur selten Aufmerksamkeit in der philosophiegeschichtlichen Sekundärliteratur gefunden. In meinem Beitrag versuche ich in den ersten beiden Abschnitten, seinen kulturellen Hintergrund seit seiner Schul- und Studentenzeit zu skizzieren und dann seine persönlichen Kontakte und Austauschbeziehungen mit Vorkämpfern der Moderne wie Franz Roh und Siegfried Giedion zu beschreiben. (...) Nach diesen Vorbereitungen wird die Frage gestellt, ob und in wieweit Carnaps Philosophie selbst als ein Teil der Bewegung der Neuen Sachlichkeit verstanden werden kann oder sogar muss. Zur Beantwortung werden eine Reihe von Charakteristika dieses Zeitgeist-Phänomens aus den – gleichzeitig mit Carnaps „Aufbau“ erschienenen Werken der Genannten – herauspräpariert und dann untersucht, ob diese Kriterien sich auch im Werk Carnaps finden. Der vorliegende Band enthält eine Sammlung von Beiträgen zum Problem der Wechselwirkung zwischen Geist und Materie, einem der zentralen Probleme europäischer Geistesgeschichte. Die Blickwinkel, die dabei eingenommen werden, sind vorrangig die der Physik und der Psychologie. Die Wechselwirkung dieser Gebiete wird so deutlich wie nie zuvor im Dialog zwischen zwei Forscherpersön lichkeiten dieses Jahrhunderts sichtbar: dem Physiker Wolfgang Pauli (1900- 1958) und dem Psychologen Carl Gustav Jung (1875-1961). In zahlreichen Brie fen und Manuskripten Paulis, die erst in den letzten (...) Jahren allgemein zugänglich wurden, finden sich bemerkenswerte und wichtige Beiträge zu diesem Dialog, die das Verständnis des Zusammenhanges von Geist und Materie in einem neuen Licht erscheinen lassen. Um den durch Pauli und Jung begonnenen Dialog fortzusetzen und weiter fruchtbar zu machen, ist das interdisziplinäre Gespräch zwischen Physikern und Psychologen nötig. Diesem Zweck diente eine von der Eidgenössischen Techni sehen Hochschule Zürich (der Hochschule, an der Pauli tätig war) und dem C.G.Jung-Institut Zürich (das Jung gegründet hat) gemeinsam veranstaltete Tagung im Centro Stefano Franscini (Monte Verita, Ascona) vom 13. bis 18. Juni 1993. Sie stand unter dem Thema Das Irrationale in den Naturwissenschaften: Wolf gang Paulis Begegnung mit dem Geist der Materie und wurde von Pier Luigi Luisi initiiert und organisiert. Xunzi is a Confucian scholar well known for his theory that no human nature is innately good. However, because it is in human nature to be greedy, it is possible to train the people through promise of reward and punishment. Xunzi has long been considered to have taught Han Feizi who has a similar notion of human nature and uses it as basis for his Legalist theories. In this paper, I compare the two philosophers based on their system of reward (...) and punishment in order to analyze the difference and the similarities between the two. Although the two philosophers share an understanding of human nature, and a system of reward and punishment based on such theories, whereas Xunzi has a belief in the possibility of edification through good influence, Han Feizi does not. Xunzi asserts that mercy is more efficient than punishment in case of petty thefts among the poor, and also that even the severest punishment cannot push a soldier to risk one’s life in battle. This goes directly against Han Feizi’s strong belief in the absoluteness of the rule of law, where no mercy is possible even for the poor, and a rigid system of punishment and reward is seen as sufficient to push the people toward risking their lives for their king. On the other hand, although Han Feizi is often criticized for his lack of regard for the people’s lives in pursuit of power for the absolute monarch, it may be seen that such unforgiving application of the law results in an orderly society which benefits the people in the long run. Therefore, it may be argued that both Xunzi and Han Feizi’s long term goal for the legal system is to bring order to society. A comparison between the two scholars proves that no one-dimensional analysis is valid, and renders it possible to understand their philosophies in a much more colorful light. (shrink)

Mead's lectures on Movements of Thought in the Nineteenth Century offer a surprise: Mead, despite having attended his lectures from 1889 to 1891, does not mention the name of Wilhelm Dilthey, who nowadays is regarded as one of the classical authors of nineteenth-century philosophy. Mead's lectures lack any sign of awareness concerning the (...) hermeneutic tradition, represented by such names as Friedrich Schlegel, Schleiermacher, Droysen and, of course, Dilthey. Nevertheless it is, as I would like to show in the following pages, quite obvious that Mead's own philosophical conception shows some striking parallels with Dilthey's thought, which can be explained partly through direct influence, partly through the similarity of the questions both tried to answer. For Mead's missing reference to the mentioned tradition, I see two complementary explanations. Firstly, the philosophical importance of the hermeneutic tradition in nineteenth-century philosophy was mainly a retrospective matter. Only after 1927, when Martin Heidegger published Being and Time, did philosophical interest in hermeneutics begin; and then it took thirty-three years more till Hans-Georg Gadamer's Truth and Method brought about a historical interest in the development of hermeneutic thought. Secondly, Mead's own passion for the development of science may have forced him to neglect these philosophical movements, which seemed to resist the autocracy of (natural) science. Wilhelm Dilthey's thought, especially around the year 189o, can systematically be interpreted as a common starting-point for two of the most important directions of contemporary philosophy: pragmatism and hermeneutics. It is but a small exaggeration to say that G. H. Mead drew out the pragmatic, Martin Heidegger the hermeneutic consequences of Dilthey's concept of [661 ] 662 JOURNAL OF THE HISTORY OF PHILOSOPHY 33:4 OCTOBER ~995 Seelenleben, understood as a "best~indige Wechselwirkung zwischen unserem Eigenleben und dem Milieu. ''1 Mead's phase-model of acting, his whole idea of understanding meaning as the result of a social process, are paralleled and partly anticipated by Dilthey, as is Heidegger's conception of Dasein as "beingin -the-world" and his attempt to understand the inner historicity of life.2 In pointing this out, I do not want to claim that Mead was conscious of the systematic relation of his theories to Dilthey's work; nevertheless I will argue that Dilthey's ideas may have served as a source of inspiration and, furthermore, that this inspiration was made possible through some important similarities in the systematic architecture of both hermeneutics and pragmatism.3 I begin by working out the historical dimension, which Hans Joas characterized as "Meads biographische Abkunft von Dilthey"4 (section l). Cet article se propose de préciser la relation entre l'épanouissement de l'autobiographie allemande de la fin du XVIIIe siècle et l'essor de la bourgeoisie en s'appuyant sur la notion de carrière de Luhmann. Cette notion rend compte de la nécessité pour l'individu, dans la société fonctionnelle bourgeoise, de trouver une identité personnelle et sociale grâce à la description de sa vie sous forme orale ou écrite. L'autobiographie de Jung-Stilling, publiée au fur et à mesure de son ascension sociale, montre (...) bien le rôle extraordinaire de ce récit dans la constitution de l'identité et l'amélioration des chances pratiques de son auteur. Au contraire, le roman psychologique, Anton Reiser, de K. P. Moritz décrit l'impossibilité du héros à poursuivre une carrière et à construire son identité à cause de son imagination débordante. Wolfgang Pauli is well recognized as an outstanding theoretical physicist, famous for his formulation of the two-valuedness of the electron spin, for the exclusion principle, and for his prediction of the neutrino. Less well known is the fact that Pauli spent a lot of time in different avenues of human experience and scholarship, ranging over fields such as the history of ideas, philosophy, religion, alchemy, and Jung's psychology. Pauli's philosophical and particularly his psychological background is not overt in his (...) scientific papers and was unknown even to many specialist scholars until a number of enthralling and perplexing documents of a close interaction between Wolfgang Pauli and the psychologist Carl Gustav Jung became publicly available in recent years. Both scholars stressed the inseparability of the physical and the psychical and called upon a sense of more openness toward the unconscious. Decades after his death, Pauli's innovative perspective and his vision of a wholeness of psyche and matter are more than ever before of great relevance. (shrink)
